[Surgical treatment of spasmodic torticollis: effectiveness of microvascular decompression].

We report two patients of spasmodic torticollis who underwent different types of surgery according to their pathogenetic mechanisms. The first case was a 42-year-old man who had two years history of right-sided antecollis. EMG of the sternocleidal muscle on the affected side showed pathological findings. Characteristic findings on the vertebral angiography were a low position of the branching of tortuous PICA. At operation, we confirmed sandwich-like compression of left accessory nerve between the PICA and vertebral artery. Decompression of the nerve from these arteries led to a remarkable relief of the symptom. The second case was a 70-year-old man whose neck had been forced to bend backward; retrocollis over two years. EMG study was negative. The vertebral angiography showed no abnormality, either. Because there was no evidence of vascular compression on both accessory nerves at surgery, we chose bilateral C1-3 anterior root section which was effective in consequence. We found so far sixteen cases of spasmodic torticollis in the literature including ours who were benefited by microvascular decompression (MVD). All of them presented antecollis or horizontal type. The characteristic operative finding was the compression of the accessory nerve located at between the dural perforation of the vertebral artery and dural attachment of C1 dentate ligament. We can therefore propose a mechanism that the compression at the junction of the central and peripheral myelin causes such a neuroexcitatory symptom as in cases of facial spasm and trigeminal neuralgia.(ABSTRACT TRUNCATED AT 250 WORDS)

[Acinic cell tumor of the bronchial gland].

The patient was a 29-year-old male with a history of recurrent pulmonary infection for the past two years. Bronchoscopy showed complete obstruction of the left sixth bronchus by a submucosal tumor. Biopsy of the tumor revealed clear cell neoplasia suggesting bronchial gland origin. Left sixth segmentectomy was done. Grossly, the tumor was 1.5 cm in diameter, solid, yellowish white in color, round in shape and protruding into the lumen and covered by the bronchial mucosa. The lung tissue and mediastinal lymph nodes were free of tumors. Histologically, the tumor consisted of sheets of clear or granular basophilic cells. Mucicarmine-negative, diastase-resistant PAS positive granules were evident in the tumor cells. Immunohistochemical stain of amylase (salivary gland type) was positive in the tumor cells. Multiple psammoma bodies were present. Histological diagnosis was acinic cell tumor of the bronchial gland. The patient is alive and well one year after removal of the tumor.

[Transurethral ureterolithotripsy under hydraulic ureteral dilatation].

Transurethral ureterolithotripsy was performed in 32 patients with ureteral stones. A rigid ureteroscope was used and for the purpose of ureteral dilatation a hydraulic ureteral dilator (Ureteromat) was used. In total 39 stones were treated and of these 30 stones (77%) were successfully extracted or disintegrated. The success rate according to the location of the stone was as follows; 50% for the upper ureteral stone, 55% for middle ureteral stone and 96% for lower ureteral stone. As a complication of the procedure a lower ureteral stricture was noted in one patient. Hydraulic ureteral dilatation is a useful method of ureteral dilatation in transurethral ureterolithotripsy.

Acute pancreatitis as a complication of Crohn's disease.

Two cases of Crohn's disease coincidental with acute pancreatitis are described. Both patients were diagnosed as suffering from acute pancreatitis by hyperamlasemia and ultrasonography. Both had received a previous diagnosis of Crohn's ileocolitis. Crohn's disease was active in one patient at the onset of pancreatitis but was inactive in the other. We saw no factor that was obviously responsible for the pancreatitis in these patients. We also review other cases of Crohn's disease accompanied with pancreatitis that have been reported previously in the literature. In six cases, no cause was established for the pancreatitis, other than the Crohn's disease itself. We assume that pancreatitis, although rare, may be one of the extra-intestinal complications of Crohn's disease.

[The effectiveness of FID-CT (functional image of dynamic CT) for the evaluation of early cerebral ischemia].

A computer program was newly developed to display the ischemic area from the serial dynamic CT scan, namely the functional image of the dynamic CT (FID-CT). The principles of FID-CT are as follows; Seven rapid-sequence dynamic CT scans were taken following a peripheral bolus intravenous injection of 40 ml of iopamidol. As the data from each scan can be separated into three consecutive segments, we obtained 21 images during 44 seconds. Time density curves of each pixel were calculated employing Gamma variate fitting method. Eight functional parameters obtained from this curve were displayed with gray-scale pixel by pixel. These eight images were functional images obtained from dynamic CT. Only three or four minutes were required to complete all the calculations. Twenty-two patients were examined with both FID-CT and 123I-SPECT. In each case, the lesion detected by FID-CT was remarkably consistent with that shown by SPECT. Two representative cases were presented. The authors believe that FID-CT is a very useful diagnostic method in the acute stage of cerebral ischemia because the method can be quickly and easily performed and it discloses the ischemic area with fair certainty.

[Sleep apnea in well-controlled myasthenia gravis].

UNLABELLED: Polygraphic monitoring of the respiratory status during sleep provides a definitive test for the presence of the sleep apnea syndrome. This syndrome has been recognized in various kinds of neurological disorders. However, as far as we know, there have been no reports of describing the sleep apnea syndrome in myasthenia gravis (MG). We conducted overnight polygraphic sleep studies of 10 patients of clinically well-controlled MG. PATIENTS: We examined 10 patients of MG (4 men and 6 women). The subtypes of MG were IIA(1), IIB(8) and V(1) according to Osserman's criteria. RESULTS: Six patients, with an average age of 47.8 and the average duration of 6.2 years, had polysomnographically obstructive and central types of sleep apnea, appearing longer than 10 seconds and more than 30 times in one night. Their mean Apnea Index (AI) was 14.6. The duration of mean apnea was 23.4 seconds and 99.0 times the average frequency. On the other hand, in four patients of MG, averaged age at 30.8 and for a 0.9-year duration, the sleep apnea syndrome was never seen. Therefore, the longest duration of MG tended to have the sleep apnea syndrome. DISCUSSION: Six out of the 10 patients of MG had obstructive and central types of the sleep apnea syndrome. Since their respiratory functions examined in a daytime were normal and the physical findings concerned with MG were well controlled by medications, our findings of nocturnal sleep apnea might be indicative of a central cholinergic system disturbance in MG during sleep.

[HLA study in 108 Japanese patients with Crohn's disease].

We attempted to clarify the association between HLA and Crohn's disease. HLA-A, -B, -C, -DR and -DQ locus antigens in 108 Japanese patients with Crohn's disease were analyzed and the results were compared with findings of 472 healthy Japanese. In patients with Crohn's disease there was a strong positive association with HLA-DR4 (chi 2 = 14.086, Pc less than 0.005, RR = 2.231), and a weak positive association with HLA-B51, -Bw54, -DRw12, -DRw13 and -DRw52. While there was a strong negative association with HLA-DR2 (chi 2 = 10.194, Pc less than 0.025, RR = 0.435), DQw1 (chi 2 = 14.680, Pc less than 0.001 RR = 0.442) and DQw3 (chi 2 = 7.760 Pc less than 0.025 RR = 0.549), and a weak negative association with HLA-B7. In conclusion, susceptibility to Crohn's disease may relate to HLA-DR4, especially HLA-DR4.1, in Japanese patients.

X-radiation-induced differentiation of xenotransplanted human undifferentiated rhabdomyosarcoma.

A serially xenotransplantable strain of undifferentiated embryonal rhabdomyosarcoma originating from the nasal cavity of a 42-year-old woman has been established in our laboratory. After radiotherapy for the tumor donor, distinct rhabdomyoblastic differentiation of the undifferentiated sarcoma cells appeared in the primary lesion, and it is a reasonable assumption that X-irradiation has a certain potentiality to induce morphologic differentiation of tumor cells. To study this possibility, tissue fragments of undifferentiated embryonal rhabdomyosarcoma that had grown to more than 10 mm after being transplanted to nude mice were selectively irradiated in situ. The degree of rhabdomyoblastic differentiation according to radiation dose was evaluated by light and electron microscopy and by immunostainability for myoglobin, creatine phosphokinase-MM, and desmin. Distinct morphologic differentiation of undifferentiated sarcoma cells could be induced by repeated X-irradiations at several-week intervals.

Natural history of gastric adenomas in patients with familial adenomatosis coli/Gardner's syndrome.

The natural history of gastric adenomas was studied in 26 patients with familial adenomatosis coli (FAC)/Gardner's syndrome. The patients were followed for an average of 6.8 years (1 year, 5 months to 15 years 4 months) and examined repeatedly using gastric radiography, endoscopy, and biopsy. Gastric adenomas were present in 13 (50%). During the follow-up period, there was a new appearance of gastric adenomas in six patients but no distinct change in number, size, and histologic features in the remaining seven patients. In a 51-year-old man, an antral adenoma that measured 13 mm in diameter and had an irregular central depression did not change in size or morphology during 4 years and 1 month, but malignant change was evident at the time of endoscopic biopsy. The findings indicate that in patients with this disease, gastric adenomas require careful repeated follow-up examinations using endoscopic biopsy and, if feasible, endoscopic removal is recommended.

Late-onset acid maltase deficiency associated with intracranial aneurysm.

The autopsy findings in a patient with late-onset acid maltase deficiency who died following rupture of a basilar artery aneurysm are reported. The patient developed muscular weakness and wasting at the age of 20 years and died 9 years later after suddenly falling into a coma. At autopsy a giant fusiform aneurysm was found in the basilar artery which had ruptured at the base of the pons, leading to a large haematoma in the pons. The cerebral arteries and arterioles showed extensive vacuolar degeneration of smooth muscle cells in the media. It is suggested that the aneurysm was induced by fragility of the arterial wall due to this smooth muscle change.

Clinical analysis of the small kidney associated with VUR.

Analysis was performed on the clinical features of children with VUR who had one or two kidneys that were at least -2 SD smaller than normal for age of its renal ratio. The prevalence of the small kidney was 15.6% (97/623 children). Of 107 small kidneys, 87 (82%) were associated with moderate (grade IV) to severe (grade V) reflux and renal scarring was evident on 95 kidneys (88%). About half of the kidneys were diagnosed as scarred atrophy and 40% as congenital hypoplasia. The renal growth after antireflux surgery was evaluated by the change of the renal ratio and it was found that about three-fourths remained small during the pre- and postoperative periods. Accelerated growth was only exceptional.

Effect of prostaglandin E2 on gamma-interferon and 1,25(OH)2D3 vitamin D3-induced c-myc reduction during HL-60 cell differentiation.

The effect of prostaglandin E2 (PGE2) on the reduction of c-myc expression during the differentiation of the human leukemic cell line, HL-60, was examined. PGE2, a potent inducer of intracellular cyclic AMP (cAMP) in HL-60 cells, augmented monocyte-associated cell surface antigens induced by human gamma-interferon (IFN-gamma) or 1 alpha,25-dihydroxyvitamin D3 (1,25(OH)2D3) in these cells. The elevation of intracellular cAMP was induced dose-dependently by PGE2, but not by IFN-gamma or 1,25(OH)2D3. Changes were also seen in functional differentiation, such as, the increase of phagocytic capability and superoxide generation. PGE2 also enhanced the reduction of c-myc expression and the down-regulation of transferrin receptor by IFN-gamma or 1,25(OH)2D3, whereas PGE2 alone did not induce these phenotypic changes. These data suggest that IFN-gamma and 1,25(OH)2D3 reduce c-myc expression of HL-60 cells by a mechanism other than the augmentation of intracellular cAMP.
