[Relation between hormone dependency and the estrogen receptor of mammary carcinoma--comparative study of rat and human mammary tumors].

A comparative study of rat and human mammary tumors was undertaken to clarify the mechanism of hormone dependency by the PAP method. In normal mammary glands, histological estrogen receptor ER (+) cells were observed in the ductal epithelium and acinal cells, while only ER (-) cells were noted in HAN. After overiectomy, 55.6% of the ER (+) cells changes to ER (-). Furthermore, histological changes were glandular atrophy, anaplastic changes, (with) the remaining ER (+) cells (being in the) insular group. Those ER (+) cancer cells seem to be different in hormone sensitivity. As for human mammary glands, proliferation of ER (+) cells was observed in papillary lesions as duct papillomatosis. In the cancer tissues as a whole, ER (+) and ER (-) cancer cells were mixed, showing a so-called mosaic pattern.

Control of phenotypic expression of cultured B16 melanoma cells by plant glycosides.

The effects of two plant glycosides, ginsenosides Rh1 and Rh2, on the growth and differentiation of mouse melanoma (B16) cells in culture were studied. These plant glycosides have a dammarane skeleton resembling a steroid skeleton as an aglycone. Ginsenoside Rh2 inhibits the growth of B16 melanoma cells, causes morphological alterations, and stimulates melanogenesis at high cellular density. When ginsenoside Rh2 was removed after 2 or 6 days of treatment, the growth rate recovered slightly but not completely, during the period of observation (4 days after removal). On the other hand, ginsenoside Rh1 does not inhibit the growth of melanoma cells even at concentrations over 100 microM but stimulates the expression of the melanotic phenotype. Ginsenosides Rh1 and Rh2, possessing a glucose molecule at C-6 and C-3, respectively, have very similar chemical structures, but their effects on B16 melanoma cells differ remarkably. While it appears that the degree of differentiation is inversely related to cell growth, the present observations suggest that the differentiation and growth capacity of this B16 melanoma subline are independent phenotypic expressions.

Multiple primary cancers subsequent to gastric cancer in Hokkaido Cancer Center.

The occurrence of multiple primary malignant neoplasms in 607 patients with gastric cancer (404 males and 203 females) and 166 patients with peptic ulcer (136 males and 30 females) was examined by follow-up study. All patients were gastrectomized and had follow-up data from 2 to 13 years. The observed number of multiple primary cancer cases was compared with the expected number calculated from person-years of cases multiplied by the sex/age-specific incidence in Hokkaido. The frequency of a second primary cancer, including synchronous cancer, in gastric cancer patients was higher than that in peptic ulcer patients, and this tendency was also confirmed by the standardized incidence in the UICC standardized population. However, this difference disappeared after subtraction of synchronous cases. Therefore, the chance of a second cancer in gastric cancer patients seemed to be the same as that in the general population at least one year after the operation.

[Treatment of small liver cancer].

Histopathological studies were conducted on the resected lesions in eight cases of small liver cancer, and therapeutic methods are discussed. Examinations revealed tumor cell infiltration into the fibrous capsule in 5/7 (71%), into extracapsular tissues in 3/7 (43%), and vascular invasion by tumor cells in 2/8 (25%) cases. Seven patients were complicated with liver cirrhosis and one patient with hepatic fibrosis. On the basis of preoperative TAE performed in three cases and studies on the resected indicating that tumor cells remained that have infiltrated into extracapsular areas, resection was considered to be necessary to attain radical cure. At 100%, the one-year survival rate was encouraging. However, two cases of recurrence would suggest that further studies are necessary on areas to be resected.

Intensive care nurse: job tension and satisfaction as a function of experience level.

It has been suggested that experienced ICU nurses are better able to cope with the complexity of their environment and, as a result, achieve greater satisfaction than their less-experienced counterparts. One hundred ten ICU nurses from a large Midwestern medical center were requested to fill out a job tension and job satisfaction questionnaire. The results showed that job tension and job satisfaction remained relatively constant as the number of years on the job increased, refuting the notion that work experience automatically facilitates a mastery of environmental demands. As expected, job tension was inversely related to job satisfaction.

Serious wound infections in burned patients.

Wound infections are a major problem in burned patients. To determine the rate of wound infection associated with initial wound size and the amount of open full-thickness wound, we prospectively studied all patients admitted within 1 week of burn injury during a 2-year period using weekly wound cultures. Wounds were treated with topical silver sulfadiazine and occlusive dressings. Burn wound excision and immediate grafting were initiated during the first postburn week. One thousand five hundred twenty-three patient weeks (483 patients) were evaluated. Serious burn wound infections developed during 55/185 patient weeks (42.3%) when the initial total burn (ITB) was greater than 40% body surface area (BSA). This decreased to 27/304 (8.9%) when the ITB was 21% to 40% and 60/1034 (5.8%) when the ITB was less than 20% BSA. Burn wound infections developed during 57/211 patient weeks (27.0%) when the initial full-thickness burn (IFB) was greater than 20% BSA. The rate of wound infection decreased to 73/776 (9.4%) when the IFB was 1% to 20% and to 12/536 (2.2%) when no IFB was present. We further analyzed the prevalence of serious wound infections in relation to the open wound size during the hospital course. Wound infections occurred during 47 of 96 patient weeks (49.0%) when the current full-thickness wound was greater than 10% BSA. The infection rate decreased to 76 of 594 (12.8%) and 17 of 833 patient weeks (2.0%) when the remaining full-thickness wound was reduced to 1% to 10% and less than 1% BSA, respectively (p less than 0.05). Early wound closure would appear to reduce the risk of serious wound infections, especially in patients with full-thickness burns.

Effect of a Ca2+ antagonist, nifedipine, on the experimental asthma mediated mainly by slow reacting substance of anaphylaxis.

In the asthmatic model mainly mediated by the endogenous slow reacting substance of anaphylaxis (SRS-A) induced by the antigen inhalation to passively sensitized guinea pigs, continuous intravenous infusion of nifedipine (Adalat) at a speed of 7 micrograms/kg/min depressed the airway open pressure by about 68% compared to the saline-treated group and produced a delay in the time to peak response. Moreover, nifedipine inhibited the response of the peripheral airway more strongly than that of the central airway. The same concentration of nifedipine inhibited the airway open pressure by about 43% compared to the saline-treated group in the asthmatic model induced by the inhalation of leukotriene C4. The effect of nifedipine on the central airway was shorter in duration than that on the peripheral airway. The inhibitory effect of nifedipine on the airway response was greater in the asthmatic model mediated mainly by the endogenous SRS-A induced by the antigen inhalation than in the asthmatic model produced by the inhalation of leukotriene C4.

[Adenocarcinoma arising in a bronchial cyst of the mediastinum].

A 39 year-old man with a mediastinal tumor was operated on. After the median sternotomy incision, a mediastinal cyst, which was connected to the right pulmonary hilus with a string, was found. The microscopic examination of the cyst revealed a bronchial cyst with ciliated columunar epithelium and muscle fibers. Adenocarcinoma was found in the mucosa of the cyst. This patient seemed to be the first reported instance of carcinoma in a mediastinal bronchial cyst.

Specific inactivation of alpha (+) molecular form of (Na+ + K+)-ATPase by pyrithiamin.

We have recently reported that pyrithiamin inhibits the activity of (Na+ + K+)-ATPase derived from neuronal but not non-neuronal tissue. In view of the recent demonstration that there are two molecular forms of (Na+ + K+)-ATPase in brain, the present study has examined which form of the enzyme is selectively inhibited by pyrithiamin. Experiments with sodium dodecyl sulfate-polyacrylamide gel electrophoresis showed that the treatment with pyrithiamin decreased the phosphorylation by [gamma-32P]ATP of alpha (+) in a dose-dependent manner while it had little effect on that of alpha. Furthermore, the treatment with pyrithiamin changed the sensitivity of the enzyme to ouabain and N-ethylmaleimide. The specificity of the action of pyrithiamin for alpha (+) was further examined in the developing rat brain (Na+ + K+)-ATPases and peripheral (Na+ + K+)-ATPases from various animals. The molecular form of rat brain (Na+ + K+)-ATPase was primarily alpha on the 14th day of gestation, while it was mainly alpha (+) on the 18th day of gestation and postnatal age. The enzyme from 14-day fetal rat brain was resistant to pyrithiamin inhibition more than other brain enzymes. In cardiac and renal (Na+ + K+)-ATPases from various animals, only the alpha molecular form was found except for dog cardiac preparation which contained alpha (+) and alpha. Pyrithiamin inhibited dog cardiac enzyme activity, while it exerted only minimal inhibition on other peripheral enzymes. It is concluded that pyrithiamin selectively inactivates the alpha (+) molecular form of (Na+ + K+)-ATPase.

Effect of methionine-enkephalin on the spontaneous electrical and mechanical activity of the smooth muscle of the rat portal vein.

In the longitudinal smooth muscle of the isolated rat portal vein, methionine-enkephalin (Met-enkephalin) increased the spontaneous contraction with a concentration as low as 10(-8)M. When the membrane activity was recorded using a microelectrode, Met-enkephalin enhanced the spike burst activity but without any effect on the resting membrane potential. Naloxone, phentolamine, atropine and reserpine pre-treatment did not inhibit the excitatory effect of Met-enkephalin on the spontaneous contraction. These results suggest that the excitatory effect of Met-enkephalin on the mechanisms involved in the automaticity may be a direct action on smooth muscle or relate to presynaptic action on a non-adrenergic non-cholinergic system.
