Reversible asymmetric septal hypertrophy in acute myocarditis. Serial findings of two-dimensional echocardiogram and thallium-201 scintigram.

Serial two-dimensional echocardiographic and thallium-201 scintigraphic findings are described in a patient with acute myocarditis diagnosed by endomyocardial biopsy. On the 4th day of illness, just before the onset of congestive heart failure, the echocardiogram showed asymmetric septal hypertrophy (IVS/PW = 16 mm/10 mm = 1.6) and thallium-201 scintigram showed the ventricular septal thickening. On the 8th day of illness, when severe congestive heart failure was seen, asymmetric septal hypertrophy disappeared (IVS/PW = 8 mm/8 mm = 1.0), the left ventricle dilated markedly (LVDd = 63 mm), and the wall motion became poor (EF = 0.24). After one month, when congestive heart failure and clinical inflammatory findings disappeared, the contractility somewhat improved (EF = 0.43), although marked left ventricular dilatation remained. Thallium-201 scintigram showed some scattered persistent perfusion defects, thinning of the ventricular septal thickening, and dilatation of the left ventricle. The right ventricular endomyocardial biopsy revealed the histologic findings of the late stage of acute myocarditis. It is concluded that transient thickening of the ventricular wall may represent early changes in acute myocarditis.

Antidiuretic effects of alpha- and beta-adrenoceptor agonists microinjected into the hypothalamic supraoptic nucleus in a water-loaded and ethanol-anesthetized rat.

Effects of catecholamines microinjected into the supraoptic nucleus in the hypothalamus on urine outflow in rats which were loaded with water and anesthetized with ethanol were studied. L-Norepinephrine, L-epinephrine and L-isoproterenol induced potent antidiuresis, showing similar time courses to each other. The ED50 values for L-norepinephrine, L-epinephrine and L-isoproterenol were approx. 25, 20 and 5 nmol, respectively. The D-isomer of isoproterenol demonstrated no significant antidiuretic activity. The effect of L-norepinephrine was inhibited strongly by preinjection of alpha- and beta-adrenoceptor antagonists. The effect of DL-isoproterenol was inhibited strongly by beta-adrenoceptor antagonists, but not affected by alpha-adrenoceptor antagonist. Among the three catecholamines, only L-epinephrine induced a tachyphylaxis in the antidiuretic effect. Visceral functions other than urine outflow such as blood pressure, respiration rate, heart rate and rectal temperature were not significantly changed by microinjecting antidiuretic doses of the catecholamines. The results demonstrated that stimulation of alpha- and beta-adrenoceptors in the supraoptic nucleus induced a potent antidiuretic effect.

Effect of activated lymphocytes on the regulation of hematopoiesis: suppression of in vitro granulopoiesis and erythropoiesis by OKT8+ Ia- T cells induced by concanavalin-A stimulation.

The effects of activated lymphocytes were studied in the regulation of in vitro hematopoiesis. Peripheral blood lymphocytes stimulated by concanavalin A (Con A) were cocultured with normal bone marrow cells in the assay system of hematopoietic stem cells. Con-A-stimulated lymphocytes and their supernatants showed significant suppression of in vitro growth of myeloid and erythroid progenitor cells (CFU-C, CFU-E, and BFU-E). Suppressive activity detected in the T-cell fraction was completely abolished by treatment with OKT3 or OKT8 monoclonal antibody and complement and 20 Gy radiation but not OKT4 or OKIa1 antibody and complement. These observations indicate that peripheral blood lymphocytes can be induced by Con-A stimulation to become suppressor T cells for myeloid and erythroid progenitor cells that are OKT8 positive, Ia negative, and radiosensitive. Together with our previous observation that CFU-C suppressor cells induced by alloantigen stimulation are radioresistant and OKT8- and Ia-positive T cells, it is suggested that in vitro hematopoiesis may be regulated by heterogeneous subpopulations of activated T-lymphocytes.

[A clinical study of radiotherapy with HCFU for advanced and recurrent breast cancers. Radiation and HCFU Study Group for breast cancer].

We have investigated the usefulness of combination therapy with radiation and HCFU for advanced and recurrent breast cancer according to a clinical controlled multicenter trial from 1982 to 1984. One hundred cases were registered and 82 of them were available. Treatment sites were the lymph nodes, skin, bone and lung, and the overall response rate was 58% in CR and 19% in PR, while the duration of remission was 18 weeks in CR. Side effects were found in 10% of the patients. Combination therapy with radiation and HCFU may be useful in multimodal treatment for advanced recurrent breast cancer.

Antithrombogenic elastomers: novel anticoagulant/complement inhibitor-controlled release systems.

We have introduced novel synthetic anticoagulant and complement inhibitors for controlled release systems with high biocompatibility. These drugs were molecularly designed for extremely high biospecific inhibition, are readily soluble in polar organic solvents, such that they have versatile applications in commonly used hydrophilic polymeric systems via an easily attainable one-step co-casting technique at a given amount of loading. Another characteristic feature of the controlled release system is that both release rate and duration are controlled by the material, formulation and fabrication variables. These are easily manipulated by the hydrophilicity of polymers, amount of loading and film thickness. The drug-impregnated system may generate a new dimension in the formulation of controlled release biocompatibility.
