On the ovarian bursa of the golden hamster. II. Intercellular connections in the bursal epithelium and passage of ferritin from the cavity into lymphatics.

Cell abutments in the ovarian bursal epithelium of the golden hamster included tight junctions, desmosome-like junctions, gap junctions and zonulae adhaerentes. The plasma membranes of adjacent epithelial cells were often closely apposed forming a zonula adherens at the apex of the intercellular space, but these did not fuse along the entire length of the intercellular space. Gap junctions and other intercellular junctions did not allow diffusion of ferritin into the junctional areas, but failed to prevent ferritin from filling the intercellular space basal to the junction. This suggested that these junctions were not continuous around the entire circumference of the cell. In general, intercellular connections in the bursal epithelium may be labile and this lability, as well as periodic distention of the bursa, may give rise to separation of epithelial cells. Ferritin injected into the bursal cavity reached lymphatics via three routes: (1) via stomata; (2) via gaps and pores; (3) via intercellular spaces. Transcellular transport did not make a major contribution to permeation of ferritin across the bursal epithelium, and ferritin particles did not enter fenestrated blood vessels within 30 minutes after injection. The variety of routes may facilitate rapid and complete drainage of fluid and cellular components from the bursal cavity.

[Clinical evaluation of combination hyperthermia and radiation therapy for superficial tumors].

The most effective treatment in hyperthermia is the combination therapy of hyperthermia with radiation therapy applied to superficial tumors. This is clear from the standpoint of biological theory as well as clinical experience. The advantage of combination hyperthermia and radiation therapy compared with radiotherapy alone is the boosting of treatment effectiveness and the expansion of efficacy against malignant tumors with reduced radiation sensitivity. Recently, even deep-seated tumors have been actively treated with combination therapy, and the effectiveness of this approach is beginning to be recognized. However, heating technique, temperature measurement and evaluation of effectiveness will have to be studied further in order to improve them.

[Syringomyelia, its pathogenesis and surgical treatment based on 4 cases' experience].

Four cases of syringomyelia, each were considered to have different pathogenesis of syrinx and presented different clinical and radiological pictures, are reported. Case I was associated with Chiari I malformation and the syrinx communicated with the fourth ventricle through the central canal, case 2 was associated with Chiari II malformation and the syrinx was not communicated with the fourth ventricle, case 3 was thought to be traumatic and case 4 to be arachnoiditis due to unknown etiology. Metrizamide CT myelography was most valuable diagnostic technique to disclose the syringomyelic cavity and its extension. The cases except case 1 showed central opacification without via fourth ventricle, suggesting transneural migration CSF as shown by Aubin et al. Surgical treatment, therefore, was different in each case to obtain normal CSF dynamics. Case 1 was treated by suboccipital craniectomy, muscle plugging to the obex and syringo-subarachnoid shunt. In case 2 syringo-cisternal shunt was done in addition to suboccipital craniectomy. In case 3 syringocisternal shunt was done after laminectomy. In case 4 syringo-peritoneal shunt was performed. All but case 4 were obtained favorable result and case 4 was unfavorable except the disappearance of girdle sensation. For traumatic or inflammatory syringomyelia with tight adhesion between pia and arachnoid membrane in subarachnoid space, syringocisternal shunt was good way to obtain normal CSF dynamics and was expected to relieve the neurological deterioration.

[Autosomal translocation and associated male infertility].

Four balanced autosomal reciprocal translocations were found through mitotic chromosome analysis among 72 subfertile males, 27 with azoospermia and 45 with sperm counts below 40 X 10(6)/ml. They were 46, XY, t(3; 20; 21) with azoospermia, 46, XY, t(14; 21) with sperm counts below 1 X 10(6)/ml, 46, XY, t(1; 19) lqh+ with azoospermia and 46, XY, t(3; 16) with sperm counts 27 X 10(6)/ml. Histological, cytogenetic and hormonal analysis were performed. Testicular biopsies from the first 3 carriers revealed complete spermatogenic arrest at the spermatocyte stage and meiotic studies of the same biopsies showed severe reduction in numbers of cells in 2nd meiotic division. In spite of severely defective spermatogenesis, serum gonadotropins of the carriers were within normal range, except for LH of the 4th case. Other chromosomal aberrations observed were 5 Klinefelter's syndrome, 2 autosomal minor variants (46, XY, 15p+ and 46, XY, 14s+) and 1 small Y.

[Hyperthermia in the treatment of cancer].

The expectation that hyperthermia would be effective in the treatment of cancer is attributable to a theoretical viewpoint based upon biological studies. Cancer cells, which characteristically are hypoxic with poor nutrition and low pH, are sensitive to temperatures over 42.5 degrees C. This is the theoretical basis which assures the effectiveness of hyperthermia when used in combination with radiation therapy. Previous clinical studies have shown that a combination hyperthermia and radiation therapy is an effective treatment for superficial tumors, thus expanding the range of cancer therapy. When used for deeply seated cancer, this combination enhances the efficiency of therapy. It is, however, necessary to improve the heating techniques and methods of thermometry. Fundamental studies are now being actively pursued in respect of combined hyperthermia and chemotherapy. However, inadequacy of clinical material hinders evaluation. The combination of whole-body hyperthermia with anti-cancer agents presents so far, unsolved problems, such as determination of applicable anatomical regions and prevention of complications.

[Clinical study of non-Hodgkin's lymphoma--results of the Kanazawa University-Kurume University Cooperative Study on Malignant Lymphomas].

Ninety-two patients with previously untreated non-Hodgkin's lymphoma (NHL) were studied multidisciplinarily. The high incidence of NHL in elderly men (older than 60 years) was confirmed. With regard to the initial site, 51 patients (55.5%) were affected in the nodal region while other patients were affected in the extranodal or Waldeyer's ring region. Eighty-two patients (89.1%) were diagnosed as having diffuse lymphoma, and 47 of these patients were further classified into large cell type (LSG classification). Surface marker analysis revealed that the incidence of B cell-type NHL was twice as high as that of T cell-type NHL. All patients were entered into a randomized trial of radiotherapy and/or combination chemotherapies (ACOP, ACOP-E and AclBEP regimens) according to clinical stage. More than 90% of the patients with early-stage NHL achieved complete remission (CR), and the CR rate for patients with advanced-stage disease was about 50%. All patients with CS I NHL are still alive at 27 months after diagnosis, and the survival rate of patients with CS II NHL at 35 months after diagnosis is 80%. However, the median survival time of patients with advanced-stage disease was estimated to be 16 months.

[Clinical evaluation of hyperthermia with radiofrequency capacitive heating in combination with radiotherapy].

Clinical studies of radiofrequency (RF) hyperthermia combined with radiotherapy have been applied on the superficial bulky tumors and deep-seated tumors. Some improvement during the local heating was achieved by applying the small pad and/or the overlay bolus onto the skin surface underlain by subcutaneous fatty tissue. The condition of effective heating on tumors had shown to have a tendency to lead a good response regarding reduction rates in a tumor size. However, for the clinical evaluation, we have considered that it should be suitable to apply such findings as one showing the necrotic degenerative changes on computed tomograms in addition to the evaluation guide on the local response.

[Hyperthermia in the treatment of cancer].

We have clinically experienced 192 cases of superficial tumors treated by combination hyperthermia and radiation therapy. Analysis of their treatment results has indicated various advantages of this combination therapy, as summarized in the following items. Mutual differences among pathological findings of treatment effectiveness are small. The smaller tumors are more effectively treated. However, even larger tumors more than 10 cm in size have good responses. There is a clear correlation between tumor temperature and treatment results. Even a small dose between 20 and 40 Gy brings a good response, and this is especially recognized in cases of recurrence after radiation therapy. Further studies are yet to be conducted for improvement of the heating device, thermal dose, and thermal mapping.

[ABEP chemotherapy in the treatment of advanced non-Hodgkin's lymphomas].

Nineteen patients with refractory non-Hodgkin's lymphoma were treated with a combination of N4-behenoyl-1-beta-D-arabinofuranosylcytosine (150-200 mg/m2), aclacinomycin (15 mg/m2), etoposide (70 mg/m2 i.v. or 100 mg/m2 p.o) and prednisolone (20 mg/m2) (ABEP regimen). Patients who achieved complete remission (CR) received 10 courses of consolidation therapy with ACOP-E chemotherapy (adriamycin, cyclophosphamide, vincristine, prednisolone and etoposide). Seven patients achieved CR and three partial remission. Twelve patients with fresh non-Hodgkin's lymphoma stage IV were treated with ABEP regimen. CR was obtained in eight patients (66.7%). The median duration of CR was 11+ alpha months. Hematological toxicity was a dose-limiting factor but this was manageable.

Regional distribution of thiamin pyrophosphokinase in rat brain.

The highest specific activity of thiamin pyrophosphokinase was found in the cerebellum, and lower activity in cerebral cortex and midbrain. The regional difference in the enzyme activity was similar to that in thiamin content and the influx rate in rat brain, suggesting that the enzyme is involved in the thiamin transport.
