Regulation of interleukin-6 expression in cultured human blood monocytes and monocyte-derived macrophages.

A culture system that allows human blood monocytes to differentiate into macrophages in vitro was used to study B-cell stimulatory factor-2/interleukin-6 (interferon-beta 2/26 kd protein) expression in mononuclear phagocytes. Using B-cell stimulatory factor-2 (BSF-2) cDNA and a polyclonal, monospecific antibody directed against human BSF-2, we find that strong interleukin-6 (IL-6) expression is initiated in cultured monocytes on stimulation with endotoxin. Maximally induced monocytic BSF-2/IL-6 synthesis (1% to 2% of total proteins secreted by monocytes) is more than ten times stronger than in terminally differentiated macrophages (approximately 0.1% of total secretory proteins). BSF-2/IL-6 mRNA was detectable as early as one hour after stimulation with endotoxin, reaching maximum levels three hours after stimulus. Interleukin-1 (IL-1) was able to stimulate IL-6 synthesis in monocytes, but not in macrophages. Tumor necrosis factor, interferon-gamma and interleukin-2 (IL-2) had no effect on IL-6 synthesis in monocytes or macrophages. We found five molecular weight forms of BSF-2/IL-6 to be secreted by monocytes of 21.5 kd, 23.5 kd, 24 kd, 26 kd, and 28 kd apparent molecular weight. The 26 kd and 28 kd forms were found to represent N-glycosylated molecules, which were not detectable on treatment of the cells with the N-glycosylation inhibitor tunicamycin. The 21.5 kd, 23.5 kd, and 24 kd BSF-2/IL-6 forms were unaffected by tunicamycin treatment. We conclude from our data that cells of the mononuclear phagocyte lineage are one of the main sites of BSF-2/IL-6 (interferon-beta 2/26 kd protein/HSF) synthesis.

[Left ventricular wall thickness in hypertrophic cardiomyopathy: comparisons of measurements by magnetic resonance imaging, cineangiography, and echocardiography].

To evaluate the usefulness of magnetic resonance imaging (MRI) in measuring regional wall thickness of the left ventricle, 11 patients with hypertrophic cardiomyopathy (HCM) were imaged. The measurements were compared with those obtained by two-dimensional echocardiography (2D-echo) and cineangiography. Regional wall thickness was ascertained at six locations including the base, middle, and apex of the interventricular septum (IVS) and left ventricular posterior wall (LVPW) from the two selected transverse sections. All of these six locations were also measured for the IVS by biventriculography and for the LVPW by left ventriculography with the left anterior oblique view. Only the base was measured for both the IVS and LVPW using 2D-echo because of the difficulty in obtaining good images for the middle and apex of the left ventricle. The measurements by MRI correlated well with those obtained by 2D-echo and cineangiography in the majority of locations. At the base, where the wall thickness was measured by the three methods, the measurements of the IVS correlated well each other (r = 0.63 to 0.81). However, a good correlation was obtained only between 2D-echo and MRI for the LVPW r = 0.75. The correlation between MRI and cineangiography was generally good for the IVS at any levels (r = 0.76 to 0.87), but not for the LVPW. This was partially due to the narrow range of data for the LVPW as well as to the difficulty in determining the epicardial border in left ventriculograms. In conclusion, regional wall thickness can be correctly measured by MRI in any locations of the left ventricle even at the apex, where measurement is difficult by other methods.

[Historical course of surgery in rectal cancer in Japan].

Rectal cancer surgery in Japan started to first step of Lisfranc type resection by T. Sato in 1887. Since H. Ito (1902) published in success of three cases of abdominosacral excision (ASE), ASE occupied the main position of rectal cancer surgery in Japan. In 1944, however, M. Kuru reported the superiority of abdominoperineal resection (APR), an orient of surgery had changed to APR from ASE with time. His operative mortality was 2.0% or less, and 5-year survival rate was more than 50%. D. Jinnai (1961) introduced endorectal pull-through operation, and in 1972 he exhibited an excellent survival and satisfying function following sphincter preserving operation, such as anterior resection and pull-through, for mid and upper rectal cancer. T. Kajitani (1975) and Y. Koyama (1977) improved the survivals through an extended pelvic nodes dissection. Today, the principle of rectal cancer surgery in Japan is a limited resection for early cancer, sphincter preserving operations for upper and mid-rectal cancer and extended pelvic nodes dissection for advanced lower rectal cancer.

[Local administration of recombinant interferon-beta to patients with cancer-associated body cavity fluids].

The effects of local administration of interferon-beta produced by recombinant DNA technology (GKT-beta, Kyowa Hakko) on malignant effusion were studied in cancer patients and evaluated on the basis of the Criteria for the Evaluation of the Clinical Effects of Solid Cancer Chemotherapy (effects on malignant effusion). Out of 13 cases of peritoneal effusion, the effects were evaluated in 9 cases. Four of these cases remained free from ascites for 4 weeks and were evaluated as complete response (CR). Administration of GKT-beta alone was performed in 4 of 7 cases of pleural effusion, and CR was obtained in 1 case each of breast cancer, gastric cancer and malignant mesothelioma. In cases of gastric cancer, CR was obtained in 5 of 10 cases. The above results suggest that GKT-beta is a very effective drug against malignant effusion.

[Clinical studies of male urethritis caused by Chlamydia trachomatis].

From January through June 1983, 178 male patients with urethritis were investigated on the etiology and epidemiological and clinical features. By using Chlamydia trachomatis direct specimen test (Micro Trak), C. trachomatis was detected in 60 (47.2%) among 127 patients with non-gonococcal urethritis, while it was detected 8 (15.7%) among 51 patients with gonococcal urethritis. The source of infection was a non-prostitute in 30% of chlamydial urethritis, while in gonorrheal urethritis only one patient was infected from a non-prostitute. Mean incubation period of chlamydial and gonorrheal urethritis was 28.5 and 9.2 days, respectively. In chlamydial urethritis, 33.3% of patients did not complain of urination pain and 20.4% showed no urethral discharge. Symptoms of chlamydial urethritis were milder than those of gonorrheal urethritis.

Right iliac metastasis occurring eight years after resection of invasive bladder cancer: a case report.

A case of transitional cell carcinoma of the bladder which metastasized to the right ilium eight years after resection of the original tumor without recurrence in the urinary tract is reported. Both the original and metastatic tumors were shown by immunoperoxidase technique to contain carcinoembryonic antigen (CEA). The patient received a combination of intra-arterial chemotherapy, irradiation and hyperthermia with partial response.

[Problems of hyperthermia in deep-seated tumors].

The results of the combined use of radiotherapy and hyperthermia for 27 cases of deep-seated tumors were evaluated, and the clinical efficacy of hyperthermia was summarized as follows: 1) Tissue image and size of tumor: Adenocarcinoma: 21 cases, 78%; and tumors 6 cm or larger: 22 cases, 83%. With radiotherapy alone, local control was impossible. 2) Effectiveness of treatment in terms of tumor size before and after treatment: CR 3 cases, PRa 7 cases, PRb and NA 8 cases each, and NE 1 case. Thus, the effectiveness of the combined use of hyperthermia is confirmed. 3) Treatment effectiveness was not correlated with temperature of tumor and thermal dose. Measurement of the deep-tissue temperature and its display must be fundamentally considered. 4) With deep-seated tumors, it is difficult to determine the effectiveness of the treatment accurately merely by the change in tumor size. In many cases, a large low-density area could be detected inside the tumor even though the size was not changed on the CT image. Standard judgement of treatment effectiveness must be determined on the basis of the change in CT image quality and histological findings of tumor. 5) Irradiation with 5000 to 7000 rads was performed in all CR and PRa cases. An adequate irradiation does is required, because it is applied to radioresistant cancer. Conformation radiotherapy is especially required, whereby X-rays are concentrated on the target region without irradiating important organs.

[Obturator foramen approach in vasovasostomy: studies on a cadaver].

We tested the obturator foramen approach first reported by Shafik in 1982, on an 84-year-old male cadaver. By this approach, the ductus deferens is by-passed through the obturator foramen for the reanastomosis in patients who require excision of a long segment of the ductus deferens. The length of the ductus deferens spared by this method was 10.5 cm. Preservation of the deferential artery and protection of the obturator nerve and vessels were considered to be essential in the operation. When the ductus deferens was cut more distal than the internal inguinal ring, this method was not feasible because the distal part of the vas was too short to come out to the upper scrotal region through the obturator foramen.

Bacteremia complicating acute leukemia with special reference to its incidence and changing etiological patterns.

Over the 15-yr period, 1972-1986, 194 episodes of bacteremia occurred in 132 patients with acute leukemia at the Third Department of Medicine, Kanazawa University Hospital, giving an incidence of 478 episodes per 1,000 hospital admissions. This incidence was at least twice as high as that in patients with chronic leukemia, malignant lymphoma, multiple myeloma or aplastic anemia, and about 40-fold higher than that in patients with all other internal diseases. The rate of occurrence of bacteremia, whether unimicrobial or polymicrobial, remained almost unchanged throughout the study period. The frequency of gram-negative bacilli decreased significantly, however, from 81% of the total isolates for the first 10-yr period to 50% for the second 5-yr period. Escherichia coli and Klebsiella pneumoniae were isolated in markedly decreasing frequency, but Pseudomonas aeruginosa and Enterobacter cloacae in relatively constant frequency. The majority of P. aeruginosa isolates belonged to a limited number of O-antigen groups, suggesting the possibility of nosocomial infection. On the other hand, the frequency of gram-positive cocci increased from 9 to 36%. Staphylococcus epidermidis, Enterococcus species, and Staphylococcus aureus emerged as important pathogens. Such a change in the spectrum of organisms was considered to coincide with the common use of the so-called second- and third-generation cephalosporins and central venous catheters. It is thus suggested that vancomycin be added to empiric antibiotic therapy, especially when gram-positive infections are clinically or microbiologically suspected, and that reducing the acquisition of P. aeruginosa from the hospital environment remains a priority in infection prevention.

[Rectal cancer therapy compatible with preservation of function].

Rectal cancer surgery causes often postoperative morbidities such as defecation disturbance, dysuria and male sexual impotence. Compatibility of cancer eradication and function preservation are the problem should be resolved in the rectal cancer surgery. Defecation function were preserved in the patients with middle and upper rectal cancer by anterior resection transsacral resection, invagination or pull-through operation. Since 1984, usage of suture instruments made it practicable to increase sphincter preserving operation up to more than 65% of rectal cancer. Postoperative 5-years survivals were 81% of anterior resection, 71% of invagination operation and 57% of pull-through. Autonomic nerves preserving operation (ANP), aimed to prevent the urinary and sexual disfunction were studied both sides of the cancer spreads and extend of nerve plexuses. And ANP were adopted to the 185 cancers, limited to the submucosa or the proper muscle coat, by Study Group of Welfare Ministry. Their postoperative disfunction decreased to 15% of urination and 21% of male potency, while 33% and 81% respectively following conventional operation. Local excision for early cancer, which are defined as mucosal or submucosal cancer are discussed.
