Extracellular matrix production by osteoblasts on bioactive substrata in vitro.

Some bone-substitute biomaterials have been classified as bioactive since they allow direct biological bonding to their surface in vivo. Using in vitro techniques, we have re-created the first stages of this biological bonding phenomenon and compared the initial, fibrillar, extracellular matrices produced by migrated primary osteoblast cell populations in contact with both dense and macroporous calcium phosphate substrata, apatite/bioactive glass composite (ABC) and 45S5 bioactive glass (BAG). The first formed fibrils in contact with these materials may be identified as collagen from their morphology as observed by scanning electron microscopy (SEM). However, the organization of this fibrillar material is significantly different on the five bioactive substrata examined. These in vitro findings may not only be related to both the surface morphology and surface chemistry of the substrata, but also correlated with their levels of in vivo bioactivity.

Interleukin-6 (B-cell stimulatory factor 2)-dependent growth of a Lennert's lymphoma-derived T-cell line (KT-3).

A T lymphoma cell line (KT-3) established from a patient with Lennert's lymphoma showed macrophage-dependent growth. Macrophage-derived factors were able to replace the macrophage functions. Experiments using a variety of cytokines demonstrated that KT-3 proliferated in response to recombinant interleukin-2 (rIL-2), rIL-4, or rIL-6 but did not proliferate in response to rIL-1 alpha, rIL-1 beta, rIL-3, recombinant granulocyte colony-stimulating factor (rG-CSF), rGM-CSF, recombinant interferon-alpha (rIFN-alpha), rIFN-gamma, recombinant tumor necrosis factor (rTNF-alpha), or native IFN-beta. Polyclonal rabbit anti-IL-6 antibody almost completely neutralized the activities of macrophage-derived factors or IL-6 but not IL-2 or IL-4. Scatchard plot analysis demonstrated that KT-3 cells indeed express IL-6 receptors. The results indicate that the macrophage-derived factor that supports the growth of KT-3 is IL-6 and suggest that macrophage-derived IL-6 may play an important role in the histopathogenesis of Lennert's lymphoma.

[Clinical application of MRI for urological malignancy. 2: Usefulness of various imaging modalities for local staging of bladder cancer; a comparison between MRI, CT and transurethral ultrasonography].

MRI of bladder cancer was accomplished in 28 patients. All of the 28 patients were simultaneously studied by CT, and 22 were also studied by transurethral US before operation. MRI is not as useful as US for local staging of bladder cancer. However, MRI is more useful for diagnosing extravesical extension and covers the deficit of the overdiagnosing tendency of US.

[Studies of clinical efficacy of ofloxacin against male urethritis caused by Chlamydia trachomatis].

A multi-institutional clinical study was performed on the efficacy of Ofloxacin against chlamydial urethritis in males between January and June, 1987. Chlamydia trachomatis was detected in 68 (38.2%) among 178 patients with male urethritis by the antichlamydial FITC monoclonal antibody technic (Micro Trak). Neisseria gonorrhoeae was isolated in 51 patients (28.7%) and C. trachomatis was also detected in 8 of them. Ofloxacin was administered at a dosage of 600 mg divided into 3 doses for 14 days. One hundred and forty-eight patients including 53 with chlamydial urethritis were evaluable for clinical efficacy. C. trachomatis did not disappear in 6 patients (13.6%) and 2 (8.3%) after the therapy for 7 and 14 days, respectively. After 7 days of therapy, both the pathogen and urethral excretions had disappeared from 79.6% of the patients with chlamydial urethritis, and from 84.8% of those with gonorrheal urethritis; and after 14 days of therapy they had disappeared from 94.1 and 90.9% of the patients, respectively.

Demonstration of antigenemia in patients with invasive aspergillosis by biotin-streptavidin enzyme-linked immunosorbent assay.

A biotin-streptavidin (B-SA) enzyme-linked immunosorbent assay (ELISA) was designated for the detection of Aspergillus carbohydrate antigens in serum. The sensitivity of the assay was considered to be the lowest concentration showing an absorbance above the mean of the pooled serum samples from normal adults (mean + SD = 0.118 + 0.018) plus 3 SD. This value was 1.6 ng/ml for the ELISA. The upper limit of negativity was determined by adding 0.055 to the absorbance of pooled serum samples from normal adults. The maximum absorbance value in 18 patients with invasive aspergillosis as proven by autopsy was 0.271 + 0.087 (mean + SD). Sixteen patients with invasive aspergillosis showed antigen levels ranging from 2.0 to 17.0 ng/ml. Among 14 patients from whom serum samples were sequentially obtained, 11 showed a progressive increase in concentration in accord with the progression of the infection. Only one serum sample from 42 normal subjects (0.101 + 0.025) showed false positivity for antigens. On the other hand, serum samples from 23 patients with other fungal infections (0.109 + 0.021), 48 hospitalized patients without infections (0.103 + 0.022), and 42 patients with bacterial sepsis (0.117 + 0.022) were negative for antigens. It was found that the sensitivity and specificity were 89% and 99%, respectively.

[Obstructive azoospermia treated by epididymovasostomy following vasovasostomy: a case report].

A case of secondary epididymal obstruction caused by vasal obstruction due to bilateral inguinal herniorrhaphy is reported. A 28-year-old patient, who had undergone right inguinal herniorrhaphy at the age of 3 and bilateral inguinal herniorrhaphy at the age of 25, was diagnosed as having obstructive azoospermia because testicular biopsy disclosed normal spermatogenesis. Vasography revealed bilateral vasal obstruction at the level of the inguinal canal. Bilateral microscopic vasovasostomy was performed but postoperative semen analysis showed azoospermia. At the operation only one sperm was found in the left vasal fluid while no sperm was found in the right. Postoperative vasography showed that the left vasovasostomy was accurate while the right vas was reobstructed. Microscopic epididymovasostomy using Silber's specific tubule technique was performed on the left side. The left epididymis was transected at its tail and numerous normal sperms were found in the epididymal fluid. Four months after the second operation, semen analysis showed normal sperm density of 34 x 10(6)/ml.

[Experience with combination chemotherapy consisting of methotrexate, vinblastine, adriamycin and cisplatin (M-VAC) in advanced urothelial cancer].

The M-VAC (methotrexate, vinblastine, adriamycin and cisplatin) regimen was used to treat 6 patients with metastatic transitional cell carcinoma of the urothelial tract. Three patients showed a partial response, all of them were made surgically disease free. Two of them are still alive 1 year and for 6 months after surgery with no evidence of disease and one other died of disease 11 months after surgery. The response in one case was no change and that in two others was a progressive disease. From our experience, we suggest that treatment with M-VAC is effective but that surgery after M-VAC appears essential for the successful management of the patients with metastatic transitional cell carcinoma of the urothelial tract.

[Combined immuno-radio-chemotherapy of head and neck non-Hodgkin's lymphoma].

Twenty-one previously untreated cases, who underwent the same protocol at our department from January 1984 until December 1986, were investigated. The following results were obtained. Non-Hodgkin's lymphoma accounted for 10.5% of all the head and neck malignant tumors at our department. The age range was from 23 to 76 years of age and had a peak in the forties. Fourteen were male and seven were female. According to the stage distribution, six cases were stage I (28.6%), nine were stage II (42.9%), four were stage III (19.0%) and two were stage IV (9.5%). Stage I and II accounted for 71.4%. By site grouping, palatine tonsil and nasal cavity accounted for 38.1%, respectively. Surgical therapy was presumably useful for a solitary lesion which resisted all conservative therapies. By combined therapy with radiotherapy, VAPE-Chemotherapy and OK-432-immunotherapy, the survival rates were 100% (stage I), 77.8% (Stage II), 50% (stage III) and 50% (stage IV). The mean survival rate was 76.2%.

A clinical index for the timing of blood cultures in febrile patients with acute leukemia.

In an attempt to define a clinical index for the timing of blood cultures in febrile patients with acute leukemia, subjective symptoms at onset of bacteremia were investigated in a total of 109 consecutive episodes. General malaise, chills, and nausea and vomiting were most frequently observed (66%, 59%, and 50%, respectively). The gastrointestinal (GI) symptoms including nausea and vomiting, abdominal discomfort and fullness, abdominal pain, and diarrhea were encountered in 72% of all the episodes, forming the second largest group next to those closely associated with high fever. These GI symptoms were usually mild and of brief durations, and their occurrence had no relation to sites of infections or etiology of bacteremia. In some cases, nausea and vomiting were aggravated by intensive antileukemic chemotherapy or massive GI bleeding. It was thus suggested that GI symptoms, particularly nausea and vomiting, concomitant with a remarkable, sometimes abrupt rise in temperature during granulocytopenia may serve as a useful index for the timing for blood collection for culture to improve the probability of detection of bacteremia.

[Current state of the problem of local hyperthermia in Japan].

The author reported the results of local RF hyperthermia in combination with radiotherapy applied to 700 cancer patients used jointly in a group of 7 Japanese institutes, the author being the chief of the group. The author also reported clinical results of thermochemotherapy applied to a limited number of patients in 4 institutes. A conclusion has been made that the use of hyperthermia, especially in combination with radiotherapy, holds promise. However, further optimization of the methods of heating and thermometry is required.
