Particular cases of Cushing's syndrome--one with adrenocortical carcinoma and the other with double adenomas.

Two particular cases of adrenocortical tumors associated with Cushing's syndrome were documented. In the first case, the steroid pattern suggested a malignancy, and the histology of the excised tumor confirmed it. In the second case, the left adrenal had two distinct tumors; one dark-brown, and the other yellow in color. Recent trends in the diagnosis and treatment of Cushing's syndrome were discussed.

Isolation and characterization of terpentecin, a new antitumor antibiotic.

A new antitumor antibiotic, terpentecin was isolated from the culture broth of strain MF730-N6. Strain MF730-N6, isolated from soil, was found to belong to the genus Kitasatosporia. The antibiotic was extracted with chloroform, purified by column chromatography using silica gel and Diaion HP-20 successively, and finally purified by high performance reverse-phase thin layer chromatography. The molecular formula of terpentecin was determined to be C20H28O6 (molecular weight, 364). The antibiotic inhibited the growth of Gram-positive and Gram-negative bacteria, and prolonged the survival period of mice bearing leukemia L-1210, P388 and Ehrlich ascites carcinoma.

[Clinicopathological studies on disseminated carcinomatosis of the bone marrow occurring through metastasis of gastric carcinoma].

Five cases of gastric carcinoma (one woman) were investigated. The average age of the patients was 44 and the clinical manifestation were lumbago, melena and fatigue. Two cases were Borrmann III, and other were Borrmann IV, IIc and IIc advanced, respectively. Three were signet-ring cell carcinoma and the other two were moderately differentiated and well-differentiated adenocarcinoma, respectively. All patients showed leukoerythroblastosis. Bone marrow specimens revealed myelofibrosis in three cases, while adenocarcinoma cells were also found in three. MHA or DIC was found in three patients, all of whom died of bleeding tendency. Mean survival time was 2.6 months. Thus, bone marrow carcinomatosis proved to be very severe.

[Diagnostic usefulness of stepwise discriminant analysis employing the values of CA125, TPA, IAP, CEA and ferritin in sera measured simultaneously for gynecological malignant neoplasms].

The values for CA125, TPA, IAP, CEA, and ferritin in sera were measured simultaneously in 68 healthy nonpregnant females and 133 patients with various gynecological diseases, and examined by stepwise discriminant analysis. The usefulness and the limits for diagnosis of various gynecological diseases were investigated for each tumor marker. Also, the diagnostic usefulness of stepwise discriminant analysis employing the values for five tumor markers in sera was studied for gynecological malignancies compared with that of measuring serum CA125 alone. Because the mean values for CA125 in sera were increased specifically in the ovarian cancer patient group compared with those of other tumor markers in sera, the measurement of serum CA125 was considered to be more useful in diagnosing ovarian cancer than that of the other tumor markers. The mean values for CA125 in sera, however, were also increased more significantly in the groups of patients with endometriosis and normal pregnancies than in the group of healthy nonpregnant females (p less than 0.005). In the stepwise discriminant analysis employing the values for CA125 and four other tumor markers in sera, the diagnostic usefulness of each tumor marker was demonstrated in the early diagnosis, the differential diagnosis, and the determination of complete remission after several therapies for ovarian cancers.

A novel lymphokine derived from human IgG Fc receptor-bearing B cells: its suppressive effect on activated T and B lymphocytes including tumour cells in vitro.

Peripheral blood FcR gamma-bearing human B cells, but neither T cells nor adherent cells, produce an immunoregulatory lymphokine after receiving the stimulation of FcR gamma by immune complexes--antibody-sensitized erythrocytes (EA). This factor suppresses polyclonal immunoglobulin (Ig) production of B cells to pokeweed mitogen (PWM) and Nocardia opaca delipidated cell mitogen (NDCM), indicating that not only B cells, but also T cells are targets for this factor. It also inhibits the proliferation of mitogen-activated mononuclear and T and B tumour cells in vitro. The inhibitory effect on tumour cell growth is cytostatic, but not cytotoxic as in the case of lymphotoxin (LT). All these suppressive effects are observed in a HLA-non-restricted manner. Irradiation (2000 rads) of FcR gamma + B cells does not inhibit the production of this suppressive factor, implying that DNA synthesis is unnecessary. Nonstimulated FcR gamma + B cells retain the precursor activity for Ig-forming cells, since mononuclear cells untreated with EA respond to the mitogens, resulting in Ig production. However, it is worthy to note that they lose the activity when stimulated with immune complexes. Thus, the property obtained from human FcR gamma + B cells is similar to, but distinct from, a murine suppressive B-cell factor (SBF) prepared by the same procedure as for the human factor. Nevertheless, the observation in the present studies on the human analogue to murine SBF suggests that this factor, tentatively termed human SBF, appears to be a novel lymphokine which is different from any other factors, including LT, and that FcR-bearing B cells play an important role in the immunoregulatory mechanism in humans, as in the case of mice.

Effector mechanisms of syngeneic anti-tumour responses in mice. I. Establishment and characterization of an exogenous IL-2-independent cytotoxic T-lymphocyte line specific for radiation-induced leukaemia RL male 1.

A CTL line (CTLL-D4) mediating specific cytolytic activity against radiation-induced leukaemia RL male 1 has been established and maintained on a long-term basis without the addition of exogeneous TCGF. This line was originally selected by the limiting dilution of MLTC cells from RL male 1-immune (BALB/c X C57BL/6) F1-nu/+(CB6F1-nu/+) spleen cells (500 cells/well) in the presence of 5% rat TCGF, 2000 rads-irradiated normal CB6F1-nu/+ spleen cells as the feeder cells, and 10,000 rads-irradiated RL male 1 tumour cells as the stimulator. After expansion only with the feeder and tumour cells, CTLL-D4 shows highly specific cytotoxic activity against RL male 1 by in vitro CMC assay, since cells such as RL male 6, RL female 8, RL female 9, P815, MOPC-315 (H-2d), EL-4 (H-2b) and YAC (H-2a) are not killed. Microcytoxicity assay of this line has revealed that CTLL-D4 comprises three subsets of T lymphocytes (100% Thy-1.2+): 15-25% Lyt-1+23-, 60-75% Lyt-1+23+ and 10-15% Lyt-1-23+. The proliferation of this line seems to depend largely upon the syngeneic MLR-like responsiveness of the Lyt-1+23- subsets of CTLL-D4 to the Ia-positive cells in CB6F1-nu/+ splenic feeder cells, and has been restricted to the H-2d-haplotype of the feeder cells. In spite of the vigorous cell proliferation by coculturing with the feeder cells alone, the cytolytic activity of this line begins to decrease after some 7 days of culture in the absence of the stimulator RL male 1 cells which have no capacity to stimulate by themselves. Thus, by long-term culture of CTLL-D4 with the syngeneic feeder cells alone, a new non-cytolytic line (D4f) was established. Mechanisms enabling the long-term maintenance of CTL activity and subset composition have been discussed in terms of cellular cooperation between the subsets of this line.

Effector mechanisms of syngeneic anti-tumour responses in mice. II. Cytotoxic T lymphocytes mediate neutralization and rejection of radiation-induced leukaemia RL male 1 in the nude mouse system.

We demonstrated the efficacy of a long-term cultured cytotoxic T-lymphocyte line, CTLL-D4, on tumour growth inhibition using athymic nude mice as recipients. CTLL-D4, specific for a unique surface determinant on a radiation-induced leukaemia RL male 1 of BALB/c origin, was obtained from the limiting dilution culture of MLTC cells performed between spleen cells of a CB6F1-nu/+ mouse immunized in vivo and RL male 1 stimulator cells, and cultured for several months in the absence of added TCGF as described in our preceding paper (Kuribayashi, 1985). The specific inhibition of tumour growth by CTLL-D4 was demonstrated both in Winn-type neutralization assay and in systemic transfer experiments. A subcutaneous inoculation of the mixture of CTLL-D4 and RL male 1 cells resulted in the complete inhibition of tumour growth, even at the effector to tumour cell ratio of 1:1, whereas non-cytolytic D4f, which was self-Ia antigen(s)-reactive, composed entirely of Lyt-1+23- T cells and derived originally from CTLL-D4 but completely lost its cytotoxic activity during culture with the irradiated syngeneic feeder cells alone, had no inhibitory effect at all. In the adoptive transfer studies, the subcutaneously established tumours were rejected by the single i.v. transfer of 2 X 10(7) CTLL-D4 cells into CB6F1-nu/nu mice. However, D4f was ineffective again in this systemic transfer system. When the effect of CTLL-D4 cells on tumour rejection in vivo was compared to that of non-cultured spleen cells hyperimmunized with RL male 1 cells, the former exhibited more rapid rejection in nude mice after i.v. transfer than the latter did, suggesting that CTLL-D4 cells also attack the tumour cells much more effectively as effectors in vivo. Thus, it is conceivable that CTLs are mainly involved in tumour rejection in this adoptive transfer system using RL male 1 tumour cells and athymic nude mice.

[A long survival case of carcinoma of the colon with hepatic metastasis following intra-arterial infusion chemotherapy].

A 61-year-old man with double colon cancers and a solitary hepatic metastasis underwent right hemicolectomy and catheterization of the proper hepatic artery in April 1979. Intraarterial infusion chemotherapy of 5-FU, to a total of 8,000 mg, and Mitomycin C, to a total of 40 mg, was performed for thirty-two days. During chemotherapy, calcification appeared and occupied the metastatic lesion. The patient is presently in good health and has been disease-free for five years and nine months. In this case, it was supposed that obstruction of the peripheral tumor vessels was the most important causative factor of tumor calcification. This case was clinically valuable because tumor calcification proved to be a marker of tendency toward cure and complete cure of cancer.

[Clinical application of local hyperthermia combined with antineoplastic agents].

At present hyperthermia shows great promise when combined with other modalities. Local thermo-chemotherapy may also be a very attractive research field, but the combination of hyperthermia and anti-cancer drugs is still poorly understood. In this review, the background and the clinical reports on heat and chemotherapy which have been reported during the last year were analyzed. Finally, our clinical experiences with local hyperthermia combined with anti-cancer drugs for gastrointestinal cancer were also reported.

[A case of primary adenosquamous carcinoma of the stomach preoperatively diagnosed by endoscopic biopsy].

A 53-year-old complained of upper abdominal pain. A diagnosis of Borrmann III type gastric cancer in the posterior part of the corpus region was made by X-ray and endoscopy, and primary adenosquamous carcinoma was preoperatively diagnosed by endoscopic biopsy. Subtotal gastrectomy was performed. The part of the adenocarcinoma adjacent to squamous-cell carcinoma was poorly differentiated. Adenosquamous carcinoma of the stomach is rare. Furthermore, preoperative diagnosis by endoscopic biopsy is extremely unusual, having been reported in 7 cases in Japan including ours. And in 5 cases out of 6, region of the adenocarcinoma was the poorly differentiated type.
