Fibrinolytic activity in experimental intracerebral hematoma.

In an attempt to investigate the role of tissue fibrinolytic activity in the resolution of intracerebral hematoma, an experimental model of intracerebral hematoma was developed in the rat. The fibrinolytic activity was studied using a histochemical fibrin slide technique. A total of 59 adult male rats were studied. Twenty-nine rats were used for developing the intracerebral hematoma model via injection of autologous whole blood into the left frontal lobe; in the remaining 30 rats, the intracerebral hematomas were studied sequentially. Intracerebral hematoma formation was unsuccessful in six (21%) of 29 rats. Four rats died in the immediate postoperative period and two showed no intraparenchymal clot. Intense fibrinolytic activity was demonstrated in the blood vessel walls of the normal brain, especially in the meninges, choroid plexus, and ependymal cell layer. In the initial stages of hematoma resolution, fibrinolytic activity was not seen in the hematoma or parenchyma except in the preexisting blood vessels. However, 3 to 5 days later, fibrinolytic activity was observed in the capillary buds surrounding the hematoma and among the infiltrating mononuclear cells. This activity increased for 7 to 10 days following formation of the hematoma and decreased after 21 to 28 days. It is concluded that tissue fibrinolytic activity associated with newly formed blood vessels appears to be important in lysis of intracerebral hematomas.

Intraoperative imprint cytology of the thyroid gland with computer-assisted morphometric analysis of cell clusters.

Imprint preparations were used in addition to frozen sections in the intraoperative diagnosis of 37 cases of benign and malignant lesions of the thyroid gland, including adenomatous goiter, follicular adenoma, follicular carcinoma and papillary carcinoma. In the imprints, the cytologic features specific for carcinoma, as compared with benign lesions, were (1) the folding of the nuclear contour, (2) the increased density of the cytoplasmic matrix and (3) the frequent appearance of cell clusters of larger size. The size and frequency of cell clusters were morphometrically analyzed by a computer image analyzer. There was an increasing number of large clusters, plus the appearance of clusters of more than 300 micron in diameter, in both follicular and papillary carcinoma. In benign lesions, on the contrary, the majority of cells were isolated or in small clusters, the diameter of which never exceeded 300 micron in diameter. These results demonstrate that (1) the imprint cytology of the thyroid gland is useful in making a rapid intraoperative diagnosis and (2) the introduction of computer-assisted quantitative analysis is of practical value in the diagnosis of malignancy.

[A case of small cell carcinoma of the esophagus].

The case of a 51-year-old man, complaining of a sense of pressure on his chest, is reported. Tests led to a diagnosis of a cancer in the midesophagus and a subtotal esophagectomy was performed. The tumor was found to be a protruded type, 8.5 X 7.0 cm in size. A histologic diagnosis revealed it to be a small cell anaplastic carcinoma. The tumor cells were found negative in a Grimelius staining and did not contain hormones such as ACTH. Some cytokeratins, however, were detected by an immunohistochemical examination, Cancer invasion into the adventitia was seen, though no lymph node metastasis or a distant metastasis was shown. Six months after operation, a recurrence in the mediastinum and in the distant lymph nodes was found. Chemotherapy and irradiation were found to be useful, and the postoperative survival period was 25 months.

Effects of fine-grained diet on annulospiral endings of muscle spindles in the masseter muscle in developing and adult mice.

In the masticatory muscles, neuromuscular spindles have a very important role in controlling the jaw movement since they act as stretch receptors in skeletal muscles. The continuous intake of fine-grained diet which is easily chewable leads to degeneration of the sensory endings of Ia fibers in many muscle spindles of the mouse masseter muscle in only 120 d after birth.

[Flow cytometry DNA analysis of a gastric cancer before and after intra-arterial anticancer chemotherapy--report of a case].

Flow cytometric DNA analysis has been performed on tumor tissue obtained by endoscopic biopsy before and after intraarterial infusion of ADM and 5Fu. The patient was a 74-year-old woman with a gastric cancer with an involvement of a para-aortic and cervical lymph nodes. Histological examination revealed a poorly differentiated adenocarcinoma, and flow cytometric DNA analysis disclosed that the DNA ploidy (DNA index) did not differ before and after intraarterial infusion of ADM and 5Fu. The fraction of the tumor cells in the S + G2.M phase of the cell cycle however, decreased from 56% to 44% with anticancer chemotherapy. A flow cytometric DNA analysis, using biopsied samples of tumor tissue, was found to be a useful tool for monitoring the effect of anti-cancer chemotherapy for solid tumors.

Effects of the infraorbital nerve-transection on the formation of Ia fibers in muscle spindles of the masseter muscle in developing mice.

The effect of the transection of the infraorbital nerve on d 20 after birth on the formation of the annulospiral ending of Ia fibers of muscle spindles in the masseter muscle was investigated in developing mice. On the 20th d after the operation, there were significant differences between control and treated animals in the ratio of muscle spindles with non-coiled endings to all muscle spindles, and the ratios of muscle spindles with non-coiled endings in the masseter muscle on the operated and sham-operated sides were about equal to each other. On the 30th d after the operation, the ratio for the masseter muscle on the operated side was approximately twice that for the one on the sham-operated side. The ratio for the operated side was about equal to that for 15-d-old control mice, and about twice that for 20-d-old control animals. Thus, the present results indicate that disorder of sensory inputs following the transection of the infraorbital nerve leads to degeneration and/or inhibition of the formation of annulospiral endings in muscle spindles of the masseter muscle, and suggest that disorder of the masticatory mechanism may be caused by the degeneration and/or inhibition of the formation of the endings in many muscle spindles of the masseter muscle.

Postnatal development of the annulospiral endings of Ia fibers in muscle spindles of the mouse temporal muscle.

The formation of the annulospiral endings of Ia fibers in muscle spindles was investigated in the temporal muscle of developing mice. From 15 to 20 d after birth, there were only a few spindles with "++" sensory endings. With growth of mice, the primary endings of the Ia fiber began to develop its spiral endings. And by the 30th postnatal day (that is, after weaning), almost all of the Ia fibers had completely coiled endings though the formation was still continuing in some spindles at this time. Formation of the annulospiral endings of the Ia fibers of muscle spindles in the temporal muscle was completed earlier than that of fibers in the spindles in the masseter muscle.

[A case of right adrenal myelolipoma diagnosed preoperatively and review of literature in Japan].

Fifty-nine years old female who complained of right back pain was diagnosed as having right adrenal myelolipoma preoperatively by ultrasound, CT and other examinations. The patient was cured by surgery. The resected tumor was 11.5 X 10.5 X 8.5 cm in size and 470 g in weight. Histopathological diagnosis was myelolipoma of Soós's type 1. Sixteen cases of myelolipoma, including our case have been reported in Japanese literature. The age of patients ranged from 20 to 60 (average 49.1) and male-female ratio was 11:5. Most patients presented with abdominal pain and abdominal mass, and had complication of obesity and hypertension. The lesion was more common on the right side (11:5). Their size was 3-21 cm (average 10.9 cm) and their weight was 10-1, 930 g (average 823 g). Adrenal myelolipoma can be certainly diagnosed preoperatively by ultrasound, CT and other examinations. Adrenal tumors showing normal adrenal endocrine function and fat density on CT examination indicate that they could be myelolipoma in their nature.

Results of transtrochanteric rotational osteotomy for nontraumatic osteonecrosis of the femoral head.

From 1976 to 1985, transtrochanteric rotational osteotomy was applied to 58 hips of nontraumatic necrosis of the femoral head. Of these, 52 hips were available for the present study. The follow-up periods ranged from one to ten years (average, 5.1 years). Satisfactory results were obtained in 69%, as there was no evidence of progressive collapse of the head. Hips that had had no collapse or slight collapse before osteotomy had a significantly better prognosis, compared with those that had marked collapse of the head. The extent of the lesion as depicted on the preoperative lateral roentgenogram also influenced the eventual function. Patient satisfaction was significantly greater when these lesions were less than 70% (31/37), compared with greater than 70% (5/15).

Steroid-induced osteonecrosis of the patella.

Two cases of previously unreported steroid-induced osteonecrosis of the patella were investigated. The first case was a 46-year-old man who had been receiving steroid therapy for three years for treatment of bronchial asthma. The second was a 32-year-old woman who had been receiving steroid therapy for nine years as treatment of systemic lupus erythematosus. Both reported slight knee pain. The roentgenographic findings revealed increased radiodensity with radiolucency of the upper one-half of the patella. A trephine biopsy revealed focal areas of dead trabecular bone.

Postnatal development of the mouse temporal muscle and effects of a fine-grained diet on the muscle spindle.

The postnatal development of the mouse temporal muscle and effects of a fine-grained diet on muscle spindles were investigated. On d 25 after birth, 3 types of extrafusal muscle fibers could be distinguished on the basis of SDH activity, but not on the basis of their diameters. And in 50-d-old mice, 3 types of the muscle fibers had different diameters each other. In intrafusal muscle fibers, the postnatal changes of the SDH activity were similar to those of extrafusal muscle fibers, but the diameter distribution showed unimodal statistical distribution from 10th to 50th postnatal day. On the other hand, in control 180-d-old mice, 80 percent of muscle spindles in the temporal muscle had complete annulospiral endings, but the ratio of muscle spindles with complete annulospiral endings was decreased significantly in mice fed a fine-grained diet. The diet also decreased the diameter of the endings significantly. These results suggest that the main changes in the morphological and functional properties of the mouse temporal muscle occur in during and early after weaning period, and that normal mastication is needed to maintain the homeostatic condition of the mouse temporal muscle.
