[Circulating immune complexes in uveitis].

The serum of 63 patients suffering from various clinical types of endogenous uveitis with unknown etiology was investigated for the presence of circulating immune complexes (CIC). None of the patients was suffering from a systemic disease usually associated with CIC. Significantly elevated CIC levels were detected in 2 women with chronic cyclitis. One of them was also suffering from paresis of the facial nerve. The investigation reported here indicates that C1q-binding CIC are of no importance in the pathogenesis of endogenous uveitis. On the one hand one should consider the possibility of local IC formation, on the other hand one should be aware of the fact that IC does not always represent a pathogenic factor but that it may also occur as a secondary phenomenon after non-immunologic tissue destruction.

Amikacin levels in human aqueous humor.

Amikacin is one of the newer aminoglycoside antibiotics. The penetration of amikacin into the human primary aqueous humor after intramuscular application was investigated. Comparing our results with reports concerning the blood aqueous permeability of gentamicin and tobramycin we could not find any significantly better penetration of amikacin into the human aqueous humor after i.m. injection. The advantage of amikacin lies in its effectiveness against bacteria resistant to other aminoglycosides.

[Immunological investigation in primary open angle glaucoma (author's transl)].

It is a matter of discussion in literature, if autoimmunological processes are involved in the pathogenesis of primary open angle glaucoma. Therefore we determined circulating Clq binding immune complexes, collagen antibodies, antinuclear antibodies and anit-nDNA-antibodies in the sera of patients with this disease. The result of our investigation and of recent papers lead to the conclusion that autoimmunological processes- are not involved in the pathogenesis of primary open angel glaucoma.

Collagen types in keratoconus.

Collagen types in normal human and keratoconus corneas were separated by salt fractionation and thermal gelation in the pepsin-soluble fraction of the lyophilized tissues. Peptic digestion indicated no significant differences between normal and keratoconus corneas. Further collagen characterization was performed using SDS-PAGE. Collagen concentration were determined via hydroxyproline. Soluble collagens from normal human cornea represent 85% collagen type I, maximally 10% collagen type III and 5% collagen type V. Soluble collagens of keratoconus corneas consist of 90% type I collagen and maximally 5% type II and type V collagen.

[True idiopathic exfoliation].

In contrast to pseudoexfoliation, true exfoliation is an extremely rare condition not associated with glaucoma. In true exfoliation the superficial layers of the anterior lens capsule are partially peeled off. This results in a fine homogenous water-clear transparent membrane floating in the anterior chamber. The etiology of this mostly bilaterally condition may be exposition to infrared rays, iridocyclitis, metallic intraocular foreign bodies and trauma. To our knowledge this is the first published report of a case of idiopathic true exfoliation of the lens capsule.

[Postoperative intraocular pressure following iris-clip lens implantation].

Within the past three years intraocular Binkhorst lenses have been implanted in 152 consecutive cases. In 36% of the intracapsularly operated cases (123 eyes) and 45% of the extracapsularly (29 eyes), intraocular pressure increased to 40 mm Hg for three to four days postoperatively. Pseudophakic-pupillary block glaucoma was not observed in any of the cases. Fourteen days postoperatively no case required glaucoma treatment. As in normal aphakic eyes, intraocular pressure dropped to subnormal values 8 weeks postoperatively in 6% of the cases.

[Immunopathogenesis of primary open-angle glaucoma].

By means of direct immunofluorescence technique the trabecular meshwork of patients with primary open-angle glaucoma, obtained at the time of filtering surgery, and the trabecular meshwork of controls were investigated for the presence of immunoglobulins and the complement component C3. In neither group were such deposits found; this indicates a lack of immunogenic mechanisms in primary open-angle glaucoma.

Metastatic Serratia marcescens endophthalmitis.

Serratia marcescens, a gram-negative rod belonging to the family of the Enterobacteriaceae may be the cause of various ophthalmic infections, especially in patients with decreased resistance. The course of these ocular infections is always severe. This is the first description of a metastatic S. marcescens endophthalmitis. It developed in a patient with chronic lymphatic leukemia after prostatectomy. The purpose of this paper is to draw attention to S. marcescens, an organism of increasing importance as an ocular pathogen.

[On the question of lead poisoning after injury by small shot (author's transl)].

On the basis of a report on a particular case the risk of lead poisoning after injury by small shots is discussed. The probability of lead poisoning is very small, when the small shots have been fired at long range or when they are placed in the connective tissue or the muscle tissue, in which case mostly a fibrous encapsulation takes place. The danger of lead poisoning is much greater when the small shots have been fired at close range, especially when a big amount of them bursts at the bone or is placed in the bone marrow where lead can better be absorbed.

[Ocular manifestations of temporal arteritis (author's transl)].

A report is presented of 15 patients with ocular manifestations of temporal arteritis. Ophthalmoscopy revealed anterior ischaemic neuropathy of the optic nerve in 12 patients, occlusion of central retinal artery in 2 cases and secondary atrophy of the optic nerve in 1 patient. No involvement of eye muscles of other ocular complications were seen. 10 patients showed occult temporal arteritis, whilst classical features were present in 5 patients. The diagnosis was proven histologically, and high-dosage cortisone therapy was instituted. 13 out of the 15 patients could be saved from blindness of both eyes. Temporal arteritis is an ophthalmological emergency which requires immediate diagnosis and therapy.

Ocular manifestation of temporal arteritis. Immunological studies.

Since immunological mechanisms may be involved in the pathogenesis of temporal arteritis, serum samples of 15 patients with ocular manifestations of temporal arteritis were investigated for the presence of circulating immune compleses (CICs) and antinuclear antibodies (ANAs). In addition, the serum levels of IgG, IgA, IgM, and the complement components C3 and C4 were determined. In none of our patients could CICs be detected. Serum samples of two of the patients were positive for ANA. The values for IgG, IgM, C3, and C4 were within normal limits. Our data indicate that circulating C1q-binding immune complexes and ANAs are not the cause of ocular manifestation of temporal arteritis. This does not exclude the possibility that other forms of circulating immune complexes or the local formation of immune complexes might play a role in the pathogenesis of this disease.

[Serum immunofluorescence examination in loa ophthalmia (author's transl)].

Loaiasis is a chronic parasitic disease caused by the filaria Loa loa.As soon as the worm reaches the subconjunctival space local irration ensues. This so-called Loa ophthalmia necessitates immediate surgical intervention which can be performed under topical anesthesia, using the slit lamp. The indirect immunofluorescence technique represents a valuable diagnostic test,since it is usually difficult to demonstrate the microfilaria in the peripheral blood.

[Differential diagnosis of ischemic optic neuropathy (author' transl)].

The levels of the immunoglobulins IgA, IgM, and IgG, as well as the complement components C3 and C4 were measured in the sera of patients with vascular pseudopapillitis. The ischemic papilledema was caused by either temporal or idiopathic arteritis. In the temporal arteritis group, the levels of IgG, IgM and C4 were significantly higher than in the idiopathic group. However, these facts are of little value for the differential diagnosis of vascular pseudopapillitis, because there is a considerable overlapping of values in the two groups.

[Nervus of ota (author's transl)].

A 27-year-old woman presented with a distinct gray-blue coloration of the left eye and the periorbital region. The right periorbital region also displayed minimal pigmentation. It was established that eye drops containing mercury had been administered for some years. Initially hydargyrosis was considered as an etiologic factor. Finally we arrived at the diagnosis of nevus of Ota on conjunction with a blue nevus on the forehead. The simultaneous occurrence of a nevus of Ota and the blue nevus suggest a common basic developmental disturbance. The nevus of Ota represents a benign dermal hyperpigmentation syndrome and has to be differentiated from melanosis bulbi.

[Eye diseases and ophthalmological care in a developing country (author's transl)].

Five million blind people in India suffer from cataract, which is a curable condition. Untreated cataract accounts for more than half of the cases of blindness in that country. Twenty percent of all cases of blindness are caused by infections, while malnutrition accounts for 27% in children. In India, ophthalmological care has been provided for many years now in eye-camps. In cataract surgery, the expression method is customary. Trachomas at all stages, infectious eye diseases, traumatic lesions, vitamin A deficiency and leprosy are also diagnosed and treated in large numbers of cases. By way of an example the authors, assistants at the First Eye Clinic of Vienna University, describe an eye-camp in Andhra Pradesh where they worked. Work of this kind can be recommended to others.

[Late results of pars plana vitrectomy in diabetics (author's transl)].

Report on 82 consecutive cases in which pars plana vitrectomy was performed because of intravitreal hemorrhage. Postoperative follow-up ranged from 3 to 4 1/2 years. Although an incipient improvement of the visual acuity was achieved in 54%, recurrent hemorrhages as well as progression of the proliferative diabetic retinopathy lead to visual deterioration of blindness in 64% of the eyes.
