[Accurate and simplified determination of ketamine in plasma by gas chromatography mass spectrometry].

An accurate and simplified method has been developed for determination of ketamine in human plasma using gas chromatography and electron impact mode mass spectrometry with selected ion recording from 0.5ml of plasma. Standards and samples of plasma underwent the same procedure of two step extraction by methanol. Ketamine concentrations in the plasma were determined from the peak in the selected ion profile of ketamine (m/e: 237). Standard curve was linear with the increasing amount of ketamine (0.63-5.0 micrograms.ml-1) in plasma with mean CV = 5.2% mean RR = 63.8% and r = 0.998. The concentration of ketamine in plasma ranged from 1.6 micrograms.ml-1 to 2.5 micrograms.ml-1 during ketamine anesthesia (2mg.kg-1.h-1) in a surgical patient.

[A case of two-chambered right ventricle complicating severe cyanosis due to tricuspid regurgitation and patent foramen ovale].

A 63-year-old female of two-chambered right ventricle (TCRV) associated with tricuspid regurgitation (TR), patent foramen ovale (PFO) and small ventricular septal defect (VSD) underwent corrective surgery successfully. She suffered severe heart failure and cyanosis with 47.7% of right-to-left shunt through PFO. The VSD was so small that no significant shunt was shown in catheterization data. Operative findings suggested that TR was caused by elongation of the chorda as a consequence of long-term pressure load of right ventricular inflow chamber. Among reported cases of TCRV, the present case is the oldest one who underwent corrective surgery successfully.

Warfarin antagonism of natto and increase in serum vitamin K by intake of natto.

Significantly elevated Thrombo-test values after natto intake were observed in patients undergoing anticoagulant therapy after valve replacement operations. Corroborative tests with humans and rabbits to check the observations showed natto to be strongly antagonistic to warfarin. The author suspected that Bacillus natto contained in natto might produce a large amount of vitamin K in the intestinal tracts after taken in, and assumed that the antagonism of natto to Warfarin was attributed to the synthesis of vitamin K by the bacterium in the organ. The author then determined vitamin K in plasma of healthy persons after intake of natto to indicate increases of serum vitamin K. In anticoagulant therapy it is important to maintain the TT value within a certain therapeutic range for a long period of time. Thus the author believes that patients under administration of Warfarin should be given special instructions in food.

[Disorders of vision and act in a child with spastic cerebral palsy].

A boy with spastic cerebral palsy, who had frequent apneic attacks during the neonatal period, showed developmental disorders; (1) cerebral poor eye sight, a disorder of visual directed attention, and associative visual agnosia (2) disorders of visuospatial perception and corporeal awareness, and (3) dyspraxia d'évolution (Stambak et al.). When the visual directed attention was completed at the age of 10 years, the integration of his cerebral higher functions was facilitated, except the following neuropsychological deficits; (1) visual line agnosia (apperceptive type) and (2) constructive apractognosia. These deficits were not compensated through the maturation of the central nervous system and through the contacts and interchanges with the outer world. MRI findings showed bilateral lesions of the white matter of occipital and parieto-occipital transitional regions, and the trunk and splenium of corpus callosum. Although this case is included in mental retardation, what differs is the fact that each "higher" function developed unevenly in this case unlike the ordinary mental retardation. In this report these disorders and deficits were discussed considering his developmental process of neuropsychological functions.

[Reaction products of sevoflurane with new soda lime-A under various conditions].

Sevoflurane is reported to react with sodalime, a common carbon dioxide absorber which resolves into several products. We measured the reaction products of sevoflurane when this anesthetic reacted with sodalime-A, a new carbon dioxide absorber under various conditions. Analysis of reaction products was done by gas chromatography using a 2 m column packed with DOP. Six peaks including sevoflurane were detected on the gas chromatogram of sevoflurane after reaction with sodalime-A and five reaction products were obtained. These peaks were from P1 with the shortest retention time to P5 with the longest retention time. When sevoflurane was sealed with sodalime-A in a test tube at room temperature, 40 degrees C and 45 degrees C, only P1 was detected and two reaction products of P3 and P5 were identified when the test tube was heated at 50 degrees C. Sodalime contains Ca(OH)2, NaOH, KOH and silicon dioxide, and sodalime-A contains Ca(OH)2 and NaOH only. We reported previously that sevoflurane reacted with KOH to produce P1 to P5 even at room temperature and KOH was the most contributing factor to produce these reaction compounds. Reaction products of sevoflurane with two types of carbon dioxide absorber were checked with gas chromatography. Degradation products except P1 with sodalime-A was less than with conventional sodalime at any conditions. P1 production was about twice to three times more than that with sodalime.

[Plasma cortisol levels during abdominal surgery under sevoflurane anesthesia: comparison between gastrointestinal and gynecological surgery].

Plasma cortisol levels during abdominal surgery under sevoflurane anesthesia were evaluated in 22 patients who ranged in ages from 37 to 65. They underwent either gastrointestinal or gynecological abdominal surgery. Anesthesia was induced and maintained with sevoflurane (1-5%) in nitrous oxide (4 l.min-1) and oxygen (2 l.min-1). Succinylcholine was administered intravenously to facilitate tracheal intubation and pancuronium was given intravenously during surgery. Lactated Ringer's solution at a speed of 10-15 ml.kg-1.hr-1 was also administered intravenously throughout the surgical procedures. Plasma cortisol levels were unchanged with the induction of sevoflurane anesthesia alone, but they increased significantly 2-3 times of the preanesthetic levels during and after surgery in both groups. However, the concentrations of plasma cortisol after recovery from anesthesia were significantly higher in the gastrointestinal group than in the gynecological group. The findings suggest that plasma cortisol levels after surgery reflect the difference in magnitude of stress response between gastrointestinal and gynecological surgery.

Amino acid sequence of Mirabilis antiviral protein, total synthesis of its gene and expression in Escherichia coli.

We have determined the complete amino acid sequence of Mirabilis antiviral protein (MAP). MAP is composed of 250 amino acids having a combined molecular weight of 27,833 and contains 23 lysine residues and 7 arginine residues. The amino acid sequence of MAP has 24% homology with the Ricin D-A chain. To carry out systematic structure-function studies of MAP, we have accomplished the total synthesis of its gene. We designed a synthetic MAP gene containing 12 unique restriction sites that were on the average 65 base pairs apart. Thirty synthetic oligonucleotides were enzymatically joined to form DNA duplexes. These were strategically synthesized to have EcoRI and HindIII cohesive ends and were cloned in pUC19. Nine blocks of the synthetic fragments were assembled in pUC19 to form the MAP gene consisting of 759 base pairs. The correctness of the connecting reactions was confirmed by step-wise sequencing of each assembled fragment as well as the total gene. When expressed under control of the tac promoter in Escherichia coli, the synthetic gene gave a protein similar to the native MAP. This was confirmed by an enzyme-linked immunosorbent assay and Western blotting analysis.

Activation of calcium ion-dependent proteinases by bradykinin in dental pulp of the rat.

The present study was aimed to examine whether BANA-degrading enzyme activities could be enhanced by bradykinin(BK) in dental pulp of the rat in vitro. The results showed that BK(0.1-10 microM) dose-dependently enhanced BANA-degrading enzyme activity at pH 7.4. The effects of BK(1 microM) were found to be most effective at both pH 7 and 8, with enhancement of the enzyme activities at a wide range of pH. The BK effects at both the pH were not inhibited by FOY-305(0.1 microM), an inhibitor of trypsin-like enzymes, differing from that at pH 6 in adrenal medulla of the rat. On the other hand, the effects of BK at both the pH were remarkably inhibited by EGTA (2 mM), followed by reversal with calcium ion (2.42 mM). These results suggested as follows: 1) there might be two kinds of BANA-degrading enzymes activated by BK in the pulp. 2) it was conceivable that BANA-degrading enzymes activated by BK were quite different from serine proteinases and were interfered with them in the pulp. 3) calcium ion might play a role in BK-induced enhancement of BANA-degrading enzyme activities which were regarded as met-enkephalin (ME) processing enzyme activities in the pulp.

Enhancement of proteinase activities by bradykinin in adrenal medulla of the rat.

In the present study, a significant positive correlationship was found between the contents of bradykinin (BK)-like and met-enkephalin(ME)-like peptides in adrenal medulla of the rat with cavity-formed incisors in vivo, and the production of ME-like peptides was increased by BK in adrenal medulla of the rat in vitro. Influence of BK on the degradation of BANA, a synthetic substrate for trypsin, by the tissue enzymes was also studied. It was found that BK (0.1-10 microM) enhanced the enzyme activities in a dose-dependent manner, and the effect of BK(1 microM) was most effective at pH 6 and 8. The BK effect was inhibited by FOY-305, an inhibitor of serine proteinases, at pH 6, but not at pH 8. However, E-64, an inhibitor of cysteine proteinases, reduced the BK effects at both pH 6 and 8. These results suggested that 1) BK was an activator for BANA-degrading enzymes which were thought as processing proteinases of ME-like peptides in adrenal medulla of the rat, and 2) there may be, at least, two kinds of BANA-degrading enzymes activated by BK, one might be a serine proteinase with optimal pH at 6, and the others might be cysteine proteinases with optimal pH at both 6 and 8.

Specific binding to methylated polynucleotides in monoclonal anti-poly(dT) antibodies from autoimmune mice.

Six monoclonal antibodies (mAbs) reactive to synthetic polynucleotide, poly(dT), were established from spontaneous autoimmune MRL/MpJ-lpr/lpr mice and male BXSB mice by spleen cell hybridization method, and were analyzed for cross-reactivity with polydeoxy-5-methylcytidylic acid [poly(dmC)] in comparison with its unmethylated counterpart poly(dC). By direct binding tests, these mAbs, all of which had preponderant binding activity to poly(dT) relative to poly(dU), were divided into two groups: (i) four mAbs showing reactivity to poly(dmC) as well as to natural DNA preparations and (ii) two mAbs with limited reactivity to poly(dT) but no binding to poly(dmC) or natural DNAs. Inhibition binding tests with these synthetic polynucleotides demonstrated that one mAb (TP-A9) in the first group reacted specifically to poly(dmC), as well as to poly(dT). In the second group, one mAb (TP-B5) showed highly specific reactivity to poly(dT) that could not be inhibited by poly(dU), poly(dC), or poly(dmC). However, another mAb (TP-C8) in the second group showed reactivity to poly(dT) that could be inhibited by poly(dU) as well as by poly(dT). Thus, these findings indicate that monoclonal anti-poly(dT) antibodies can cross-react with poly(dmC) with different specificities and suggest that the methylated base may be one of the major antigenic sites of DNA molecules recognized by anti-DNA antibodies spontaneously produced in autoimmune mice.

Construction of a secretion vector production of peptide hormones in Escherichia coli (extracellular production of calcitonin as fused protein).

A new secretion vector, pEAP84 which contained a unique restriction site (BglII) at the 3' end of the penicillinase gene to produce a fused protein, and the Ex-kil region to make the outer membrane permeable, was constructed from pEAP82. A recombinant plasmid p84h06, which contained a synthetic gene for human calcitonin with a cyanogen bromide cleavage site at the junction site of the fused protein, was constructed and introduced into Escherichia coli. The hybrid protein produced in E. coli carrying p84h06 was secreted into the culture medium. The amino acid composition of this product was consistent with that deduced from the DNA sequence. Mature calcitonin was obtained following cyanogen bromide cleavage of the fused protein.

The third cellulase of alkalophilic Bacillus sp. strain N-4: evolutionary relationships within the cel gene family.

The third cellulase gene (celC) of Bacillus sp. strain N-4 was cloned in plasmid pBR322 and was located within a 5.5-kb HindIII fragment. The cellulase encoded by this fragment had an Mr of about 100,000 and showed optimum activity around pH 9. These properties were different from those of the enzymes encoded by the celA and celB genes of the same organism. The amino acid sequence deduced from the nucleotide sequence was found to be highly homologous to the CEL-F enzyme from Bacillus sp. strain No. 1139 [Fukumori et al., J. Gen. Microbiol. 132 (1986) 2329-2335]. An evolutionary relationship observed among the four cellulases of alkalophilic Bacillus strains and that of Bacillus subtilis endoglucanase suggested that ancestral genes for alkaline and neutral cellulases diverged early in the evolution of these enzymes.

Construction of a chimeric series of Bacillus cyclomaltodextrin glucanotransferases and analysis of the thermal stabilities and pH optima of the enzymes.

The cyclomaltodextrin glucanotransferase (CGTase, EC 2.4.1.19) gene from the alkalophilic Bacillus sp. strain no. 17-1 was cloned in Escherichia coli. The cloned CGTase gene consisted of a single open reading frame which would encode a polypeptide of 713 amino acids, and the first 27 amino acid residues comprised a signal peptide. The nucleotide sequence and the amino acid sequence of this CGTase (CGTase 17-1) gene had strong homology with those of the CGTase (CGTase 38-2) gene previously cloned in our laboratory from the alkalophilic Bacillus sp. strain no. 38-2, although the enzymic properties of the CGTase 17-1 were distinct from those of the CGTase 38-2. To analyse those enzymic properties further, we constructed 12 chimeric CGTases using three restriction nuclease sites and compared the enzymic properties of the chimeric CGTases. The N-terminal part of the enzyme was important for heat stability, and the pH-activity profile was influenced by both the N- and the C-terminal parts. A third segment was less important for enzymic properties.

Metabolism of amniotic fluid dopamine by fetal membranes.

The concentrations of L-dopa, three catecholamines and 3,4-dihydroxyphenylacetic acid (DOPAC) were measured both in the amniotic fluid and the fetal urine obtained at term elective cesarean section, using high performance liquid chromatography with electrochemical detection. Dopamine was the major compound in the fetal urine, while DOPAC predominated in the amniotic fluid. Monoamine oxidase (MAO) activity was detected in the fetal membranes by the method of Wurthman and Axelrod. MAO activities in the amnion, chorion and decidua were 0.023 +/- 0.003, 0.180 +/- 0.05 and 2.69 +/- 0.66 nmol/mg protein/min (mean +/- SE), respectively. Incubation experiments, using fetal membranes with human amniotic fluid, demonstrated that dopamine in amniotic fluid was metabolized to DOPAC by fetal membranes. Additional incubation experiments with pargyline, a MAO inhibitor, showed that it strongly inhibited the conversion of dopamine into DOPAC. These results suggest that the dopamine in amniotic fluid was metabolized to DOPAC by MAO in the fetal membranes. This mechanism may regulate the dopamine concentration in amniotic fluid.

Experience with the ultrasonic surgical aspirator in a cavernous hemangioma of the cavernous sinus.

A patient with a cavernous hemangioma of the cavernous sinus was operated upon using the Cavitron Ultrasonic Surgical Aspirator (CUSA). Intracapsular subtotal removal of the tumor was accomplished efficiently with the CUSA. At the end of the procedure, however, the CUSA penetrated not only the capsule of the tumor, but also the wall of the internal carotid artery. Advantages and disadvantages of CUSA surgery for cavernous hemangiomas of the cavernous sinus are discussed.
