[Therapeutic effect of Robaveron tablet (KN-7) on neurogenic bladder].

Therapeutic effect of Robaveron Tablet (KN-7) was studied on 128 patients suffering from neurogenic bladder. Robaveron Tablet was administered 6 tablets daily for 3 weeks. The effect was evaluated by the residual urine, subjective symptoms, cystometric findings, urethral pressure profile and uroflowmetry. Remarkable decrease of residual urine was observed regardless of position of injury. The pressure amplitude defined as the difference between the maximum voiding pressure and the maximum resting pressure increased on any group of neurogenic bladder. The effectiveness, rated as moderately improved or better was 76.9% for brain disorders, 58.3% for upper motor neuron lesion and 75.7% for lower motor neuron lesion of spinal injuries, 58.3% for peripheral nervous injuries and 100% for others. No remarkable abnormality was observed in the results of side effects or clinical laboratory tests.

Eosinophilic spongiosis in bullous pemphigoid.

Eosinophilic spongiosis is accepted as one of the histopathological features of pemphigus. We found eosinophilic spongiosis in six (24%) of 25 patients in whom erythematous lesions, which were in the vicinity of blisters of bullous pemphigoid, were found. This finding was associated with eosinophilic infiltration of the upper dermis and peripheral eosinophilia.

[Comparative double-blind trial of KN-7 tablet and Robaveron injection in the treatment of neurogenic bladder].

The clinical effectiveness, safety and usefulness of KN-7 tablet as a new oral application of the prostatic extract, on urinary dysfunction of neurogenic bladder were compared with those of Robaveron injection by the double-blind test method. In the study, 2 tablets t.i.d. and a shot of intramuscular injection 1 ml a day were given successively for 3 weeks. A total of 233 cases were reported from 37 facilities belonging to the KN-7 Clinical Research Group. Some of them were excluded or dropped out. The number of cases used for analysing the effectiveness, safety and usefulness were 214, 232 and 215, respectively. There was no bias between the two groups with a significant homogeneity in the background. In the overall clinical effectiveness, the effective rate including excellent, moderate and slightly effective was 76.9% with KN-7 and 77.4% with Robaveron. In the clinical usefulness, the rate of usefulness of slightly useful or above was 75.0% with KN-7 and 75.7% with Robaveron. There was no significant difference between the two groups in the clinical effective and useful rates at a significant level of 5%. Side effects were observed in 1 of the 114 (0.9%) patients given KN-7 and 8 of the 118 (6.8%) patients given Robaveron. The incidence of adverse reactions with KN-7 was significantly lower than that with Robaveron. Based on the results, it was concluded that KN-7 tablets, 2 tablets t.i.d., would be as effective and useful as a Robaveron injection 1 ml daily and safer than the latter in the treatment of neurogenic bladder.

[Coagulation-fibrinolysis abnormalities in acute stage of subarachnoid hemorrhage (Part 1)--With special reference to the relation between cerebral vasospasm and fibrinopeptides A and B beta].

It is well known that abnormalities of coagulation and fibrinolysis frequently take place during the course of cerebrovascular diseases. In this paper, coagulation and fibrinolytic studies were performed during the course of acute stage through chronic stage of subarachnoid hemorrhage. Tested items were partial thromboplastin time, prothrombin time, FDP, alpha 2-plasmin inhibitor, antithrombin III, fibrinogen, besides, fibrinopeptide A (FPA), and fibrinopeptide B beta (FPB beta) which were being worthy of note. Blood were sampled from peripheral vein (V) and internal jugular vein at jugular bulb (J). And, moreover, cerebrospinal fluid (L) were collected as possible as we could for measuring FPA and FPB beta. The obtained results were summarized as follows; Within forty-eight hours from the onset of subarachnoid hemorrhage, FPA-V, J,L and FPB beta-V, J,L were statistically higher than those of control. FPA-J and FPB beta-V, J, within forty-eight hours from the onset were statistically higher in the cases with brain death than in the survived cases. On the third to fifth day from the onset when so called cerebral vasospasm became apparent to begin, FPA and FPB beta had a tendency to be higher than other periods. Increase of fibrinogen delayed from the peaks of FPA and FPB beta showing the peaks at the seventh to the fourteenth day from the onset of subarachnoid hemorrhage. In three cases with symptomatic vasospasm, FPA and FPB beta showed maximal values two to four days prior to the appearance of symptomatic cerebral vasospasm. Other tests were all within normal limits.(ABSTRACT TRUNCATED AT 250 WORDS)

[Retrovesical sarcoma (rhabdomyosarcoma): report of a case and a review of 46 cases in Japan].

The patient was a 2-year-old boy, who was brought to our clinic with the chief symptom of urinary retention. We suspected retrovesical tumor by digital examination, intravenous pyelography and retrograde cystography. Surgery was performed and the bladder and prostate including the tumor were removed. Ureterocutaneostomy was constructed on both sides. The pathohistological diagnosis was rhabdomyosarcoma (embryonal type). Thereafter, chemotherapy and irradiation were applied, but the patient died of cachexia which was clinically caused by a local recurrence, fourteen months after the operation. Forty-six cases of retrovesical sarcoma were collected from the recent Japanese literature, and reviewed.

Further characterization of presynaptic beta-adrenoceptors in guinea-pig pulmonary arteries.

Presynaptic beta-adrenoceptors were further characterized in spiral strips of guinea-pig pulmonary arteries preloaded with [3H]norepinephrine. l-Metoprolol (3 X 10(-6) M) inhibited isoproterenol (3 X 10(-7) M)-induced increases in 3H efflux by transmural field stimulation, whereas the d-isomer produced no inhibition. However, IPS 339, H 35/25, butoxamine and metoprolol (3 X 10(-6) M) antagonized salbutamol (3 X 10(-7) M)-induced increases in the parameter, whereas acebutolol, bevantolol and practolol (3 X 10(-6) M) produced no antagonism. Presynaptic beta-adrenoceptors in guinea-pig pulmonary arteries appear to have characteristics similar to those postsynaptic classical beta-adrenoceptors.

Effects of adenosine on [3H]norepinephrine release from perfused mesenteric arteries of SHR and renal hypertensive rats.

Adenosine (10-30 microM) inhibited the 3H-efflux evoked by sympathetic nerve stimulation in the perfused rat mesenteric arteries preincubated with [3H]norepinephrine. The inhibition was smaller in the prehypertensive (5 weeks old) as well as hypertensive (15-18 weeks) SHR (spontaneously hypertensive rats), compared with age-matched normotensive Wistar Kyoto rats. However, it was not diminished in Wistar rats rendered hypertensive by left renal artery occlusion. Thus, a diminished adenosine-mediated presynaptic inhibition of adrenergic transmission appears to be genetically inherent in SHR.

The nucleus reticularis dorsalis: a region sensitive to physostigmine.

Microinjection of physostigmine (3 micrograms) into the nucleus reticularis dorsalis of the rat produced hypertension. Little or no increases in blood pressure were found at the other areas of the medullary reticular formation. The pressor effect following injection of physostigmine into the nucleus reticularis dorsalis was sensitive to atropine (1 microgram) but resistant to hexamethonium (5 micrograms), similarly injected. Atropine (1 microgram) bilaterally injected into the nucleus decreased pressor responses to intravenous physostigmine (100 micrograms/kg). These data may indicate that there are muscarinic receptors responsible for pressor effects in the nucleus reticularis dorsalis. The medullary cholinoceptors may be involved in the systemic physostigmine effect on blood pressure.

Differential effects of diazepam upon vestibulo- and visual-oculomotor responses in the rabbit.

The effects of intravenously administered diazepam (0.6 mg/kg) on vestibulo-ocular reflex (VOR), optokinetic nystagmus (OKN), optovestibular reflex (OVR) and their after-nystagmus were examined in rabbits. These reflexes were evoked by velocity step of 20 degrees/sec of chair or drum rotation. Slow phase eye velocity (SPEV) of OVR shows algebraic summation of those of VOR and OKN. Although SPEVs of VOR, OKN and OVR significantly decreased at 10 and 30 min after diazepam injection (p less than 0.05, t-test), OKN shows most distinctive reduction. SPEV reduction of OVR after diazepam administration was also equal to the algebraic summation of VOR and OKN reductions.

Comparative study of caloric responses induced by different stimulus intensities.

Bithermal caloric responses induced by 20, 50 and 240 ml water irrigations were compared in 111 healthy Japanese adults. Distribution mode of max. slow phase eye velocity (SPEV), duration and max. frequency showed normal pattern in the logarithmic scale but not in the regular scale. All three nystagmus parameters of 240 ml caloric test showed most linear distribution compared with two other caloric tests in the probit plots of logarithmic scale. When the mean, coefficient of variation and normal range (mean +/- 2 SD) were compared between two modes of distribution, the most consistent difference was found in the range which was biased to the side of large number with little difference in the width of the range. When one standard deviation of CP- and DP-indexes were compared, these values were smallest in max. frequency, however, the best negative correlation between variation (SD) of CP- and DP-indexes and stimulus intensity was found in SPEV.
