A modified guinea pig testing technique was developed for the detection of weak allergens and allergenicity of materials unsuitable for testing by intradermal injection. This test involved the use of Freund's complete adjuvant to stimulate the immune system of the animal, and external application instead of intradermal injection of the test compound in the induction stage. The allergenicity of Sudan III, Brilliant Lake Red R and Sudan I was tested by this procedure. In the dose-effect study of Sudan I, the dose dependency of a positive reaction of the induction and challenge concentrations was recognized. The test was compared with three other guinea pig sensitization tests. The results obtained with this test correlated well with those obtained with the guinea pig maximization test.

Two hundred one patients (138 boys, 63 girls), 1 month to 11 years old, with Kawasaki disease underwent coronary angiography, ultrasonic tomography, myocardial imaging, and biopsy of right ventricular myocardium. Aneurysms of epicardial coronary arteries, mostly left, occurred in 26 cases (12.9%). Degeneration and proliferation of endothelium, edema, scarring, and fibrosis, slight to moderate, involved the tunicae of intramural small vessels in all cases. These changes tended to abate with time. The basic lesion of Kawasaki disease, round-cell myocarditis and fibrosis, involved all cases and persisted. Disarrangement, abnormal branching, disarray, and hypertrophy of myocytes correlated significantly with myocarditis, but not with epimural or intramural coronary angitis. As a consequence of the myocardial changes, it is suggested that some cases may terminate as myocardiopathy. Scoring criteria for vascular and myocardial changes in biopsy specimens are included.

A patient's condition was initially diagnosed as constrictive pericarditis, but an echocardiogram revealed the presence of an intracardiac tumor. The tumor was a slow-growing leiomyosarcoma that originated in the left internal iliac vein, extended into the inferior vena cava, and reached the right ventricle. This tumor was considered to be a recurrent uterine leiomyosarcoma that had been resected 4 years before. After a second resection of this tumor, the patient was living a normal life 61/2 years postoperatively, in spite of recurrence in the pelvis and lung. We reviewed 12 cases of leiomyosarcoma in the world literature that reached the heart through the inferior vena cava. We added our case and evaluated the clinical features of all 13. This is the first successfully resected case of leiomyosarcoma of this nature so far as we could determine.

Coronary cine-angiography (CAG) was performed in 290 cases with the history of Kawasaki disease, in whom 57 cases (19.6%) were found to have coronary involvements. In 42 out of these 57 cases, myocardial imaging with thallium-201 (201Tl) was performed, and abnormal findings were seen in 6 cases. Five of these 6 cases showed not only coronary aneurysms, but also coronary obstructive lesions on CAG, while the remaining case showed only coronary aneurysms. The decrease of the uptake of 201Tl by the myocardium appeared to be mainly due to the decrease of regional myocardial blood flow, especially in those cases with obstructive lesions. Myocardial imaging with 201Tl seems to be more sensitive than stress electrocardiography by treadmill to detect myocardial ischemia in the patients with coronary obstructive lesions, although it seems less sensitive than stress electrocardiography to detect coronary arterial stenosis.

Two-dimensional echocardiographic studies were performed in 23 patients with coarctation of the aorta, 5 with interruption of the aortic arch, 6 with supravalvular aortic stenosis, 3 with truncus arteriosus, 2 with anomalous origin of the right pulmonary artery from the ascending aorta and 2 with aorto-pulmonary septal defect. The diagnosis was confirmed in each patient by surgery and/or cardiac catheterization and angiography. Visualization of the junction of isthmus and the descending aorta was possible in 18 of 23 patients with coarctation by two-dimensional echocardiography. In 13 patients correct diagnosis was obtained prospectively and in 2 patients coarctation was detected retrospectively. In only one of the 5 patients with interruption correct diagnosis was obtained and in 3 patients it was difficult to differentiate interruption from coarctation. In 5 of the 6 patients with supravalvular aortic stenosis, visualization of the area of obstruction was possible to two-dimensional echocardiography. In 2 of the 3 patients with truncus arteriosus, in whom none of the 2 had anomalous origin the right pulmonary artery from the ascending aorta and one of the 2 had aorto-pulmonary septal defect, correct diagnosis was obtained prospectively by two-dimensional echocardiography. Two-dimensional echocardiography may offer a useful noninvasive method for the direct visualization of aortic obstructive lesions and aortic malformations.
