In Hokkaido, 113, 612 women were subjected to mass screening of breast cancer from 1973 to 1983. Out of them, 74, 285 (56.3%) were examined once and the remainder twice or more: 4,798 (4.2%) were advised to receive further precise examinations and 99.8% of them underwent the second examination. Breast cancer was detected in 377, in which 291 (77.2%) were detected by the first examination and 33 (3.8%) were so-called "interval cancer" with the mean interval of 19.3 months. The detection rate was 0.3%. Histologically non-invasive cancer was seen in 26 (6.9% of 377). Despite a recent significant increase in detection rate of the earlier stage of breast cancer, the present study showed no evidence of a decrease in advanced cancer. The prognosis of the patients who had curable operation revealed 86.9% in 5 year survival rate and 40% in 10 year survival rate.

The follow-up study of 16 patients with farmer's lung and 35 farmers having antibody to thermophilic actinomyces was performed. The following results were obtained. Three patients out of 16 patients with farmer's lung were readmitted to the hospital. The patients out of 11 believed that the dust respirator is useful for prevention against the development of farmer's lung. Any new patient was not found out from farmers having antibody to thermophilic actinomyces in 1985.

A new type of anticancer agent with an amphiphilic nature, poly(styrene-co-maleic acid)-conjugated neocarzinostatin (Smancs), was dissolved in lipid contrast medium Lipiodol (Smancs/Lpd, Gelbet Co., Paris, France). This medium was injected arterially and found to be an invaluable tool for highly sensitive computerized tomography (CT) image analysis of tumors. After the administration, CT images revealed high-density areas which corresponded to the location and size of liver cancer, the smallest being 4 mm in diameter. The deposition pattern of Smancs/Lpd in liver cancers by CT was classified into three types. In type A, Lipiodol was distributed relatively even in the tumor lesion, whereas in type B it was accumulated predominantly around the tumor's periphery although the central portion remaining low in density. A few cases exhibited type C pattern, which was a mixture of types A and B. Type A was found essentially in primary liver cancer, and types B and C in secondary liver cancer. Thus, the CT pattern was found to be useful for differential diagnosis. For a sufficient therapeutic effect, 0.08 ml of Smancs/Lpd per cm2 of the maximal CT cross-section of the tumor area was found to be necessary. As a routine protocol after Smancs/Lpd administration, CT scanning was recommended for primary liver cancer initially at 1 week and then once every month. Secondary liver cancer required more frequent CT follow-ups after the administration (on the third day, after 1 and 2 weeks, and every month) due to relatively rapid disappearance of the stain than in the primary liver cancer.
A successful transplantation of soybean agglutinin (SBA) and sheep red blood cell (SRBC)-fractionated, maternal marrow in a patient with severe combined immunodeficiency (SCID) is reported. The engraftment of HLA-haplotype mismatched marrow cells was obtained without apparent graft versus host disease (GVHD). With immunological reconstitution the patient recovered from a BCG infection, which might have been caused by a BCG inoculation before his bone marrow transplantation.
Serum bile acids and their conjugates were analysed in 20 breast-fed infants with prolonged jaundice. The mean total bile acid levels in serum were increased in the breast-fed infants with jaundice, as compared with those in either breast- or bottle-fed infants without jaundice. However, there were no significant differences between the groups. All the breast-fed infants examined, regardless of association with jaundice, had a bile acid pattern dominated by taurine conjugates (the ratio of glycine- to taurine-conjugated bile acid, G/T ratio, less than 1.00). In contrast, the bottle-fed infants without jaundice had a pattern dominated by glycine conjugates (G/T ratio, more than 1.00). Among the breast-fed infants with jaundice, the mean G/T ratio in those who had serum bilirubin levels over 10 mg/100 ml was significantly lower than that in those who had serum bilirubin levels of less than 10 mg/100 ml. The altered bile acid metabolism might be associated with the pathology of breast milk jaundice.
Entry of the KUN strain of human rotavirus into MA 104 cells was studied by electron microscopy. Virus particles attached to the cell membrane appeared to be almost exclusively double-shelled virions. These attached virions followed two distinct pathways into the cytoplasm depending on pretreatment with trypsin. Using infectious rotavirus which had been pretreated with trypsin, the viral nucleoids passed directly into the cytoplasm within 5 minutes after inoculation, through dissolution of the viral capsid and cell membrane. Using non-infectious rotavirus that had not been pretreated with trypsin, phagocytosis or pinocytosis occurred in which virions were sequestered into lysosomes 20 minutes after virus attachment to the cell membrane. After being sequestered, uncoating of the rotavirus virions within lysosomes was seen, but it did not result in release of the genome. On the basis of these observations it was concluded that when virions were pretreated with trypsin, virus replication occurred following the direct passage of viral nucleoids into the cell cytoplasm. However, mere phagocytosis of virus particles into cell lysosomes, which occurred when trypsin-untreated virus was used, does not appear to be related to rotavirus replication.
When a high NaCl concentration was used to decrease selectively the synthesis of cell proteins, the synthesis of most cellular polypeptides was greatly diminished relative to human rotavirus proteins. Thus, in the presence of 150 mM excess NaCl, 11 viral polypeptides were clearly identified. However, hypertonic conditions also reduced viral protein synthesis to a different extent with individual proteins. No significant changes in viral protein synthesis occurred during incubation under the hypertonic condition for up to 6 h, and infectious virus yields of MA104 cells incubated in the hypertonic medium did not differ from the yields of untreated MA104 cells. These results indicate that hypertonic conditions provide a useful tool for qualitative studies of viral protein synthesis in human rotavirus infected cells.
Labeling patterns were classified after double bone labeling of four male beagles, 10 months of age. Calcein and oxytetracycline were given on the 18th and the 7th day prior to simultaneous iliac and 11th rib biopsies. Undecalcified sections stained with the Villanueva bone stain were studied by epifluorescence microscopy. Five structures were identified and classified: the first or green label, the interlabel layer of mineralized bone, the second or yellow label, the post double-labeled mineralized bone layer, and osteoid seams. Doubly plus singly labeled surface equalled 40.8 +/- 8.4% of the total trabecular surface of the ilium. Doubly labeled surface as a percent of the total labeled surfaces equaled 55.5% in trabeculae and 68.1% in osteons, whereas green first-singly labeled surface equaled 24.2% and 11.9%, respectively, and yellow second-singly labeled surface equaled 20.3% and 20.0%, respectively. Unequivocal examples appeared in both biopsy sites of all four dogs of bone-forming systems that lacked one or the other label, or both, and also of systems in which cessation of mineralization or of new matrix formation occurred between the two labels and between the second label and the day of biopsy. The findings prove that the On-Off states in active bone-forming sites that have been postulated by other investigators do exist. Since widely different labeling patterns appeared in different bone-forming centers in the same bone and the same animal, a local factor rather than a systemic one should control those differences at the level of the BMU.

Serum bile acid conjugates in breast-fed infants with prolonged jaundice were analyzed by a newly developed procedure using high-performance liquid chromatography with fluorescence labeling. Major bile acids were cholate and chenodeoxycholate conjugates. Some of the breast-fed jaundiced infants had high levels of serum bile acid conjugates (greater than 25 mumol/L), but the mean levels of individual bile acid conjugates found in jaundiced breastfed infants were not significantly different from those in breast-fed infants without jaundice. The glycine- to taurine-conjugated bile acid ratio in breast-fed jaundiced infants was significantly lower than in breast-fed nonjaundiced infants or bottle-fed nonjaundiced infants. In breast-fed infants, the portion of taurine-conjugated bile acids increased in proportion to serum bilirubin levels. These findings suggest that alteration in conjugated bile acid patterns of breast milk jaundice is related to an increased enterohepatic circulation of bile acids as well as bilirubin in infants fed on breast milk that contains high amounts of taurine.
CFU-E growth from fractionated bone marrow cells of 5 children with Diamond-Blackfan syndrome was studied. In all patients, CFU-E growth was reduced in mononuclear cell-rich fraction. In 2 of the 5 patients, CFU-E growth was returned to normal by the depletion of E-rosette forming cells or monocytes from mononuclear cell-rich fraction. In the patient whose CFU-E growth returned to normal by the depletion of E-rosette forming cells, co-cultivation between bone marrow buffy coat cells and autologous bone marrow E-rosette forming cells resulted in a significant decrease of CFU-E growth, and there was a significant increase in CFU-E growth by treatment with monoclonal antiserum to OKT4. We concluded that immunologic causes such as cellular factors may play a role, at least in part, in the pathogenesis of Diamond-Blackfan syndrome.
This study assesses the diagnostic value of Lipiodol (iodized oil) and computed tomography (CT) in detecting hepatocellular carcinoma (HCC). Twenty-four patients who were suspected of having HCC received injections of a small amount of Lipiodol, along with an antitumor agent, in the hepatic artery following routine celiac angiography. CT scans obtained 7-10 days after Lipiodol administration demonstrated HCC in distinct contrast to the surrounding noncancerous parenchyma. In particular, the CT-Lipiodol procedure disclosed many small HCC lesions that were not shown by celiac angiography, scintigraphy, CT with and without contrast medium enhancement, and ultrasonography. Although this procedure may miss very small or highly fibrotic lesions, it is recommended for patients suspected of having HCC and for patients for whom hepatic resection is being considered.
One strain of atypical rotavirus, morphologically indistinguishable from known rotaviruses but lacking the common group antigen of typical rotavirus detectable by an enzyme-linked immunosorbent assay (ELISA), was isolated from one of 152 Ecuadorian infants with acute rotavirus gastroenteritis. The genome analysis by polyacrylamide gel electrophoresis revealed a unique RNA pattern clearly distinct from that of typical rotaviruses.
