[Successful resection and reconstruction of the left atrium, right atrium and atrial septum under extracorporeal circulation in a patient with invasive pulmonary carcinoma].

A 69-year-old man was admitted to our hospital with a complaint of cough and hemoptysis. His chest X-ray showed an obstruction of the right intermediate bronchus and resultant atelectasis of the middle and lower lobes. Cytological examination by bronchoscopy showed squamous cell carcinoma. Although the cancer involvement of the left atrium could not be clearly defined by preoperative chest CT scan, the cancer invasion to the left atrial wall was recognized intraoperatively. Right pneumonectomy along with resection and patch reconstruction of the left atrium, right atrium and atrial septum was performed under extracorporeal circulation. Postoperative hemodynamic state was stable, and echocardiography showed normal volume of the left atrium. Histological examination of the resected specimen showed moderately differentiated squamous cell carcinoma with the involvement of the left and right atrial wall. The resected margins of the left and right atria were free of malignancy. Although he had been clinically in good condition and free from any sign of cancer recurrence, he died of aspiration pneumonia five months after the surgery.

[Significance of fiberoptic bronchoscopy after pulmonary resection of lung cancer for early detection in second lung cancer].

By the end of 1988, seventy five cases after pulmonary resection for lung cancer and one case after pulmonary resection for basal cell hyperplasia were examined with fiberoptic bronchoscopy as postoperative follow up. Endobronchial tumor was detected in 14 cases. In seven cases, endobronchial tumor was located at site far from the surgical bronchial anastomosis. These tumors were suspected as second lung cancer as long as examined by fiberoptic bronchoscopy. Histological type of all of these was squamous cell carcinoma. Re-operations were performed in five cases and two of them were early lung cancer. In 6 cases (13.6%) among 44 cases examined with fiberoptic bronchoscopy after pulmonary resection for central type squamous cell carcinoma, second lung cancer was detected. In the case of central type squamous cell carcinoma, usually the tumor appears to be of same histological type and at same location. Among those cases those who smoke heavily should be separated as high risk group. If this group is followed with fiberoptic bronchoscopy or sputum cytological examination, it is more possible to detect second lung cancer at an early stage.

Relationship between mitogen receptors in peripheral blood lymphocytes and blastogenic responses to mitogen.

The relationship between the optimum concentration of mitogen which induces lymphocyte blastogenic response and the receptor occupancy by mitogen was investigated. The receptor occupancies which induced maximal blastogenic activity in equine, bovine and canine peripheral blood lymphocytes (PBL) were 31.1 per cent, 26.5 per cent and 38.4 per cent with phytohaemagglutinin-P, and 48.2 per cent, 17.9 per cent and 24.5 per cent with concanavalin A, respectively. The data clearly show that each animal species had its own optimum concentration of mitogen for stimulation of PBL. Optimum concentration for blastogenesis and number of binding sites of each mitogen had a good correlation with each other for all three species.

[Treatment of malignant lymphoma of the small intestine in aged patients over 70].

Case histories of three elderly patients with a malignant lymphoma of the small intestine are described. Case No. 1 involved a 80-year-old male with a diffuse malignant lymphoma of medium size that was associated with a jejunal perforation. On the 64th day after surgery, he died of heart failure during the course of chemotherapy. Case No. 2 was a 76-year-old male with a diffuse malignant lymphoma of medium size in the ileocecum. He left hospital after surgery had relieved his symptoms, although 8 months after he developed active pulmonary tuberculosis during the course of chemotherapy. Case No. 3 involved a 76-year-old female with a diffuse malignant lymphoma of the mixed cell type in the terminal ileum. She died of heart failure complicated by a pulmonary disease on the 36th postoperative day. The therapeutic results of treating the malignant lymphoma in these 3 aged patients were all unfavorable. Thus, it is felt that special care should be taken in treating aged patients with this kind of disease.

[Retrospective study of tumor growth and progression in gastric leiomyosarcoma; a care report].

Presented is the case of a 58-year-old female who had been found to have a submucosal tumor of the stomach during a group medical examination 7 years earlier. Since that time, the proliferative course of the tumor has been regularly observed and the diagnosis of a gastric smooth muscle sarcoma was finally determined after an endoscopic examination performed this year. On the basis of X-ray photographs taken periodically, the mass gradually proliferated from 1.3 to 3.5 cm in diameter during the observation period of 2392 days. According to the doubling time (DT) calculation of Horai et al, the DT was 0.69. The proliferation in this case was very slow and this seems to be a characteristic of a highly differentiated smooth muscle sarcoma of the stomach.

Effects of vanadate on tyrosine phosphorylation and the pattern of glycosaminoglycan synthesis in rabbit chondrocytes in culture.

The present study examined the effects of high doses of vanadate on glycosaminoglycan (GAG) synthesis and tyrosine phosphorylation in rabbit chondrocytes in confluent cultures. Although 6 microM vanadate increased the incorporation of [3H]glucosamine into chondroitin sulfate proteoglycans twofold, 40-60 microM vanadate suppressed this incorporation fourfold. Although 6 microM vanadate had little effect on [3H]glucosamine incorporation into hyaluronate, 40-60 microM vanadate increased this incorporation threefold. Chemical analyses confirmed that the increase in [3H]glucosamine incorporation into hyaluronate and the decrease in the incorporation into chondroitin sulfate proteoglycan correlated with increased hyaluronate content and decreased chondroitin sulfate content in the cell layers of vanadate-transformed cells. Chondrocytes exposed to 40-60 microM vanadate became typically transformed spindlelike cells. Furthermore, vanadate, at 6 and 60 microM, increased the overall level of phosphotyrosine by 8- and 31-fold, respectively, and 60 microM vanadate enhanced phosphorylation of many phosphotyrosine-containing proteins. These observations suggest that vanadate induces transformation-associated changes in the pattern of GAG synthesis when it induces excess phosphorylation on tyrosine in chondrocyte proteins.

Anomalous origin of anterior choroidal artery.

We report a very rare case of anomalous origin of anterior choroidal artery. In our case anterior choroidal artery arises from internal carotid artery proximal to the posterior communicating artery.

Effect of FK-506, a novel immunosuppressive drug on murine systemic lupus erythematosus.

A novel immunosuppressive compound extracted from the fermentation broth of Streptomyces tsukubaensis belongs to the macrolide family. We gave this drug (FK-506) to MRL/lpr and NZB X NZW F1 (B/W F1) mice, an animal model of human systemic lupus erythematosus (SLE), to investigate its effect on the course of the disease. This drug showed potential to prolong the lifespan, to reduce proteinuria, and to prevent the progression of nephropathy. Appreciable differences in the levels of anti-dsDNA antibodies between treated and control animals were nil.

Interleukin 1: a regulator of chondrocyte proliferation.

Human interleukin 1 beta (IL-1 beta), at low concentrations (0.3-1 ng/ml), decreased by 95% the efficiency of colony formation in soft agar by rabbit articular chondrocytes. Furthermore, interleukin 1 (IL-1) suppressed by 50-60% the incorporation of [3H]thymidine into DNA in high density chondrocyte cultures on plastic dishes in the presence of 10% serum or fibroblast growth factor, although it increased twofold this incorporation in 0.3% serum alone. This suggests that IL-1 directly inhibits mitogenic response of differentiated chondrocytes to growth factors. In contrast, IL-1 stimulated [3H]thymidine incorporation into DNA in rabbit fibroblasts in the presence of growth factors. The selective suppression of chondrocyte replication by IL-1 may play an important role in cartilage destruction associated with chronic inflammatory joint diseases.

IgG class anti-cardiolipin antibody as a possible marker for evaluating fetal risk in patients with systemic lupus erythematosus.

To identify the correlation between incidence of anti-phospholipid antibodies and fetal prognosis in pregnant SLE patients, we measured the amount of anti-cardiolipin antibody in their sera, using solid-phase enzyme immunoassay (EIA) methods. Findings in the group having poor obstetric results (fetal loss group) and in those with a history of full-term births (live birth group) were compared with regard to other anti-phospholipid antibodies. The incidence of IgG class anti-cardiolipin antibody was 60% in the fetal loss group and 19% in the live birth group, (P less than 0.05). The incidence of the other anti-phospholipid antibodies, including lupus anticoagulant and biological false-positive serological test for syphilis (BFP-STS), did not differ significantly between the two groups. Therefore, the presence of IgG class anti-cardiolipin antibody may prove to be a useful marker for evaluating fetal risk in SLE patients.

Role of Ca2+ channels in the ability of membrane depolarization to prevent neuronal death induced by trophic-factor deprivation: evidence that levels of internal Ca2+ determine nerve growth factor dependence of sympathetic ganglion cells.

Sympathetic neurons depend on nerve growth factor (NGF) for their survival both in vivo and in vitro; these cells die upon acute deprivation of NGF. We studied the effects of agents that cause membrane depolarization on neuronal survival after NGF deprivation. High-K+ medium (greater than or equal to 33 mM) prevented cell death; the effect of K+ was dose-dependent (EC50 = 21 mM). The protection by high K+ was abolished either by withdrawal of extracellular Ca2+ or by preloading the cells with a Ca2+ chelator. The involvement of Ca2+ flux across membranes in high-K+ saving of NGF-deprived neurons was also supported by experiments using Ca2+-channel antagonists and agonists. The Ca2+ antagonists nimodipine and nifedipine effectively blocked the survival-promoting effect of high K+. The Ca2+ agonists Bay K 8644 and (S)-202-791 did not by themselves save neurons from NGF deprivation but did strongly augment the effect of high K+; EC50 was shifted from 21 mM to 13 mM. These data suggest that dihydropyridine-sensitive L-type Ca2+ channels play a major role in the high-K+ saving. The depolarizing agents choline (EC50 = 1 mM) and carbamoylcholine (EC50 = 1 microM), acting through nicotinic cholinergic receptors, also rescued NGF-deprived neurons. The saving effect of nicotinic agonists was not blocked by withdrawal of extracellular Ca2+ but was counteracted by a chelator of intracellular Ca2+, suggesting the possible involvement of Ca2+ release from internal stores. Based on these findings we propose a "Ca2+ set-point hypothesis" for the degree of trophic-factor dependence of sympathetic neurons in vitro.

Relationship between anti-IgE autoantibody and severity of bronchial asthma.

To determine whether anti-IgE autoantibody (aIgE) is involved in modulating allergic asthma, we examined the relationship between the titers of aIgE and severity of asthma in 63 allergic asthmatics. The titer of aIgE in patients' sera was 14.4 +/- 17.9 units (mean +/- SD). Thirty-one patients had aIgE greater than 5 units (26.4 +/- 22.1 units), whereas 32 patients had aIgE less than 5 units (2.0 +/- 0.9 units). Patients with high aIgE (greater than 5 units) were less severe than those with low aIgE (less than 5 units) (1.65 +/- 0.42 and 2.19 +/- 0.52 of severity score, respectively; p less than 0.01). There was an inverse correlation between the titers of aIgE and the severity scores (r = -0.328; p less than 0.01). Patients with high aIgE also had lower bronchial reactivity to methacholine than those with low aIgE (556 +/- 292 and 170 +/- 182 micrograms/ml of threshold dose of inhaled methacholine, respectively; p less than 0.005). There was a significant correlation between the titers of aIgE and the threshold dose of methacholine (r = 0.499; p less than 0.01). These findings suggest that aIgE plays a protective role in bronchial hyperreactivity in allergic asthma.

Angiographic follow-up study of cerebral arteriovenous malformations with reference to their enlargement and regression.

Twenty patients with cerebral arteriovenous malformations (AVMs) were followed up by angiography. Of the 20 AVMs, 16 were nontreated and the remaining 4 were residual. The follow-up periods between the two examinations were from 5 to 28 years, with a median interval of 15 years. At follow-up, the size of the AVM had increased in 4 patients (20%), decreased in 4 (20%), totally regressed in 4 (20%), and was unchanged in 8 (40%). In cases of enlarged AVMs, the patients were all young: from 0 to 11 years old at first angiography and from 6 to 30 at second angiography. The decrease or total regression of the AVM occurred in older patients (over 30 years old at second angiography), and the AVMs were relatively small and fed by single or a few feeders. The patient's age and the size of AVM should be considered when operative indications are discussed.

Intracranial extradural pressure monitoring after direct operation on ruptured cerebral aneurysms.

Intracranial extradural pressure (ICP) was monitored by using a miniaturized transducer for an average period of 8 days after direct operation in 55 patients with ruptured cerebral aneurysms. Acute stage operation and drainage of cerebrospinal fluid were simultaneously performed in most of the patients. In many grade I patients with satisfactory cisternal drainage, ICP was monotonously stable, with faint pressure waves. When the drained fluid volume decreased or drainage was removed, however, ICP was elevated moderately, with associated pressure waves present. There were no A-waves observed in any of the patients. Decreases of mean ICP and disappearance of pressure waves were found 2 to 3 hours after infusion of 200 or 300 ml of 10% glycerol or 20% mannitol in patients without drainage or with an inadequate drain, but were not found in patients with a good drain. In a patient showing diffuse severe vasospasm, a rapid elevation of ICP caused by marked brain edema was observed. In patients with residual aneurysms or incompletely clipped aneurysms, ICP increased immediately after rupture of these aneurysms. The daily mean ICP was higher in patients with a clinically poor condition and/or severe subarachnoid hemorrhage on admission than in those with a good condition and/or mild subarachnoid hemorrhage, in spite of a functional drain. There was a poor outcome in about half of the patients showing maximum daily mean ICP greater than 30 mm Hg or frequent B-waves. No complications caused by ICP monitoring were found.(ABSTRACT TRUNCATED AT 250 WORDS)

The inhibitory effects of colchicine, vinblastine and cytochalasin D on lymphocyte blastogenic responses.

The inhibitory effects of colchicine, vinblastine and cytochalasin D on blastogenic responses of equine, bovine and canine peripheral blood lymphocytes (PBL) were investigated. These drugs inhibited blastogenic responses of each PBL. The concentrations for 50% recovery in PBL blastogenic response of colchicine and vinblastine were lower in equine and canine PBL than in bovine PBL. This suggested that microtubules may be more concerned in blastogenic response of equine and canine PBL than of bovine PBL. On the other hand, the concentration for 50% recovery in PBL blastogenic response of cytochalasin D was almost the same in the PBL of each animal. This meant that microfilaments made only a small contribution to the lymphocyte blastogenic response.

Role of lymphocytes in dogs experimentally re-challenged with canine transmissible sarcoma.

The role of lymphocytes in dogs re-challenged with canine transmissible sarcoma (CTS) was investigated histologically and immunologically. Tumors were detected on the 3rd day, began to regress on the 9th day and disappeared on approximately the 15th day after the secondary transplantation (AST). The CTS cells were surrounded by lymphocytes which had infiltrated from the early stage, degenerated and ultimately disappeared. Most of the lymphocytes expressed thymocyte antigen (Ta) on the cell membrane surface. The numbers of lymphocytes and Ta-positive lymphocytes in peripheral blood increased gradually AST. The blastogenic responses of peripheral blood lymphocytes (PBL) induced by 3 kinds of mitogens were elevated strikingly from the early stage AST. These results suggest that Ta-positive lymphocytes play an important role in vivo in the regression of tumors and that the immunological activity of PBL is stimulated immediately AST.
