Heterogeneity of histiocytes in primary lung cancer stained with anti-S100 protein, lysozyme and OKT 6 antibodies.

The heterogeneity of histiocytes in primary lung cancer was investigated by immunohistochemical methods using anti-S100 protein and anti-lysozyme antibodies on paraffin sections and OKT-6 monoclonal antibody on frozen sections. T-zone histiocytes (Langerhans cells and their precursors stained by anti-S100 protein and/or OKT-6 monoclonal antibodies) heavily infiltrated tumor tissues and regional lymph nodes in cases of moderately or well-differentiated adenocarcinoma, especially in areas of papillary growth or bronchiolo-alveolar pattern. These cells were interspersed amongst tumor cells showing dendritic figures and were occasionally present in squamous cell carcinoma. However, they were seldom found in other histological types, particularly in small cell carcinoma and carcinoid tumor. The distribution of these cells was different from that of lysozyme-positive macrophages. In this connection, T-zone histiocytes were thought to have a different immunological function from that of the monocyte-macrophage series against lung cancer. The closely similar reactivity of T-zone histiocytes with anti-S100 protein and OKT-6 monoclonal antibodies indicated that these two markers can be employed for paraffin and frozen sections, respectively.

[Cushing's disease caused by adrenocortical nodular hyperplasia--diagnosis and surgical treatment].

Six cases of Cushing's disease, in which perplex diagnostic problem was encountered, were reported. In the endocrinological examination two cases were not suppressed by a high dose of dexamethasone (8 mg), and the other four cases showed complicated clinical findings due to adrenocortical nodular hyperplasia. Among those four cases with adrenocortical nodular hyperplasia two of them were of functionally autonomic and showed different attitude in responding endocrine assay. When the serum cortisol is not suppressed by a high dose of dexamethasone, differential diagnosis from other causes, such as Cushing's syndrome due to adrenocortical macronodular hyperplasia, becomes quite important for surgical treatment. Usually plasma ACTH in the case of Cushing's disease is slightly increased over the normal range and, on the other hand, it is lower than normal value in the case of Cushing's syndrome due to autonomic adrenocortical nodular hyperplasia because of negative feed back phenomenon. However, in some cases of Cushing's disease plasma ACTH is within normal range or even lower than normal level, and, furthermore, it is not extremely unusual to find a case of adrenocortical macronodular hyperplasia with slightly increased plasma ACTH level over normal range. Those cases offer particularly complex problem in making diagnosis and therapeutic indication. According to our clinical experiences with those six cases, selective venous sampling is found quite useful in differentiating Cushing's disease from other causes and ACTH value in selective venous sampling from inferior petrosal vein is remarkably increased in the cases due to pituitary ACTH secreting adenoma. Cushing's disease with adrenocortical nodular hyperplasia reduces the ACTH value in the sampling to the lower normal range as the adrenocortical nodular hyperplasia progresses.(ABSTRACT TRUNCATED AT 250 WORDS)

[Measurement of the liver and spleen volume by single photon emission computed tomography].

Organ volume was experimentally and clinically determined using single photon emission computed tomography (SPECT). In organ phantom filling with 99mTcO4-, the error was less than 3.3% in the volume over 400 ml but 11% at 200 ml. In 28 cases of patient, liver and spleen volume were measured by SPECT and X-CT. Good correlation was founded between SPECT volume and X-CT volume (r = 0.99, n = 49), although SPECT volume were 16% higher than X-CT volume (y = 1.16 x + 11.6, x: X-CT volume, y: SPECT volume).

Distribution of blood group antigens A, B, H, and I(Ma) in mucus-producing adenocarcinoma of human lung.

The distribution of blood group antigens A, B, and H and their precursor antigen I(Ma) in the mucus of lung cancer patients was studied histochemically. Antigens A, B, and H compatible with the ABO status of the patients were expressed in the mucus of bronchial gland mucous cells and bronchial goblet cells except in a few patients presumed to be nonsecretors , while antigen I(Ma) was not expressed at all in these cells. In contrast, compatible antigens A, B, and H were decreased or not present in cancer cells but antigen I(Ma) accumulated. Accumulation of antigen I(Ma), which resulted from incomplete synthesis of antigens A, B, and H, was a tumor-associated phenomenon in the mucus of the lung and was found in all patients examined. However, the degree of this accumulation varied from patient to patient, and tumor cells showed marked heterogeneity even in an individual case. In addition, accumulation of antigen associated with loss of antigens A and B was demonstrated in cancer cell mucus of patients with a blood group status other than O. Expression of incompatible blood group antigen reactive with anti-A serum (A-like antigen) also was detected in cancer cell mucus of blood group B patients.

[A case of alpha-fetoprotein-producing gastric carcinoma with unique macroscopical configuration].

A gastric carcinoma with unique macroscopical configuration was resected from a 63-year-old male patient. Serum level of alpha-fetoprotein (AFP) was markedly elevated (19,000 ng/ml), and AFP-positive cells were found in the carcinomatous tissues. The cytoplasm of the carcinoma cells appeared to be watery-clear by light microscopy, however, electron microscopy revealed abundant amorphous substances in their cytoplasmic matrix. The unique macroscopical configuration of the gastric carcinoma may reflect its biological resemblance to hepatocellular carcinoma. Such a case may be rare, but AFP production, per se, seems to be a rather universal phenomenon of gastric carcinomas.

Laparoscopic ultrasonography. Diagnosis of liver and pancreatic cancer.

We tested two kinds of laparoscopic ultrasonography having a 3.5 MHz and a 5.0 MHz, linear assay, generated into 45-mm-long and 13-mm-wide laparoscopic tip. Twenty studies were performed in 6 patients with HCC, 2 patients with pancreatic cancer and others. It is clearly demonstrated that this apparatus provides sufficient resolution real-time ultrasound visualization of a target organ on which ultrasonic probe is placed under direct vision.

Curious manifestations in cavernous hemangioma of the liver.

We report a cavernous hemangioma of the liver with some curious findings. Angiography showed a hypervascular lesion and the arterioportal shunt which commonly is regarded as a pathognomonic sign of malignancy. Laparoscopy showed a well-defined, reddish-purple elevated lesion with a central umbilication. As the patient was a painter and dealing with vinyl chloride monomer, we suspected the possibility of hemangiosarcoma. The lesion proved to be a cavernous hemangioma with central fibrous replacement probably in consequence of thrombosis of some cavernous spaces.

[Clinical studies on netilmicin].

Netilmicin (NTL), a new semisynthesized aminoglycoside, was evaluated in 11 episodes of infection in 10 patients, who had severe underlying diseases, such as acute myocardial infarction, cerebral infarction, malignancy and hepatic cirrhosis. The infection was bacteremia in 3 cases, urinary tract infections in 3 cases and respiratory tract infections in 5 cases. NTL was administered intramuscularly at a dose of 100 mg twice a day for 3 to 14 days. Overall clinical efficacy was only 40%, including excellent in 2 cases, good in 2 cases, fair in 3 cases and poor in 3 cases. Bacteriologically, 2 episodes of E. coli, 2 of S. marcescens and 1 of K. pneumoniae were eradicated, whereas, 2 of P. aeruginosa were decreased, and 1 of K. pneumoniae and 1 of P. rettgeri were persisted. Transient eosinophilia was observed in 1 case, and also nephrotoxicity was encountered in 1 case.

Villous tumor of the stomach associated with adenocarcinomas--a histochemical study of mucosubstances.

A case is described in which both a moderately elevated villous adenoma associated with adenocarcinoma at the posterior wall and an independent gastric carcinoma at the anterior wall were present. These two lesions were surrounded by the mucosa of incomplete intestinal metaplasia. Consecutive sections of the villous adenoma associated focally with carcinoma and the independent cancer were stained by the periodic acid-Schiff (PAS), pH 2.5 alcian blue (A1-B1), high iron diamine pH 2.5 alcian blue (HID-AB) and carcinoembryonic antigen peroxidase-antiperoxidase (CEA-PAP) methods. Villous adenoma was weakly positive in the PAS stain but negative in A1-B1 and HID stains, showing that it did not produce mucin. On the other hand, the intestinal metaplasia and cancerous lesions were positive in PAS, A1-B1 and HID stains, indicating production of the intestinal type of mucin. The villous adenoma accompanied by malignant changes was positive by the CEA-PAP method. This result shows the biological property of villous adenoma that they can easily change into malignancy. These three lesions in our case are considered to have originated independently from the primordial cells and to have developed the differences in mucin production in the process of cell development.

Multiple primary cancers in 5,456 autopsy cases in the National Cancer Center of Japan.

Multiple primary malignant neoplasms in 5,456 consecutive autopsy cases from 1962 to 1981 in the National Cancer Center were analyzed. There were 285 (5.2%) double primary cancers, 58 (1.1%) triple or more, and 65 (1.2%) minute cancers of the thyroid or prostate gland. Higher incidence of second cancer is observed in cancers of the oropharynx, intestine, larynx, uterus, bladder, and thyroid. Organ association between the two cancers was present in certain organs; for example, there was a tendency for upper gastrointestinal tract cancers to be associated with lower gastrointestinal tract cancers. Influence of histologic subtypes in multiple cancer cases is discussed for lung cancer.

Enhancing effect of hydrocortisone acetate administration on the content of A-type particles in Ehrlich ascites tumor.

The incidence of intracytoplasmic A-particles was investigated in neoplastic and nonneoplastic cells of Swiss/ICR mice. The concentration of the virus-like particles decreased in the order of 1) invasive Ehrlich 4N ascites clone 1 tumor, 2) noninvasive Ehrlich 4N ascites clone 3 tumor, and 3) nonneoplastic cells of tumor-free mice. Four successive injections of hydrocortisone acetate, 1 mg/day/mouse, when administered after tumor inoculation, almost tripled the cellular content of A-particles in Ehrlich clone 1 tumor (P less than .001), whereas the same treatment given before tumor inoculation did not affect the particle content. The enhancing effect of the hormone was found to be dose-dependent (P = .0027). Possibly, hydrocortisone acetate accelerates the formation of A-particles by stimulating directly the virus-generating apparatus of clone 1 tumor cells.

[Characteristics of urinary steroid excretions in ovarian cancer patients].

We performed a case-control study to investigate the urinary steroid excretions in ovarian cancer patients. The use of gas-liquid chromatography permitted the estimation of 14 urinary steroids. Data on 139 urine specimens from 36 cancer patients were compared with those on age-matched controls. A general depression of adreno-corticosteroid excretions was consistently observed in the cancer patients as compared with the control women. The possible role of observed steroidal disorders in the genesis of ovarian neoplasia is discussed in light of the epidemiological and biological aspects of this tumor.
