Diagnosis of small pancreatic carcinoma.

A retrospective analysis was performed to evaluate the clinical symptoms and abnormal test findings in small pancreatic carcinoma. Five hundred and thirty-six cases of pancreatic carcinoma with the histology of duct cell carcinoma were collected from 14 medical centers in Japan. In 440 of the cases, tumor size was measured at the time of laparotomy or from the resected specimen. Three hundred and seventy-seven patients (86%) had a carcinoma larger than 3.0 cm; only 30% of these were resectable. Sixty-three patients (14%) had a carcinoma of 3.0 cm or less, with resectability of 97%. Detecting a tumor of "3 cm or less" with a high probability of resectability is the objective of early diagnosis with the resulting possibility of a cure. In most cases these small carcinomas were found easily when obstructive jaundice was present (73%). However, the estimated occurrence of obstructive jaundice associated with carcinomas of 3 cm or less was only 10% among the total cases of pancreatic carcinoma studied. Therefore, it is necessary for early diagnosis to detect carcinomas of 3 cm or less presenting without jaundice. The symptoms of small carcinoma without jaundice are weight loss, anorexia, upper abdominal pain, back pain and a palpable abdominal mass. Among the various available examinations, endoscopic retrograde cholangiopancreatography, computerized tomography and ultrasonography were valuable in diagnosing these small carcinomas.

[The metastatic patterns of gastric cancer from its histopathological characteristics in the primary lesion--particularly in relation to peritoneal dissemination and liver metastases].

The purpose of this study was to catagorize the metastatic patterns of advanced gastric cancer into liver and peritoneal metastases from the stromal behavior of the primary lesion in 278 patients. According to our data, peritoneal disseminations were encountered 5.0% of the medullary type, 30.0% of the intermediate type and 38.0% of the scirrhous type, while the corresponding figures for liver metastases were 28.8%, 6.0% and 1.9%, respectively. The differences among the three types were significant with both patterns of metastasis. From the present data, we assume that the histological pattern in the stroma of primary gastric cancer shows the biological behavior of cancer growth and metastatic patterns, particularly in close relation to peritoneal dissemination and liver metastases.

Analysis of DNA ploidy patterns of gastric carcinomas of Japanese.

Ploidy patterns of gastric carcinomas of 54 Japanese patients, ranging in age from 28 to 82 years, were determined by cytofluorometry. The gastric cancers were divided into four basic ploidy patterns; the first pattern was a diploid mode, the second was a heteroploid mode, the third was a mosaic pattern of a diploid and a heteroploid mode, and the fourth was a mosaic of two or more different heteroploid modes. The incidence was 68.4%, 14.8%, 12.9%, and 3.7% in the first, second, third, and fourth patterns, respectively. All the cancers contained polyploid populations, whereas the early cancers tended to contain a lesser amount of polyploid cells than the advanced ones. Diffuse-type cancers and well-differentiated, intestinal-type adenocarcinomas were mostly unimodal cancers of the diploidy or heteroploidy, whereas poorly differentiated adenocarcinomas were often mosaic cancers of several different ploidies. There was no correlation between ploidy patterns and ages and sexes of patients. For 11 cases, labeling indices with 3H-thymidine were determined. Labeling indices of the intestinal-type and diffuse-type cancers were 12% to 27% and 7% to 19%, respectively.

Genesis of resting microglia in the gray matter of mouse hippocampus.

The genesis of resting microglia in the gray matter of mouse hippocampus was studied by 3H-thymidine autoradiography in combination with electron microscopy. Newborn mice were injected with 3H-thymidine singly or repeatedly at different postnatal stages, and killed shortly after the injection or after various intervals. Tissue specimens of the hippocampus at CA1 and CA2 were processed for light and electron microscopic autoradiography. The results showed that at least 91% of glial cells in the stratum radiatum of the hippocampus are produced after birth. About three-fourths of astroglia in this area are produced before the sixth postnatal day, and a larger part of resting microglia are formed after the ninth postnatal day. Morphological transition can be traced from either proliferating cells in the stratum radiatum at late postnatal days to resting microglia, or from those in early postnatal days to astroglia. A continuous morphological transition was observed between the proliferating cells at the late postnatal days (microglial production period) and those at the early postnatal days (astroglial production period). The latter retain some fine structural characteristics similar to small glioblasts in the subependymal layer. These findings strongly suggest that resting microglia, as well as astroglia, are derived from glioblasts, and are of neurectodermal origin.

Malignant parotid gland tumors and facial nerve paralysis.

During the last three decades, 586 cases of parotid gland tumor have been extirpated in our clinic. The tumors were malignant in 170 cases and, of these, facial nerve paralysis was observed at first examination in 59 patients. This paper deals with the retrospective study of these cases and is chiefly concerned with the histological types and facial nerve paralysis.

Electrocardiographic changes following electroconvulsive therapy.

This paper reports a study of the effects of ECT (electroconvulsive therapy) upon cardiac function (as judged by electrocardiographic changes) in 24 patients who were given 139 treatments in all. Of the 24 patients 13 were studied over a 24-h period using monitoring equipment. No potentially dangerous arrhythmias were encountered. Modified ECT was found to give rise to sinus tachycardia, which was considerably more prolonged than previously reported. The tachycardia was shown in some cases to be accompanied by a depression of the ST segment and may hence be potentially harmful to those with pre-existing ischaemic heart disease. In such patients consideration should be given to the prior administration of beta-adrenergic blocking drugs and/or oxygen.

Immunohistochemical studies of blood monocytes infiltrating into the neonatal rat brain.

Brains of normal rats ranging in age from newborn to adult were observed with immunofluorescence technique using anti- granulomonocyte antiserum. For the first 10 days after birth, many cells with positive fluorescence were found in the white matter, the subependyma , the extra-parenchymal spaces, and the leptomeninx , but very few in the gray matter. They were mononuclear, rich in cytoplasm, and globular or irregular in shape. After about day 10 p.n., the positive cells decreased in number and became slender. However, there was no change in the distribution pattern. After about 3 weeks of age, no positive cells were detected in the brain parenchyma, except for very rare necrobiotic ones. It was suggested that blood monocytes infiltrate into the brain parenchyma of normal neonatal rat, but only for a while in the limited areas (white matter and subependyma ). They have the morphology and distribution of the "ameboid microglia" of neonatal brain. These monocytes disappear from the brain finally by the end of month 1 p.n.

Serum lipase response to cerulein secretin stimulation test in patients with pancreas-associated diseases as measured by sensitive colorimetric assay (a BALB-DTNB method).

We have compared responsiveness of serum lipase and amylase activity to the pancreatic exocrine stimulation with cerulein and secretin (CS test) in normal subjects and patients with pancreas-related and other diseases. The lipase and amylase activities were measured by a sensitive colorimetric method, the BALB-DTNB method and the Caraway method, respectively. The percentage of positive lipase and amylase response cases was as follows: confirmed chronic pancreatitis (N = 22), 27 and 14%; suspected chronic pancreatitis (N = 37), 46 and 32%; pancreatic cancer (N = 16), 44 and 25%; biliary tract diseases (N = 11), 14 and 14%; miscellaneous (N = 11), 0 and 18%; normal subjects (N = 13), and partial pancreatectomy (N = 5), 0 and 0%, respectively. The serum lipase response cannot be regarded as specific for pancreatic diseases because the lipase response cases were found in biliary tract diseases as well. However, in view of frequent, fast, and intense responsiveness to the CS test, the serum lipase activity measured by the BALB-DTNB method may be more useful than the serum amylase as an auxiliary diagnostic aid for suspected pancreatitis which might develop into confirmed chronic pancreatitis or cancer of the head or body of the pancreas.

Bilateral facial microsomia associated with separation of the posterior portions of the maxilla. A case report.

A case of bilateral facial microsomia associated with separations of the posterior portion of the maxilla is presented. The X-ray findings revealed hypoplasia of the condyles, absence of the coronoid processes of the mandible, and displacement of the pterygoid processes. Cephalometric analysis revealed a characteristic feature of the facial skeleton a rotation clockwise to the anterior cranial base.

An immune adherence assay for discrimination between etiologic agents of hemorrhagic fever with renal syndrome.

Antigens of the viruses that cause hemorrhagic fever with renal syndrome (HFRS) and antibodies to these antigens were titrated in an immune adherence hemagglutination (IAHA) test and a CF test. Vero-E6 cells infected with the SR-11 strain of HFRS virus, an isolate from rats associated with a laboratory outbreak in Japan, and the 76-118 strain of Hantaan virus, the etiologic agent of Korean hemorrhagic fever, were used as antigens in the IAHA, CF, and indirect immunofluorescent antibody (IFA) tests. Sera from patients with different types of HFRS were tested against both strains of virus. Titers of antibody detected by the IFA test were similar for the two strains, but the IAHA test discriminated between antibodies to the two types of HFRS virus. Antigenic differences between HFRS viruses were suggested by the results of the IAHA test.

Depression and ventricular enlargement.

Compared with controls, 46 depressed patients showed increased ventricular-brain ratio (VBR). Patients with poor outcome, single episode, onset at 50 years or over, or without melancholia were found to have greater VBR than controls.

Reliability of clinical assessment of blunted affect.

The affect of 20 chronic schizophrenic in-patients was assessed by two psychiatrists using two rating scales. The interview recordings were reviewed three times--firstly with the auditory information only (the "audio review"), secondly with the visual information only (the "visual review") and thirdly with both (the "audio-visual review"). The "blunted affect score" correlated strongly with the total score of the verbal items of scales, but did not when a single mode of information was given. Since the absolute values of the "blunted affect score" were lower in the "visual review" than in the live interview and in the "audio-visual review", it was speculated that speech tended to bias the assessment of affect.
