[Treatment of four cases of B-cell lymphoma].

Four cases of B cell lymphoma, seen at the Department of Dermatology, Miyazaki Medical College, for the previous approximately 10 years, were reviewed. Case 1 was characterized by a localized tumours lesion on the head; the histologic picture was of the follicular, medium sized cell type. Complete remission for 8 years has been obtained after electron beam therapy and surgical removal of the tumor. Cases 2 and 3 had a histologic picture of diffuse, large cell type and were assumed to have a bad prognosis with standard chemotherapy. In addition to conventional chemotherapy, they were placed on noncross-resistant alternating combination chemotherapy with medium dosages of MTX, VP-16, VDS, PCZ MXT, PEP and ACR, so as not to develop cross resistance. Case 2 has been in complete remission for 11 months and case 3 for 7 months. Case 4 was a nodal lymphoma with skin lesions. This patient was treated mainly by local radiation therapy, but later died with leukemic changes and renal failure. Cutaneous B cell lymphoma should be treated vigorously and carefully, since it has a poorer prognosis than that of lymphoma of lymph node origin.

[Coordinate regulation of immune and inflammatory responses by cytokines].

In response to antigenic stimulation, T cells and macrophages secrete a set of glycoproteins termed lymphokines and monokines such as IL-1, IL-2, IL-3, IL-4, IL-5, IL-6, GM-CSF, G-CSF, IFN-alpha, IFN-gamma, TNF-alpha, and lymphotoxin. These glycoproteins mediate immune and inflammatory responses by regulating proliferation, differentiation and maturation of lymphocytes, hemopoietic cells and endothelial cells. Along with factors such as M-CSF, IL-7, LIF, IFN-beta and TGF-beta produced by stromal cells, all these molecules are collectively called cytokines. Most cytokines are pleiotropic and have multiple biological activities. Accordingly, regulatory network is formed between lymphoid cells, hemopoietic cells and endothelial cells. All or a part of this network can be activated in different ways by unique combinations of cytokines. IL-3, IL-4 and IL-5 produced by TH2 cells enhance whereas IFN-gamma produced by TH1 cells suppresses IgE mediated allergic response. Through production of a spectrum of cytokines, T cells and macrophages play a vital role in coordinating the function of different body compartments in immune and inflammatory responses. The activation of cytokines genes in T cells and macrophages is transient. Therefore, an inflammatory response that involves proliferation and maturation of hemopoietic progenitor cells may be restricted to the site of cytokine production. The inducible nature of the hemopoiesis associated with inflammatory response is in contrast to the constitutive, steady state hemopoiesis that occurs in the bone marrow microenvironment in the absence of immunological stimuli.

Analysis of GFA-protein mRNA expression in developing bovine brain by in situ hybridization and northern blot hybridization.

Using bovine brains of adult and developmental stages, the time and place of the appearance of mRNA for glial fibrillary acidic protein (GFA-protein) were studied by in situ- and Northern blot-hybridization. Double-stranded cDNA labeled with [3H]dCTP or [32P]dCTP was used as the probe for this mRNA. To compare the location of GFA-protein mRNA and GFA-protein itself on serial sections. GFA-protein immunohistochemistry was used. By in situ hybridization with adult bovine brain. GFA-protein mRNA was detected in astroglia, most of which were in the white matter. The distribution of these astroglia by in situ hybridization was consistent with the findings by GFA-protein immunohistochemistry and Northern blot hybridization, indicating that each techniques were specific. Concerning fetal stages, GFA-protein mRNA could be detected in the brain of a fetal calf with a body length of 28 cm by in situ hybridization using the 32P-labeled probe, and the mRNA was localized in the subpial area and the fornix. These results indicated that glial maturation first became recognizable at least in the subpial area and the fornix in the brain of a fetal calf measuring 28 cm. In this fetal brain, GFA-protein mRNA was almost undetectable by Northern blot hybridization. This suggested that in situ hybridization was more sensitive and useful for the analysis of gene expression than Northern blot hybridization, when the target mRNA is present in only a limited area, such as in the brain.

[Study of chromogenic substrate on protein C activity assay--in patients treated with warfarin].

Protein C is an anticoagulant protein that circulates in blood as a zymogen of a serine protease. Recently the exogenous Protein C activator has been obtained from the venom of Agkistrodon Contortrix Contortrix. We also reported a functional assay method which activates Protein C with PROTAC and then measure the amidolytic activity with chromogenic substrate (S-2366) by COBAS FARA. At present, S-2366 (Glu-Pro-Arg-pNA), CBS 65-25 (Lys-Pro-Arg-pNA) and SPECTROZYME PCa (Lys-Pro-Arg-pNA) are used as the chromogenic substrate for activated Protein C. We studied which substrate is more suitable for amidolytic activity assay for Protein C by PROTAC in 99 patients under long-term Warfarin therapy and 29 as normal subjects. Our results indicate that SPECTROZYME PCa and CBS 65-25 are unsuitable for Protein C activity assay with PROTAC. Because change of absorbance was detected in Protein C deficient plasma and we couldn't gain good correlation between amidolytic activity of activated Protein C measured with these chromogenic substrates and antigenicity of Protein C by EIA. On the other hand, S-2366 is the most specific for activated Protein C and we also obtained good correlation against Protein C antigen.

Fiberoptic transurethral lithotripsy prefers smaller impacted ureteral calculi rather than large renal stones.

A total of 30 patients with urolithiasis underwent 35 sessions of fiberoptic transurethral lithotripsy (f-TUL) involving electrohydraulic lithotripsy. There were 11 patients with nephrolithiasis including 4 with staghorn calculi and 19 patients with ureterolithiasis which were all located above the pelvic brim. A fiberoptic nephroureteroscope (URF-P) was successfully introduced up to the stone in all but 2 cases (93.3%). Of the 28 patients with successful introduction, 21 received a single session of f-TUL for complete fragmentation of their stones, which measured less than 16 x 9 mm by radiography. Three patients with larger renal stones (greater than or equal to 17 x 9 mm) underwent 2 sessions of f-TUL for complete fragmentation. The remaining 4 patients who had staghorn calculi (greater than or equal to 38 x 33 mm) received 1 session of f-TUL with partial fragmentation; then 3 were sent to an ESWL clinic with good results, while the other patient received 2 more sessions of f-TUL and formed a stone street. Attempts were subsequently made to destroy the latter by ESWL but were unsuccessful. In conclusion, f-TUL appears to be most suitable for smaller impacted ureteral calculi above the pelvic brim.

[Tetralogy of Fallot revealed by autopsy in an elderly subject].

A 62-year-old women had a history of cyanosis and easy fatigability from school age but had refused medical examinations. She was admitted to our department with exacerbation of dyspnea at the age of 59. Laboratory tests showed hypoxia complicated with polycythemia with 35 mmHg of PaO2, 8.5 x 10(6) erythrocytes per mm3. The systolic pressure of the right ventricle was 155 mmHg. RI angio demonstrated a high degree of the right-to-left shunt at the ventricular level. The patient was given oxygen therapy at home without diagnosis of tetralogy of Fallot (TOF) and eventually died at the age of 62. Tetralogy of Fallot was diagnosed at autopsy. Pulmonary stenosis of this case was located in infundibulum. The lining of the infundibular inlet was constituted of the thick fibrous tissue which remarkably augmented the pulmonic stenosis. Although the autopsy findings were considered to be relatively mild TOF, the death was presumed to be caused by development of the infundibular stenosis due to formation of fibrous tissue. This is the oldest patient in Japanese medical literature to be identified by cardiac catheterization and autopsy finding as a case of TOF.

[Diagnosis of malignancy in the urinary tract and prostate gland from the aspect of hematuria].

During the 10 years from 1979 to 1988, 245 patients with urinary tract or prostate gland malignancy were diagnosed and treated at our Department. 245 malignancies were composed of 5 groups, i.e., 50 renal cell carcinomas (RCC), 17 renal pelvic and ureteral tumors (RPUT), 117 bladder tumors (BLT), 59 prostate carcinomas (PRC) and 2 urethral carcinomas (URC). On the basis of chief complaint or reason leading to diagnosis, and of grade of hematuria, evaluation was performed in 4 groups except for group URC. With regard to the chief complaint or reason leading to diagnosis, asymptomatic gross hematuria was most frequently seen at 40%, 59% and 74% in groups RCC, RPUT and BLT, respectively. In group PRC, asymptomatic gross hematuria was seen at 8%, although dysuria and urinary retention were the most popular. Regarding grade of hematuria at the first visit, urine without any RBC (55%) and microscopic hematuria (39%) were the commonest in group RCC, while moderate degree of microscopic hematuria (5-20/hpf) was most frequently seen in group RPUT (44%). On the contrary, gross hematuria (38%) was the commonest in group BLT. Patients in group PRC showed gross and microscopic hematuria at 25%, though it was not the commonest one. In conclusion, it should be noted that no RBC may be seen on the first visit even if the chief complaint was gross hematuria in patients with urinary tract malignancy.

Delay of astrocyte reaction in the injured cerebral cortex of hypothyroid mouse.

Reactive change in the number of astrocytes in the stabbed cerebral cortex (layers II-VI) of the hypothyroid mouse was studied quantitatively over a 4-day period after stabbing, by immunohistochemistry for glial fibrillary acidic protein (GFAP) and S-100 protein. The reactive increase in the number of GFAP-positive astrocytes was delayed in the hypothyroid mouse, compared to that in the euthyroid mouse. The number of S-100-positive astrocytes neither increased nor decreased significantly in the hypothyroid and the euthyroid mice. [3H]Thymidine autoradiography showed that the time course and intensity of the reactive proliferation of astrocytes were the same in the hypothyroid mouse as in the euthyroid mouse. These results indicate that hypothyroidism delays the reactive expression of GFAP-antigen and delays the transformation of GFAP-negative astrocytes into GFAP-positive reactive astrocytes.

Reactions of S-100-positive glia after injury of mouse cerebral cortex.

Reactions of glial cells after stab wounding of mouse cerebral cortex were studied by [3H]thymidine autoradiography combined with immunohistochemistry for S-100 protein. S-100-positive cells in the stabbed cortex had the light and electron microscopic characteristics of astrocytes, and they showed remarkable hypertrophic changes 4 to 5 days after stabbing. There were many cells labeled with [3H]thymidine in the stabbed cortex from 24 h to 8 days after stabbing, and the number of labeled cells was maximum at 48 h. A few of the labeled cells were S-100-positive, and the labeled S-100-positive cells were seen 24 h to 6 days after stabbing, mostly after 72-96 h. By successive injections of [3H]thymidine for 6 days after stabbing, about 90% of labeled cells were S-100-negative, and about 90% of S-100-positive cells were unlabeled with [3H]thymidine. The increase in number of S-100-positive cells by day 6 after stabbing was not statistically significant (P greater than 0.05). These results suggest that reactive proliferation of astrocytes is a minor phenomenon in gliosis of injured cerebral cortex, in contrast with their remarkable reactive hypertrophy.

Characterization of human interleukin-3 receptors on a multi-factor-dependent cell line.

Recombinant human interleukin-3 (hIL-3) was radioiodinated by Bolton-Hunter method with maintenance of biological activity. Using 125I-hIL-3, hIL-3 receptors were characterized on a multi-factor-dependent cell line TF-1. Equilibrium binding studies revealed the existence of a single class of binding sites (667 +/- 306 sites/cell) with a Kd of 173 +/- 25 pM. Affinity labeling of TF-1 cells with 125I-IL-3 yielded two bands of 150 kDa and 85 kDa, implying molecular weights of 135 kDa and 70 kDa for the hIL-3 receptors.

The interaction of alkynyl carboxylates with serine enzymes. A potent new class of serine enzyme inhibitors.

The recently reported alkynyl esters, propynyl benzoate and propynyl p-methoxybenzoate, were found to interact with a variety of serine enzymes. alpha-Chymotrypsin was inhibited very rapidly by an equivalent amount of the esters. Trypsin, elastase and pronase were also inhibited by the esters. On the other hand, liver esterase started to hydrolyze the alkynyl esters rapidly, but the enzyme became inhibited during the course of reaction. The inhibited enzymes exhibited slow reactivation which could be considerably enhanced by hydroxylamine.
