[Albumin therapy for patients with increased intracranial pressure: oncotic therapy].

Albumin plays an important role in maintaining an adequate plasma colloid osmotic pressure (COP). On the basis of the Starling's law, we postulated that elevation of plasma COP produced by infusion of serum albumin would result in the withdrawal of cerebral interstitial fluid. To test the hypothesis that an oncotic gradient would produce cerebral dehydration and result in reduction of intracranial pressure (ICP), we infused intravenously 25% solution of salt-poor human serum albumin, 2g/kg over 60 minutes for 10 patients with increased ICP. In these 10 patients, the degree of brain edema was not severe but mild to moderate by judging from CT scan findings. In addition to ICP, various parameters such as mean arterial blood pressure (MABP), central venous pressure (CVP), pulse rate, PaCO2 and urine volume were measured before, during and after albumin infusion. Hematological studies including COP, total protein, albumin-globulin ratio, protein fractions, hematocrit (Ht), blood urea nitrogen (BUN), creatinine and electrolytes were also performed before, during and after administration of albumin. The raised ICP (27.4 +/- S.E. 1.6 mmHg) decreased significantly down to 19.1 (+/- 1.9) mmHg and remained stable for at least 240 minutes without any changes of MABP, pulse rate and PaCO2. CVP elevated significantly and remained at higher levels in comparison with control despite increase of urine output after administration of albumin. This results suggested that an adequate circulatory volume was maintained. A significant increase of total protein, serum albumin concentrations and albumin-globulin ratio was noted.(ABSTRACT TRUNCATED AT 250 WORDS)

Purification, characterization and development of radioimmunoassay of human liver ribonuclease.

Human liver ribonuclease (RNase) was purified 36000-fold into an electrophoretically homogeneous state by column chromatography on phosphocellulose, gel filtration, poly(G) affinity chromatography, and heparin affinity chromatography. The molecular weight of the RNase estimated by SDS disc electrophoresis was 19,500. RNase was a heat- and pH-stable protein, and optimum activity was obtained at pH 7.0. The radioimmunoassay (RIA) for human liver RNase has been developed and the assay was shown to be sensitive (20 ng/ml), reproducible and specific. A good parallel relationship was observed between the standard curve and the dilution curves for serum and urine. No cross-activity was demonstrated between human liver and pancreatic RNase (less than 1%). In 44 normal subjects, the mean serum concentration of liver RNase determined by the RIA was found to be 99.4 ng/ml (SD +/- 66.3).

Genetic and immunologic analysis on moya-moya.

The genetic and immunologic abnormalities associated with the pathogenesis of moya-moya were assessed in 23, 13 children and 10 adults with angiographically diagnosed moya-moya. In HLA-A, -B, -C stereotyping, an association was found of AW24, BW46, and BW54 with relative risks of 3.83, 6.50, and 3.58 respectively. Natural T cell toxic autoantibody was detected by FACS analysis in sera from five out of 23 patients. Millipore filter assay for autoantibody against double-stranded DNA revealed higher than normal binding in sera from four out of 18 patients. Anti-vessel antibody which might be responsible for vascular change associated with moya-moya was not detected in any of the 23 patients studied. Significant association of the disease with certain HLA types, in addition to the presence of natural T cell toxic autoantibody and anti-double-stranded DNA antibody in patients' sera, supports the theory that genetic and immunologic disturbances may underly the pathogenesis of moya-moya.

Cerebrospinal fluid edema: a rare complication of shunt operations for hydrocephalus. Report of three cases.

Three cases are presented in which a rare complication occurred after a shunt operation for hydrocephalus. On postoperative computerized tomography (CT) scans, extensive low-density areas appeared in the white matter along the ventricular catheter. After shunt revision, gradual resolution or disappearance of the low-density area was clearly demonstrated on CT. In one patient, a collection of cerebrospinal fluid (CSF) was confirmed at operation and appeared to lie in the extracellular spaces of the white matter. The phenomenon is considered to be localized CSF edema, different from porencephaly.

Changes in serum immunoreactive pancreatic elastase 1 in acute pancreatitis.

Serum immunoreactive elastase 1 content was significantly elevated in acute pancreatitis. The mean content of the control group was 1.79 ng/ml (SD: 0.62 ng/ml), while that of patients with acute pancreatitis was 23.02 ng/ml (SD: 9.44 ng/ml). The increased content of pancreatic elastase 1 in acute pancreatitis continued for several days after onset, even though serum amylase activity had returned to normal.

Purification and immunological characterization of human pancreatic ribonuclease.

Three alkaline RNases were purified from human pancreatic juice to near homogeneity as judged by sodium dodecyl sulfate and native polyacrylamide gel electrophoreses. The molecular weights of these RNases determined by sodium dodecyl sulfate electrophoresis were 27,000, 19,000, and 13,000. The activities of these three RNases were increased by addition of Mg2+, Ca2+, Co2+, K+, Na+, spermine, and spermidine and decreased by the addition of heparin, Cu2+, and Zn2+. These RNases showed higher hydrolytic activity toward polycytidylic acid than toward polyguanylic acid, polyuridylic acid, or polyadenylic acid. The three human pancreatic RNases were immunologically identical but differed from human liver RNase, as shown by Ouchterlony double-diffusion test. Radioimmunoassay of human pancreatic RNase showed that immunologically similar RNases are present in human saliva, and serum.

[Combination chemotherapy of primary gastric lymphoma with vincristine, endoxan (cyclophosphamide), prednisolone and adriamycine (VEPA)].

A new combination chemotherapy(VEPA) was applied to 9 patients with advanced and/or non-resectable primary gastric lymphoma. Among 6 patients with measurable disease, there were 3 (50%) complete responses and 1 (17%) incomplete response. None of 3 patients with lymph node and/or anastomosis involvements at the gastrectomy have relapsed after 13 months to 32 months. Complications during chemotherapy with VEPA were mild myelosuppression and gastrointestinal disturbances, and severe alopecia. Only one paralytic ileus with vincristine occurred, but no cardiotoxicities with adriamycin were observed. This study indicates that VEPA therapy is effective in the patients with primary gastric lymphoma.

A new strain (KW-1) of human chronic myeloid leukemia transplantable in lasat mice.

Heterotransplantation of cells from a patient with chronic myeloid leukemia into athymic nude mice and athymic asplenic lasat mice was performed. A serially transplantable tumor cell line (KW-1) was obtained in lasat mice, although the "take" rate was low. The tumor was composed of myeloid cells showing various degrees of differentiation and maturation. The cellular characteristics, the availability of lasat mice and the effect of whole body irradiation or pretreatment with anti-asialo GM1 on heterotransplantation of myeloid cells are described and discussed.
