Liver metastasis nineteen years after surgery for typical bronchial carcinoid.

A resected case of metastatic liver carcinoid is presented. A 62 year-old woman, who had undergone removal of a typical bronchial carcinoid 19 years before, was found to have a well-defined, oval hepatic tumor on ultrasonography. The resected specimen was a hard and solid tumor, which was microscopically diagnosed as a carcinoid. Histological review of the previously resected lung tumor revealed that the liver tumor was a metastasis from the primary bronchial carcinoid. The patient is alive without recurrence 42 months after hepatectomy. This case suggests that typical bronchial carcinoid, a slowly growing tumor, may metastasize to distant sites after many years, and that re-excision of metastatic lesions may prolong survival time.

Undifferentiated spindle cell carcinoma of the extrahepatic bile ducts: a case report.

Spindle cell carcinoma is a rare tumor that generally occurs in the upper digestive tract. We report an 81-year-old man with spindle cell carcinoma located in the extrahepatic bile ducts, resulting in obstructive jaundice. The patient died 10 months after operation due to local recurrence. The literature on this rare disease is reviewed and discussed.

Neurological manifestations in Japanese Ama divers.

Repetitive breath-hold (BH) diving can lead to accumulation of nitrogen (N2) in blood and tissues, which may give rise to decompression illness (DCI). An unusual condition is "Taravana", the diving syndrome reported by Cross in the 1960s. That report generated wide discussion as to whether BH diving can cause DCI. Paulev was the first person to suggest the link between DCI and BH diving. He, a submarine medical officer developed symptoms of DCI after a series of BH dives, having proceeded the dives by spending time in a hyperbaric chamber at 20 meters for 8 minutes. Recently four professional Japanese BH divers (Ama) with histories of diving accidents were reported. Magnetic resonance imaging of these divers detected cerebral infarcts localized in the watershed areas of the brain. A survey conducted on their island revealed that many Ama divers had experienced stroke-like events. A clinical feature of DCI in BH diving is that the damage is limited to the brain. Although the mechanisms of brain damage in BH diving are unclear, N2 bubbles passing through the lungs or the heart so as to become arterialized are most likely to be the etiological factor.

Development of alumina ceramic transcutaneous connector to prevent skin exit site infections around CAPD catheters.

It is very important to prevent skin exit site infections in order to continue continuous ambulatory peritoneal dialysis (CAPD) safely over the long-term. The silicon material conventionally used can result in sinus formation between the skin and the catheter, and may trigger exit site infections. To resolve this problem, we employed a bioinert, rigid, and nonporous transcutaneous connector made of alumina ceramic. Earlier long-term animal experiments with alumina ceramic placed in soft tissue demonstrated the outstanding biocompatibility of this material. The transcutaneous connector was designed with a simple cylindrical configuration; in order to make sufficient contact between this connector and the patient, a silicon tube was made into a swan neck catheter with a disk-shaped Dacron cuff at the upper position in order to reinforce adhesion to soft tissue. Ten CAPD patients (maximum time on dialysis of 14 months) have used the new CAPD catheter system with no exit site infections to date. Furthermore, there has been virtually no downgrowth of the skin around the connector, while the connector and tissue have remained in very close contact.

Hypocalcemic effect of 24R, 25-dihydroxyvitamin D3 in rats.

The physiological significance of 24R,25-dihydroxyvitamin D3 [24R,25 (OH)2D3] is still controversial. The effects of 24R,25-(OH)2D3 on serum calcium level were examined in rats. While 24R,25-(OH)2D3 had no effect on serum calcium level in normal rats, 200 micrograms/kg of 24R,25-(OH)2D3 showed significant hypocalcemic effect in rats pretreated with vitamin D3 (changes of serum calcium level compared with control group were 1.3 and 1.5 mg/dl at 24 and 36 hours respectively) and also showed a significant antagonistic effect against the action of 1 alpha,25-dihydroxyvitamin D3 (1 alpha,25(OH)2D3) in the thyroparathyroidectomized rats (changes of serum calcium compared with control group were 0.5 and 0.8 mg/dl at 12 and 24 hours respectively). It was concluded that the effect of 24R,25-(OH)2D3 on serum calcium differs from that of 1 alpha,25-(OH)2D3.

Cross-sectional assessment of renal function in the inhabitants of a cadmium-polluted area.

To understand the development of cadmium (Cd)-induced renal tubular dysfunction, cross-sectional assessment of renal function in the inhabitants of the Cd-polluted Jinzu River basin in Toyama, Japan, was carried out. Sixty-seven men and 78 women, aged 56 to 71 years whose urinary beta 2-microglobulin (beta 2-m) exceeded 1,000 micrograms/g creatinine, were examined and divided into five groups according to their values of fractional excretion of beta 2-m (FE beta 2-m). Comparisons were made between six groups including the inhabitants of a non-polluted area as a reference group. Significantly increased values of fractional excretion of urate (FEUA), calcium (FECa), sodium (FENa), and chloride (FECl), serum creatinine and beta 2-m, and urinary protein were detected even in the subjects with FE beta 2-m level below 1 percent compared with the reference group. Fractional phosphate reabsorption (percent TRP) was significantly decreased in the subjects with FE beta 2-m level above 10 percent. Creatinine clearance (CCr) decreased with increase in FE beta 2-m, and the subjects with FE beta 2-m above 30 percent showed significantly decreased CCr values compared to those with FE beta 2-m below 30 percent. In summary, FEUA, FECa, FENa, and FECl, as well as low-molecular-weight proteins such as beta 2-m, are increased in the early stage of Cd-induced renal tubular dysfunction. Although percent TRP was maintained by the decreased CCr, there was a marked decrease in percent TRP in subjects with progressive stage showing FE beta 2-m level above 10 percent. No sex difference in renal tubular dysfunction was found in this cross-sectional study.
