The metabolism of digitoxin in hepatic cirrhosis of human subjects.

Digitoxin, 1 mg, was orally administered to 12 cirrhotic patients (the 6 in a clinically compensated and the 6 in a decompensated state) and their serum concentrations were measured at 6 hours and everyday through 7 days by radioimmunoassay. The serum half-life of digitoxin in these groups and normal subjects, determined from the serum concentrations, was as follows: 4.7 +/- 0.55 days for controls, 4.9 +/- 0.45 days for the compensated group and 5.3 +/- 0.35 days for the decompensated group. No statistically significant difference could be found in half-life among these groups. (P less than 0.2). The same dose of digitoxin was orally administered to 6 cirrhotic patients and 6 control subjects and their left ventricular systolic time intervals, LVET and QS2, were determined at 6 hours and every morning for 7 days. Cardiac responses, exhibited by decrease in the systolic time intervals, in both control subjects and cirrhotic patients dissipated in fair parallel during the ensuing 4 days and returned to base line level by 5 days. From these both biological half-life and physiological effect, it may be concluded that overall metabolism of digitoxin in cirrhotic patients is not disturbed.

Acrodermatitis enteropathica-like eruption association with parenteral nutrition.

This is a case report of a patient with acrodermatitis enteropathica-like eruption that occurred during parenteral nutrition. The skin lesions were similar to those of acrodermatitis enteropathica, but there were no other major symptoms such as diarrhea and alopecia. It was noticed that the lack of linoleic acid in the diet might be the cause of lesions.

Correlation between myocardial and plasma concentration of digitoxin and its metabolites with special reference to digitoxin radio-immunoassay.

According to our prevous study, the metabolites of digitoxin show nearly dentical affinity with digitoxin antibody. The present study reveals relatively constant ratio of heart to plasma in total 3H-counts, and the concentration of metabolites are ver similar between the two throughout the time investigated which indicates digitoxin concentration of plasma as determined by radioimmunoassay reflects well that of myocardium, although it may contain metabolites.

Ventricular tachycardia induced by supraventricular extrasystoles.

Electrocardiography was performed in a newborn boy without organic heart disease, in whom supraventricular extrasystoles with varying coupling were seen. The supraventricular extrasystoles were occasionally followed by ventricular extrasystoles. The coupling intervals of ventricular extrasystoles to the preceding supraventricular extrasystoles were also considerably variable. Ventricular tachycardia occurred following comparatively late coupled ventricular extrasystoles. This is the first known report on ventricular tachycardia following comparatively late coupled ventricular extrasystoles in a newborn infant. This strengthens our previous suggestion that such ventricular tachycardia can be caused by longitudinal dissociation in the reentrant pathway of extrasystoles.

Delayed enhancement of hepatocellular carcinoma on dynamic CT: sign of extrahepatic collaterals after transcatheter arterial chemoembolization or transcatheter arterial chemoinfusion.

BACKGROUND: We examined the factors of delayed enhancement of hepatocellular carcinoma (HCC) on dynamic computed tomography (CT). METHODS: Dynamic CT and angiography were compared in 113 patients who had undergone transcatheter arterial chemoembolization or transcatheter arterial chemoinfusion and were suspected of developing new HCCs. RESULTS: Eight of 113 patients had HCC nodules that enhanced gradually from the arterial phase to the portal venous phase on dynamic CT and were fed by extrahepatic arteries on angiography. The feeding artery was an omental branch of the splenic artery in one lesion, an omental branch of the gastroduodenal artery in five, and the intercostal artery in two. CONCLUSION: We believe that the recurrence of HCCs delayed enhancement on dynamic CT because the hepatic artery was hidden and long narrow extrahepatic collaterals fed the tumor.

MR imaging of peripheral cholangiocarcinoma.

A prospective study was performed to compare MR spin-echo (SE) sequences [repetition time/echo time (TR/TE) 2,000/80, 500/44 ms], unenhanced CT, and rapid intravenous contrast enhanced CT in eight consecutive patients with peripheral cholangiocarcinoma. All the tumors (ranging from 5 to 9.6 cm in size) were detected with all four techniques. Tumor contrast, however, was qualitatively greatest on long TR/TE SE images. With long TR/TE SE images, tumors were demonstrated as well-demarcated homogeneous regions of high signal intensity, and the anatomic relations between tumors and intrahepatic blood vessels were easily perceived. Detection of small intrahepatic metastatic foci was best on long TR/TE images. Tumor invasion of the portal vein's branches was also best seen on long TR/TE SE images. These results indicate that long TR/TE SE sequence is the most effective initial screening method in demonstrating the presence and determining resectability of peripheral cholangiocarcinoma.

Isolation of dermatophytes from house dust on a medium containing gentamicin and flucytosine.

Dermatophytes are usually isolated from house dust by using actidione-chloramphenicol Sabouraud glucose medium (ACS medium). We have prepared a new medium, 5FC medium, by adding gentamicin sulphate (GS) and 5-fluorocytosine (5FC) to ACS medium to achieve more efficient isolation of dermatophytes from house dust. 5FC medium was more effective in isolating dermatophytes than ACS medium or ACS medium supplemented with GS alone. Trichophyton rubrum could be grown from 13 out of 19 house dust samples from the homes of patients with tinea pedis (68.4%), and T. mentagrophytes could be grown from 17 out of 21 (81.0%). Two of 20 house dust samples from the home of a control family without dermatophytosis grew only one colony of dermatophytes in 5FC medium; the rest of the samples showed no growth. The number of colonies isolated on 5FC medium was much higher than on ACS medium (5.3 vs. 2.0 for T. rubrum and 17.2 vs. 2.1 for T. mentagrophytes). In addition, the size of the isolated colonies was much larger than that on ACS medium. Thus, 5FC medium can be regarded as a useful tool for isolating dermatophytes from various contaminated samples.

Demonstration of dermatophyte dissemination from the infected soles using the foot-press method.

We have designed a new direct isolation method, the foot-press method, to survey dissemination of dermatophytes from the infected soles. A total of 56 untreated patients with tinea pedis were examined. The infected soles of 42 patients were pressed onto actidione-chloramphenicol-5-fluorocytosine (5FC)-gentamicin sulphate Sabouraud glucose medium prepared in a large culture dish; the culture media were then incubated at 25 degrees C. Dermatophytes were isolated in 30 out of the 42 patients, while no dermatophytes could be grown from 10 healthy controls. The number of isolated colonies from each patient ranged from 1 to 97 (mean +/- SD, 11 +/- 20). The isolation frequencies were higher in the patients of hyperkeratotic type and in those with tinea unguium, while causative organisms and the extent of the lesions did not affect the results of the footpress method significantly. In order to reveal the morphology of disseminated dermatophytes, 1 x 1 cm pieces of culture media were cut out from culture dishes after pressing infected soles and were examined microscopically. Dermatophyte-like spores or hyphae, most of which were detached from cornified cells, could be seen in 10 out of 14 patients. Subsequently performed slide cultures isolated dermatophytes from approximately 70% of the pieces on which dermatophyte-like fungi were observed.

Dual colour fluorescein analysis of peripheral blood T cells in auto-immune chronic active hepatitis.

Both CD4 and CD8 T cells are subdivided into two phenotypically distinct sublineages via another two T cell markers, Leu-8 and CD11b antigens. The proportions of these four T cell subsets, CD4+Leu8+, CD4+Leu8-, CD8+11b+ and CD8+11b-, were studied in patients with auto-immune chronic active hepatitis (CAH) and compared with disease controls (hepatitis B surface antigen positive chronic active hepatitis) and healthy controls. We found that the proportion of CD4+Leu8+ cells was significantly reduced compared with controls (P less than 0.01), whereas those of the other cells were almost identical in all 3 groups. The absolute number of these CD4+Leu8+ cells was also lower than that of controls (P less than 0.01). Thus, the present study suggests that a reduced number of CD4+Leu8+ cells is associated with the aberrant immune response in auto-immune CAH.

Bacterial colonization of the upper respiratory tract of patients with primary lung cancer and nonmalignant lung disease.

We examined the bacterial colonization of the upper respiratory tract of 110 patients with primary lung cancer (PLC), 75 patients with nonmalignant lung diseases (NMLD) and 45 healthy volunteers (HV), comparing the sensitivity of expectorated sputum, and throat and nasal swabs. The frequency of bacterial colonization of the upper respiratory tract was significantly higher in the PLC patients (59.1%) than in NMLD patients (37.3%, p < 0.01) and HV (37.8%, p < 0.01). The frequency of gram-negative colonization was significantly higher in PLC patients than in the other subjects (p < 0.01). Expectorated sputum and nasal swab were the most sensitive for detection of whole bacteria and methicillin-resistant Staphylococcus aureus in the patients with PLC. Our results showed that PLC patients are significantly more frequently colonized by bacteria in their upper respiratory tracts and that a combination culture of expectorated sputum and nasal swab is suitable to estimate the bacterial colonization of the upper respiratory tract in the patients.

Effect of 24R,25-dihydroxyvitamin D3 in experimental renal insufficiency.

This placebo-controlled study in JCL-Wistar rats showed that 100 micrograms/kg 24R,25-dihydroxyvitamin D3, when given for 12 days consecutively, restored immune function which had deteriorated following aminonucleoside-induced nephrosis. Urinary creatinine excretion in five-sixths nephrectomized rats was also shown to be increased by 10 micrograms/kg 24R,25-dihydroxyvitamin D3. Hypophosphataemic effects in both these cases of experimentally-induced renal insufficiency was also shown following administration of 24R,25-dihydroxyvitamin D3.

Effect of 24R,25-dihydroxyvitamin D3 in experimental diabetic rats.

The results are presented of a controlled study in male Wistar rats into the effects of 24R,25-dihydroxyvitamin D3 on blood glucose levels, bone calcium content and ADP-induced platelet aggregation in streptozocin-induced diabetes mellitus. Blood glucose levels were shown to be decreased by 10 micrograms/kg 24R,25-dihydroxyvitamin D3. The reduced bone calcium content associated with diabetes mellitus was returned to normal levels with both 1 and 10 micrograms/kg 24R,25-dihydroxyvitamin D3. It was also shown to exhibit dose-dependent anti-platelet activity. The data suggest that 24R,25-dihydroxyvitamin D3 might have potential as a mild therapeutic agent in the treatment of osteoporosis and platelet hyperactivity associated with diabetes mellitus.

Effects of vitamin D3 metabolites in ovariectomized rats.

The effects of two vitamin D3 metabolites, 24R,25-dihydroxyvitamin D3 and 1 alpha,25-dihydroxyvitamin D3, were investigated in ovariectomized rats. The amount of ash in the femur on a defatted dry weight basis was significantly greater in rats treated with 1 microgram/kg 24R,25-dihydroxyvitamin D3, or 0.01 or 0.1 microgram/kg 1 alpha,25-dihydroxyvitamin D3 than in the controls. The concentration of bone gla protein in serum and amounts in the femur were significantly greater in rats treated with 1 or 10 micrograms/kg 24R,25-dihydroxyvitamin D3, but not those given 1 alpha,25-dihydroxyvitamin D3 compared with the controls. These results suggest that 24R,25-dihydroxyvitamin D3 increased bone mass probably through the stimulation of bone formation.

Pharmacologically stimulated portal flow measurement by magnetic resonance imaging for assessment of liver function.

PURPOSE: To evaluate pharmacologically stimulated portal flow measured by magnetic resonance (MR) imaging for assessment of liver function. MATERIALS AND METHODS: Pharmacologically stimulated portal flow was measured by phase contrast MR imaging in 27 patients when they were undergoing abdominal angiography for liver tumors or gall bladder cancer. The patients included 11 cases of liver cirrhosis and eight of chronic hepatitis. Pharmacological stimulation was done by infusion of 10 microg/Kg of nicardipine hydrochloride into the superior mesenteric artery through an angiographic catheter. We examined the correlation between stimulated or non-stimulated portal flow and biochemical liver function tests. RESULTS: Correlation coefficients and their corresponding p values between non-stimulated portal flow and the indocyanine green residual rate at 15 min after injection (ICG R15), serum albumin (ALB), total bilirubin (TB), cholinesterase (CHE), and hepaplastin test (HP) were--0.414 (0.056), 0.296 (0.134), -0.570 (0.002), 0.289 (0.153), and 0.321 (0.126), respectively, whereas those between stimulated portal flow and ICG R15, ALB, TB, CHE, and HP were--0.561 (0.007), 0.411 (0.033), -0.509 (0.007), 0.445 (0.023), and 0.494 (0.014), respectively. CONCLUSION: Stimulated portal flow showed better correlations with biochemical liver function tests than non-stimulated portal flow. It is suggested that stimulated portal flow measurement is more useful for the evaluation of liver function than non-stimulated portal flow measurement.
