Effect of 19-norandrostenololylaurate on serum testosterone concentration, libido, and closure of distal radial growth plate in colts.

The long-term effect of anabolic steroid was investigated in 3 experiments. In experiment I, 500 mg of 19-norandrostenololylaurate was given to 5 colts and a dose of 100 mg to another 5 colts every 3rd week. Six colts served as untreated controls. The animals were 12-16 months old at the start, and 24 months at the end of treatment. In experiment II, a dose of 1 mg/kg was given every 3rd week to 4 colts and 0.3 mg/kg every week to another 4 colts. Six colts served as controls. The colts were treated from 7 months to 12 months of age. In experiment III, 1 mg/kg of steroid was given every 3rd week to 3 foals between 3 and 8 months of age. Three foals were used as controls. Libido and hCG-induced serum testosterone concentrations were studied after the cessation of treatments, up to 3 or 4 years of age. Closure of the right distal radial growth plate was determined between 21 and 36 months of age. Treated colts had lower testosterone levels 4.5 months after cessation of treatment in experiment I (p < 0.001) and experiment II (p < 0.05) when compared with the corresponding control groups. Two years after treatments in experiment I, hCG-induced testosterone levels were higher in treated colts than in untreated controls, but the difference was not statistically significant. The treatments had no effect on libido. The anabolic steroid treatment did not cause premature closure of epiphyseal growth plates in any of the experiments, but closure appeared to be delayed. It was concluded that anabolic steroids have long term effects on reproduction. Their influence on serum testosterone can last for years after cessation of treatment, and they can delay the closure of growth plates which can cause increased susceptibility to cartilage injury during exercise.

Intrauterine fluid accumulation in oestrous mares.

Intrauterine fluid (IUF) was collected using a tampon from mid-oestrous mares (n = 57) with and without ultrasonically detectable accumulations of free intraluminal fluid. Bacteria were cultured and neutrophils counted from all samples (n = 57). Total protein concentration, trypsin-inhibitor capacity (TIC), and plasmin, beta-glucuronidase (B-Gase) and N-acetyl-beta-D-glucosaminidase (NAGase) activities were determined in 27 IUF samples. The motility of spermatozoa in the presence of IUF, IUF extended with Kenney's medium (1:1) and Kenney's medium alone was analysed in 9 samples using a Hamilton-Thorn motility analyser. Thirty-five mares were inseminated immediately after collection of IUF, and every second day until ovulation. Embryos were recovered nonsurgically 6 days after ovulation. After embryo transfer, fluid accumulations were recorded during oestrus and an endometrial biopsy specimen taken (n = 53). In the beginning of oestrus, fluid accumulations were detected in 39% (22/57) of mares, while on the day when IUF was collected, fluid accumulations were observed in 26% (15/57) of mares. The fluid was anechogenic, and in 80% of the mares located in the uterine body. None of the mares exhibited cytological or bacteriological evidence of acute endometritis. Total protein concentrations, TIC and B-Gase activities in IUF were statistically significantly lower in mares with fluid accumulations (n = 14) than in mares without fluid accumulations (n = 13) (p < 0.01). The addition of undiluted IUF to extended semen significantly reduced total and progressive motilities, path velocities and percentages of rapid spermatozoa (p < 0.05) in vitro. On endometrial biopsy, fibrosis was found to be more prominent (p = 0.025) in mares with fluid accumulations (n = 9) than in mares without (n = 44). It was concluded that anechogenic fluid accumulations during oestrus were associated with compositional changes in IUF. Although IUF had negative effects on spermatozoal motility in vitro, the presence of fluid accumulations at the time of insemination did not affect embryo recovery rates.

Noninvasive risk modeling after myocardial infarction.

The aim of this study was to extract and combine non-invasive risk parameters from the signal-averaged electrocardiogram (SAECG) and heart rate variability (HRV) based on 24-hour ambulatory electrocardiography to optimize the prognostic value for arrhythmic events after acute myocardial infarction. A prospective series of 553 men < 66 years of age enrolled in the Post-Infarction Late Potential study were analyzed. Within 2 to 4 weeks after acute myocardial infarction, all patients underwent SAECG and 24-hour ambulatory electrocardiography before hospital discharge. During 6 months of followup, 25 patients (4.5%) experienced arrhythmic events (sustained ventricular tachycardia, n = 11; ventricular fibrillation, n = 7; sudden cardiac death, n = 7). The predictive power of SAECG and HRV parameters was assessed using a Cox proportional-hazards model. In HRV analysis, the most significant differences between patients with and without arrhythmic events were observed for the beat-to-beat parameter root-meansquare of successive RR differences [RMSSD]): 25.7 +/- 16.9 ms in patients with arrhythmic events versus 34.1 +/- 18.6 ms in patients free of arrhythmic events (p = 0.004). Time domain analysis of the SAECG showed the QRS duration to be most significantly different in both patient groups: 106.4 +/- 18.7 ms (arrhythmic events) versus 95.3 +/- 18.7 ms (no arrhythmic events) (p = 0.001). Based on the Cox regression model, RMSSD and QRS duration were demonstrated to be independent significant risk factors (regression coefficient for QRS duration: cq = 0.014 +/- 0.006 ms(-1), p = 0.014; for RMSSD: cr = -0.041 +/- 0.016 ms(-1), p = 0.009). Based on the regression coefficients, an analytic risk model was developed describing the arrhythmic risk as a function of QRS duration, RMSSD, and time after infarction. We conclude that the combination of beat-to-beat changes of heart rate measured by RMSSD and QRS duration from the SAECG enhances noninvasive risk stratification after myocardial infarction.

Registration and display of brain SPECT and MRI using external markers.

Accurate anatomical localisation of abnormalities observed in brain perfusion single-photon emission computed tomography (SPECT) is difficult, but can be improved by correlating data from SPECT and other tomographic imaging modalities. For this purpose we have developed software to register, analyse and display 99mTc-hexamethylpropyleneamine oxime SPECT and 1.0 T MRI of the brain. For registration of SPECT and MRI data external skin markers containing 99mTc (220 kBq) in 50 microliters of coconut butter were used. The software is coded in the C programming language, and the X Window system and the OSF/Motif standards are used for graphics and definition of the user interface. The registration algorithm follows a noniterative least-squares method using singular value decomposition of a 3 x 3 covariance matrix. After registration, the image slices of both data sets are shown at identical tomographic levels. The registration error in phantom studies was on average 4 mm. In the two-dimensional display mode the orthogonal cross-sections of the data sets are displayed side by side. In the three-dimensional mode MRI data are displayed as a surface-shaded 3 D reconstruction and SPECT data as cut planes. The usefulness of this method is demonstrated in patients with cerebral infarcts, brain tumour, herpes simplex encephalitis and epilepsy.

Predictive value of wavelet correlation functions of signal-averaged electrocardiogram in patients after anterior versus inferior myocardial infarction.

OBJECTIVES: This study sought to evaluate the prognostic value of wavelet correlation functions of the signal-averaged electrocardiogram (ECG) for arrhythmic events in patients after myocardial infarction. BACKGROUND: Wavelet transform of the signal-averaged ECG has been shown to be a nonstationary analysis technique describing the time evolution of frequency spectra throughout the QRS complex. To quantify the wavelet transform, we introduced the new concept of the wavelet correlation function. METHODS: The relation among wavelet correlation functions, ventricular late potentials and the site of infarction was investigated in 769 men < 66 years old who survived the acute phase of myocardial infarction (351 [46%] anterior, 418 [54%] inferior infarctions). Signal-averaged ECG recordings were obtained 2 to 3 weeks after infarction. During 6 months of follow-up, 33 patients (4.3%) experienced a malignant arrhythmic event. Wavelet correlation functions of the signal-averaged ECG were evaluated in a time-frequency plane ranging from 25 ms before QRS onset to 25 ms after QRS offset in the frequency range between 40 and 100 Hz. RESULTS: Patients with an anterior infarction had lower mean wavelet correlation coefficients (p < 0.001) and a lower incidence of ventricular late potentials than patients with an inferior infarction (32.3% vs. 42.7%, p = 0.003). The combination of wavelet correlation functions and late potentials increased the total predictive accuracy from 52% to 72% for inferior and from 64% to 76% for anterior infarctions. CONCLUSIONS: Spectral changes in the signal-averaged QRS complex are more prominent in anterior than inferior infarctions. Combination of late potential analysis and wavelet correlation functions increases the prognostic value for serious arrhythmic events after myocardial infarction.

Classification of lung sounds in patients with asthma, emphysema, fibrosing alveolitis and healthy lungs by using self-organizing maps.

The performance of the self-organizing map (SOM), an artificial neural network, was evaluated in the classification of lung sounds. Patients with asthma (n = 8), emphysema (n = 8) and fibrosing alveolitis (n = 8), and patients with healthy lungs (n = 8) were selected for the study. Fast Fourier transform (FFT) spectra from midinspiratory breath sounds recorded at the right lower lobe area were used to construct feature vectors in the learning and classification process of SOM. The sound segments did not contain wheezing sounds. The lung sounds of 25/32 (78%) patients were classified correctly, with an overall kappa (kappa) value of 0.71. The agreement between the clinical and proposed diagnoses based on classification of lung sounds was good among patients with emphysema (kappa = 0.92) and those with healthy lungs (kappa = 0.83), but only moderate among patients with asthma (kappa = 0.52) and fibrosing alveolitis (kappa = 0.54). This is due to the limitations in distinguishing breath sounds of asthmatics without wheezing sounds from those with crackles in fibrosing alveolitis by the spectral pattern alone. The results indicate that SOM based on FFT spectra is potentially useful in the classification of lung sounds, e.g. in health screening or in differential diagnosis of pulmonary disorders. To enhance the performance of SOM, other features of lung sounds should be combined with FFT spectra.

Averaged and time-gated spectral analysis of respiratory sounds. Repeatability of spectral parameters in healthy men and in patients with fibrosing alveolitis.

STUDY OBJECTIVE: To obtain a basis for assessment of changes in breath sound spectra in patients with pulmonary diseases, short-term and day-to-day repeatability of spectral parameters was studied. DESIGN: Breath sounds were recorded simultaneously from the trachea and from the chest twice at an interval of 15 min (short-term repeatability) and of 1 to 3 days (day-to-day repeatability). During recordings, air flow at the mouth was controlled, the target inspiratory and expiratory peak flow being 1.25 L/s. Inspiratory and expiratory breath sound spectra were averaged over 7 to 10 successive respiratory cycles. The repeatability of sound intensity (RMS), frequency of maximum intensity (Fmax), and median frequency (F50) was analyzed with analysis of variance. PARTICIPANTS: Short-term repeatability was studied in 10 healthy nonsmoking men (age 25 to 44 years), and day-to-day repeatability was studied in 10 healthy nonsmoking men (age 23 to 41 years) and in 12 patients with clinically stable fibrosing alveolitis (age 35 to 82 years). RESULTS: Short-term coefficient of variation (CoV) of Fmax and F50 was 2.6 to 6.7% when recorded from the chest, and 6.2 to 8.7% when recorded from the trachea. Day-to-day CoV of Fmax and F50 in healthy subjects was 4.7 to 8.5% and 5.0 to 8.7% recorded from the chest or from the trachea, respectively. Inspiratory day-to-day variation in those parameters was higher in patients with fibrosing alveolitis. CoV of RMS was high, ranging from 18 to 47% in different subject groups and sampling situations. CONCLUSIONS: Repeatability of F50 of averaged flow-controlled lung sound spectra is good both in healthy subjects and in patients with fibrosing alveolitis. Thus, F50 of respiratory sound spectra may be useful in monitoring of changes induced by respiratory diseases and interventions. These results emphasize the importance of standardization of recording conditions and of analyzing techniques.

Recovery rate and quality of embryos from mares inseminated at the first post-partum oestrus.

The pregnancy rate is lower in mares inseminated at the first post-partum (p.p.) oestrus (40-50%) compared with pregnancy rates in subsequent oestrous cycles (55-65%). The causes of the lowered pregnancy rate are not fully understood. The aim of the present study was to examine if embryonic defects could be one of the reasons for lowered pregnancy rate. A total of 23 p.p. and 14 non-lactating control mares were flushed 7 days after detection of ovulation. Embryo recovery rate was 48% and 71% in p.p. and control mares, respectively (p = 0.16). Embryos were photographed, measured, graded and stained with fluorescein diacetate to assess their viability. Thereafter embryos were bisected and stained with Hoechst 33342 to count the cell nuclei. Embryos in both groups were equally viable and the cell numbers were not significantly different. According to morphological evaluation all embryos were classified as excellent or good. Embryos aged 7.3 to 7.6 days (+/-0.25 days) were smaller in the p.p. group than in the control group (p < 0.05). Forty-seven (9/19) and 8% (1/13) of the uterine swabs, taken before the first insemination, yielded bacteria and neutrophils in p.p. and control mares, respectively. The amount of neutrophils and/or bacteria had no statistically significant effect on embryo recovery rate (p > 0.10). Recovery of embryos was not related to histological findings in uterine biopsies taken after embryo recovery. Embryo recovery rate in p.p. mares (48%) was similar to previously reported foal heat pregnancy rates (40-50%). Hence, early embryonic death in utero would not be the most likely reason for lowered pregnancy rate in mares inseminated at the first p.p. oestrus. Sperm transport and oviductal conditions by the time of the first p.p. oestrus would need to be studied to clarify the role of fertilisation failure as the cause of lower pregnancy rate in mares inseminated at foal heat.

Effect of timing of frozen semen insemination on pregnancy rate in mares.

Thirty-four mares were inseminated with frozen semen from one stallion during 2 oestrous cycles, every 48 h until ovulation took place and within 12 h after ovulation. Semen was frozen using the Colorado method. The insemination dose was from 200 to 400 x 10(6) progressively motile spermatozoa. Ovaries were examined every 12 h to determine time of ovulation. Examination for pregnancy was carried out using ultrasonography, 15 days after ovulation. Thirty-five per cent of mares inseminated < 24 h and 23% of mares inseminated between 24-48 h before ovulation were pregnant (p = 0.388). The pregnancy rate in all mares inseminated before ovulation was 30%. In the mares inseminated within 12 h of ovulation, it was 18% (p = 0.253). Younger mares (aged 4-10 yr) had a higher pregnancy rate (59%) than older mares (aged 11-15 yr) (23%), but the difference was not statistically significant (p = 0.057).

Serum progesterone levels in mares in winter and during transitional periods.

Weekly blood samples were collected for serum progesterone determination from 68 mares during the period from September 1990 to June 1991. A total of 78% (53/68) of the mares became anoestrous: 88% (15/17) of mares with foals and 75% (38/51) of dry mares (p = 0.323). Six mares of the 15 which continued cycling showed persistent corpus luteum for 77 days on average (range 42-106 days). Anoestrus began between September and February, most frequently in December, and ended in January to June, most frequently in March. Mares with foal at foot became anoestrous in 2 separate groups, in September and in December. Anoestrus ended most frequently in February in foaled mares. The mares with foals differed significantly from the other mares both at onset (p = 0.0084) and end (p = 0.0218) of anoestrus. However, the effect of the stable was also significant at onset (p = 0.0386) and end (p = 0.0170) of anoestrus. Since all mares with foals were kept in one stable and dry mares in 4 different stables, the effect of stable and possible differing lighting conditions cannot be separated from the foal effect. The length of anoestrus was on average 101 days for lactating mares and 95 days for dry mares (p = 0.6691). In foaled mares its length was on average 159 days in mares which had entered anoestrus as early as in September-October and 62 days in mares entering anoestrus in December (p = 0.001). The effects of breed and age were not significant in any of the parameters.

Sperm-induced leukocytosis in the equine uterus.

The objective of this study was to investigate the inflammatory reaction induced in the equine uterus by insemination with fresh and frozen semen. Eleven groups (6 to 8 mares per group) were studied during 2 breeding seasons. The mares were inseminated using raw semen, frozen semen, extended fresh and frozen semen, concentrated fresh semen, seminal plasma and seminal extenders only. One group was bred naturally. Six hours after insemination, the uteri were flushed with 50 ml of phosphate-buffered saline (PBS). Seventeen out of 104 samples (16%) exhibited slight bacterial growth. Neutrophil concentrations were significantly (P<0.05) higher in all treated mares than in the controls. Mares infused with PBS, seminal extenders or the supernatant from centrifuged frozen-thawed semen exhibited only a mild neutrophil response. Insemination with frozen semen resulted in higher neutrophil concentrations than insemination with extended fresh semen (means of 59 vs 5 million neutrophils/ml; P<0.05). Highest neutrophil counts were found after insemination with frozen semen or concentrated fresh semen. Bacterial contamination of uteri was insignificant 6 hours after breeding. Neutrophilia seems to be induced by spermatozoa rather than bacteria. The intensity of the neutrophil reaction seems to depend on concentration and/or volume of inseminate.

High-resolution and signal-averaged electrocardiography to separate post-myocardial infarction patients with and without ventricular tachycardia.

High-resolution and signal-averaged ECG, 24 h Holter recording and ejection fraction were used to separate post-myocardial infarction patients with and without ventricular tachycardia (VT) among 150 individuals: 26 patients with an old myocardial infarction and documented sustained VT, 104 patients with an acute myocardial infarction without sustained VT, who were followed-up for 2 years, and 20 healthy volunteers. Bipolar orthogonal XYZ leads were recorded, high-pass filtered at cut-off frequencies of 25, 40, 60, 80 and 100 Hz, and combined to vector magnitude square root of X2 + Y2 + Z2. The filtered QRS duration, the root-mean-square voltages of different time intervals and the durations of low amplitude signals under different thresholds, both from the initial and terminal QRS, were calculated. The sensitivity and specificity of each parameter alone and in every combination of two, three and four parameters (17 million different combinations) were computed both from non-averaged and averaged data. The best separation was achieved by 12 combinations all including four signal-averaged ECG parameters, with a sensitivity of 81% and a specificity of 79%. The parameters represented most were: filtered QRS duration at 25 Hz, RMS voltage of the last 50 ms at 25 Hz, terminal LAS duration at 80 Hz, and RMS voltage of the last 20 ms at 80 Hz. Parameters of the initial QRS complex did not improve either the sensitivity or the specificity of the method. In logistic regression analysis, the best combinations of four signal-averaged ECG parameters separated VT patients better (P < 0.001) than non-sustained ventricular tachycardia at Holter (P = 0.001); left ventricular ejection fraction (P = 0.01); or age (P = 0.006). Parameters calculated from averaged data gave better results than parameters calculated from non-averaged data.

Changes in frequency spectra of breath sounds during histamine challenge test in adult asthmatics and healthy control subjects.

Air-flow standardized breath sounds were recorded at the chest and at the trachea during histamine challenge test and after subsequent bronchodilation in 12 asthmatics and 6 healthy controls for spectral analysis, to be compared with simultaneous changes in spirometric variables. Of all the lung sound variables measured, the changes in median frequency of the power spectrum (F50) of tracheal expiratory sounds were found to correlate best (r = 0.853, p < 0.0001) with changes in FEV1. The increase of F50 during histamine challenge was significantly larger in asthmatics than in healthy control subjects (p < 0.005). The provocative dose of histamine inducing a decrease of 15 percent in FEV1 (PD15FEV1) and the provocative dose causing an increase of 30 percent in tracheal expiratory F50 (PD30F50) were significantly related (r = 0.754, p = 0.012). In asthmatics, the breath sound frequency distribution in terms of median frequency reflected acute changes in airways obstruction with high sensitivity and specificity. The present method for breath sound analysis can be applied for patients with limited cooperation during bronchial challenge tests.
