Correlation between endoscopic retrograde pancreatogram and postmortem.

Endoscopic retrograde pancreatogram and postmortem pancreatograms were compared in 6 dogs. The main duct, branches and acini of the pancreas were opacified adequately in that order with infusion of each 1 ml of contrast medium on 4 occasions in endoscopic retrograde pancreatography, and infusion of each 0.1 ml of the contrast medium on 4 occasions in postmortem pancreatography, respectively. The two pancreatograms were almost identical to the appearance of the main duct, branches and acini. The results suggest that a postmortem retrograde pancreatogram in order to investigate the correlation between the pancreatogram and the histological findings of the pancreas.

Subtotal versus total parathyroidectomy with forearm autograft for secondary hyperparathyroidism in chronic renal failure.

Forty-three patients with chronic renal failure and secondary hyperparathyroidism underwent parathyroidectomy; 20 of the 43 underwent subtotal parathyroidectomy (Group A) and 23 patients underwent total parathyroidectomy and parathyroid autotransplant in the forearm (Group B). Postoperative clinical improvement was similar in both groups. In the immediate postoperative period eight patients in Group A who had severe bone changes and 21 patients in Group B needed supplemental calcium administration. The grafted tissues in all cases functioned well; reimplantation of the cryopreserved parathyroid tissues was unnecessary. One case in each group showed a recurrence. One patient in Group A was submitted to reexploration of the neck with a lateral approach. The other patient in Group B underwent excisions of the transplanted parathyroid tissues on three separate occasions under local anesthesia. The second operation was definitely easier and safer to manage after a total parathyroidectomy with autotransplantation to the forearm.

[Diagnosis and surgical treatment of renal hyperparathyroidism].

Forty-three patients with chronic renal failure and secondary hyperparathyroidism underwent parathyroid surgery. The first 20 patients were submitted to subtotal parathyroidectomy, and the last 23 patients underwent total parathyroidectomy and parathyroid autotransplantation in the forearm. Non-invasive image diagnosis, CT, ultrasonography and scintigraphy are valuable for 1) making a definite diagnosis of secondary hyperparathyroidism, 2) locating the tumor 3) determining the effectiveness of treatment, and 4) differentially diagnosing thyroid tumors. For surgical treatment, we recommend total parathyroidectomy and autotransplantation in the forearm because the second operation for recurrence may be done more safely and easily than after subtotal parathyroidectomy.

[Blood levels of estradiol, FSH and LH in women with climacteric syndrome--conjugated estrogen therapy].

To study the types of patients with climacteric syndrome who respond to conjugated estrogen therapy, we investigated the results of 1- to 2-month therapy in 52 patients by comparing their pre- and post-drug level of blood estradiol (E2), FSH and LH as well as comparing information through a questionnaire on menopausal complaints listed according to Kupperman. Predrug E2 in the patients studied was lower than normal, but the lowering was not significantly specific to any particular climacteric symptom. Blood FSH was higher in the patients complaining of hot flushing, sweating, depression, feeling of something sticking in the throat, and decreased sexual desire, whereas blood LH was higher in the patients with hot flushing and sweating. Changes in various symptom were investigated in relation to hormonal changes found after conjugated estrogen therapy. In the patients whose E2 was increased and FSH and LH were decreased after the therapy, hot flushing, cold sensation, excitability and insomnia were ameliorated at a high rate. Numbness was favorably treated in the patients responding with increased E2, whereas shoulder stiffness, fatigability and headache was reduced in those responding with decreased LH.

[Influence of PSK on the metabolism of FT-207].

The influence of PSK on the metabolism of FT-207 was studied in patients with gastric cancer. The 5-FU concentration in the blood was determined 15 min, 30 min, 1 hour and 3 hours after intravenous injection of FT-207, 400mg. The blood level of 5-FU remained constant in 86% of patients after administration of PSK for 7 days, but decreased in 14% of patients. After administration of PSK for 8 to 14 months, no change in the blood level of 5-FU was detected. These results suggest that PSK has no distinct influence on the activation of FT-207.

[Renal angiomyolipoma: report of 2 cases of tuberous sclerosis in female siblings].

Two cases of renal angiomyolipomas in female siblings associated with tuberous sclerosis were reported. The first case was a 47-year-old housewife who visited the hospital because of severe right flank pain. Bilateral renal angiomyolipomas and hemorrhage in the right angiomyolipoma were recognized by computerized tomography, ultrasonography and renal angiography. After 2 months she had another episode of left flank pain and was diagnosed to have hemorrhage in the left renal tumor. The pain remissed by conservative treatment. Bilateral nephrectomy and hemodialysis would be necessary if she were to have a third attack. The second case was her 36-year-old sister with the chief complaint of left flank pain and genital bleeding. CT and renal angiography suggested a large angiomyolipoma in her left kidney and a small one in her right kidney. Left nephrectomy, right partial nephrectomy and hysterectomy were performed to prevent intratumoral hemorrhage. The histopathological diagnosis was angiomyolipoma of kidneys, uterus and lymphnodes of right renal hilus. Although pre-operative differentiation of angiomyolipoma from renal cell carcinoma has been difficult, recently diagnosis has become possible by CT, ultra-sonography and angiography.

Recurrent gastric carcinoma--analysis of 100 in-patients--.

Between 1964 and 1981, 100 in-patients with recurrent gastric carcinoma following curative resection were investigated in the Second Department of Surgery, Kyushu University Hospital. The disease-free interval after initial operation and the advancement of the carcinoma correlated. The mean disease free interval of cases with local recurrence was 4 years and 1 month, that is 2 years longer than in cases of peritoneal dissemination or hematogenous metastasis. The disease free interval and the survival time after recurrence also correlated. Patients with a relapse more than 2 years after initial operation survived statistically longer than did those with a relapse in less than 2 years (p less than 0.01). One and 2 year survival rates after recurrence in the second resection group was 41.4 per cent and 17.2 per cent, respectively. Almost all patients in the other group died within one year after recurrence. The average survival in patients on a combination of chemotherapy and resection of the recurrent tumor was about 2 years and longer than in cases of resection but without chemotherapy.

Late results of postoperative long-term cancer chemotherapy for the gastric cancer patients subjected to curative resection.

Postoperative long-term cancer chemotherapy (PLCC) with a combination of Mitomycin-C (MMC), FT-207 and PSK (an immuno-stimulant) was prescribed for gastric cancer patients subjected to curative resection. The 5 year survival rates for patients with stage III and stage IV cancer were 58.3 per cent and 50.0 per cent in the PLCC groups, 48.0 per cent and 15.4 per cent in MMC groups, and 46.3 per cent and 13.3 per cent in no chemotherapy groups, respectively. In stage IV, the survival rate in PLCC group was significantly higher than that in MMC or no chemotherapy group (p less than 0.05). In the PLCC group, there was a tendency toward a dose-dependent effect in each group, and 5 year survival rate of stage III group administered over 60 mg of MMC, 60 g of FT-207 and 270 g of PSK was 70 per cent, such being remarkably higher than 46.3 per cent in those given no chemotherapy (p less than 0.07). There was no drug related death and only a slight leukopenia and hepatotoxicity occurred in some patients.

Combination chemotherapy enhances survival of patients with unresectable gastric cancer.

Of 177 Japanese patients with a gastric cancer which could not be resected and seen at our institution during the period from 1964 to 1979, 153 were investigated with regard to the efficacy of anticancer agents, in terms of prolongation of life. The average survival time was 23 weeks in the combination chemotherapy group (57 cases), 17 weeks in the single drug chemotherapy group (42 cases) and 13 weeks in no chemotherapy group (54 cases). Three and 6 month survival rates in the overall patients were 57.1 per cent and 16.7 per cent for single drug chemotherapy group, and 37.0 per cent and 11.1 per cent for no chemotherapy group, while in the combination chemotherapy group, the rates were higher at 64.9 per cent and 29.8 per cent, respectively (combination chemotherapy vs. no chemotherapy group, p less than 0.05). In patients with peritoneal dissemination, hepatic metastasis and carcinomatous ascites, there was a significant difference in survival rates between those prescribed combination chemotherapy and those given no chemotherapy (p less than 0.05). Of 57 in the combination chemotherapy group, 6 and 9 month survival rates were 45.5 per cent and 22.7 per cent in the postoperative long-term cancer chemotherapy (PLCC) group (22 cases), such being higher than other combination chemotherapy group (35 cases), 22.9 per cent and 11.4 per cent, respectively. There was a significant difference in the survival rates between the two groups (p less than 0.05).

Adrenergic regulation of estradiol uptake and cyclic GMP content in rat uterus.

The presence of adrenergic regulation of estradiol uptake in rat uterus was studied by using alpha- and beta-adrenergic blockers in normal or Ca2+-free Tyrode's solution at 30 degrees C in vitro. Cyclic GMP content was also investigated under these conditions. By the stimulation with noradrenaline (4 X 10(-7) M), Ca2+-dependent 3H-estradiol uptake was increased in both cytosol and nuclear fractions. These increases were blocked in the presence of phentolamine (4 X 10(-7) M) but not by propranolol (4 X 10(-7) M). Increase in uterine cyclic GMP contents were shown under the condition in which 3H-estradiol uptake was stimulated. These results suggest that adrenergic alpha-stimulation increased uterine estradiol uptake and that consequently, increased estradiol-receptor complexes elevated uterine cyclic GMP levels. The changes in uterine cyclic GMP may serve as an index of estrogen action.
