Immunochemical properties of immunoglobulin G conjugated with lactate dehydrogenase.

Quantitative binding affinities of immunoglobulin G (IgG) for the five isoenzymes of lactate dehydrogenase (LD; EC 1.1.1.27) were determined for IgG isolated from three patients' sera that contained LD-IgG complexes. These three IgGs formed soluble complexes with four (LD 1, 2, 3, 5) of the five LD isoenzymes in two of the patients and in the third they bound with three (LD 2, 3, 4) of the five LD isoenzymes without inhibiting the enzyme activity or precipitating with the enzyme molecules. When rabbit antibody to human IgG was added to these sera, the complexes between the LD isoenzymes and the patients' isolated IgG were completely precipitated. The equilibrium constants for the respective isolated IgG complexes with LD-3 were 0.102, 2.58, and 7.49 X 10(8) L/mol. In these three cases, LD-3 evoked the strongest response from the prepared IgG, demonstrating that the site of antigen recognition of LD-linked IgG was not associated with the structure of individual H and M subunits.

[Laser acupuncture therapy for pain and vague complaints in neurosurgery].

The value of diode laser acupuncture for pain and other vague neurological complaints has been studied in chronic patients of head trauma and of cerebrovascular disease or neuralgia in head and neck regions. Gallium-Aluminium-Arsenide (Ga-Al-As) diode laser pain attenuator PANALAS-4000 manufactured by Japan Medical Laser Laboratory was used in the following specifications; wave length: 830 nm, power: 20-40 mW, frequency: 5-50 c.p.s., pulse duty: 0.9. The laser acupuncture was applied to some of the representative Chinese meridian points, local pain points and related nerves for 1 to 7 times in a week. It was given in 310 patients in our neurosurgical department. 175 of 227 patients who complained of pain and 12 of 39 patients who complained of numbness showed improvement within 24 hours, another 24 patients with pain and 16 patients with numbness showed improvement after 24 hours. No side effect was noticed. The duration of the effectiveness was about 2 or 3 days. It is very useful to apply the laser acupuncture for pain and vague complaints in neurosurgery.

Clinical significance of acidic pI isoenzyme of ribonuclease in human serum.

Employing isoelectric focusing with an Ampholine column, the presence of three major pI isoenzymes (basic, neutral and acidic) of human ribonuclease (RNase) is demonstrated. The acidic pI isoenzyme was observed in pancreatic and hepatic cancer cells and in fetal pancreas and liver tissues, and was frequently detected in the sera of patients with pancreatic and hepatic cancer. This isoenzyme was not, however, detected in the sera of non-cancerous patients. In pancreatic cancer tissues, other pI isoenzymes, except for the acidic one, were markedly suppressed and the isoelectrophoretic patterns of the cancerous tissues closely resembled those obtained from fetal pancreatic tissue. These results suggest that the acidic pI isoenzyme could represent one of the carcinofetal proteins and that the detection of this pI isoenzyme in the patient's serum may be useful for the diagnosis of pancreatic and hepatic cancer.

Examination of the subcellular distribution of tripeptide aminopeptidase and evaluation of its clinical usefulness in human serum.

Following electrophoretic separation, we assayed tripeptide aminopeptidase (EC 3.4.11.4) in human tissues and sera free of interferences by other aminopeptidases. Tripeptide aminopeptidase is distributed in various human tissues, with the highest activity observed in liver and lymphocytes. The highest specific activity of the enzyme was observed in the soluble fraction prepared from liver, and 65% of the enzyme activity in the original homogenate was recovered in this fraction. Cancerous and fetal tissues showed lower enzyme activities than normal adult tissues. Elevations of tripeptide aminopeptidase were observed in sera of patients with liver disorders, leukemias, and autoimmune diseases.

Evaluation of cytosolic aminopeptidase in human sera. Evaluation in hepatic disorders.

Employing the quantitative electrophoretic separation of cytosolic "LAP" activity (EC. 3.4.11.1), the authors investigated the clinical significances of cytosolic and total "LAP" activities in the sera of patients with several different hepatic disorders and elucidated the previously confusing results of aminopeptidase measurements in clinical laboratories. The dominant aminopeptidases measured in human sera by L-leucinamide as substrate are both cytosolic "LAP" and microsomal aminopeptidase (EC. 3.4.11.2). Total "LAP" activity means the sum of these two aminopeptidases. Different substrate specificities are observed in these two enzymes located in other subcellular fractions and different increasing rates of these enzymes are observed in various hepatic diseases. In cases of acute hepatic cell damage, cytosolic "LAP" accounts for a large percentage of total "LAP" of a patient's serum. On the other hand, in cases of elevation of hepatobiliary enzymes, microsomal aminopeptidase constitutes a large part of total "LAP" activity, and the ratio between cytosolic and total "LAP" activity is very low.

Inhibitory influence of amino acids on secretagogues induced exocrine secretion in isolated perfused rat pancreas.

Influence of amino acids upon pancreatic exocrine secretion has been investigated in the isolated perfused pancreas of rats. Arg produced significant and dose-related inhibition of pancreatic juice flow, protein output and amylase output evoked by CCK-PZ (1.25 pM). The secretory response evoked by CCK-PZ was inhibited by other amino acids (Ala, Asp, Asn, Gly, Ile, Leu, Lys, Met, Phe, Pro, Thr, Trp, Val, in each 20 mM). A similar inhibitory pattern was observed using 10 mixed amino acids of 2 mM each (Pro, Phe, Thr, Met, Lys, Asp, Leu, Trp, Val, Gly). Gly at a concentration of 20 mM produced significant inhibition of exocrine secretion evoked by ACh (50 nM) or GRP (36 pM). The inhibitory response induced by amino acids could not be repeated by using exogenous insulin (1 microM) and glucagon (280 nM). The inhibitory response was also not changed by increased extracellular Ca (5 or 10 mM). However, Gly (20 mM) produced inhibition of exocrine secretion evoked by Ca reintroduction into a pancreas which was pretreated with A 23187. It was suggested that the inhibitory effects of some amino acids on exocrine secretion are mainly caused by suppression of Ca influx in a stimulus-secretion coupling process.

Role of surgery in hypertensive intracerebral hematoma. A comparative study of 305 nonsurgical and 154 surgical cases.

The indications for surgery in hypertensive intracerebral hematoma are still controversial. The reason for this may be: 1) lack of adequate and comparable data in conservative and surgical therapy from the same institution; 2) lack of adequate close follow-up monitoring over an extended period of time; or 3) lack of proper classification of hematomas for comparison of results from different institutions. The authors have treated 459 cases of hypertensive intracerebral hematoma between October, 1975, and July, 1983. The hematomas have been classified according to their mode of extension on computerized tomography. The long-term outcome was assessed on the basis of activity of daily living. Putaminal hematomas were classified as mild, moderate, severe, and very severe. In general, there was no significant difference in outcome between the surgical and nonsurgical cases; however, the outcome in the moderate and severe hematomas was found to be a little better for the surgical cases in some restricted areas. Thalamic and pontine hemorrhages were classified as mild, moderate, or severe. If the hematoma is localized to the thalamus or pons, and if it extends to the midbrain, there is no indication for surgery; however, in patients with moderate hematomas, the prognosis showed a variable outcome, and the indications for surgery were questionable. In cerebellar hematomas, the authors propose that even a hematoma with a diameter greater than 3 cm might show a good outcome with nonsurgical therapy.

Estimation of the gene frequency of lactate dehydrogenase subunit deficiencies.

To detect the frequency of lactate dehydrogenase (LDH) subunit deficiency, screening for LDH subunit deficiency was performed on 3,776 blood samples from healthy individuals in Shizuoka Prefecture by means of electrophoresis. The frequency of heterozygote with LDH-A subunit deficiency was found to be 0.185%, and with LDH-B subunit deficiency, 0.159%. The frequencies of both subunit deficiencies were not significantly different. Gene frequencies of LDH subunit deficiencies were calculated by the simple counting procedure, and the results are as follows: gene frequency of LDH-A subunit deficiency was 11.9 X 10(-4), and that of LDH-B subunit deficiency, 7.9 X 10(-4). In addition, the second case in the world of a homozygous individual with LDH-A subunit deficiency was detected by this screening. This case with regard to the characteristics of LDH-A subunit deficiency are summarized herein.

Sex differences in bone resorption: a scanning electron microscopic study of mouse parietal bones.

Bone resorption surfaces formed by osteoclasts can be identified as rough areas by scanning electron microscopy (SEM). In this study, the endocranial surfaces of mouse parietal bones were examined by SEM at 1, 4, 6, 8, 14 and 20 weeks of age in an attempt to understand the bone resorbing activity after the cessation of bone growth. On the inner surface of the parietal bones, the rough areas, composed of a group of 8 to 30 microns wide concavities, can be divided into two distinct groups (type I and type II). The surface of type I areas appeared irregularly rough and those of type II revealed feather, fire-flame, or wave-like patterns due to characteristic arrangements of the concavities. The rough areas occupying the inner surface of the parietal bones made up about 60% of the surface area at 1 week and about 5% at 4 weeks in both sexes. The rough areas were all type I at this age. After puberty, the rough areas in males occupied about 10% of the inner surface until 20 weeks and consisted of large type I and small type II areas. In females, the proportion of the rough areas increased after puberty and occupied about 40% of the endocranial surface at 14 and 20 weeks. The type I areas were much larger, increasing in size with age and eventually occupying a large proportion of the rough areas. The sex differences in the rough areas were reversed by gonadectomy. The results suggest that male and female gonadal products, possibly sex hormones, have respective inhibitory and stimulatory effects on the formation of bone resorptive surfaces, and may be responsible for the sex differences in the size and morphological features of the areas on the bone surfaces.
