Potentiation of cholecystokinin-induced amylase release by peptide VIP in guinea pig pancreatic acini.

The mechanism of the potentiating effect of vasoactive intestinal peptide (VIP) on cholecystokinin (CCK-8)-induced amylase release was studied in isolated and perifused pancreatic acini of the guinea pig. VIP (30 pM-10 nM) potentiated CCK-8 (100 pM)-induced amylase release. Unexpectedly, VIP inhibited CCK-8-induced intracellular Ca2+ oscillations. Forskolin (10 microM), an activator of adenylate cyclase, potentiated CCK-8 (100 pM)-induced amylase release with a time course similar to that observed with VIP. Caffeine (20 mM) inhibited both amylase release and Ca2+ oscillations in response to CCK-8, suggesting that inhibition of Ca2+ oscillations does not necessarily lead to a potentiation of amylase release. When intracellular Ca2+ concentration ([Ca2+]c) was raised by thapsigargin (10 microM), a selective inhibitor of Ca(2+)-ATPase in the endoplasmic reticulum (ER), VIP (10 nM) induced significantly greater amylase release than that induced by VIP alone. When [Ca2+]c was lowered by preincubation with BAPTA-AM (25 microM), a cell-permeant Ca2+ chelator, VIP-induced amylase release was completely abolished. These results suggest that VIP, in spite of its inhibitory action on Ca2+ oscillations, facilitates a Ca(2+)-dependent process distal to the increase in [Ca2+]c to potentiate CCK-8-induced amylase release.

Tolosa-Hunt syndrome with unusual clinical courses--two case reports.

Two cases of Tolosa-Hunt syndrome with unusual clinical courses are described. The clinical onset of the first patient was typical, but was unresponsive to steroid hormone treatment. The symptoms resolved spontaneously after 1 year. Computed tomography (CT) and magnetic resonance (MR) imaging revealed enlargement of the affected cavernous sinus, which did not change after the symptoms resolved. The clinical presentation in the second patient included marked pupillary dilatation, a sign which is absent in the classical criteria. CT showed no abnormal findings, but MR imaging revealed a mass located in the medial side of the cavernous sinus extending to the lateral wall, which might have caused the pupillary dilatation. After steroid therapy the symptoms disappeared and MR imaging showed remarkable reduction of the mass in the cavernous sinus. Tolosa-Hunt syndrome usually responds promptly to steroid therapy and rarely manifests marked pupillary dilatation. Such variation in presentation adds to the difficulties in diagnosis and treatment of this disease of unknown etiology.

Intracerebral infiltration by monoclonal plasmacytoid cells in Waldenstrom's macroglobulinemia--case report.

A 68-year-old female presented with Waldenstrom's macroglobulinemia with infiltration into the cerebral parenchyma manifesting as increased confusion, memory loss, and disorientation. She had a past history of Waldenstrom's macroglobulinemia treated 3 years before. Magnetic resonance imaging showed a high intensity area on T2-weighted images in the left frontal lobe extending to the corpus callosum which was well enhanced by gadolinium-diethylenetriaminepenta-acetic acid. Direct infiltration of neoplastic cells was confirmed by biopsy. Immunohistochemical examination showed that mature plasmacytoid cells in the cerebral parenchyma were immunoglobulin M and lambda light chain antigen positive, but immature lymphocytes in Virchow-Robin space were negative. Monoclonal proliferation was confirmed by southern blot analysis. She became symptom free and the size of the lesion was dramatically reduced after 40 Gy irradiation. She showed no evidence of recurrence 3 years after irradiation. As no effective chemotherapy regimen for Bing-Neel syndrome has been established, irradiation is worth considering when neuroimaging suggests intracranial infiltration of neoplastic cells.

Inhibition of stimulus-specific neutrophil superoxide generation by alpha-tocopherol.

Alpha-tocopherol but not 2-carboxy-2,5,7,8-tetramethyl-6-chromanol (trolox or CTMC) and 2,2,5,7,8 pentamethyl-6-hydroxy chromane (PMC), derivatives of alpha-tocopherol, inhibited the superoxide (O2-.) generation of rat peritoneal neutrophils (RPMN) induced by phorbol 12-myrisate 13-acetate (PMA). ID50 for neutrophils obtained from the peritoneal cavity of rat and guinea pig was about 1microM. This concentration, however, was much lower than that for the inhibition of PMA-activated phospholipid-dependent protein kinase (PKC) (ID50 = 30 microM). The alpha-tocopherol sensitive O2-. generation was also observed in neutrophils induced by dioctanoylglycerol (diC8) and calcium ionophore A23187 but not by formylmethionyl-leucyl-phenylalanine (FMLP), opsonized zymosan (OZ) and sodium dodecyl sulfate (SDS). The pattern of inhibition by alpha-tocopherol was quite similar to that of staurosporine, a specific inhibitor of PKC. The alpha-tocopherol content of RPMN was 12 ng/10(6) cells and a linear increase to 200 ng/10(6) cells by addition of alpha-tocopherol to the cell suspension corresponded with an increased inhibition of O2-. generation. These results indicate that both the chemical structure and the content of alpha-tocopherol might be important factors in O2-. generation by neutrophils.

[Concept of isoenzymes in clinical points of views].

In the fields of clinical medicine, the word "isoenzyme/isozyme" was widely and easily used for the heterogeneities of plasma enzymes. The word "isozyme" seemed to be synonym for the multiple forms of enzymes. The Committee of Biochemical Nomenclature in IUPAC/IUB proposed six categories for multiple forms of enzymes in 1776. In this paper, additional four categories were proposed for the understanding the clinical significance of enzyme heterogeneities. These categories were follows, 1) enzyme linked immunoglobulins. 2) protease modified heterogeneities of plasma enzymes. 3) membrane associated plasma enzymes. 4) different enzymes reacted for the same substrates. Including these four classes, "isozyme", the multiple forms of enzymes in clinical uses were clearly categorized for clinical purposes.

[Liver, bile ducts and pancreatic diseases].

A fundamental guideline for the use of test results concerning liver, bile duct and pancreatic diseases was proposed in 1991 from the Japan Society of Clinical Pathology (JSCP). This guideline was principally based on the document of 1988 from the Committee on liver function tests of the Japanese Society of Gastroenterology (JSG). The document from the JSG was revised in May, 1994. Also a guideline for selection of markers of hepatitis virus in hepatic disorders, was proposed in January, 1994 from the same Committee of JSG. Here, we reevaluated and discussed the JSCP guideline as taking into consideration the two 1994 JSG documents.

[Clinical usefulness of 3-D CT endoscopic imaging of cerebral aneurysms].

Usefulness of endoscopic imaging of cerebral aneurysms is presented. 3D-luminal images were obtained using a new processing technique which extracts CT numbers in the boundary region between the vessel wall and contrast media filling in the vascular lumen. Clinical application of this technique to complicated large cerebral aneurysms showed that, with this 3D-CT endoscopic images and MRA, anatomical details of cerebral aneurysms such as the orifice of the aneurysm, intraluminal thrombus, and calcification of the wall could be clearly demonstrated. We operated on two large, complicated aneurysms after obtaining 3D-CT endoscopy images of the aneurysms. Such information was found to be very useful when operating on difficult and complicated cerebral aneurysms.

Automated homogeneous liposome-based assay system for total complement activity.

We developed an automated homogeneous immunoassay, based on immune lysis of dinitrophenyl (DNP)-labeled liposomes, for measuring total complement activity. Liposome lysis caused by complement activity was detected spectrophotometrically from entrapped glucose-6-phosphate dehydrogenase activity. Complement activity in human sera was quantified by comparison with a calibration curve. For ease of application to fully automated routine clinical analyzers, we adopted a two-reagent system, one reagent containing a homogeneous population of small DNP-labeled liposomes and one containing antibody/substrate. This system required calibration only once a week. Within-run and between-run CVs were 0.4-1.3% (n = 10) and 1.8-4.7% (n = 10), respectively. Serum results were linear upon dilution (with saline) over a twofold range. Bilirubin, hemoglobin, Intrafat, and serum proteins such as rheumatoid factor, M protein, IgG, and IgA did not affect the assay results. The results (y) correlated well with those from a hemolytic complement activity test (x): y = 1.05x - 1.14, r = 0.92, on 66 samples in the range < 10- > 50 kU/L. This method should therefore be of great use for the determination of complement activity.

[The assessment of quick turnaround service in clinical laboratories].

The quick turnaround service of laboratory data has been a basic and fundamental service of clinical laboratories. To realize this service, there are three basic requirements in the hospital and laboratory information systems. The first is cooperation with the hospital information system providing bar-coded label sampling system and test requisition from the information system terminal. The second is real time quality assurance system fully supported from the laboratory information system. The third is random and sequential automated analyzer supported by the bar-code system. In 1994, we set up an automated system in our clinical laboratories. The effect of the quick turnaround service of reporting was analyzed and is discussed herein. Employing this system, the quick turnaround service system was established for 31 items of biochemistry, glucose, Thrombo test and complete cell counting of hematology. Furthermore, these items were available within 30 minutes before medical consultation. Effective sample conveying system, quick sample preparation without clotting and arrival information system of laboratory data as a mailing system are further requirements for this quick turnaround service system.

[Clinical usefulness of helical-scanning CT for the evaluation of arteriosclerotic carotid lesions].

We examined arteriosclerotic carotid lesions in 76 patients using helical scanning CT (HES-CT), and evaluated the clinical usefulness of this method. A high speed slip-ring X-ray CT system was used. Scanning of the neck was performed for a 30 second period following intravenous bolus injection of non-ionic contrast medium, while couch top movement was 2.0 mm/sec. Multiplannar reconstruction images (MPR-image) and 3-dimensional surface images (3D-image) were reconstructed from the continuous raw data. MPR-images offered axial, coronal and sagittal images in which the lesion could be seen from any direction, and 3D-images that could be freely rotated were obtained by using a track ball and monitor. Eighteen cases were also evaluated by conventional angiography. Excellent HES-CT images were obtained in 73 cases, showing occlusion in 13, stenosis in 34, plaques without calcification in 15 and plaques with calcification in 74 vessels. A good correlation was obtained between HES-CT and angiogram in most cases, and in 6 cases, HES-CT was superior in the detection of stenosis, because it enabled us to observe the lesion from various directions. These results suggested that HES-CT was a minimally invasive, useful diagnostic method for the evaluation of arteriosclerotic carotid disease.

Image-oriented rule generating tool for medical knowledge base.

This paper describes an automatic programming tool for the end users in the medical knowledge representation. When end users work with the rule generating tool (RGT) that we have designed, they can represent an idea using an Image-oriented interface such as a picture, movie, audio reference or two dimensional bar code. Such representations can be valuable tools for medical study, diagnosis, decision making and treatment monitoring to supplement the expertise of medical personnel. The RGT will automatically generate rules in the logic programming language Prolog and then add them to the knowledge base.

Concomitant use of prostaglandin E1 and prednisolone; inhibition of superoxide anion production by polymorphonuclear leucocytes.

We investigated the effect of prostaglandin E1 (PGE1) or prednisolone on superoxide anion production by polymorphonuclear leucocytes (PMN) stimulated by opsonized zymosan (OZ). The production of superoxide anion by PMN was significantly suppressed by treatment with either PGE1 or prednisolone. Concomitant use of PGE1 and prednisolone suppressed it significantly more than PGE1 or prednisolone alone. Both PGE1 and prednisolone suppressed the Ca2+ influx into PMN, which was a crucial event for production of superoxide anion in stimulation by OZ. The effect of concomitant use of PGE1 and prednisolone in vivo was confirmed in rat peritonitis. The additional use of PGE1 with prednisolone may provide a strategy to reduce the amount of prednisolone required in treatments.
