[Developmental changes in normal images of 123I-IMP brain SPECT in young children].

In an attempt to determine the normal evolution of N-isopropyl p-I-123-iodoamphetamine (123I-IMP) single photon emission computed tomography (SPECT), we selected 12 children (an age range of 1 month-3 year) who were diagnosed as normal in follow-up between 6 month-3 year by neurological examination, X-ray computed tomography, electroencephalography and 123I-IMP SPECT in our 78 cases. SPECT of the brain was performed 30 minutes after intravenous administration of 74 MBq (2 mCi) 123I-IMP using a rotating gamma camera equipped with a 30-degree slant-hole collimator. In neonates, a predominant activity was observed in the basal ganglia and thalamus, with only slight parietal and occipital activity. Frontal cortex was barely recognizable. Under 1 year, cortical activity was gradually increased and clearly visible. After 1 year, SPECT images were similar to those of adult. In conclusion there was an important modification of the regional perfusion pattern in relation to brain maturation.

Search for immunobiological parameters predictive of clinical effects of OK-432 in patients with malignant ascites.

Although OK-432, a potent BRM, has been known to induce the remarkable improvement of clinical conditions in cancer patients through its strong effects on their immune capabilities, no specific immune parameters have been identified to best predict the clinical outcome after the OK-432 treatment. In an attempt to identify early parameters indicative of the clinical effects, we have administered 0.1 mg of OK-432 intraperitoneally to a total of 12 patients with malignant ascites and examined peritoneal fluid and peripheral blood obtained on 4 days before, 1, 3, and 7 days after the OK-432 injection using various immunobiological assays. Four weeks later, clinical improvements were evaluated by the disappearance of malignant cells from and/or substantial decrease in ascites. Four patients (responders) showed the improvements while 8 patients (nonresponders) showed no clinical evidence for improvement. In a few parameters among the many examined, significantly different patterns of changes were noted between responders and nonresponders. Thus, in nonresponder patients MO and T cell population returned to an initial low level after early increases (on days 1 and/or 3), while they remained increased day 1 through 7 in responders. In responder patients, the cytotoxicity of peritoneal mononuclear cells against K562 and Daudi cells were augmented on day 7, but not in nonresponder patients. The in vitro stimulation of the mononuclear cells with OK-432 enhanced the cytotoxic activity and induced the interferon (IFN) production in the responders but not in nonresponders. These parameters will be useful for the early prediction of the expected clinical effects of OK-432.

Gallium scintigraphy in neoplastic angioendotheliosis of the lung.

A case of neoplastic angioendotheliosis of the lung is reported. A 56-year-old man presented with a 2-month history of fever, increased serum lactic dehydrogenase, and arterial hypoxemia. Chest radiography revealed diffuse reticulonodular opacity, whereas CT scanning showed diffuse small nodular areas of increased attenuation with indistinct borders. Diffuse pulmonary uptake of Ga-67 was noted in scintigraphy. Combination chemotherapy designed for lymphoma induced a remission of the disease.

Cinematic three-dimensional surface display of cardiac blood pool tomography.

A method of three-dimensional cinematic display (3D cine) of cardiac blood pool tomography is described. ECG-gated transaxial blood pool imaging was obtained from a set of projection images that were collected from 32 images with 10 ECG-gated images per projection during a 180 degrees arc of a rotating gamma camera. A surface contour of the blood pool was determined by a set of isocount lines (40-55% of the maximum pixel counts) of the transaxial images. 3D cine was made by a depth-shading method, in which brightness of a given point on the contour was set proportional to the distance between the viewing plane and the point and to the incident angle formed by the viewing line and the surface of the point. In 15 patients, 3D cine showed hypokinesia, akinesia, dyskinesia, ventricular aneurysm, and opposite motions of the atria and ventricles. Diagnoses of left ventricular motion by 3D cine agreed well with those by echocardiography and contrast left ventriculography.

[The group study of diagnostic efficacy of cerebro-vascular disease by I-123 IMP SPECT images obtained with ring type SPECT scanner--the ROC analysis on the diagnosis of perfusion defect and redistribution].

We performed two image reading experiments in order to investigate the diagnostic capability of I-123 IMP SPECT obtained by the ring type SPECT scanner in cerebro-vascular disease. Fourteen physicians diagnosed SPECT images of 55 cases with reference to clinical neurological informations, first without brain XCT images and second with XCT images. Each physician detected perfusion defects and redistributions of I-123 IMP and assigned a confidence level of abnormality for these SPECT findings by means of five rating method. From results obtained by ROC analysis, we concluded as follows: (1) Generally, I-123 IMP SPECT is a stable diagnostic modality in the diagnosis of cerebro-vascular disease and the image reading of XCT had no effects on the diagnosis of SPECT on the whole of physician, (2) However, there were unnegligible differences among individuals in the detectability of findings and the effect of XCT image reading, (3) Detectability of redistribution of I-123 IMP was lower than that of perfusion defect and inter-observer variation in the diagnostic performance for redistribution was larger than that of perfusion defect. The results suggest that it is necessary to standardize diagnostic criteria among physicians for redistribution of I-123 IMP.

Studies by pancreatography of ductal changes induced by administration of pancreatic carcinogen in two dogs.

To assess the temporal changes of the pancreatic duct following the administration of N-ethyl-N'-nitro-N-nitrosoguanidine (ENNG) in two dogs, serial pancreatography was performed. They received intraductal administration of a total of 595 and 500 mg ENNG in each one, over the periods of 12 and 13.5 months, respectively. In one dog, sequential changes of the main duct were observed, a small round filling defect developed a circumferential defect and became a severe stenosis associated with proximal dilatation of the duct. While no gross tumors were macroscopically detected at autopsy, continuous lesions with features of hyperplasia, atypical hyperplasia, and cancerous change of duct epithelial cells were microscopically seen. In the other one, a small round filling defect was detected by pancreatography, which was histopathologically hyperplasia of pancreatic duct, rather than cancerous cells. The present dog model for induction of pancreatic duct carcinomas appears useful for elucidating clinico-pathological changes occurring during cancer development.

[Clinical significance of I-123 IMP brain SPECT in children with brain diseases].

Single photon emission computed tomography (SPECT) of the brain using N-isopropyl p-I-123-iodoamphetamine (I-123 IMP) was performed in 43 children with suspected brain diseases. Forty-three children (25 males and 18 females), with an age range of 24 days-15 years (mean: 6.6 years), were included in the study. Six patients were subsequently diagnosed as normal. Early SPECT of the brain was performed 30 minutes after intravenous administration of 74-111 MBq (2-3 mCi) I-123 IMP using a rotating gamma camera equipped with a 30-degree slant hole and medium energy collimator. Transverse images were reconstructed by Shepp-Logan filtered back projection method with attenuation correction after spatial filtering using an 8th order Butterworth-Wiener filter. Findings of I-123 IMP SPECT were compared with those of X-ray computed tomography (CT) and electroencephalography (EEG). The results showed that 1) In I-123 IMP SPECT, abnormality was found in 30 out of 37 children with brain diseases. The incidence of abnormal findings in the 37 patients was 81% in I-123 IMP SPECT, 61% in X-ray CT, and 78% in EEG., 2) In both cryptogenic and secondary epilepsy, the incidence of abnormality was higher in I-123 IMP SPECT than in X-ray CT. (70% and 94% vs 50% and 81% respectively). Epileptic foci detected by EEG did not correspond with defects found using I-123 IMP SPECT in 27% of the patients., 3) In asphyxiated infants, a high incidence of abnormality was observed on both I-123 IMP SPECT (86%) and X-ray CT (86%).(ABSTRACT TRUNCATED AT 250 WORDS)

[The experience of a new double-catheter introducer for simultaneous bilateral internal iliac angiography].

We developed a new 8F catheter-introducer system designed for simultaneous insertion of two catheters into each internal iliac artery via a single puncture. This introducer has a common distal limb and a pair of proximal limbs connected in a Y-shaped fashion. Each of the proximal limbs has its own check valve and flushing side arm, through which one can pass two catheters separately. We succeeded in placing each of two catheters at desired sites in three cases of pelvic angiography. This system will be of great help in pelvic angiography and infusion-chemotherapy when bilateral catheterization is required.

[Clinical results of UFT enteric-coated granule therapy under cooperative study (phase II study). Tokyo Cancer Chemotherapy Cooperative Study Group].

We have carried out the Phase II study by oral administration of UFT enteric-coated granules (UE) against various malignant tumors. Out of 45 patients entered in the study, 40 patients were evaluable: 36 patients among them had measurable lesions. PR (partial response) were obtained in two (11.1%) out of 18 patients with gastric cancer and one (50.0 %) out of 2 patients with lung cancer. Total response rate was 8.3%. Side effects by UE were slight; that is, anorexia (13.3%), nausea and vomiting (8.9%), generalized malaise (8.9%) and so forth. UE, as a long-term therapy, can be considered a useful drug against cancers with less side effects.

[Generation of brain scintigram reports with a speech recognition device controlled by a personal computer].

Radiology reporting system using a speech recognition apparatus was applied to generation of reports on brain scintigram and SPECT. The number of words or phrases that were selected for the reporting system by the authors was 259. Rates of correct recognition at the first, second, and third or more utterance of the voices were 91.1, 5.0, and 3.9%, respectively. The recognition rates decreased from 96.6 to 88.0% when the mean lengths of the words or phrases varied from 6.4 to 3.3. Net time for the generation of 8 reports by the system was 21 minutes 26 seconds, while that by the traditional dictation-transcription method was 15 minutes and 53 seconds. This system seems to be useful because of much longer input time expected for an average doctor, who usually is not well trained in using a Japanese-word processor, and convenience for database management offered by the system.

Factor analysis of multigated cardiac blood pool scintigram for the measurement of left ventricular ejection fraction.

Left ventricular ejection fraction (EF) was measured by factor analysis (FA) of multigated cardiac blood pool scintigram in 38 consecutive patients, and compared with that measured by the variable ROI method (EFVROI) with automated left ventricular contour detection. FA was automatically performed without operator intervention with a success rate of 100%. The correlation of EF with EFVROI was significant in the group of 22 patients with normal wall motion (r = 0.65, p less than 0.001), and the entire group of patients (r = 0.70, p less than 0.001), but not significant (p = 0.19) in the group of 16 patients with abnormal wall motion. In conclusion, left ventricular ejection fraction can be estimated by factor analysis of MUGA in patients with normal wall motion.

Scintigraphic and CT findings of Tietze's syndrome: report of a case and review of the literature.

A case of Tietze's syndrome is reported. A 43-year-old woman, who had experienced right anterior chest pain and tender swelling of the right first costosternal junction for seven months, showed increased accumulation of the right first, the right fourth, and the left first costochondral junction on bone imaging. Ga-67 imaging showed increased accumulation at the right first costosternal junction. CT showed sclerosis of the sternal manubrium, partial calcification of costal cartilage, and soft tissue swelling. Biopsy of the right first costal cartilage showed chronic inflammation with fibrosis and ossification. Increased uptake of bone gallium imaging is consistent with Tietze's syndrome.

[Clinical assessment of uneven ventilation by factor analysis of Xe-133 washout scintigram].

Factor analysis (FA) was applied to 50 consecutive Xe-133 washout lung scintigrams, and its findings were compared with dynamic scintigrams of Xe-133 washout (DSW). Mean transit time (MTT) of radioactive xenon measured by FA also was compared with that measured by height/area (H/A) or two-compartment analysis (2CA). In 12 patients with normal DSW, FA separated and located two factors: one represented background activity, and the other uniform washout from the lung. In 31 out of 38 patients (82%) with abnormal DSW, FA separated the same areas where the washout was delayed on DSW. In 5 (13%) patients, FA diagnosed more areas of delayed washout than DSW. FA failed in 2 (5%) patients, but this failure was due to small number of counts of pixels. Diagnosis by FA was seemed easier than diagnosis by DSW in 95% of all patients by the authors' impression. MTT by FA (y) correlated with MTT by 2CA (x) in small groups of patients: in 9 patients in whom only one MTT was obtainable by FA (r = 0.75, p = 0.02, y = 4.2x + 1.9), and in 8 patients of normal DSW in whom two MTT's were obtainable by FA (r = 0.77, p = 0.02, y = 0.69x + 6.2, y; shorter MTT by FA between the two). MTT by FA did not correlate with MTT by H/A. In conclusion, FA is useful for diagnosing uneven Xe-133 washout from the lung.

Preliminary observation on pancreatic duct adenocarcinoma induced by intraductal administration of N-ethyl-N'-nitro-N-nitrosoguanidine in dogs.

Pancreatic duct adenocarcinoma was induced by intraductal administration of N-ethyl-N'-nitro N-nitrosoguanidine (ENNG) in two mongrel dogs. A dog received a total dose of 595 mg of ENNG during 12 months and was sacrificed. Duct obstruction was detected by pancreatography and duct adenocarcinoma was found. Another dog was given a total dose of 350 mg of ENNG during 8 months and was sacrificed 26 months after the first administration of the carcinogen. Duct adenocarcinoma was found. No pancreatic tumors were found in 2 dogs given intraperitoneal N-nitrosobis(2-oxopropyl)amine at a total dose of 4000 mg or in 2 dogs given Tween 60 only. These results suggest that the direct presence of a carcinogen in the pancreatic duct was able to induce duct adenocarcinoma in dogs.

Vibrio cholerae O1 adherence to villi and lymphoid follicle epithelium: in vitro model using formalin-treated human small intestine and correlation between adherence and cell-associated hemagglutinin levels.

Formalin-fixed human small intestinal mucosa possessing villi and lymphoid follicle epithelium of Peyer's patches at the mucosal surface was used to test the adherence ability of clinically isolated strains of Vibrio cholerae O1. V. cholerae O1 grown on CFA agar for approximately 3 h at 37 degrees C had various levels of cellular hemagglutinins (HAs) and manifested adherence abilities that were roughly correlated with the cellular HA levels, irrespective of cellular HA types. V. cholerae O1 adhered better to epithelium over ileal lymphoid follicles than to epithelium of jejunal or ileal villi. Cells of different morphology which constituted lymphoid follicle epithelium were almost equal targets for adherence. In contrast, V. cholerae O1 grown on CFA agar for approximately 20 h at 37 degrees C in many cases had lower levels of cellular HAs and adherence abilities. Contrary to the above observations with cellular HAs and adherence, piliation of V. cholerae O1 was rather more extensive at approximately 20 h of incubation at 37 degrees C than at approximately 3 h of incubation at 37 degrees C. L-Fucose inhibited adherence to a varied extent depending on the cellular HA types, while D-mannose enhanced adherence in some strains. Heating of V. cholerae O1 diminished adherence ability. This adherence model system provides a tool by which various V. cholerae O1 strains can be preliminarily tested for adherence ability and site in human small intestine.

[UFT + MMC and UFT + ACNU therapy in advanced gastric cancer].

The effects of UFT + MMC and UFT + ACNU therapy on advanced gastric cancer were compared by randomized controlled trial in 12 institutions. Unresectable and postoperatively recurrent patients were divided into PS 0-2 and 3-4 groups. After classification according to cancer spread, such as localized type, hepatic metastatic type, ascitic type and distant metastatic type, regimens of (A) UFT 375 mg/m2/day + MMC 5 mg/m2 for 1-2 W or (B) UFT + ACNU 60 mg/m2/W/x2 (with a 4-week interval) were administered for as long as possible. As a result, among a total of 104 cases, responses were recognized in 7 out of 32 cases (21.9%) treated with regimen A and in 5 out of 25 cases (20.0%) treated with regimen B, giving a total of 57 evaluable cases excluding 33 incompletely evaluated cases and 14 ineligible cases. This study therefore demonstrated no significant difference between the two regimens. The median survival of patients treated with regimen A was 120 days, and that of patients treated with regimen B, 155 days. There was no significant difference between the two regimens. As side effects, UGI symptoms were recognized in 35.4% of regimen A patients and in 37.1% of regimen B patients. Bone marrow suppression appeared in 39.6% of regimen A patients and in 54.3% of regimen B patients.

Common duct carcinoma and obstruction in female hamsters treated with N-nitrosobis(2-oxopropyl)amine and/or cholecystectomy.

Detailed histologic observations were performed on the head of the pancreas of hamsters treated with 10 mg/kg body weight N-nitrosobis(2-oxopropyl)amine (BOP) once a week for 6 weeks with or without cholecystectomy. Cholecystectomy was performed 5 weeks before starting BOP initiation. The incidence of head cancers was 100% and cholecystectomy did not affect pancreatic carcinogenesis by BOP. Common bile duct dilatation was produced by advanced pancreatic head carcinomas and microadenocarcinomas in common duct. Micro-adenocarcinomas were not macroscopically detected since the tumors were located in the lumen of common duct.

[Clinical study of the NCC-ST-439 EIA kit using serum from patients with various types of cancer and benign disease (1)].

The amounts of antigens in the serum of patients with various types of cancer and benign diseases were measured in order to clinically evaluate the NCC-ST-439 EIA kit. From the results of measurements in 583 healthy persons, cut-off values of 4.5 U/ml for males and females 50 years or older and 7.0 U/ml for females 49 or younger were set. The false-positive rate for healthy persons at this time was 5.0%. The positive rates for malignant tumors at these cut-off values were 58.9% for pancreatic carcinoma, 53.1% for cholangiocarcinoma, 41.9% for mammary carcinoma, 36.6% for rectal and colonic carcinoma, 33.3% for hepatocellular carcinoma and 35.3% for carcinoma as a whole. The false-positive rates for benign diseases were 14.0% for pancreatic disease, 9.7% for liver disease, 8.0% for biliary duct disease and 8.3% for benign diseases as a whole. These rates were low when compared with those for other tumor markers. These results showed that NCC-ST-439 is a tumor marker with a high degree of specificity.
