Long term survival case of small (oat) cell carcinoma of the rectum.

A long term survival case of small (oat) cell carcinoma of the rectum in a 39-year-old female is presented. She complained of anal pain and occasional anal bleeding. The tumor was located at the anterior wall in the lower rectum. Biopsy specimens revealed a carcinoid tumor. She underwent trans-anal local resection for the first time in December, 1980. Macroscopic findings of the resected specimen showed a small nodule, 0.4 by 0.4 by 0.5 cm, with yellowish cut-surface. Microscopically, the tumor deeply invaded the submucosal layer. The appearances were indistinguishable from pulmonary small (oat) cell carcinoma. Since lymphatic permeations were moderately recognized in the tumor, she underwent radical operation (Miles' operation) with lymphadenectomy. Microscopic findings of the resected rectum revealed an intramural metastatic lesion with marked lymphatic permeations in the submucosal layer 2 cm distant from the primary lesion. Up to date, there is no evidence of local recurrence or liver metastasis. Small (oat) cell carcinoma of the rectum easily metastasizes lymphogenously through the lymph system from an early stage of the development. Wide surgical resection will be needed to give a long term survival even if the tumor is extremely small.

Thyrotropin response to thyrotropin-releasing hormone in rats with hypothalamic knife cuts.

Regulation of the pituitary-thyroid axis in rats with hypothalamic knife cuts has been studied. Complete hypothalamic deafferentation, either limited to the median eminence and the arcuate nucleus, or including parts of the dorsomedial nucleus and the whole ventromedial nucleus caused an increase in thyrotropin (TSH)-releasing hormone (TRH)-induced TSH secretion. Using an immunocytochemical procedure, a few TRH-positive fibers were observed within the median eminence of the larger island, while almost no fibers were identified in the smaller island. The exaggerated TSH response to TRH appeared within 3 days after the surgery and lasted for at least 1 week, when blood thyroxine (T4) level was significantly lowered. Exogenously injected T4 could inhibit such responses of TSH in the deafferented rats in a dose-related manner. These results support the hypothesis that the increase in the TSH response to TRH following hypothalamic deafferentation is due, at least in part, to the lowered thyroid hormone level in the blood.

Intraoperative ultrasonography in screening for liver metastases from colorectal cancer: comparative accuracy with traditional procedures.

As a new screening procedure, intraoperative ultrasonography using high-resolution real-time instruments was used routinely in 84 colorectal cancer operations to scan the entire liver for liver metastases. In 10 operations (11.9%), intraoperative ultrasonography identified 14 previously unrecognized metastatic tumors, all of which were less than 2 cm in size and were nonpalpable. The sensitivity of this technique in detecting metastatic liver lesions (97.8%) was significantly superior (p less than 0.01) to that of preoperative ultrasound (41.3%), computed tomography (47.8%), and surgical exploration (58.7%), while the specificity of each test was comparable (approximately 90%). High-resolution intraoperative ultrasonography is safe, simple to perform and more accurate than preoperative imaging and surgical exploratory methods. Therefore its use is encouraged during colorectal cancer surgery for screening of liver metastases.

A new technique for esophageal reconstruction by combined skin and muscle flaps after failure in primary colonic interposition.

A new procedure is presented for esophageal reconstruction using the skin flap combined with the overlying muscle flap in a situation in which preceding cervical esophagostomy and antesternal colostomy have been performed. A conventional musculocutaneous flap may be used for the second stage of the operation. However, our method has several advantages. The skin canal, which is the most important part of this reconstruction, is created using the skin of the anterior chest wall without transferring cutaneous tissue from other places. Therefore, the operative procedure becomes simpler and the blood supply to the skin canal is more assured. While blood of the wall of the canal is supposed to be supplied initially from internal mammary and intercostal arteries, the muscle flap over the canal may subsequently provide more blood to the skin canal. In addition to sufficient blood supply, the length of a suture line for creating the skin canal is minimal, and, thereby, the possibility of postoperative leakage can be diminished. Because both esophagostoma and colostoma are not damaged in the second stage of the operation, strictures at these sites are not likely to occur. Furthermore, the muscle flap serves as a physical protector for the canal, and presents a suitable bed for the split thickness graft. Our two stage operation using combined skin and muscle flaps after antesternal colostomy is a safe, simple and assured technique for the compromised colonic segment at primary esophageal reconstruction. This procedure is also applicable to the patient in whom use of a stomach tube for esophageal replacement is attempted but primary cervical esophagogastrotomy is not possible.

[Effectiveness of multidisciplinary treatment combined with immunotherapy in esophageal cancer--analysis of 287 cases of carcinoma of the thoracic part of the esophagus in Kyushu and Yamaguchi districts].

The effectiveness of surgical adjuvant immunochemotherapy was studied on 287 patients with thoracic esophageal cancer in the Kyushu and Yamaguchi districts. Pre- and postoperative subcutaneous administrations of BCG, 10 mg once a week and an oral administration of PSK, 3 g a day, for as long as possible after surgery were instituted. The control group was designated as the one which neither PSK nor BCG was given. Most patients received chemotherapy and/or radiotherapy. Significant elongation of survival time was observed in immunotherapy patients with stage III and in those with non-curative resection, both having no lymph node metastasis.

[The criteria of radiotherapy for treating carcinoma of the esophagus].

The criteria of radiation therapy is given for the treatment of a carcinoma of the esophagus, said criteria decided by a committee of the Japanese Association for Radiotherapy Systems at the request of the Japan Radiological Society. The indication for radiation therapy is described and adapted to the new TNM Classification, based on the depth of the invasion of the tumor. The depth of the tumor invasion is divided into 4 stages and the radiological images for each are shown. To evaluate the effect of radiotherapy, cases are divided into curable and non-curable groups, according to the degree of changes of the radiological findings after irradiation, and each group is further subdivided into absolute and relative categories.

Growth hormone release induced by electrical stimulation of the basolateral amygdala, observed in pentobarbital anesthetized rats.

The aim of this study was to clarify the neural pathway leading to the growth hormone (GH) release when the basolateral amygdala (ABL) was electrically stimulated. Concentric bipolar stimulating electrode was implanted in the unilateral ABL. Blood samples were taken from a cannula implanted into the right atrium via the right external jugular vein. Electrical stimulation of the ABL for 10 min caused a significant increase in plasma GH level from resting value 27.5 +/- 5.7 ng/ml (mean +/- S.E.M.) to 62.2 +/- 7.5 ng/ml at the termination of stimulation. This increase in GH level was markedly augmented to 152.0 +/- 23.0 ng/ml after lesion of the periventricular hypothalamic nucleus (Pe), where somatostatinergic neurons send their axons to the median eminence. Lesion of the stria terminalis (st) fully or partly abolished GH release induced by ABL stimulation. These results suggest that stimulation of the ABL accelerates GH secretion. The st is an essential pathway for this release, whereas the activity of Pe-neurons is rather inhibitory to this release.

Effect of polyamines on in vitro reconstitution of ribosomal subunits.

The effect of polyamines on in vitro reconstitution of Escherichia coli 30S and 50S ribosomal subunits has been studied. Spermidine stimulated the reconstitution of 30S particles from 16S rRNA lacking the methyl groups on two neighboring adenines and total proteins of 30S subunits at least 1.6-fold. The reconstitution of 30S particles from normal 16S rRNA and total proteins of 30S subunits exhibited only slight spermidine stimulation. However, the optimal Mg2+ concentration of the reconstitution was decreased from 20 mM to 16 mM in the presence of 3 mM spermidine. In the absence of spermidine the assembly of 30S particles from normal 16S rRNA was more rapid than the assembly from 16S rRNA lacking the methyl groups on two neighboring adenines. The reconstitution of 50S particles from 23S and 5S rRNA and total proteins of 50S subunits was not influenced greatly by spermidine. Gel electrophoresis results, from reconstitution experiments of 30S particles from 16S rRNA lacking the methyl groups on two neighboring adenines and total proteins of 30S subunits, showed that the assembly of S1 and S9 proteins to 23S core particles was stimulated by spermidine during reconstitution. The relationship of polyamine effects on in vitro ribosome assembly from its constituents to in vivo ribosome assembly is discussed. The reconstitution of Bacillus subtilis 30S particles from 16S rRNA and total proteins of 30S subunits was also stimulated approximately 1.3-fold by 3 mM spermidine.

Detection of unrecognized liver metastases from colorectal cancers by routine use of operative ultrasonography.

The importance of diagnosis and detection of liver metastases cannot be overemphasized for the treatment and prognosis of colorectal cancers. As a new diagnostic technique, operative ultrasonography has been performed during 33 operations for colorectal cancers including three operations for metachronous liver metastases. Of these, in five patients (15.2 percent) ultrasonography using 5- or 7.5-MHz instruments identified metastatic tumors that had not been diagnosed during preoperative imaging studies or at exploration. Most of these tumors were approximately 1 cm in size and nonpalpable. Cases of these five patients are presented in this report. High-resolution operative ultrasonography is considered to be a valuable method for detection of unrecognized metastatic tumors and for precise localization and spatial assessment of these hepatic lesions. Because it is safe, simple, and highly sensitive, the routine use of operative ultrasound is encouraged during surgery upon colorectal cancers for systematic screening of metastatic liver tumors.

Polyamine binding sites on Escherichia coli ribosomes.

To determine the binding sites of polyamines on Escherichia coli ribosomes, ribosomal proteins crosslinked with polyamines through bifunctional reagents were analyzed. When 1,5-difluoro-2,4-dinitrobenzene was used as a crosslinking reagent, spermine was bound to S3, S8, S9, L1, L2, L3, L5, L6, L13, L18, L24, and L27 proteins. When dimethyl suberimidate was used, spermine was bound to S1, S3, S4, S5, S7, S8, S9, S15, L1, L2, L3, L6, L18, and L24 proteins. In addition to crosslinking with the above proteins, spermidine, when crosslinked with dimethyl suberimidate, bound to S2, S14, S20, L4, L5, L9, L13, and L16 proteins. The relationship between the binding site(s) of polyamines on ribosomes and the function of polyamines is discussed.

Formation of a compensatory polyamine by Escherichia coli polyamine-requiring mutants during growth in the absence of polyamines.

The amounts of normal and compensatory polyamines of polyamine-requiring Escherichia coli mutants grown in the absence of polyamines were determined. Although aminopropylcadaverine, a compensatory polyamine, was synthesized by MA135 (speB) and DR112 (speA speB), no aminopropylcadaverine or only small amounts of aminopropylcadaverine were synthesized by EWH319 (speA speB speC speD) and MA261 (speB speC), respectively. The average mass doubling times of MA135, DR112, MA261, and EWH319 grown in the absence of polyamines were 113, 105, 260, and 318 min, respectively. The correlation of these values with the sum of spermidine plus aminopropylcadaverine suggested that aminopropylcadaverine is important for cell growth in the presence of limiting amounts of normal polyamines. This hypothesis is supported by the results of aminopropylcadaverine stimulation of the in vitro synthesis of polyphenylalanine and MS2 RNA replicase and of its stimulation of the growth of MA261. For the following reasons, it was concluded that aminopropylcadaverine was synthesized preferentially from cadaverine made by ornithine decarboxylase: aminopropylcadaverine was synthesized in relatively large amounts in cells (MA135 and DR112) which possess ornithine decarboxylase; ornithine decarboxylase catalyzed the decarboxylation of lysine in vitro, and the in vivo formation of aminopropylcadaverine was inhibited by an inhibitor of ornithine decarboxylase.

Normal stomach wall and gastric cancer: evaluation with high-resolution operative US.

Operative ultrasound examinations were performed using a 7.5-MHz transducer during nine operations for advanced gastric cancers and during six for early cancers. Normal stomach wall showed five layers that corresponded to histologic structures. Fourteen of 15 tumors, including nonpalpable early cancers, were localized with ultrasonography. In addition, both depth of penetration and lateral wall extension of the gastric cancers were precisely determined with this technique. Our preliminary results indicate that high-resolution operative ultrasonography may become a diagnostic aid during gastric cancer operations.
