Sexually transmitted diseases in pregnant women in Lusaka.

The prevalance of sexually transmitted diseases has been known to be high in many African countries and preliminary investigations in Lusaka revealed a similar trend in Zambia. This paper presents an analysis of diagnoses made in 170 symptomatic pregnant women referred to the sexually transmitted diseases clinic and the screening results of 163 pregnant women in a routine antenatal examination. A high prevalance of gonococcal infection as well as syphilis was discovered in pregnant women. The implications of these findings on the possible complications and outcome of pregnancy are discussed and remedial measures suggested.

Phaeochromocytoma in pregnancy: report on two cases and a review of the literature.

Two cases of phaecochromocytoma in pregnancy are described. One case was diagnosed at autopsy and in the other case the tumour was successfully removed. The world and particularly African literature on the subject is briefly reviewed. The clinical feature diagnosis and management are discussed in the context of the reported cases.

Some aspects of coagulation and fibrinolysis in non-pregnant and pregnant Zambians.

Platelet counts, fibrinogen level, prothrombin time and the level of fibrinogen degradation products (F.D.P.) were studied in a group of Zambian women before pregnancy, during pregnancy, in labour and soon after delivery. The platelet count in the non-pregnant Zambian women were found to be lower than caucasian standard and there were significant falls in the platelet count in the second trimester of pregnancy, labour and after delivery. The fibrinogen level did not rise during pregnancy. The prothrombin time remained normal. The F.D.P. levels were found to be significantly higher in third trimester of pregnancy and after delivery than in labour. It is suggested that further study should be done of the other blood clotting factors which rise during pregnancy and the F.D.P. levels in the Zambian women.

Advanced abdominal pregnancy.

Two cases of advanced abdominal pregnancy have been reviewed and two live babies have been delivered. Emphasis has been given to suggestive clinical features which may help in the pre-operative diagnosis of this rare condition.

A review of 79 cases of eclampsia at University Teaching Hospital, Lusaka.

Seventy-nine cases of eclampsia (incidence 0.22%) are reviewed. The corrected maternal and perinatal mortality rates were 12.6% and 28.3%. Maternal and perinatal mortality were directly related to number of fits, severity of eclampsia, delay in onset of treatment, and duration of labour. Whereas the maternal mortality was 23 per cent in patients delivered vaginally and 8.6 per cent in patients delivered by caesarean section, the perinatal mortality was almost the same in both types of delivery.

Conjoint twins (1).

Two cases of Conjoined twins are reported. The etiology, incidence, diagnosis and management are discussed. It is suggested that if conjoined twins are diagnosed before labour, of if there is dystocia, Caesarean section is the treatment of choice.
