Intraabdominal granuloma reaction in rats after introduction of maize-starch powder.

A total of 32 Wistar rats were given 1, 10, 100 and 1000 mg glove powder (Biosorb) intraperitoneally for 4, 11, 18 or 25 days. Four control rats received physiological saline. Examination of the abdominal cavity displayed granulomatous inflammation which was clearly dose-dependent in the experimental animal, but not in the controls. A biphasic time-sequence of the granulomatous reaction was observed in those rats receiving 100 and 1000 mg Biosorb with a minimum at day 18. The mean size of the granules (8.1 microns) within the inflammatory tissue was almost identical with the size of powder granules found on the external surface of the gloves (8.8 microns). X-ray microanalysis demonstrated maize-starch additives of magnesium and aluminium. The study indicates a foreign body reaction to maize-starch. In addition, immunological factors may play a role later in the development of the disease.

Dysplasia in colorectal adenomas related to the presence of O-acylated sialic mucin and to morphometric measurements.

In a study of 41 surgically removed colorectal adenomas of unselected size, the dysplasia was graded as mild in 9, as moderate in 22, and severe in 10 cases. Twenty-four of the lesions were classified as tubular adenomas and 17 as tubulo-villous. The presence of O-acylated sialic mucin was for each of the adenomas assessed as normal (19 cases), intermediate (13 cases) or absent (9 cases). The absence of O-acylated mucin was in the present study found to be highly predictive for severe dysplasia (89%). The morphometric measurements of epithelial structures showed that nuclear shape factor and nuclear stratification index were most significantly related to the grades of dysplasia. In a multivariate analysis, the grading of O-cylated sialic mucin most significantly contributed to the grade of dysplasia, and to a lesser degree, nuclear shape factor and nuclear stratification index. The presence of villous elements and the size of the adenomas had no independent influence on the grade of dysplasia in this statistical analysis.

Mortality in rats on long-term parenteral nutrition.

The long-term effect of total parenteral nutrition (TPN) was studied in rats. A failure to thrive developed and all animals died within 40 days of TPN. Autopsy invariably demonstrated the increased weight and volume of the liver and spleen. Multiple fat emboli were found in the lungs of all animals, and multiple epithelioid cell granulomas in the liver of all and in the spleen of most animals. Chronical low grade infection could not be demonstrated. Microorganisms were not found in blood or tissue specimens and endotoxin could not be demonstrated in plasma samples.

Parenteral nutrition via the portal vein in rats.

A rat model was designed for continuous infusion of complete parenteral nutrition (CPN) via the hepatic portal vein. The purpose was to evaluate possible side effects compared with conventional continuous systemic CPN via the superior caval vein, which bypasses the liver. Observed side effects of transcaval CPN (increase of liver weight and of intraabdominal fatty tissue, granulomatous infiltration of liver and spleen, vascular changes and fat droplets in the lungs) were less when the nutritional substances were infused into the portal vein.

Cancer of the uterine cervix in Norway by histologic type, 1970-84.

A total of 6,202 patients with cancer of the uterine cervix was reported to the Cancer Registry in Norway from 1970 to 1984. Squamous cell carcinoma was reported in 86.1% of all cases, adenocarcinoma in 9.5%, and undifferentiated cancer in 3.6%; the heterogeneous group of "other malignant neoplasms" was 0.8% of all cases, most of which were sarcomas. During the 15-year period the average annual age-adjusted incidence rates for both squamous cell carcinoma and undifferentiated cancer decreased by 30 and 79%, respectively, whereas adenocarcinoma increased by 38%. The increase of adenocarcinoma was mostly confined to females 20-34 years of age. The decrease in incidence rates of undifferentiated cancer was observed in all age groups, whereas the fall in incidence rates for squamous cell carcinoma was demonstrated in females above 35 years of age only. Of all patients with squamous cell carcinoma, 55% were diagnosed in clinical stage I. Females with adenocarcinoma and undifferentiated cancer were diagnosed in stage I in 60 and 38% of the cases, respectively. The 5- and 10-year relative survival rates were highest for patients with squamous cell carcinoma at all stages, whereas a significantly less favorable prognosis was found for females with adenocarcinoma and undifferentiated cancer of the uterine cervix.

Risk of carcinoma following gastric operations for benign disease. A historical cohort study of 3470 patients.

The risk of stomach cancer was analysed in a cohort of 3470 patients who had had gastric surgery for benign disease between 1900 and 1969. In 87 patients (2.2%) stomach-stump cancer was diagnosed in the follow-up period 1970-84. By comparison with the total incidence of stomach cancer in the same region during the same time period, the observed versus expected ratio in the post-surgery group was 2.1 (p less than 0.001) and did not differ between men and women. At 5-10 years postoperative the risk of cancer was no different from that in the total population, whereas after 40-45 years it was 7.3-fold higher. The risk was unrelated to primary diagnosis or type of operation.

Risk of colorectal cancer in adenoma-bearing individuals within a defined population.

Assuming that all colorectal cancers develop from preexisting adenomas, the annual conversion rate, defined as the number of cancers occurring each year as a percentage of all adenoma-bearing individuals, was determined. The number of adenoma-bearing individuals in the living population of northern Norway was estimated in each cohort for the period 1974-76 by determining the prevalence of colorectal adenomas in an autopsy population of 271 consecutive cases, representative of the population of the area with regard to underlying causes of death. During the 10-year period 1974-1983 a total of 656 colorectal cancers were recorded among an estimated number of 26,419 adenoma-bearing individuals aged over 35 years. The annual conversion rate was found to be 0.25%, indicating that an average adenoma-bearing individual is only at a moderate risk of colorectal cancer. The annual conversion rates for individuals having large adenomas, or adenomas with villous structures, or severe dysplasia were roughly estimated to be 3%, 17% and 37% respectively, assuming that colorectal cancer develops from one of these sub-groups of adenomas only.

A morphometrical analysis of dysplasia in small adenomas of the large intestine.

In a morphometrical study of 40 colorectal adenomas less than 10 mm in diameter and of 10 specimens of normal mucosa, it was found that nuclear size, perimeter and shape-factor of the epithelial cells were significantly different in neoplastic when compared with normal tissues. The same was found for the volume fraction of stroma, gland spaces, goblet and non-goblet epithelium, for the gland diameter, nuclear stratification height and stratification index, but not for the epithelial height nor the total volume fraction of the epithelium. In a multivariate analysis, stratification index and nuclear size of the epithelial cells contributed most significantly to determination of the histological grade of dysplasia in adenomas. By the combination of these two morphometric variables 75% of all adenomas could be correctly allocated to the grade of dysplasia.

Prevalence and morphological features of adenomas of the large intestine in individuals with and without colorectal carcinoma.

The presence of adenomas in 164 surgical specimens of the large intestine from 81 males and 83 females with colorectal carcinomas was compared with the presence of adenomas in a general necropsy population in northern Norway. In the group of patients with colorectal carcinomas the observed prevalence of single or multiple adenomas was 5.9 times higher than expected from the prevalence in the general necropsy population. The observed-to-expected ratios of all adenomas (4.7), adenomas with villous elements (4.6), adenomas with moderate or severe grades of dysplasia (4.9) and adenomas 10 mm or larger (5.5) were also higher in the surgical series. The study indicates that factors which initiate the growth of adenomas and, to a minor degree, factors which promote the growth of adenomas to a large size are more prevalent in individuals with colorectal carcinoma than in the necropsy population. Factors determining the presence of villous elements and the grades of dysplasia of adenomas seem to operate similarly in adenoma-bearing patients with and without colorectal carcinoma.

Adenoma↗

The age-, sex-, and site-specific occurrence of adenomas and carcinomas of the large intestine within a defined population.

The prevalence of adenomas at autopsy is compared with the incidence of colorectal carcinomas within a defined population in northern Norway. The study shows that females are more inclined to develop adenomas than males at ages up to 55 years, which corresponds with the higher cancer incidence in younger females than males before the age of 65 years. At older ages males have both a higher prevalence of adenomas and a higher incidence of carcinomas than females. When the data are analysed in accordance with anatomical segment and age, a change in predilection site of adenomas from the distal part of the large intestine at younger ages to the proximal part at older ages was found in both sexes. A similar relative change in the subsite distribution was also found for carcinomas, but only in males, and about 10 years later.

Long-term continuous intralipid infusion in the thoracic duct in rats.

Intralipid was continuously infused into the thoracic duct for 12 days in rats. All remained clinically fit and had satisfactory weight gain only for the first week. Autopsy showed significantly increased lung weight in all rats and in some there were lymphatic fistulas and dilated thoracic duct. Microscopy revealed fat droplets, vascular changes and thickening of alveolar septa in the lungs. Epithelioid cell clusters were found in liver and spleen in some of the rats and Kupffer cell increase in most. Thus commercially available fat emulsion for parenteral use, when delivered by the natural route for natural chylomicrons, caused undesirable derangements in rats.

Ectopic thyroid tissue in the head and neck.

A review has been performed of thyroid ectopia seen during a 10 year period. There were five cases of total ectopia; three of these were lingual thyroids, one situated in the perihyoid region had been clinically diagnosed as a thyroglossal cyst, while the fifth presented as a tumor in the lateral neck. In the same period 41 thyroglossal cysts were removed. None of these showed uptake on technetium scintigraphy. Thyroid tissue was present in the cyst walls in 41% of cases. Scintigraphy should be performed routinely if the clinical picture is at all compatible with thyroid ectopia.
