[A case of remission of metastatic gastric cancer lesions in the chest and abdominal wall by 5'-DFUR administration].

52-year-old female was weakened and bed-ridden by metastasis of gastric cancer to the chest and abdominal wall, and peritonitis carcinomatosa 2 years after gastrectomy. One week after administration of 5'-DFUR, 1200 mg/d, p.o., a clinical effect was seen. In 3 weeks metastatic lesions had diminished in size, ascites had decreased and the patient had regained her appetite and put on weight. She recovered sufficiently to be able to do some daily work. Pathological degeneration and necrosis of cancer cells were observed. Although she died 8 months after remission, 5'-DFUR was effective in improving her condition and prolonging her life.

[An analysis of treatments of recurrent breast cancer in relation to the lesion].

One hundred fifty-four patients being treated for recurrent breast cancer were studied for response rates and prognosis according to first recurrent lesions in radiation, chemotherapy, hormone therapy and combined therapy groups. It was important to consider the sites of the recurrent lesions in the treatments. Although there was no significant difference in prognosis among the first recurrent therapy groups, the overall cumulative five-year survival rate of the patients who responded (to the initial treatment) was significantly better than that of those who did not. As for the treatment of recurrent breast cancer, it is important to consider hormone receptor, lesion and age and to choose the effective mode of treatment for the first (recurrence of the) disease.

[Changes in placental enzymatic activities in relation to estrogen production during pregnancy].

To study the mechanism of production of estrogen during pregnancy, the following in vitro and in vivo studies were undertaken; 1) Weight of human placenta and total estradiol (E2) levels in the maternal peripheral vein were measured at different weeks of gestation. 2) Changes in E2 levels after DHA-S 100mg loading were calculated at the 1st, 2nd and 3rd trimesters. 3) Steroid enzyme activities including sulfatase, 3 beta-HSD and aromatase in placenta obtained in the 1st, 2nd and 3rd trimesters were measured. Results were as follows; 1) The weight of the human placenta increased gradually as gestation progressed. Twofold placental weight was noticed from the 2nd to the 3rd trimester. 2) Total E2 in the maternal peripheral vein increased steadily, being 4.07 +/- 1.74 ng/ml at the 1st trimester, 27.72 +/- 11.67 ng/ml and 104.12 +/- 57.89 ng/ml at the 2nd and 3rd trimesters respectively. The increase in E2 from the 2nd to the 3rd trimester was greater than that of the placental weight. 3) Increases in E2 following DHA-S loading were 1.01 ng/ml at the 1st, 29.2 ng/ml at the 2nd and 98.2 ng/ml at the 3rd trimester. 4) No significant differences were observed between placental 3 beta-HSD and sulfatase activities in the placenta obtained at the three different stages of pregnancy, while aromatase activity was found to be significantly higher in the placenta of the 3rd trimester than that of 2nd trimester. These results indicate that the remarkable increase in estrogen production in the 3rd trimester may be explained partially by increased aromatizing enzyme activity in the placenta.

[Epidemiological studies on pancreatic cancer].

An epidemiological study on pancreatic cancer was performed, using for evaluation reports from the Ministry of Public Welfare and the National Pancreatic Cancer Registration Committee and cases experienced at the First Department of Surgery in Kobe University Hospital. The incidence of pancreatic cancer was found to have increased over the lost few decades. Aged subjects with family histories of malignant diseases and cases having past histories of chronic pancreatitis were affiliated with the high-risk group for this disease. Cigarette smoking and beverage consumption were also shown to be increased risk factors for pancreatic cancer. Poor results obtainable with the types of treatment presently available for pancreatic cancer indicated the clinical significance of applying screening methods for subjects in the high-risk group.

[Clinico-pathological studies of non-palpable breast lesions with mammographically detected microcalcifications].

Fourty-two patients with suspected non-palpable cancer of the breast due to microcalcifications detected by mammography were studied. Histologically, carcinoma was found in 17 cases (40.5%) and benign lesions in 25 cases (32 foci) at biopsy or operation. Eight cases of these carcinomas were noninvasive and seven were microinvasive. Axillary metastasis was present in only one case. Sites of calcifications were confined to the area of carcinoma in 12 patients; both within a carcinoma and adjacent benign lesions in 3; only within the adjacent benign diseases in two. The features of microcalcification was not always useful to differentiate accurately the benign from the malignant. Therefore, local excision is essential to ensure histological diagnosis in the non-palpable breast lesions with microcalcification.

[Concurrent medullary carcinoma adjacent to papillary carcinoma of the thyroid--a clinicopathological and electron microscopic study].

This paper describes an extremely rare case of the simultaneous occurrence of medullary and papillary thyroid carcinoma. A 31-year-old woman had two different coexistent tumors, i.e., medullary carcinoma (1.2 X 1.5 cm) adjacent to papillary carcinoma (2 X 1.8 cm) in the right lobe. The two different tumor cells revealed the characteristic features of each on the microscopic, histochemical and ultrastructural findings. Therefore, they must be derived from different origins. One lymph node contains metastases of mixed medullary and papillary carcinoma. Such an unusual case of concurrent medullary and papillary carcinoma seems to be interesting as to the histogenesis.

X-ray diffraction studies of enkephalins. Crystal structure of [(4'-bromo) Phe4,Leu5]enkephalin.

In order to investigate the structure-activity relationship of [Leu5]- and [Met5]enkephalins, [(4'-bromo)Phe4, Leu5]-, [(4'-bromo)Phe4, Met5]- and [Met5] enkephalins were synthesized and crystallized. The crystal structure of [(4'-bromo) Phe4, Leu5]- enkephalin was determined by X-ray diffraction method using the heavy atom method and refined to R = 0.092 by the least-squares method. The molecule in this crystal took essentially the same type I' beta-turn conformation found in [Leu5]enkephalin [Smith & Griffin (1978) Science 199, 1214-1216). On the other hand, the preliminary three-dimensional Patterson analyses showed that the most probable conformations of [(4'-bromo)Phe4,Met5]- and [Met5]enkephalins are both the dimeric extended forms. Based on these insights, the biologically active conformation of enkephalin was discussed in relation to the mu- and delta-receptors.
