Measurement of allergens associated with dust mite allergy. II. Concentrations of airborne mite allergens (Der I and Der II) in the house.

Assays of mite allergens (Der p I, Der f I and Der II) in the air of houses became feasible with the use of a low-noise air sampler and a sensitive radioimmunoassay described previously. The levels of the airborne allergens Der I (Der p I + Der f I) and Der II in the living room of 10 houses during usual domestic life were very low, 29.5 and 6.3 pg/m3, respectively, with a Der I: Der II ratio of 4.7:1. At the time of bedmaking, they greatly increased, about 1,000-fold, to 30,900 and 12,600 pg/m3, respectively, with a Der I: Der II ratio of 2.5:1. The amounts of Der I and Der II in the floor dust of the living room were 2,040 and 2,690 ng/g of fine dust, respectively, with a Der I: Der II ratio of 0.8:1. Der I seemed more prone to become airborne than Der II.

Some pharmaceutical properties of 3-hydroxypropyl- and 2,3-dihydroxypropyl-beta-cyclodextrins and their solubilizing and stabilizing abilities.

3-Hydroxypropyl- and 2,3-dihydroxypropyl-beta-cyclodextrins (3-HP- and DHP-beta-CyDs) with different degrees of substitution (D.S.) were prepared and their pharmaceutical properties were investigated. The aqueous solubility of 3-HP- and DHP-beta-CyDs was much higher than that of the parent beta-CyD and the dissolution of DHP-beta-CyD in water was endothermic. The acid- and alpha-amylase-catalyzed hydrolysis rates of 3-HP- and DHP-beta-CyDs were slower than those of the parent beta-CyD. The hemolytic activity (human erythrocytes) and local irritancy (rabbit muscle) of DHP-beta-CyD were considerably less than those of natural, methylated or other hydroxyalkylated beta-CyDs, and decreased with increasing D.S. The ability of the hydroxyalkylated beta-CyDs to remove cholesterol and proteins from human erythrocytes decreased with increasing D.S., and correlated well with their hemolytic activity. 3-HP-beta-CyD was a more effective solubilizer for poorly water-soluble drugs than the parent beta-CyD, and its stabilizing effect on chemically instable drugs was higher than that of the parent beta-CyD. The above data suggest a considerable pharmaceutical potential of 3-HP- and DHP-beta-CyDs as parenteral carriers.

[MR findings of metastatic brain tumors].

Fifteen cases with metastatic brain tumor(s) were examined on a 1.5T MR system and the results were evaluated along with images of contrast-enhanced CT. Gd-DTPA-enhanced T1-weighted imaging (T1WI) showed the best detectability of lesions and was followed by contrast-enhanced CT, T2WI, and non-enhanced T1WI in that order. No correlation was found between the MR signal intensity and the histological classification of the tumors. In differential diagnosis with glioblastoma, several signal characteristics were encountered. Low or isointensity as well as the discrimination of tumor from edema on T2WI was considered to suggest the diagnosis of metastatic tumor.

[Subdural hematoma from ruptured arteriovenous malformation; demonstration by MR imaging].

We studied MRI of 15 cases of arteriovenous malformation (AVM) with special reference on the coexistence of the subdural hematoma. All ruptured cases (6 cases) were associated with intracerebral hematoma, and half (3 cases) of them were coexisted with subdural hematoma. In one of them, the presence of the subdural hematoma promoted us to perform angiography which revealed the AVM.

[Re-operation of pectus excavatum].

We performed surgical reconstruction on 1655 cases of deformed thoracic cage, we later operated again on 11 of these to repair postoperatively re-deformed anterior chest walls. Based on these experiences, we have concluded as follows. 1: Postoperative recurrence of funnel chest deforming is mainly due to insufficient resection of costal cartilages. In particular transection of the sternum at low levels during sternal turn-over procedure results in postoperative recurrence of depression in the upper anterior chest wall. 2: In young children who have undergone sternal turn-over procedure, the first and second costal bones and cartilages overgrow and protrude anteriorly, and in compensation their junctions to the sternum recess posteriorly. This results in a depression in the upper anterior chest wall. 3: We recommend sternal turn-over with overlapping of the sternum for repair of postoperative funnel chest deformity. Through this procedure, the extent of resection of costal cartilages can easily be determined and the depression of the anterior chest wall satisfactorily reconstructed. 4: In re-do surgery, we obtained pathological evidence confirming our clinical experience that our sternal turn-over technique does not interfere with blood circulation or development of the turned-over sternum even though the sternum is not connected to the rectus abdominus muscle pedicle, preserved internal mammary vessels, or anastomosis of the internal mammary vessels.

Drug solubilizers to aid pharmacologists: amorphous cyclodextrin derivatives.

Conversion of crystalline alpha-, beta-, or gamma-cyclodextrins into amorphous mixtures of water soluble derivatives yields non-toxic solubilizers which dissolve drugs through the formation of inclusion complexes. From these types of compounds 2-hydroxypropyl ethers of cyclodextrins have presently been investigated and the ranges for the safe use in working with (a) receptor binding assays on membrane preparations, (b) cells in vitro, and (c) parenteral use in mice were established for these compounds. The drugs which were investigated were dissolved in amounts linearly proportionate to the concentration of the solubilizers used and did not precipitate upon dilution by aqueous media. These solubilizers may considerably facilitate pharmacological evaluation of new, water insoluble potential drugs.

Clinical characteristics and long-term prognosis of patients with variant angina. A comparative study between western and Japanese populations.

To determine the factors influencing the prognosis of variant angina, the clinical characteristics and long-term prognosis of 158 consecutive Japanese patients were examined and compared with those in previous major western studies (Pisa, Montreal, and Duke studies). The Japanese patients were characterized by relatively low prevalences of coronary risk factors, significant coronary stenoses and previous myocardial infarction. Survival and survival without myocardial infarction for the entire group or for the subpopulation with significant coronary artery disease were significantly better in the Japanese population than in the western populations; however, in the subpopulation without significant coronary artery disease, the prognosis was excellent in all four studies. If the prevalence of coronary artery disease was corrected for the Japanese population, there would be no difference in the prognosis between the Japanese and the western populations. It is concluded: (1) the overall prognosis of variant angina may be better in Japanese patients, and (2) coronary artery disease appears to be the strongest prognostic factor for assessing the differences in the prognosis between the Japanese and the western populations.

Amorphous water-soluble cyclodextrin derivatives: 2-hydroxyethyl, 3-hydroxypropyl, 2-hydroxyisobutyl, and carboxamidomethyl derivatives of beta-cyclodextrin.

The pharmaceutical usefulness of natural, crystalline cyclodextrins can be improved by chemical conversions into water-soluble, amorphous mixtures of their derivatives. Reaction of beta-cyclodextrin with 2-chloroethanol, 3-chloropropanol, isobutylene oxide, or iodoacetamide yielded the title compounds. Distributions of the substitution degree were close to symmetrical and relatively narrow. The average substitution degrees increased with the amount of alkylating reagent used in the preparation. The number of components (half-width of distribution) increased with increasing average substitution degree. Further, distributions of the substitution degree were measured in glucose derivatives after hydrolysis of 2-hydroxyethyl, 2-hydroxypropyl, and 2-hydroxyisobutyl-beta-cyclodextrin. The results show an uneven distribution of substituents around the cyclodextrins, suggesting that growth of oligoglycol side chains and/or clustering of substituents on one glucose residue occurs.

[A study of the cardiotocograms recorded by a separated 1MHz ultrasound transducer].

Fetal heart rate records were obtained in 12 antepartum and 13 intrapartum cases, with an ultrasonic Doppler transducer with five separated crystals and a conventional transducer. The loss of the FHR record due to increased noise in 30 minutes before delivery ranged from 1.1 to 23.2% (6.4 +/- 6.5%) in the separate type transducer, and it was 0.6 to 29.9% (10.5 +/- 7.6%) in the conventional one. During the 5 minutes before delivery, it was 3.3 to 58.0% (20.2 +/- 18.5%) in the separate type, and 3.3 to 70.0% (31.6 +/- 19.5%) in the conventional one. Clearer FHR records were obtained with the use of the separate type than the conventional one (p less than 0.05). Noise was mostly recorded in the 5 minutes before delivery. In the NST, the loss of the FHR record due to maternal posture change decreased when the new transducer (p less than 0.05) was used. The loss was 0 to 56.7% (14.9 +/- 18.2%) with the new type, and 2.0 to 100% (42.9 +/- 36.4%) with the conventional one for the maternal posture which showed the worst record. By using the separate type transducer, maternal posture need not to be limited in the NST, and the somatic and mental discomfort is less than when the conventional one is used.

[Studies on the cardiotocograms recorded by a new system with 1 MHz ultrasound and autocorrelation heart rate meter].

Improved FHR records were obtained by the use of a new 1 MHz ultrasound transducer and autocorrelation heart rate meter. The results were compared with conventional 2.5 MHz continuous wave ultrasound device. The rate of the loss of FHR recording was 0 to 6.6% (2.8 +/- 2.1%) in the new device, and it was 2.1 to 23.5% (8.7 +/- 6.2%) in the old one. The loss of FHR record in the 2nd stage of labor was 3.2% when 1 MHz ultrasound and new device were used, and it was 2.0% with direct fetal ECG signal using scalp electrode and the same device. More clear record was obtained with use of ultrasound at a crowning of fetal head than that of direct fetal ECG. In the use of 1 MHz ultrasound, 51% cases in 55 patients showed undisturbed FHR record 1 minute before delivery of the fetus. Slightly noisy but analysable FHR record was obtained in 36%. Hence, the FHR records were clearly evaluated until 1 minute before delivery of the infant in 87% of the cases. Maternal heart rate signal was mixed with FHR in 2 cases in routine intrapartum ultrasonic FHR monitoring. However, the maternal signal disappeared after moving the transducer to other position where fatal heart beat was more clearly detected.

Effect of L-threo-3,4-dihydroxyphenylserine (L-DOPS), an immediate precursor of norepinephrine, on the cerebral blood flow in rats.

L-threo-3,4-Dihydroxyphenylserine (L-DOPS), a norepinephrine (NE) precursor, 3 mg/kg, i.v., increased the cerebral blood flow (CBF) in both the striatum and hippocampus as well as the mean arterial blood pressure (MABP) in urethane-anesthetized rats, as NE infusion did. The L-DOPS induced increase in CBF was inhibited by benserazide (3 mg/kg/hour), a peripheral aromatic L-amino acid decarboxylase inhibitor, and propranolol (3 mg/kg, i.p.), a beta-adrenoceptor blocker as well. These results suggest that the effects of L-DOPS may be attributed to the action of NE formed from L-DOPS, and the action may be mediated by stimulation of beta-adrenoceptor.

[Clinical study of carumonam].

To evaluate the clinical efficacy of carumonam (CRMN, AMA-1080), the drug was used in the treatment of 10 patients including 4 with pneumonia and each with acute tonsillitis, chronic bronchitis, Mycoplasma pneumonia, primary atypical pneumonia (PAP), chronic pyelitis, and acute cystitis. Since beta-lactam antibiotics were not active against Mycoplasma pneumonia and PAP, these diseases were excluded from the clinical efficacy evaluation of CRMN. Responses were excellent in 1 patient and good in 7. Side effects were not observed. The laboratory test recognized slight elevations of GOT, GPT and eosinophilia in 1 patient and a slight leucopenia in another upon the administration of CRMN.
