Clinical report of diabetic patients treated by chronic hemodialysis.

Twenty one diabetic patients were treated by chronic hemodialysis (HD) in the seven year period from 1975 to 1982 in our Department. The actual one-year and five-year survival rate for diabetic patients treated by maintenance HD were 76% and 43%, respectively. Blood urea nitrogen and serum creatinine levels at the HD induction were 111.95 +/- 28.51 mg/dl and 10.73 +/- 4.56 mg/dl, respectively. There were complications in the form of blindness in 8 cases (38.1%), peripheral nerve impairment in 17 cases (81.0%), myopathy in 8 cases (38.1%), diabetic gangrene in 3 cases (14.3%) and glaucoma in one case (4.8%).

Separation of mono-, di-, and trihydroxy stereoisomers of bile acids by capillary gas-liquid chromatography.

Capillary gas-liquid chromatographic separation was studied for the complete set of the 26 theoretically possible isomers of mono-, di-, and trihydroxylated 5 beta-cholanic acids, which differ from one another in the number, position, and configuration of hydroxyl groups at C-3, C-7, and/or C-12 in the nucleus, as well as for some of their related acids. The bile acid samples were chromatographed as their methyl ester-trimethylsilyl (TMSi) ether derivatives and analyzed on three capillary columns coated with nonpolar OV-1, slightly polar OV-17, and polar SP-2340 as liquid phases. The retention times on capillary gas-liquid chromatography (GLC) responded dramatically to the minor structural differences, and almost complete separation of the positional and stereochemical isomers was achieved by the combined use of SP-2340 and OV-17 (or OV-1) capillary columns.

Complication of renal neoplasms with cystic disease: an indication for nephrectomy in potential transplant recipients.

Thirty-five (1.1%) out of a total of 3,133 autopsied cases of renal tumors were complicated by renal malignant tumors and cystic renal disease. They accounted for 1.4% of 2,417 cases of renal malignant tumors and for 4.7% of 751 cases of cystic renal disease. Among the 35 complicated cases, 10 cases or 28.6% were free of metastasis. The incidence of complicated cases and those without metastasis aged over 60 years or over was 12.2% and 29.2% respectively, while the 10-59 year-old age group displayed a lower incidence of 3.6% and the same incidence of 27.3% respectively. In 26 out of 35 cases the malignant tumors was renal adenocarcinoma with no evidence of metastasis in nine cases (34.6%). Although it is unknown whether cystic renal disease undergoes malignant changes, not all of the patients undergoing chronic hemodialysis should be indicated for nephrectomies since such patients with renal malignancies are mostly elderly and are frequently free from metastases.

Report on blood access for hemodialysis in the Department of Transplantation, Tokai University School of Medicine and a satellite hospital.

There were 316 patients being treated by maintenance hemodialysis in the Department of Transplantation, Tokai University School of Medicine and its satellite hospital (Bohsei Clinic, Medical Treatment Cooperation, Showakai) as of December 31, 1981. Forty eight male (23.1%) and 39 female (35.7%) patients were operated on for external shunts. One case of external shunt was functioning well 48 months later. No patient was dialysed through external shunts on December 31, 1981 in our Department or the satellite. The majority of patients, who were maintained on hemodialysis with an arteriovenous (native vein) fistula available, experienced no difficulties. Nearly 50 percent of cases with the first fistula were functioning on December 31, 1981. There were some complications with fistulas such as pseudoaneurysms, infections, the sore thumb syndrome or the steal syndrome. We experienced 14 hemodialysis cases using expanded polytetrafluoroethylene (E-PTFE) grafts and one case using femoral artery repositioning as blood access.

Brain metastasis of renal adenocarcinoma.

Among 1,828 autopsy cases of adenocarcinoma reviewed, 177 cases (9.7%) were found to have brain metastasis, and their autopsy records were analyzed to investigate the mode of dissemination of the cancer to the brain. Brain metastases were most frequent in the age group of 50-59 years but showed no significant difference in incidence between sexes or location (right or left) of the primary lesion. The cases with brain metastases had greater frequencies of metastases in the lungs, heart, thyroid and skull than cases without brain metastases. Lung metastasis was most frequent in cases with two-organ metastases, and lung metastasis with involvement of the hilar lymph nodes in cases with three-organ metastases. Metastasis to the brain was significantly more frequent in nephrectomized cases than in non-nephrectomized cases. Metastases to the contralateral kidney and peritoneum were significantly more frequent in nephrectomized cases with brain metastasis. Possible routes of dissemination of the cancer to the brain are discussed.

Renal function after nephrectomy in renal donors.

Six months to 5 years after nephrectomy in renal donors, creatinine clearances and PSP tests of 34 cases had recovered to 81.8% and 68.2% respectively of prenephrectomy performance. Blood urea nitrogen, creatinine and uric acid levels had increased to 24.9%, 36.7% and 24.3% respectively of prenephrectomy values. There was compensatory hypertrophy of the remaining kidney. The mean increase in size of the remaining kidney was 12.0% x 17.8%. There were early complications in 11 (32%) of the donors, in the form of pneumothorax in six cases, wound infection in two cases, hepatitis in two cases and urinary tract infection in one case. There was one late complication of neuralgic pain in the scar region.

Metastasis of urothelial tumors of the bladder: correlation between sites and number of organs involved.

Urothelial tumors of the bladder in 2,561 autopsy cases were classified according to the number of organs involved in metastasis, and a comparison was made concerning the frequency of the metastases in various organs. In cases with single organ metastasis, lymph node and lung involvement was low (20% and 14% respectively), but it increased as the number of metastatically involved organs increased. The frequency varied depending on the number of organs involved. Three basic types of metastases were established. Type 1: An increase in metastases was seen rectilinearly in accordance with the number of organs involved. Metastases to the lymph nodes (overall), lungs, liver, bones (overall), rectum and retroperitoneal lymph nodes belonged to this group. Type 2: The frequency was low in cases with metastasis to one or two organs, but markedly increased when three or more organs were involved. Metastases to the adrenals, kidneys and pancreas belonged to this group. Type 3: The frequency was almost unchanged, regardless of the number of organs involved. Metastases to the prostate and perivesical lymph nodes belonged to this group. The number of metastatically involved organs is useful in analyzing the mode of metastasis.
