[Fundamental and clinical evaluations of 9, 3"-diacetylmidecamycin in the field of surgery (author's transl)].

The distribution of sensitivity to 9,3"-diacetylmidecamycin (MOM) was examined of 10 strains of Staphylococcus aureus isolated from foci in our hospital. A level of less than 3.13 mcg/ml was seen distributed in the majority of 7 cases, and it could be assumed that MOM would be sufficiently effective against infections by Staphylococci in clinical practice. In the comparison with other antibiotics popularly used, the sensitivity of the 10 strains to them was comparable to those against MDM, but were slightly inferior to cephalexin (CEX) and lincomycin (LCM), and slightly better than ampicillin (ABPC). The evaluation of clinical results revealed that, of the 20 cases with surgical and superficial infections, 6 and 10 were markedly and moderately effective, respectively, while 3 and 1 were fairly and poorly effective, respectively. It was therefore concluded to be an effective remedy for treatment of surgical and superficial infections in which Gram-positive cocci share the majority of the etiological factors. Of the side effects, there was none showing symptoms in digestive tracts or abnormalities such as rash, nor was there any patient for whom further administration of the agent had to be suspended due to side effects. No abnormality was seen in the test results between the premedication and postmedication findings on the hemogram, or hepatic and renal functions. It was therefore conjectured that this agent has a large margin of safety in its application.

Estimation of state and amount of phenylalanine residues in proteins by second derivative spectrophotometry.

The second derivative absorption spectra of serum albumin, insulin, ribonuclease and lysozyme were measured under various conditions to determine the state and amount of their phenylalanine residues. The second derivative spectra of these proteins were very similar to that of phenylalanine in the region between 245 and 270 nm where tryptophan and tyrosine residues caused no appreciable interference. Denaturation of proteins with urea or guanidine hydrochloride caused decrease in the intensity of the second derivative spectra, but scarcely affected the positions of peaks and troughs. The amounts of phenylalanine residues in proteins calculated from a second derivative spectra of denatured proteins coincided well with those reported in the literature. The states of the phenylalanine residues in the proteins could be deduced from the change in optical intensity on denaturation.

An experimental study on histamine H2-receptor antagonist on calcium, gastrin and histamine induced gastric acid secretion in rat.

The suppressive effect of histamine H2-receptor antagonist, cimetidine, on gastric secretion was investigated in Ghosh-Schild rat. The study above was done in basal state under infusing normal saline (1 ml/h) and stimulated state by histamine-di-chloride (3.5 mg/kg-h), tetragastrin (50 mcg/kg-h) or calcium chloride (4 mg/kg-h). Dose related increase of cimetidine (1.7, 3.5, 7.0 and 14.0 mg/kg-h) were observed and correlated with the degree of inhibition of acid secretion. Cimetidine had a potent inhibitory activity on either basal and stimulated acid output by the agents above. Basal acid secretion was completely abolished by 3.5 mg/kg-h of cimetidine to the level of anacidity. The degree of inhibition by the same dose of cimetidine was different among the agents used as stimulant on acid secretion and it followed in the order of calcium, gastrin and histamine subsequently. This study indicated that histamine H2-receptor participated the gastric secretion induced by either gastrin or calcium other than histamine itself. This fact indicated the important role of endogenous histamine in gastric secretion induced by calcium and gastrin.

Cross-reaction between the mycelial galactomannas of three Hormodendrum strains and the mannans of two Candida albicans strains of different serotypes, A and B.

Cross-reactivity between galactomannans of three representative Hormodendrum strains, H. pedrosoi IFO 6071, H. compactum IFO 6726, and H. dermatitidis IFO 6421, and the mannans of two Candida strains, C. albicans NIH A-207 (serotype A) and C. albicans NIH B-792 (serotype B), were investigated by means of agar-gel double diffusion, quantitative preciptin reaction, and passive cutaneous anaphylaxis. The following results were obtained. (i) Antisera to whole cells of three Hormodendrum strains were completely inactive in all in vitro antigen-antibody reactions to mannans of C. albicans NIH A-207 and C. albicans NIH B-792, whereas antisera to both C. albicans strains were found to be cross-reactive against the three Hormodendrum galactomannans. (ii) The results of cross-passive cutaneous anaphylaxis tests using guinea pigs sensitized with antiserum of three Hormodendrum and two Candida seemed to be consistent with those of the vitro assay. (iii) The reactivities of the corresponding acid-resistant core moieties of the three Hormodendrum galactomannas against two anti-C. albicans sera were nearly identical to those of the parent galactomannans in both in vitro and in vivo tests.

Studies of fibrinolytic activity of uremic and longterm hemodialytic patients with special reference to fibrinolytic inhibitor.

Fibrinolytic activities on cases with chronic renal failure, 13 cases at pre and post introduction of hemodialysis and 40 cases at pre and post hemodialysis of maintenance hemodialysis with 38 normal controls were investigated. The plasminogen activator of untreated chronic renal failure was lower than controls, yet increased with the introduction of hemodialysis. On the other hand the antiplasmin was lowered with the introduction. In cases with maintenance hemodialysis, the plasminogen activator was lower than controls, but was elevated with hemodialysis. The level of antiactivator was higher in uremia of either pre or post hemodialysis than controls. The levels of alpha 2-macroglobulin and alpha 1-antitrypsin were reduced at prehemodialysis state comparing to controls, and yet, increased with hemodialysis, respectively. The low molecular weight antiplasmin and antiactivator, molecular weight below 30,000 were separated with Sephadex G-50 gel filtration of plasma. The low molecular weight fibrinolysis inhibitors of plasma with untreated uremia were elevated comparing to controls, but decreased with the hemodialysis. The removable fibrinolytic inhibitors were indicated, however, the ratio of the low molecular weight fibrinolytic inhibitors to the total fibrinolytic inhibitors were little.

Innervation of the canine inferior vena cava. Distribution of adrenergic and cholinergic excitatory fibers among the embryologically distinct segments.

The innervation of three embryologically distinct segments of the canine inferior vena cava was investigated. These segments were termed A (supradiaphragm), B-C (intrahepatic and that between liver and renal veins), and D (infrarenal). Strips were cut from these segments, and their isometric tensions were recorded. Transmural electrical stimulation induced contractile responses in circular strips from segment B-C (66.5% of the maximum norepinephrine-induced response) and in those from D (14.4%), but not in A. These responses almost completely disappeared in the presence of phenoxybenzamine. In segment B-C, however, the remaining small contraction was markedly enhanced by neostigmine and abolished by atropine. The same phenomenon was also observed in the contraction remaining after reserpinization. Longitudinal strips from segment C responded similarly. Concentration-response curves of circular strips for acetylcholine were shifted by neostigmine markedly to the left only in segment B-C, while no significant shift occurred in A and D. It was concluded that the adrenergic innervation is remarkably dense in B-C, sparse in D, and probably lacking in A. In addition, a cholinergic excitatory innervation is present in segment B-C.

Acute infantile gastroenteritis caused by rotavirus in Japan.

We had an outbreak of acute infantile gastroenteritis accompanied by milky-white stool (called Hakuri in Japanese) during the winters of 1976 and 1977. Stool specimens collected from 72 cases of Hakuri were studied by negative-staining electron microscopy. Rotavirus was detected with a very high frequency (89%). Rotavirus obtained from one of the patients was isolated and passaged in cultures of primary human embryonic kidney cells. Viral antigens could be detected in the cytoplasm of the cells by indirect immuno-fluorescence. The fluorescence-positive cells increased in number with repeated passage. Serum anti-viral activities in 11 patients were titrated by indirect immuno-fluorescence, using the cells infected with the passaged rotavirus. All 11 patients developed IgM responses in the convalescent phase. However, in 4 of the 11 patients, no IgG responses were detected even 2--3 weeks after the onset of illness. The reinfection which has occasionally be seen in our country may be related to these poor IgG responses.

The effect of motilin on the gastrointestinal propulsion in rats.

We studied the gastrointestinal propulsion in unanaesthetized rats by 51Cr method that permits the simultaneous determination of two functions of the gastric emptying and the intestinal propulsion. The results obtained are as follows: 1) The semi-logarithmic presentation of the gastric emptying showed a linear relationship with the time. 2) When synthetic motilin was subcutaneously injected at the dose of 1 microgram/kg, the gastric emptying was promoted to a statistically significant extent.

Relationship between phosphate content and immunochemical properties of subfractions of bakers' yeast mannan.

The mannan of bakers' yeast (Saccharomyces cerevisiae) was fractionated on a column of diethylaminoethyl-Sephadex into five subfractions. Phosphate content of these mannan subfractions was proportional to the concentration of NaCl solutions used in the chromatographic separation. Quantitative precipitin reactions showed that the serological reactivities of the subfractions were proportional to the content of phosphate. The result of acetolysis study showed that the amounts of mannotetraose and phosphate-containing oligosaccharide fractions increased proportionally to the acidity, whereas the amount of mannose decreased inversely. The results from quantitative precipitin reaction tests and acetolysis study demonstrated that both phosphate contents and multiplicity of branching moieties of mannan subfractions increased proportionally, i.e., micro-heterogeneity concerning the acidity comprised in the parent bulk mannan is not attributable merely to the coexistence of molecular species containing different amounts of phosphate but also to the presence of more of the branching moieties.
