Fibrinolytic activity in biopsied colonic mucosa was examined in patients with ulcerative colitis, and most cases were found to have increased tissue fibrinolysis -- due mainly to tissue plasminogen activator -- in the affected mucosa. Five cases, 4 with elevated tissue fibrinolysis and 1 normal, were treated with an antifibrinolytic agent, tranexamic acid (trans-AMCHA) administered as an enema, to inhibit fibrinolysis of the affected mucosa directly. In patients with elevated mucosal fibrinolysis, 2 showed complete remission after tranexamic acid enema alone, and there was one remission in response to combination treatment with oral prednisolone. One case with slightly elevated mucosal fibrinolysis showed clinical improvement, although the radiological findings were unchanged. No response was observed in one case with normal tissue fibrinolysis. It is concluded that tranexamic acid may show a therapeutic effect in ulcerative colitis with elevated mucosal fibrinolysis when administered via an enema, which allows direct contact of the drug with the affected mucosa.

Investigation was made on absorption, excretion and clinical responses following intravenous drip infusion of amikacin (AMK) and the following results were obtained. 1) Serum concentrations of AMK were determined after completion of a 1 hour-course of its intravenous drip infusion at a dose of 200 mg, involving 2 patients with normal renal function and 1 patient with impaired renal function. In the patients with normal renal function, a mean peak serum concentration of 15.9 micrograms/ml was reached at the termination of the intravenous drip infusion and, then, gradually declined to 0.97 microgram/ml at 6 hours after completion of the intravenous drip infusion. The half-lives were 1.64 and 1.41 hours, respectively. In the patients with impaired renal function, the elimination of AMK from blood flow was markedly delayed and the serum concentration was 5.52 micrograms/ml even at 6 hours after completion of the intravenous drip infusion. The half-life was prolonged, its value being 5.98 hours. 2) The mean urinary excretion rate for the patients with normal renal function was 67.7% within 7 hours, while the patient with impaired renal function had a lower rate of 14.0%. 3) Ten infectious patients with hematological malignant diseases were treated with AMK intravenous drip infusion. Clinical responses were excellent in 6, good in 2, fair in 1 and poor in 1. Thus, 80% of the patients responded to AMK. 4) In order to check the side effects of AMK, investigation was made on the results of various clinical laboratory tests and the subjective and objective findings. But, no side effects attributed to AMK were observed. Therefore, it is thought that the intravenous drip infusion of AMK will be very useful when attention is paid to infusion time, dose and interval.
Hemopoietic function of the marrow and splenic stroma in C57BL/6 mice aging from 2 to 24 months were evaluated with the use of subcutaneous implantation method. Although there was no significant difference in the concentration as well as the total number of CFU(S) in femoral marrows between aged and young mice, the rates of CFU(S) repopulation in the femoral marrows from aged donors were significantly lower than in those from young donors when grafted subcutaneously in the 2 month-old syngeneic hosts. On the other hand, the femoral marrow grafts from 2 month-old donors were constantly repopulated irrespective of the age of the hosts. An essentially similar finding was obtained in the study of spleen grafts. It is suggested that the defective hemopoiesis, if any, in aged mice is primarily caused by the changes in hemopoietic stroma rather than those in hemopoietic stem cells.

Role of lipid peroxidation on lysosomal instability in liver tissue was investiaged in an experimental model of D-galactosamine hepatitis in rats fed on vitamin E (V.E) deficient diet. Administration of D-galactisamine to V.E deficient rats resulted in a sudden increase of serum glutamic oxaloacetic transaminase (sGOT), glutamic pyruvic transaminase (sGPT), lipid peroxide value, as well as beta-glucuronidase and acid phosphatase activity examined as markers of lysosomal enzymes, when compared with control rats fed on V.E supplemented diet. Lipid peroxide in the liver tissue also showed significant increase in V.E deficient rats. In contrast, beta-glucuronidase and acid phosphatase in the liver tissue were found to decrease in V.E deficient rats by the administration of D-galactosamine, indicating that the enzymes in the lysosome were entirely released outside the liver cells as a result of cell destruction. It is concluded that the increase of lipid peroxide causes the instability of lysosomal membranes and releases various kinds of hydrolytic enzymes to lead further to cell damage. V.E might act on inhibiting lipid peroxidation to stabilize lysosomal membranes.

Streptococcal preparation OK-432 (Picibanil), clinically being used as an immunopotentiator, has been shown to activate the complement system either through the classical or the alternative pathway in vitro [15]. In this experiment, OK-432 was found to increase serum complement level in guinea pigs, and in human without malignancy, when investigated by hemolytic assay using sensitized sheep erythrocytes (EA) for the classical pathway activity and unsensitized rabbit erythrocytes (RaE) for the alternative pathway activity. Assay of complement components revealed a significant increase in C3, but decrease in Clq, while no specific tendency was observed in C4, C5, C9, properdin, C3 activator and Cl-inhibitor. These evidences suggested that OK-432 might potentiate immune response of the host by elevating serum complement level, in addition to activate the complement system.
