Long-term follow-up study of cerebral palsy children and coping behaviour of parents.

The purpose of this study was to understand: (a) the feelings, thoughts and actions of parents at the time their children were diagnosed as having cerebral palsy; (b) the crisis periods in raising their children; (c) the important persons who supported the parents during the 'acceptance' phase; and (d) the roles of mothers and fathers in raising the children. The subjects were 28 mothers and fathers who had sons or daughters with cerebral palsy. The latter offspring were aged 22-29 years at the time of this study. They were interviewed at their homes with a semi-structured method retrospectively. The results showed that most parents became aware of their children's disability in infancy and most of their children were diagnosed as having cerebral palsy by around 2 years of age. The mothers' reactions to the diagnosis were emotional and those of the fathers were realistic in coping with the problem. Although crisis periods arose throughout the periods of growth and development of the children, the infancy period was the more critical for mothers and the toddlerhood, school-age and adolescence periods were more critical for fathers. The important support people were the spouses. The mothers took care of the children while the fathers provided an income; however, some fathers withdrew from competing for achievement in their jobs.

Giant cell fibroblastoma. A case report.

A case of giant cell fibroblastoma occurring in the knee of a 16-month-old girl is reported. The ill-defined subcutaneous tumor measuring 2 x 2 cm was composed of diffusely proliferating spindle-shaped tumor cells with scattered, atypical multinucleated giant cells in a myxoid or collagenous background and irregularly branching sinusoid-like tissue spaces. In addition to floret-type giant cells, a few osteoclast-like giant cells were present in a cellular area where tumor cells were focally arranged in a storiform pattern. Immunohistochemically, the tumor cells gave positive reactions for only vimentin and actin. In spite of the high recurrence rate of this type of tumor, the course of the patient after excision of the tumor has been uneventful. It is important to distinguish this rare, peculiar fibrous tumor from other soft tissue tumors including some sarcomas.

Grating Discs: a simplified method of testing vision in infants and young children.

We have developed a simple new method of evaluating visual function in young children about 15 to 36 months of age. Our Grating Discs system consists of six discs with a striped pattern and one homogeneous disc. One striped disc and the homogeneous disc are presented simultaneously, and the examiner observes whether the subject can discriminate between the two. The patterned discs have vertical stripes of different widths from 16 to 0.5 mm. At a testing distance of 57 cm, the visual resolution required for discriminating the grating ranges from 96 min of arc (0.01) to 3 min of arc (0.33) in one-octave steps; at a testing distance of 114 cm the required resolution ranges from 48 min (0.02) to 1.5 min (0.67). Using this system, we evaluated the visual acuity of normal infants and young children. Their ability to discriminate the finer gratings improved with increasing age, and more than 90% of the normal subjects in the 36-month group tested at 0.67. We also examined young children with known ocular pathologies and compared the results with acuity measurements obtained with the preferential looking test. The latter method gave acuities slightly better than those from the Grating Disc system, but the finding with the two techniques were generally in good agreement. Grating Discs are a simple, compact, effective tool for rapid assessment of visual function in young children.

[An in vitro study on the treatment of complicated cystitis using an automatic simulator].

A computer simulation of a Kidney-bladder model was prepared for automatic control of the urine concentration of antibacterial drug administrated. The following results were obtained in an investigation of the problems of complicated cystitis in vitro using ofloxacin. 1) It was suggested that the time required for elimination of the bacteria was prolonged as the level of complication of the disease state in the urinary tract increased. 2) In the model of highly complicated cystitis caused by urinary flow stagnation, recurrences were observed in vitro even with administration of a large dose of ofloxacin. 3) The time required for a elimination of bacteria tended to become shorter as the dose of the antibacterial agent increased. 4) The time until the start of regrowth of the bacteria tended to be prolonged as the dose of the antibacterial agent increased. 5) It was suggested by our in vitro experiment that if the background disease became more complicated, it would be more difficult to treat, as in the case of complicated cystitis.

[Detection of human papillomavirus DNA in the normal genitalia of men].

The study of human papillomavirus (HPV) in the genitalia of all normal males is difficult, therefore cases of chronic prostatitis, a common disease treated in urological clinics, were chosen to identify HPV DNA by using Vira Type (Life Technologies Inc.). Smears of glans and sulcus coronalius of 177 subjects, showed a HPV positive rate of 3.4%, while 86 cases of those cases were negative for HPV in urethral smears. The lack of clinical findings suggests that HPV is an asymptomatic infection. In a follow up examination of 5 HPV positive cases some weeks later, smears of glans, sulcus coronalius and urethra were all negative for HPV. Examination by Vira Type showed that HPV disappeared spontaneously in these cases.

[Study on auxotype and serotype of Neisseria gonorrhoeae].

The auxotypes of 592 strains of Neisseria gonorrhoeae isolated in Sapporo between 1980 and 1989 were investigated. The serotypes of some of these strains were also studied, and the relation between the auxotype and the sensitivity to Amoxicillin (AMPC) was analyzed. The results were as follows. 1. The auxotypes of Neisseria gonorrhoeae isolated in Sapporo were similar to those distributed in Southeast Asia. 2. In a study of the auxotypes by year, the frequency of proto and Pro- decreased, while the frequency of AHU- increased. 3. The serotypes of all strains of Neisseria gonorrhoeae investigated were WII/WIII, there was no WI serotype. 4. With respect to the relation between auxotypes and sensitivity to AMPC, the AHU- strains were more sensitive than proto or Pro-.

[Study on new quinolone-resistant strains isolated from urine--annual changes of its frequency and the relation to drug volume].

There is concerned that the new quinolone-resistant strains have increased along with its widespread usage. We analysed the annual changes in frequency of ofloxacin-resistant strains isolated from urine in the past four years at two different types of hospitals, department of urology in Sapporo Medical College and Muroran City Hospital, since the usage of these agents seem to be related to the annual changes of the frequencies. The results were summarised as follows: 1) In the two hospitals, drug volume of the new quinolones had been increased, in particular, the past six years from 1984. 2) The annual changes in frequency of ofloxacin-resistant Staphylococcus aureus have been increasing from 0 to 41.2 percent in Sapporo Medical College and 16.7 to 96.7 percent in Muroran City Hospital. The frequency of ofloxacin-resistant Pseudomonas aeruginosa also have been increasing from 24 to 66.7 percent in Sapporo Medical College and 37.5 to 81.8 percent in Muroran City Hospital. 3) The frequency of ofloxacin-resistant indole positive Proteus spp. and Serratia marcescens for four years at Sapporo Medical College (indole positive Proteus spp.: 0-1.8 percent, Serratia marcescens: 10-43 percent) was very different from that at Muroran City Hospital (indole positive Proteus spp.: 65-82 percent, Serratia marcescens: 71-100 percent). The difference seems to be caused by the hospital acquired infection.

[The infection rate of Chlamydia trachomatis in young adult men without urogenital symptoms--screening test using urine sediments].

The detection of C. trachomatis antigen in first-voided urine sediments has recently been achieved by means of IDEIA CHLAMYDIA (IDEIA, Novo Nordisk), an EIA kit using monoclonal antibodies. Therefore, this kit was used as a screening test to examine the infection rate of C. trachomatis in young adult men without symptoms. The titers of serum IgA and IgG antibodies to C. trachomatis were also determined. 1. Antigen detection from first-voided urine sediments of young adult men without urogenital symptoms (141 cases): The detection rate by IDEIA was 5.0% (7/141). Three of the 7 cases which were positive for antigen in first-voided urine sediments were reaffirmed as having asymptomatic C. trachomatis urethritis, since they were also revealed to have C. trachomatis in the urethra. 2. The positive rates of serum antibodies: The titers of serum antibody were determined in 128 cases out of the 141 cases. The positive rates of IgA and IgG were 6.3% (8/128) and 35.9% (46/128) respectively. The positive rates of IgA and IgG antibodies were significantly higher in cases with positive antigen in first-voided urine sediments than in those which were negative. These results indicate that this kit is useful for antigen detection. 3. The screening test revealed asymptomatic C. trachomatis infections in 5% of young adult men, suggesting the extensive spread of the infection. The screening test using first-voided urine sediments will be useful in public health.

[Serotypes of Chlamydia trachomatis in urogenital infection in Sapporo].

The distribution of serotypes of Chlamydia trachomatis in urogenital infection in Sapporo was studied by typing 30 isolates with Monoclonal antibodies and by testing patient sera from 134 individuals with the micro-immunofluorescence test. 1. Among 30 isolates, the majority of strains were identified to be types D and E (23%), followed by G (13%), F (10%), H (10%), I (7%) J (7%) and K (7%) in this order. 2. In 134 patients sera, the most frequent antibody types were B/ED (64.8%), followed by C/J (13.3%), G/F (7.8%), H/I/J (7.1%) and K (2.3%). Thus, serovar D and E were the same predominant type of Chlamydia trachomatis in urogenital infection in Sapporo, as in the USA and Europe.

[Studies on the detection rate of Chlamydia trachomatis in married and unmarried pregnant women--field survey in Hokkaido].

Chlamydia trachomatis infection in the general population has recently been attracting attention. In this regard, we examined the incidence of positive antigen in the genital organs in pregnant female women in order to investigate the prevalence of this infection. EIA (Chlamydiazyme) was performed in 5,000 married pregnant women, and in 317 unmarried women who underwent artificial termination of pregnancy. The study was carried out between June 1986 and September 1989 in Sapporo City (3,932 subjects), Kushiro City (328 subjects), Muroran City (280 subjects), Kitami City (357 subjects), Kucchan Town (220 subjects) and Urakawa Town (200 subjects). 1. Among married pregnant women, the detection rate of C. trachomatis was 6.1% (222/3,666) in Sapporo City, 7.3% (24/328) in Kushiro City, 6.1% (17/280) in Muroran City, 7.8% (24/306) in Kitami City, 6.8% (15/220) in Kucchan Town and 7.5% (15/200) in Urakawa Town, showing no particular difference according to the area. The overall detection rate was 6.3% (317/5,000). 2. The rates of detection of C. trachomatis antigen by EIA were 22.9% (61/266) and 15.7% (8/15) for the unmarried women who underwent artificial termination of pregnancy in Sapporo and Kitami, respectively. Both rates were higher than those in the married pregnant women. This result suggests the prevalence of C. trachomatis among young women showing a high level of sexual activity. 3. Among married subjects, the detection rate was 21.3% for those in their late teens, 8.9% for those in their early 20s, 6.0% for those in their late 20s, 3.7% for those in their early 30s, and 2.9% for those 35 years or older; thus, the younger they were, the detection rate became elevated. The above findings show that latent epidemics of C. trachomatis infection are present to a considerable extent among young women.

[Study of the effect of combination therapy of human granulocyte-colony stimulating factor (G-CSF) and amikacin on experimental pyelonephritis induced by Pseudomonas aeruginosa in neutropenic mice].

We reported the prophylactic and therapeutic effect of human granulocyte-colony stimulating factor (G-CSF) on mice with ascending pyelonephritis induced by Pseudomonas aeruginosa (G-group). In the cyclophosphamide-treated neutropenic mice, the prophylactic administration of G-CSF (2 micrograms/day/mouse) yielded a lower incidence of infection than that of saline alone. However, the therapeutic administration of G-CSF (2 micrograms/day/mouse) did not produce decreases of the rate, suggesting that this type of administration had no effect on infection. Thus, we investigated the effect of the combination therapy of G-CSF and Amikacin. In neutropenic mice, the therapeutic administration of G-CSF alone and Amikacin (20 mg/kg) alone did not produce decreases of incidence of infection. But, combination administration of these yielded a lower incidence of infection. These results suggest that synergy of bactericidal effects of neutrophils accelerated by G-CSF with Amikacin, and a combination of these have a therapeutic effect on bacterial infection in neutropenic mice.

Effect of optical defocus on the steady state pattern reversal visual-evoked response.

The effects of optical defocusing with convex lenses on the amplitudes of pattern reversal visual-evoked response (PVER) were investigated. With the large check size and high-contrast pattern, PVER amplitudes showed a linear decrease in response to initial defocusing up to +5 to +6 dptr. As the degree of defocus increased, the PVER amplitude, though irregular, continued to show reliable, definite responses up to +25 dptr. With the intermediate check size and pattern contrast, PVER amplitudes displayed a linear decrease, but unlike the large-check/high-contrast condition, diminished to noise level after a defocus of greater than +4 to +5 dptr. From these results, we speculate that two phases in this function curve derive from the large-check/high-contrast condition: first, the contrast- or contour-dependent phase, and, second, the luminosity/movement-dependent phase.

Synthesis of 7-thio-substituted 4-oxoquinoline-3-carboxylic acids with antibacterial activity.

A series of C-7 thio-substituted 1-cyclopropyl-1,4-dihydro-4-oxoquinoline-3-carboxylic acids were prepared and tested for their antibacterial activity. Structure-activity relationships associated with the C-5 and C-7 substituents were discussed. Among the C-7 substituents including alkylthio, arylthio, heteroarylthio, and cyclic aminothio groups, a 2-aminoethylthio group was the best for enhancing in vitro antibacterial activity. The C-5 variants increased activity in the order OH less than F less than H less than NH2. Of compounds prepared in this work, 5-amino-7-(2-aminoethyl)thio-1-cyclopropyl-6,8-difluoro-1,4-dihydro-4 -oxo-quinoline-3-carboxylic acid (18) was the most active.

The effects of new cytochalasins from Phomopsis sp. and the derivatives on cellular structure and actin polymerization.

The effects of ten 10-phenyl-[11]cytochalasins produced by Phomopsis sp. including novel compounds having 5,7- or 6,7-glycol structures and their derivatives, on the cell morphology and actin distribution in C3H-2K cells, as well as on lymphocyte capping and actin polymerization, were examined. The structure-activity relationship reported in the previous papers has been confirmed. The novel glycol type compounds showed little or no activity, suggesting the importance of the perhydroisoindol-1-one nucleus for the manifestation of the cytochalasin actions.

Mechanical and electrical properties of smooth muscle cells and their regulations by endothelium-derived factors in the guinea pig coronary artery.

Effects of high K+, acetylcholine (ACh), 9,11-epithio-11,12-methanothromboxane A2 (STA2), thrombin and endothelin were investigated on smooth muscles of the guinea pig coronary artery in intact and endothelium-denuded tissues. In intact tissues, ACh transiently inhibited but ATP produced maintained inhibition of the STA2-induced contraction. However, in the endothelium-denuded tissue, ACh produced contraction and ATP inhibited the STA2-induced contraction. In intact tissues, thrombin produced dual actions on the STA2-induced contraction with an initial relaxation followed by contraction. In endothelium-denuded tissues, thrombin enlarged the STA2-induced contraction without transient relaxation. In intact tissues prepared from both proximal and distal regions, endothelin showed the same dual action as observed with thrombin, whereas higher concentrations of endothelin showed only contraction. In endothelium-denuded tissues, endothelin consistently produced contraction. In intact tissues prepared from proximal and distal regions, ACh produced a biphasic response (initial hyperpolarization and subsequently generated depolarization). The amplitudes of both potential changes occurred in a membrane potential-dependent manner. In endothelium-denuded tissues, ACh depolarized the membrane in both tissues. In intact and endothelium denuded tissues, ATP hyperpolarized the membrane in inverse proportion to the membrane potential level, whereas thrombin and endothelin consistently depolarized the membrane. The results indicate that ACh acts on endothelial and smooth muscle cells, and the former releases both EDRF and EDHF. ATP only acts on smooth muscle cells and hyperpolarizes the membrane. STA2, thrombin and endothelin act on both endothelial and smooth muscle cells. STA2 and endothelin may release EDRF but not EDHF, and thrombin may release EDRF and endothelin.

Intracranial and intraspinal dissemination from pineal yolk sac tumor treated by PVB therapy--case report.

A 15-year-old male was admitted because of diplopia and persistent headache. Postcontrast computed tomography (CT) revealed a homogeneously enhanced large mass, 3 x 4 cm in size, in the pineal region and moderate obstructive hydrocephalus. A right ventriculoperitoneal shunt was installed. At that time, the serum alpha-fetoprotein (AFP) level increased to 23,036 ng/ml, but the level of serum beta-subunit of human chorionic gonadotropin was less than 0.2 ng/ml. These data indicated the tumor to be a pure yolk sac tumor. Following cisplatin-vinblastine-bleomycin (PVB) therapy and whole-brain irradiation (50 Gy), the tumor disappeared on CT, although the AFP level did not return to normal. Eight months after the completion of initial therapy, he had lumbago. Spinal magnetic resonance imaging revealed a metastatic mass at the L5-S2 levels, which was subtotally removed and histologically diagnosed as yolk sac tumor. Postoperative local irradiation (30 Gy) was performed. Seven months after the operation, spinal dissemination at the Th7 level occurred and, 1 month later, intracranial dissemination in the left cerebellopontine angle was detected. He died 25 months after the first admission. PVB therapy did not prevent spinal dissemination in this case.

HLA-DP-positive T cells in patients with systemic lupus erythematosus.

HLA-DP+ T cells in peripheral blood from 23 patients with systemic lupus erythematosus (SLE) were examined using two-colour flow cytometry analysis. A marked increase of HLA-DP+ T cells was observed in patients with SLE (20.5-98.7%; 59.8 +/- 20.8%) in comparison to normal subjects (1.3-20.6%; 11.1 +/- 7.2%), and the ratio of these cells greatly exceeded that of the HLA-DR+ T cells (6.5-49.1%; 21.5 +/- 12.7%). This high frequency of HLA-DP+ T cells in patients with active SLE decreased with prednisolone therapy. When the lymphocytes from normal subjects were stimulated with PHA in vitro, HLA-DP+ T cells increased from 1.8 to 59.2%. Therefore, it appears that the HLA-DP antigen expression on T cells is a practical marker for monitoring changes in the proportion of activated T cells in patients with SLE during the course of therapy.
