Platelet-activating factor biosynthesis in rat vascular smooth muscle cells.

The ability of platelet-activating factor (PAF) receptor antagonists to protect rats from the cardiovascular collapse induced by large doses of endothelin 1 led us to examine the capacity of rat cultured vascular smooth muscle cells to produce PAF and also to evaluate its potential functional roles in this cell type. Adenosine triphosphate and the vasoactive peptides, endothelin 1, angiotensin II, and arginine vasopressin, each elicited an increase in the PAF level in extracts of rat cultured vascular smooth muscle cells as determined by bioassay. PAF was not detectable (above 20 fmol/mg protein) in the supernatants of these cells. The identity of the bioactivity as PAF was confirmed by GC/MS which indicated that more than 80% of the PAF was 1-O-hexadecyl-2-acetyl-3-sn-glyceryl-phosphorylcholine. Exogenous PAF (100 nM) elicited increases in intracellular calcium that were inhibited by WEB 2086 (10 microM). Endothelin 1, at a concentration which stimulated PAF synthesis, (1 nM), elicited increases in intracellular calcium levels that were not inhibited by WEB 2086 (10 microM). Thus, endogenous PAF is unlikely to be involved in the endothelin-1-induced calcium increases. Although WEB 2086 (3-100 microM) inhibited concentration dependently fetal calf serum (10% v/v) induced [3H]-thymidine incorporation, reaching a maximum effect at 30 microM of 40-50% reduction, in parallel experiments WEB 2086 had no effect on serum-induced increases in cell numbers. We conclude that PAF is produced and retained by cultured rat vascular smooth muscle and that it is unlikely to contribute to the signaling of increases in intracellular calcium or proliferation.

Sustained nonischemic ventricular tachycardia during dobutamine stress echocardiography.

Dobutamine stress echocardiography has become widely accepted as a safe, reliable and cost effective modality for the evaluation of patients with suspected myocardial ischemia or for prognostic stratification and outcome assessment in patients with known coronary artery disease. while the benefits of this means of cardiovascular testing are very clearly apparent, it is important to understand and recognize possible complications. This paper discusses the occurrence of sustained, nonischemic ventricular tachycardia during dobutamine stress echocardiography in the absence of functional, physiologic or anatomic evidence of coronary artery disease or cardiomyopathy.

Blood pressure experience and risk of cardiovascular disease in the elderly.

For the 1254 persons in the Framingham Heart Study who survived to age 65 without prior cardiovascular disease or prior use of antihypertensive medications, significant univariate correlates of the development of cardiovascular disease after age 65 included (1) the systolic blood pressure at age 65, (2) the average systolic blood pressure before age 65, and (3) the slope of blood pressure change up to age 65. After controlling for the systolic blood pressure at age 65, average pre-age 65 blood pressure remained significant (p less than 0.05) and the slope of the pre-age 65 blood pressure was marginally significant (p = 0.06). Even after controlling for the mean of up to three blood pressure measurements at age 65, an average systolic blood pressure of 160 mm Hg or greater before age 65 was an independently significant predictor of the post-age 65 development of cardiovascular disease (rate ratio = 1.79; 95% confidence interval = 1.04, 3.07). These data suggest that even after performing multiple measurements at a single examination, knowledge of past systolic blood pressure history, especially if it averages 160 mm Hg or greater, adds a small but statistically significant increment in predicting future cardiovascular disease in the elderly.

Involvement of platelet-activating factor in endotoxin-induced priming of rabbit polymorphonuclear leukocytes.

Endotoxin primes polymorphonuclear leukocytes (PMNs) for increased superoxide anion (O2-) generation in response to the chemotactic peptide, formyl-methionyl-leucyl-phenylalanine (fMLP). We have investigated the role of platelet-activating factor (PAF) in the priming of rabbit PMNs by endotoxin. In non-primed PMNs, fMLP stimulated PAF generation, but none was detected in the supernatant, whereas in primed PMNs PAF generation increased 17-fold and a large proportion of the total PAF formed was released. There was a close relationship between the concentration-response curve for increases in PAF synthesis and that for enhanced O2- generation. The possibility that PAF was causally linked to enhanced O2- generation in primed PMNs was investigated using compounds previously reported to reduce PAF generation, namely the serine protease inhibitor, tosyl-phenylalanine chloromethylketone, and the phospholipase A2 inhibitor, bromophenacylbromide. Both compounds reduced PAF generation and release in endotoxin-primed PMNs and prevented the enhancement of O2- generation. We examined the possibility that PAF acted in an autocrine fashion to enhance O2- generation following its release from fMLP-stimulated PMNs. However, rabbit PMNs, unlike human PMNs, were not primed by exogenous PAF (0.1-1000 nM) over a concentration range extending beyond that covering the concentrations of PAF determined in the supernatants of fMLP-stimulated, endotoxin-primed PMNs. We conclude that PAF release is unlikely to play a role in priming induced by endotoxin in rabbit PMNs, but our data are not inconsistent with a role for cell-associated PAF in this phenomenon. Furthermore, the release of a large amount (80%) of the total PAF from primed, but not non-primed PMNs suggests a cell-cell mediator role for PAF only in primed PMNs.
