A comparative study of IgA nephritis in Japan and Germany. An approach to its geopathology.

A comparison of clinical and pathological features in 357 Japanese cases and 29 German cases of primary IgA nephritis was performed. Male population was higher than female in the German cases (2.6:1), while it was almost even in the Japanese (1.07:1) (p less than 0.05). Age distribution was similar. At the time of detection of the disease, asymptomatic patients were more frequent in Japanese (78%) than in German cases (60%) (p less than 0.05). The most common findings were mild mesangial proliferative glomerulonephritis in both series (65% in Japanese, 61% in German cases), but severe histological changes were more frequent in German patients (25%) than in the Japanese (16%). The incidence of IgA nephritis in primary glomerulonephritis was 38.8% in Japanese and 11.9% in German cases, and the incidence in mesangial proliferative glomerulonephritis was 65.6% in Japanese and 29.2% in German patients (p less than 0.01). It is considered that the divergent incidence of IgA nephritis may be a real geopathological event, although its variability may be intensified by different indications for biopsy or different population. It is suggested that various possible pathogenetic agents may be irregularly distributed throughout the world, which may contribute to the divergent clinical and pathological manifestations.

Effectivity of zotepine in refractory psychoses: possible relationship between zotepine and non-dopamine psychosis.

We reported the results of a survey of patients who suffered from schizophrenia with predominate hallucinatory and delusional states, and who had been unresponsive to a variety of antipsychotics like haloperidol but responded more favorably to zotepine. In 10 of the 22 zotepine-responsive patients in this study, there was marked improvement with zotepine. Considering the results from previous drug treatment, the phenothiazines (especially levomepromazine) surpassed the butyrophenones in efficacy, suggesting that zotepine might resemble levomepromazine clinically. After administration of zotepine, cenesthesic hallucination, behavior caused by hallucination, egorrhoe, affective symptoms, and catatonic symptoms were markedly improved. However, insight into disease and negative symptoms were minimally improved after administration of zotepine. Zotepine was effective in the refractory psychoses due to its potent action on the delusional dynamics, in spite of producing little marked improvement in such main symptoms as hallucination and delusion. The authors speculate that zotepine's potent activity at serotonin-1 receptors may lead to a beneficial effect in refractory psychoses, just as in the case of its antimanic effect.

Parathyroid adenoma imaged by gallium-67 citrate. A case report.

A parathyroid adenoma imaged by Ga-67 citrate in a 17-year-old man with primary hyperparathyroidism and a palpable solid tumor in the neck is presented. Although preoperative examination and intraoperative findings suggested a parathyroid carcinoma, histologic studies showed a parathyroid adenoma with predominant chief cell type.

Microwave surgical treatment of the prostate: clinical application of microwave surgery as a tool for improved prostatic electroresection.

Microwave surgery as a tool for improved prostatic electroresection is introduced. Prior to electroresection the prostate was coagulated with 2,450 MHz of microwave which was emitted from the bipolar electrode of a specially designed probe. We conducted a comparative study of 35 patients with bladder neck obstruction. There was a microwave coagulation group treated with subsequent transurethral resection (TUR) and a conventional TUR group. Both groups were analyzed for the amount of blood loss, irrigant absorption and the frequency of complications. Both during and following surgery, the former group had a significant reduction in blood loss and had no complications. We conclude that the combination procedure of microwave coagulation and TUR can minimize the disadvantage of formal TUR and may be of value in the treatment of patients with both prostatic obstruction and of hemorrhagic diathesis as well as in high risk patients.

[Treatment of severe uveitis with cyclosporin A].

The results of a treatment with ciclosporin A in 4 patients with severe uveitis (2 cases with Behçet's disease, 2 cases Vogt-Koyanagi-Harada disease) are presented. Ciclosporin A produced a rapid regression of exudates in the ocular fundus and of the inflammatory signs at the anterior segment in the 4 cases. However, at the same time, side effects including the deterioration of the hepatorenal function developed. Interruption of the treatment with ciclosporin A led to the development of inflammation at the anterior and posterior segments in cases 2 and 4.

Oral leukoplakia and costochondral hyperplasia induced by diethylnitrosamine in hamsters exposed to cigarette smoke with or without dietary vitamin C.

Male Syrian golden hamsters receiving 12 weekly subcutaneous injections of diethylnitrosamine (DEN) were subjected to cigarette smoke-inhalation and fed a diet or without 1% vitamin C supplement for a period of 58 weeks. Another group was a sham-smoked control and was not fed vitamin C. Tissues of the oral cavity and costal cartilage were examined by light and/or scanning electron microscopy. Oral leukoplakia and costochondral hyperplasia occurred with high frequency in all groups treated with DEN. Leukoplakic lesions were found in the palate, tongue, and pharynx; the early change was focal erosion with mild epithelial hyperplasia and inflammatory cell infiltration. Advanced lesions had marked mucosal thickening due to acanthosis, parakeratosis, hyperkeratosis, and submucosal infiltration of lymphocytes and plasma cells. Precancerous lesions were noted in tongue and pharynx. Scanning electron microscopy of tongues revealed destruction of filiform papillae. The incidence of leukoplakic lesions were higher in smoke-exposed hamsters than in controls, but the incidence in vitamin C-supplemented hamsters was low when compared with the smoke-exposed hamsters without vitamin C. Costochondral hyperplasia was initiated by thickening of the perichondrium followed by proliferation of chondrocytes. Costochondral hyperplasia appeared earlier, and the incidence was higher in the vitamin C-supplemented hamsters. It could not be determined whether costochondral hyperplasia was the primary lesion induced by DEN or secondary change.

Trigeminal sensory evoked potentials in patients with trigeminal neurinoma: report of two cases.

The trigeminal sensory evoked potentials (TEPs) in two patients with trigeminal neurinoma are reported. In contrast to the two cases previously reported by others, in which the preoperative TEPs were normal, the two cases reported herein showed significant changes on the affected side: the sensory thresholds were greater and the peak latencies of P19 and N28 were longer on the affected side than those on the normal side. Our results confirm that a TEP measurement is very useful in a preoperative examination of patients with trigeminal neurinoma, when the function of the trigeminal nerve should be evaluated objectively.

Pontine urine storage center in the dog.

Electrical stimulation of the presumptive pontine urine storage center, areas ventral to and/or lateral to the pontine micturition center, elicited an increase in threshold volume at which reflex micturition occurred in response to bladder filling with physiological saline, suggesting a facilitatory effect on the urine storage function. The electrolytic lesions made in the stimulated areas were histologically identified to distribute at the nucleus reticularis pontis oralis, Kölliker-Fuse nucleus and subcoeruleus. These nucleus seemed to work as a pontine urine storage center and to perform a reciprocal action with the pontine micturition center.

Spontaneous tumors in F344/DuCrj rats from 12 control groups of chronic and oncogenicity studies.

The types and incidences of spontaneous tumors in F344/DuCrj rats were examined in 960 males and 959 females served as the control groups of separate twelve 2-year chronic and oncogenicity studies carried out during a 1978-1983 period. The major tumors occurred at more than 5% incidence were leukemia (mononuclear cell), testicular interstitial cell tumor, preputial gland adenoma, pituitary anterior adenoma, thyroid C-cell adenoma, adrenal pheochromocytoma and subcutis fibroma for males, and leukemia, uterine endometrial polyp, pituitary anterior adenoma, thyroid C-cell adenoma and mammary gland adenoma/fibroadenoma in females. Analyses on age-related occurrence of tumors revealed that the incidences of most of the major tumors in males attained already to the plateau between 85 and 97 weeks of age while those in females showed a steep rise during the last interval of observation, 98 to 110 weeks of age. An intralaboratory heterogeneity in incidence was observed in the thyroid C-cell adenoma and the adrenal pheochromocytoma for males, and the anterior pituitary adenoma for females.

A clinical evaluation of transcatheter arterial embolization on hepatocellular carcinoma.

TAE was performed in 49 cases of HCC for clinical assessment of the usefulness. This procedure was aimed to subside intraperitoneal hemorrhage in 8 cases among them and to provide a possible antitumor effect in the remaining 41 cases. The prognosis in the 8 cases of intraperitoneal hemorrhage was 54.9 +/- 41.1 days including one case survived as long as 116 days. The prognosis in the remaining 41 cases was 12.6 +/- 8.3 months in group A, 7.8 +/- 6.1 months in group B and 1.6 +/- 1.3 months in group C according to Child's classification, while it was 2.8 +/- 2.5 months in 1st branch occluded, 9.4 +/- 6.1 months in IInd branch occluded and 19.2 +/- 6.7 months in IIIrd branch occluded group according to the portal vein occlusion due to tumor thrombi suggesting that a more prolonged survival was attained with more favorable degree of Child's classification and less affected portal embolization. The cumulative survival time (by Kaplan-Meier's methods) was 6 months in 89% of the cases examined, 1 year in 59%, 2 years in 34% and 3 years or more in 11%, indicating significantly higher survival as compared to our TAI group. Angiographic re-opening of tumor vessel within 3 months was observed in 46.7%. TAE on HCC was useful both for the purpose of antitumor effect and of hemostasis. The degree of Child's classification and severity of portal occlusion at the initial examination may closely relate to the prognosis. Thus, angiography should be repeated within 3 months following the first TAE at least.
