Multiple congenital epulides in a newborn: with special reference to reported cases in Japan.

This report describes a case of multiple epulides which simultaneously occurred on the maxilla and the mandible of a 6-day-old female baby. The clinical findings, diagnostic procedure, surgical treatment, histopathology, and 2-year follow-up of this case are presented. Based on the clinicostatistical and histopathologic investigations with special reference to the pertinent Japanese literature, a possible histogenetic etiology of this entity is discussed. Any odontogenic component might play an important role in the development of this tumor. This case of multiple epulides is considered to be very rare.

[The evaluation of ultrasonic oocyte collection techniques for IVF-ET--comparing the transabdominal and transvaginal methods].

In order to evaluate the effectiveness and safety of ultrasonic oocyte collection for IVF-ET, a comparative study was performed on the results of IVF-ET by the transabdominal and transvaginal methods in 145 cycles of 51 patients. The oocyte collection rate for the transvaginal method was significantly higher than that for the transabdominal method (77.1% vs. 68.7% per follicle, p less than 0.01). The majority of the oocytes were collected in three follicular flushings. The fertilization rate for oocytes collected by the transabdominal method was significantly higher than that for those collected by the transvaginal method in cases of tubal damage and endometriosis (81.1% vs. 65.6% per oocyte, p less than 0.01). There was no significant difference between the signs of complications seen with the two methods. Six successful pregnancies were obtained by IVF-ET in our hospital (4 cases by the transabdominal method, and 2 cases by the transvaginal method). Two of these cases have already delivered full term healthy male infants. In conclusion, these results indicate that ultrasonic oocyte collection has been established as the most effective method for IVF-ET by means of either the transabdominal or the transvaginal methods.

Pancreatitis in the annulus of annular pancreas demonstrated by the combined use of computed tomography and endoscopic retrograde cholangiopancreatography.

Annular pancreas, an uncommon condition, is often hard to diagnose before operation. We report a case of annular pancreas with pancreatitis of the annulus that was diagnosed by the use of computed tomography followed by endoscopic retrograde cholangiopancreatography. An upper gastrointestinal series and duodenoscopy revealed stenosis of the second portion of the duodenum. Computed tomography revealed enlargement of the annulus of the pancreas. Two years later, follow-up computed tomography demonstrated calcifications of the annulus, although the remaining pancreas seemed to be normal.

[Analysis of hepatic hemodynamics by a new method of per-rectal portal scintigraphy using 99mTcO4- (direct intramural administration of radioisotope)].

To depict of porto-systemic collaterals clearly, and to analyze of hemodynamics of liver, we developed new method of per-rectal portal scintigraphy (direct intramural administration of 99mTcO4- by 23G needle). And we used this method in patient with liver diseases (acute hepatitis: 5, chronic hepatitis: 7, liver cirrhosis: 25 cases). From time activity curve of the liver and the heart, liver/heart ratio; index of porto-systemic shunt via inferior mesenteric vein (IMV) and first flow ratio(k); index of portal blood flow from IMV pathway/index of hepatic total blood flow were calculated. In our method, the images of portal vein, liver, heart, especially porto-systemic collaterals were visualized more clearly than enema methods. The liver/heart ratio was significantly lower in patients with liver cirrhosis than that in non-cirrhotic patients (p less than 0.01), which indicated that patients with liver cirrhosis had more porto-systemic collaterals than non-cirrhotic diseases. The k was more lower in liver cirrhosis than in acute hepatitis (p less than 0.01). And the k was also more lower in chronic hepatitis than in acute hepatitis (p less than 0.1), which indicated that portal blood flow via IMV reduced in early stage of chronic liver diseases. In conclusion, new method of per-rectal portal scintigraphy has more advantage for analysis of hepatic hemodynamics than enema methods.

Charge distribution of plasma IgA in IgA nephropathy.

The spectrotype of plasma IgA in patients with IgA nephropathy was studied by isoelectric focussing. Densitometry of the gels showed a significant increase in the anionic region at isoelectric points (pI) 4.7-5.1 (P = 0.02) and a reduction in the cationic region pI 5.8-6.0 (P = 0.03) in patients (n = 15) compared with controls (n = 8). Measurement of the IgA concentration in eluates of sequential slices of the gels showed that the ratio of anionic-to-cationic IgA, using pI 5.6 as the dividing point, was significantly greater in patients (n = 10) than in controls (n = 10) (P = 0.03). Two different methods of analysis have therefore demonstrated an increased proportion of anionic and decreased proportion of cationic IgA in the plasma of patients with IgA nephropathy.

[Succinylcholine neuromuscular blockade in a case of pseudocholinesterase variant].

Since there is little or no pseudocholinesterase at the motor end plate, the neuromuscular blockade by succinylcholine is terminated by its diffusion away from the end-plate into extracellular fluid. Pseudocholinesterase (ChE), therefore, influences the duration of action of succinylcholine (SCC) by controlling the rate of hydrolysis before it reaches the end-plate. SCC neuromuscular blockade can be prolonged in the presence of an atypical form of ChE. A 37 year old male with the deficiency of ChE activity underwent pancreatico-duodenectomy under neuroleptanesthesia. The mode, duration and treatments of neuromuscular blockade following succinylcholine (SCC) were studied using train-of-four (TOF). SCC was administered 6 times (total 170 mg) during the operation. Prolonged apnea was noted after the first intravenous injection of SCC (60 mg, 1 mg.kg-1). It was found that spontaneous resolution of the neuromuscular block had four phases and there was a pronounced fade of the TOF response. After the second injection of SCC (30 mg), a remarkable fade of the TOF was detected and the character of the block was considered to be a desensitizing block (phase II block). The block was treated by intravenous infusion of fresh frozen plasma (FFP) (400 ml) which contained ChE, at the rate of 231 +/- 56 IU/l/37 degrees C begun shortly after the fifth injection of SCC (20mg). The duration of the block was remarkably shortened but the recovery curves were almost unchanged, indicating the diffusion of SCC away from the end-plate and/or that receptor-channel is presumably slow. The neuromuscular blockade was fully reversed by additional injection of FFP (400 ml) before extubation.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of marked exercise-induced ST segment depression in anterior precordial leads with inferoposterior left ventricular aneurysm and significant stenosis of left anterior descending artery].

A 71-year-old man was admitted to a community hospital and diagnosed as having acute infero-posterior myocardial infarction. Severe ST segment depression occurred in anterior precordial leads with only 50 m walking, but he had no chest pain. He was then admitted to our hospital. Left ventriculography showed infero-posterior dyskinesis and aneurysm formation. Coronary angiography revealed triple-vessel disease including 90% stenosis at distal site of left anterior descending artery. Exercise thallium-201 myocardial scintigraphy by ergometer showed no filling defect in the anterior segment, though severe ST depression appeared in anterior precordial ECG leads. Infero-posterior segment showed persistent defect. We performed intravenous digital ventriculography at rest and during atrial pacing. Anterior wall motion during pacing was shown to be normal by amplitude and phase analysis. At the same time, the motion of the inferior wall was seen as abnormal and ST segment depression on anterior precordial leads appeared. We considered that in this case the anterior ST depression did not mean anterior myocardial ischemia but might be due to dyskinetic movement of the infero-posterior aneurysm.

[Transurethral microwave coagulation of bladder cancer in elderly patients].

Since April 1985, 72 patients with bladder cancer have been treated by transurethral microwave coagulation (TUMC). In this procedure, tumors were irradiated with microwave energy of 100 watts for 2-16 minutes. A total of 20 patients were 70 years old or older, including 5 who were more than 80 years old. These 20 patients have been followed for 6 to 32 months with a median follow-up of 14.7 months. After TUMC, tumors completely disappeared endoscopically and histologically in all patients. Four patients had heterotopic recurrences. In all patients, serious complications were not seen during and after TUMC. Our clinical experience revealed that TUMC was the useful therapeutic method for bladder cancer in elderly or poor-risk patients. In this paper, we describe the technique of TUMC and preliminary clinical results.

[Tuberculosis in patients undergoing hemodialysis].

The incidence of tuberculosis is very high in patients treated by hemodialysis particularly in the early stage of hemodialysis. The diagnosis of tuberculosis in dialysis patients was obscured as symptoms were nonspecific and extrapulmonary involvement was seen frequently. We investigated cell mediated immunity in dialysis patients in relation to the period of dialysis. The data indicate that dialysis patients show the following immunological impairments; 1) lymphopenia, 2) decreased B cell, 3) alteration of T cell subset, 4) decreased reaction of PPD skin test, 5) decreased T cell activity, 6) decreased IL-2 production, 7) decreased PHA induced lymphocyte blastogenesis, 8) decreased NK cell. Decreased immunologic host defence mentioned above may contribute to the high incidence of tuberculosis in the early stage as well as in the maintenance phase of dialysis.

The effect of RO15-1788 on cardiovascular depression caused by fentanyl and diazepam.

Cardiovascular depression occurring when diazepam is combined with fentanyl has been investigated using the benzodiazepine antagonist RO15-1788 in the dog. After the initial administration of fentanyl (40 mcg/kg), the mean arterial pressure (MAP) decreased to 89% of its control value. Following the administration of diazepam (1.2 mg/kg), the MAP and the total peripheral resistance (TPR) decreased significantly, to 75% and 83% of their control values respectively. After the administration of RO15-1788 (0.4 mg/kg), the MAP increased significantly to 90% and the TPR to 102% of their control values and, lastly, the administration of naloxone (40 mcg/kg) increased the MAP to 108% of its control value. No relationship was found between the changes in the catecholamines and the changes in the MAP after the administration of fentanyl, diazepam, and RO15-1788. The mechanism of circulatory depression when diazepam was used with fentanyl is interpreted as being a peripheral vasodilatory effect of diazepam acting by way of the benzodiazepine receptors since RO15-1788 was found to antagonize this effect.

Re-evaluation of normal human urinary proteins fractionated by one-dimensional sodium dodecyl sulphate-polyacrylamide gel electrophoresis.

Proteins in normal human urine were clearly fractionated into 26 bands with molecular weights from 14,000 to 230,000 by means of one-dimensional sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) coupled with silver staining. The main band contained uromucoid, and the second main band had albumin. However, when urine samples from healthy persons were electrophoresed in the absence of SDS using polyacrylamide gel or agarose gel, or a cellulose acetate membrane, albumin but not uromucoid, frequently formed the main protein band. It is suggested that this is due to the complexing of uromucoid subunits to form a large molecule which cannot penetrate into the gel. In order to correctly fractionate all the proteins contained in normal human urine, it was concluded that it was best to treat a urine sample with SDS with pre-condensation, fractionate it by SDS-PAGE and stain fractionated proteins by a highly sensitive method such as silver staining.
