Application of Doppler color flow imaging method on the detection of vesicoureteral reflux.

The doppler color flow imaging method was employed to detect the vesicoureteral reflux. The ultrasound examination was performed by using the doppler color flow imaging system. Ultrasonic sector scan was carried out by the use of the 3.75 MHz phased array and/or conbex array transducer with a transabdominal approach. The flow in the bladder from the ureteral orifice to the bladder was detected with a red color indicating the direction towards the probe and the flow from the bladder towards the ureteral orifice during the vesicoureteral reflux was detected with a blue color indicating the direction away from the probe. The doppler color flow imaging method appears to be a useful diagnostic procedure for the detection of the vesicoureteral reflux.

Multiplicity of spontaneous lung tumors in CD-1 mice.

The multiplicity of the spontaneous lung tumor in CD-1 mice was investigated in 413 males from 6 different control groups of 2-year carcinogenicity studies with agrochemicals. Among a total of 152 lung tumor bearers, 95 (62.5%) were seen with 1 nodule, 36 (23.7%) with 2 nodules, and 14 (9.2%) with 3 nodules. Three males bore 4 nodules and each of 5, 7, 9 and 10 nodules was seen in one male, respectively. In one control group 20 out of 30 lung tumor bearers had 2 or more nodules presenting a significant heterogeneity among the control groups in terms of the multiplicity of lung tumor nodule.

A case of interstitial pneumonia antedating rheumatoid arthritis.

We describe a case report of an interstitial pneumonia in which pulmonary involvement of the both lower lobes mimicked idiopathic interstitial pneumonia (IIP) or bronchiolitis obliterans organizing pneumonia (BOOP). Its appearance antedated clinically apparent rheumatoid arthritis by one month and a half.

[The desensitization therapy in children with nasal allergy to house dust].

A study about the desensitization therapy carried out on children with nasal allergy to house dust who were under 15 years old. The good effect was seemed in 35.7% of patients with the desensitization for 3 months, and 62.7% for 6 months, 90.0% for 12 months, 92.7% for 24 months. The patients with the desensitization for over 6 months had a considerable effect, as if they dropped out. In the questionnaire to patients who dropped out with no contact to us, the effect of this therapy was seemed on 83.3% in those who answered questions, as so we considered that the sensitization for children with nasal allergy was very important as the basic therapy. Refer to the efficacy of this therapy, it was seemed to be recommended to children who were about from 7 to 9 years old, or those who had allergic rhinitis for a long time. At the point of 6 months after starting HD desensitization, the patients who reacted intensively to the provocation test had not significantly the efficacy compared with those who reacted slightly, but other clinical data were not significantly concerned with the efficacy of this therapy.

The thallium-201 time activity curve in thyroid cancer: a role for the detection of metastases.

The Tl-201 Cl time activity curves were evaluated in 17 patients with histologically proven benign nodules and papillary carcinoma of the thyroid gland. In the dynamic studies, the uptake at the region of interest was counted every 10 sec for 20 min after the intravenous injection of 2mCi Tl-201 Cl. The curves from frame 21 to 100 were transformed to linear plots for clinical evaluation. Papillary carcinoma could not be differentiated from benign lesions by means of the values of the washout rate. However, the "relative washout rate" in papillary carcinoma was lower than that of benign lesions (P less than 0.05). Although the average values of the "relative extrapolated uptake" in papillary carcinoma appeared to be greater than those in benign lesions, the difference was not statistically significant. The difference in the washout rates between lymph node metastases and local recurrences, and pulmonary metastases of the papillary carcinoma was significant (P less than 0.05).

[Long-term administration of tegafur in postoperative adjuvant chemotherapy in gastric cancer patients. Serial analysis of serum tegafur and 5-fluorouracil concentrations, and influence from PSK combination].

We investigated the pharmacokinetics of long-term (2-year) daily oral administration of FT-207-E or UFT for postoperative adjuvant chemotherapy in 85 gastric cancer patients by measuring serum FT-207 and 5-FU concentration serially. There were no significant changes in serum FT-207.5-FU concentration during observation period between FT-207-E and UFT groups. There was different operative modality between partial gastrectomy (Billroth I) and total gastrectomy, but no significant difference in serum FT-207.5-FU concentration between the two groups, and no significant serial changes in serum FT-207.5-FU concentrations were observed in both groups. Conversion from FT-207 to 5-FU measured by serum concentration was not suppressed by combination with the biological response modifier PSK.

The effects of 24% paraquat (1,1'-dimethyl-4,4'-bipyridylium dichloride) on hemodynamics, blood gases, plasma lactate and plasma catecholamines in dogs.

The cause of death after ingestion of a large dose of paraquat is unclear. We investigated the cardiovascular effects of paraquat injection in 7 dogs that were anesthetized with urethane and alpha-chloralose and maintained on controlled ventilation. After control measurements, paraquat (20 mg/kg) was injected iv. Average concentrations of plasma paraquat were 128 micrograms/ml at 15 min (point 1) and 44 micrograms/ml at 4 hr. The average time of survival was 5.5 +/- 2.2 hr. Comparison was made of control values, 15 min (point 1), and 2.5 +/- 1.0 hr before death (point 2). Pulmonary capillary wedge pressure (+33%) and pulmonary vascular resistance (PVR) (+28%) increased, but no change occurred in mean arterial pressure (MAP) or cardiac output (CO) at 15 min. Mean arterial pressure (-50%) and cardiac output (-61%) decreased, while total vascular resistance (+56%) and PVR (+256%) were increased at point 2. Arterial pH decreased from 7.394 and 7.307 (point 1) to 7.143 (point 2). The concentration of plasma catecholamines and lactic acid increased significantly at both points 1 and 2. Renal, adrenal and myocardial tissues were studied by light and electron microscopy. The outstanding feature was early swelling of mitochondria and disruption of the contents in the presence of intact mitochondrial membrane.

[A case of Kennedy-Alter-Sung syndrome associated with external ophthalmoplegia--therapeutic efficacy of fluoxymesterone].

We reported a case with Kennedy-Alter-Sung syndrome (KAS) associated with bilateral external ophthalmoplegia. The patient had movement disturbance of bilateral infra-oblique muscles. The doll's eye phenomenon was not noted. It was suggested that the external ophthalmoplegia was due to the involvement of the oculomotor nucleus that innervated infra-oblique muscle. The serum levels of testosterone and gonadotropin were high, suggesting that the feminization of KAS patients was caused by androgen insensitivity. The feminization of KAS patients is similar to the incomplete form of testicular feminization syndrome except that they do not have feminization of genitals. Therefore, we proposed that abnormalities of androgen receptors might play a role in the pathogenesis of KAS. Fluoxymesterone therapy significantly improved the muscle weakness of the extremities of the patient, even though the therapeutic efficacy was shown temporarily. The therapeutic efficacy of fluoxymesterone for muscle weakness supports our hypothesis.

[Ultrasonic evaluation of two cases of islet cell tumor of the pancreas].

Small pancreatic tumor is difficult to be diagnosed. Two cases of pancreatic islet cell tumors were examined by ultrasonography. One case was islet cell carcinoma, the other insulinoma. In both cases, ultrasonography was useful in detecting the tumor, judging the location of tumor during surgical operation and deciding the surgical procedure.

Immunological differences between squamous cell cancer patients treated with and without biological response modifier (OK-432): evaluation at a period of more than one year after the last treatment.

The cellular immunity of peripheral blood was analyzed to evaluate the efficacy of OK-432 between 3 groups of patients with oromaxillary squamous cell cancer, which were composed of 14 patients in the pretreatment stage, 8 patients who were free from cancer more than one year after the last cancer therapy and had been continuously treated with OK-432, and 14 patients who had undertaken cancer therapy without OK-432 and were also free from cancer for at least one year. As parameters of cellular immunity, leucocyte counts, percent composition of T-cells, percent compositions of T-lymphocyte subsets (OKIal+, OKT3+, OKT4+, and OKT8+) and the ratio of OKT4+ and OKT8+, PPD skin test, and lymphocytic blastogenesis induced by PHA and Con-A, were examined. The results showed no significant difference between the patients treated with and without OK-432. Nor was there any significant difference between the pretreatment group and the posttreatment groups with and without OK-432 treatments in any of the examined immunologic parameters.

Clinical features and diagnosis by recombinant DNA techniques of familial amyloid polyneuropathy in Japan.

Diagnosis of familial amyloid polyneuropathy (FAP) was investigated by recombinant DNA techniques using Southern blot hybridization in 49 constituents of FAP pedigrees originated from Nagano and Hiroshima Prefectures in Japan. The results were correlated with clinical features of the patients. Twenty-nine patients originated from Ogawa Village in Nagano Prefecture of the central part of Japan had typical clinical features of Andrade type FAP including polyneuropathy with autonomic disorders. The variant prealbumin gene with a substitution of methionine (Met) for valine (Val) at position 30 was detected in their DNA analysis (positive DNA diagnosis). In 5 cases from Nagano and Hiroshima Prefectures who had clinical features of Andrade type FAP but no family history, DNA diagnosis was positive. In another 5 members of FAP pedigrees originated from Ogawa Village without clinical features of FAP, DNA diagnosis was positive. Two cases originated from Iiyama City in Nagano, who had central nervous system disorders resembling spinocerebellar degeneration with spastic paraplegia besides clinical features of Andrade type FAP, had the Val-Met interchange variant prealbumin gene. In a case from the same Iiyama family who had only the central nervous system disorders without clinical features of FAP, DNA diagnosis was negative. These findings indicate that the presence of the variant prealbumin gene closely linked to the clinical manifestations of Andrade type FAP. The diagnostic DNA analysis was considered to be useful for the diagnosis of Andrade type FAP for sporadic cases as well as so far nonsymptomatic gene carriers. In conclusion, FAP patients originated from Iiyama City and Ogawa village were considered to have the same mutation despite the variances in clinical features.

[Evaluation of the gastroesophageal reflux (GER) in infancy analysed with the subscore of the continuous lower esophageal pH monitoring (pH-monitoring)].

Since GER in infancy tends to disappear as patients grow, operation should be selected carefully. Data of the pH-monitoring collected from 114 cases for last 8 years were analysed. Thirty five cases were observed without operation in view of their younger age (under 6 months) or mild degree of GER evaluated on the basis of clinical signs, radiological, manometric, pH-metric and fiberscopic examination. Four in 5 aggravating cases were compared with 8 improving cases selected from the non-operated 60 cases. The four pH-subscores calculated by Johnson's method were compared between 2 groups. The means and standard deviations of the subscores of the number of the pH less than 4.0 greater in duration than 5 min. were 42.4 +/- 14.791 in the aggravating and 5.7 +/- 0.766 in the improving: those of the duration of the longest episode of pH less than 4.0 were 7.4 +/- 1.996 and 2.5 +/- 0.335. Difference between the both subscores were statistically significant (p less than 0.05). Therefore those subscores can be used as a parameter by which early operation may be chosen rather than subjecting the patient to a prolonged no-operative treatment.

Interaction of macrophages with rheumatoid articular cartilage.

These experiments investigated the in vitro interaction of macrophages and rheumatoid articular cartilage. When rheumatoid articular cartilage samples were incubated with normal peripheral blood mononuclear cells (PBM) for 1 hour or 12 hours in vitro, there were large numbers of PBM attached to the articular surface, and some PBM invaded the cartilage during the 12 hour incubation period. Conversely, in cartilage from osteoarthritic and normal subjects in the control, only a few PBM were attached to the articular surface after incubation with PBM. Under electron microscopic observation, macrophages were tightly attached to the surface and within the eroded cartilage. Light and electron microscopic observation with immuno-peroxidase staining of the rheumatoid cartilage surfaces showed the deposition of immunoglublins adjacent to the attached macrophages, suggesting the presence of immune-complexes as chemoattractants. These results suggest that macrophages in a rheumatoid joint may play an important role in the formation of pannus, resulting in the destruction of cartilage to interact with immune-complexes trapped in the superficial region of the rheumatoid articular cartilage.

[Genetic studies of Duchenne muscular dystrophy families using the dystrophy families using the dystrophin cDNA].

The cDNA probes for the Duchenne muscular dystrophy (DMD) gene can detect deletions in over 50% of affected males and provide a highly accurate diagnostic test in the affected families. We present the results of our recent molecular genetic studies of five DMD families (six affected males and 16 non-affected family members) by using the dystrophin cDNA. Five molecular deletions were identified in the DNA samples of six affected males by studying abnormalities of Hind III and Bgl II fragments detected by the entire dystrophin cDNA. We could not find any abnormality in a family by the same DNA analysis. Deletion was found at two different loci; in two of six probands, it was located near 5' region of the gene seen by cDNA segment 1-2 a, while in the other three, near the center of the gene covered by cDNA segment 8. In three females of the five families, we detected novel fragments, the size of which was altered due to molecular deletion. These results greatly facilitated diagnosis of the carrier status in females. We studied also the dystrophin location in the biopsied skeletal muscle (femoral muscle) in a family, by means of indirect immunofluorescence staining using antiserum against dystrophin. Dystrophin was positive in the surface membrane of muscles from a non-affected brother, mother and sister, but it was absent in the muscle from a DMD proband.

[Contribution of the argon laser in iridotomy using the Yag laser].

Yag laser iridotomy was performed immediately after argon laser application in 59 eyes with chronic angle closure glaucoma, occludable angle and secondary glaucoma. Postoperative intraocular pressure was elevated temporarily more than 5 mmHg on 86.4% of patients. Iris haemorrhages occurred in 17% of cases when laser Yag iridotomy was carried out with a power of 4 to 6 mJ. Prior argon laser seemed to prevent iris haemorrhages.
