Surface marker analysis of atypical cells, present at elevated levels in the peripheral blood of a 2-yr-old patient with familial erythrophagocytic lymphohistiocytosis (FEL), was performed with monoclonal antibodies. The atypical cells expressed T11, T8, Ia and LeuM2 antigens, markers of suppressor/cytotoxic T lymphocyte and monocyte. The monoclonal antibody-defined surface markers were thus distinguishable from those of other lymphoproliferative disorders clinically resembling FEL.
Involuntary activity of transferred intercostal motor units was examined in patients with brachial plexus injury. Since the internal intercostal nerves were detached from the thorax to reinnervate the musculus biceps brachii, it was possible to record pure intercostal motor activity in humans. Respiratory activity was seen in the latter part of the expiratory phase, thus dividing the phase into two substages (E1 and E2) by the onset of the activity. CO2 rebreathing prolonged the duration of the intercostal motor activity and increased the tidal activity as determined from the integration curve. There was a close linear correlation between these two variables. These observations indicate that expiratory activity and its duration are actively controlled in humans.
This report describes a primary carcinoid tumor of the testis arising in a 19-year-old boy. Symptoms suggestive of a carcinoid syndrome were not noted in this case. The pathological specimen did not contain teratomatous components. Histochemically, the argyrophil reaction was positive. The immunohistochemical finding of immunoreactive cells for 5-hydroxytryptamine (serotonin) was also positive. To our knowledge, this is the youngest case to be reported.

Conjugates of mitomycin (MMC) with an affinity-purified horse anti-human alpha-fetoprotein (AFP) antibody were prepared by direct conjugation or by indirect conjugation mediated by human serum albumin; 1a-[4-(N-succinimidoxycarbonyl)butyryl]-MMC was used in the preparation. In therapeutic experiments with athymic nude mice inoculated with the human AFP-producing gastric carcinoma OSS, the conjugates caused a greater inhibition of the tumor growth than did an unconjugated mixture of anti-AFP antibody and MMC or similar conjugates with normal horse immunoglobulin. The AFP serum level and its ratio to tumor volume were low in the group of mice treated with anti-AFP conjugate. The enhanced drug activity of the anti-AFP conjugates was not observed against human gastric carcinoma YSS (no AFP production) in nude mice; this suggests the involvement of the specific antibody-antigen interaction in the activity of the anti-AFP conjugates.
To examine the effects of transient hyperprolactinemia on in vitro fertilization and embryo transfer, 61 cycles in 50 euprolactinemic ovulatory women with irreparable tubal diseases were stimulated with clomiphene (CC) alone or CC and human menopausal gonadotropin followed by human chorionic gonadotropin (hCG). Serum prolactin (PRL) increased after hCG administration with peak values of 45.4 +/- 4.2 ng/ml on the day of laparoscopic oocyte aspiration. The highest serum estradiol (E2) concentration was found on the day before PRL peak and serum progesterone (P) began to increase after hCG injection concomitant with the PRL rise. The group having 50 ng/ml or more of PRL (34 cycles) had significantly higher levels of E2 during preovulatory and early luteal phase compared to those of the group having less than 50 ng/ml of PRL (27 cycles) but there was no significant difference between the P levels in the two groups. In the higher PRL group 72 (62.1%) of 116 collected oocytes were fertilized and 6 (20.0%) conceived. In the lower PRL group 45 oocytes (58.4%) of 77 were fertilized and 3 (12.5%) became pregnant. These data suggest that elevated serum PRL concentrations may have no effect on fertilization of oocytes in vitro or embryonic development.

Of 52 male bladder cancer cases treated with radical cystectomy, 12 cases (23.9%) were incidentally diagnosed as stage A prostatic cancer. Histological analysis was done according to the "general rules for clinical and pathological studies on prostatic cancer." The average age of the prostatic cancer cases was 62.0 years with a range of 50 to 78 years. Three cases were of well differentiated adenocarcinoma, and 9 cases were of moderately differentiated adenocarcinoma. Six cases were at pT1, 3 at pT2, and 3 at pT3. One case was stage A1, and 11 were stage A2 cases. Perineural and capsular invasions were found in 3 cases each. Neither lymphatic nor vascular invasion was found in any case, and no evidence of seminal vesicular, urethral, bladder and rectal invasion was found in any case. Lymph node metastasis was not found in any of the 3 pelvic lymph node dissected cases.

Cytochemical and immunological studies of increased eosinophils were performed in three children with marked eosinophilia. These children were diagnosed as having non-Hodgkin's lymphoma, transient eosinophilia, and hyper-eosinophilic syndrome. All showed marked eosinophilia, with eosinophil counts of more than 5,000/mm3 in the peripheral blood and with eosinophils in all maturational stages in the bone marrow. In contrast to normal eosinophils, the increased eosinophils in three patients on cytochemical analysis showed moderately to strongly positive reactions for staining with alpha-naphthyl-butyrate esterase. Also, the mature eosinophils in the patient with lymphoma showed a strongly positive reaction for periodic acid-Schiff staining. The positive reaction for alpha-naphthyl-butyrate esterase and periodic acid-Schiff stain indicates the presence of eosinophilic proliferative disorders in these cases. Surface marker analysis of peripheral eosinophils demonstrated that an increased proportion of eosinophils expressed My7, a marker of granulocytes, in two patients with transient eosinophilia or hyper-eosinophilic syndrome, but not in a patient with lymphoma. In addition, 30-40% of eosinophils expressed Ia antigen, a differentiation marker of eosinophils, in two patients with lymphoma or hypereosinophilic syndrome. Thus, eosinophils in transient eosinophilia showed mature features, and those in hypereosinophilic syndrome showed immature features immunologically. Whereas, eosinophils in lymphoma may have a dysmaturity on the expression of Ia and My7. Cytochemical and immunological analyses of increased eosinophils may play an important role in clarifying the etiology of eosinophilia, especially its association with malignancy or other proliferative disorders.

The in vitro and in vivo antibacterial activities of a new aminoglycoside antibiotic, arbekacin (HBK), against methicillin-cephem-resistant Staphylococcus aureus (MRSA) were compared with those of gentamicin (GM), netilmicin (NTL) and amikacin (AMK). The results obtained were summarized as follows: Compared to other aminoglycoside antibiotics, HBK had the highest antibacterial activities against clinically isolated MRSA (46 strains). Therapeutic effects of HBK against experimental systemic infections with MRSA in mice, were superior to those of GM, NTL and AMK. The ED50's of GM, NTL and AMK were more than 2 mg/mouse. Therapeutic effects of HBK against experimental subcutaneous infections with MRSA in mice were also superior to those of GM, NTL and AMK.

Portal hemodynamics were studied in 69 patients with cirrhosis and 29 patients with idiopathic portal hypertension to investigate the effects of an operative procedure for varices that consists of transabdominal esophageal mucosal transection, paraesophagogastric devascularization, pyloroplasty, and splenectomy. Portal venous flow measured by the pulsed Doppler flowmeter in 14 patients with cirrhosis and nine patients with idiopathic portal hypertension, who underwent operation 2-5 yr earlier, was significantly reduced compared with that in unoperated 49 patients with cirrhosis and 17 patients with idiopathic portal hypertension who had esophageal varices (410 +/- 158 versus 660 +/- 263 ml/min in cirrhosis; 443 +/- 185 versus 912 +/- 189 ml/min in idiopathic portal hypertension). In nine patients (six cirrhosis, three idiopathic portal hypertension), portal venous flow and portal vein pressure were measured before and after operation. In patients with cirrhosis, portal vein pressure did not change significantly postoperatively even though portal venous flow was reduced. In contrast, portal vein pressure decreased in two patients with idiopathic portal hypertension in whom portal venous flow was reduced. Portal vein pressure was elevated in one patient with idiopathic portal hypertension in whom portal venous flow was increased postoperatively as a result of resection of a large gastro- and splenorenal shunt conducted additionally.
A follow up study was carried out on 24 children whose mothers were treated with HLA immunization as therapy for habitual abortion. Among 350 couples who had experienced recurrent spontaneous abortions registered at this clinic, 117 women were immunized with their husbands' leucocytes. Seventy became pregnant and so far 24 have delivered. The sex ratio of the babies was 16 males to 8 females. The average body weight and height at birth were 3,294 +/- 88.5 g (mean +/- S.E., male) and 3,079 +/- 114.4 g (female), 50.0 +/- 0.38 cm (male) and 49.0 +/- 0.60 cm (female), respectively. Average gestational duration was 39.0 +/- 0.23 weeks and mean labor hours were 7.0 +/- 0.73 without any complication except two cesarean sections. Four out of 24 children were more than 2 years old and 12 were less than one year old. All of them were interviewed or investigated by questionnaires on the developments of body weight, height, sensory organs as well as dental development after birth. Emotional and other physical developments and history of disease after the neonatal period were also examined. Though basic immunological studies will be required in future, all investigations indicate that no abnormal sign normal developments were found in these children, indicating further application of this therapy for habitual abortion.
