Fungal endophthalmitis following intraocular lens implantation. A surgical epidemic.

Thirteen cases of fungal endophthalmitis were caused by Paecilomyces lilacinus. In all cases an intraocular lens was inserted that had been sterilized in sodium hydroxide and neutralized in sodium bicarbonate. In 12 of the 13 cases, it was absolutely determined that a manufacturer's lot 128 was the neutralizing solution used. The same fungus that caused the endophthalmitis was cultured from several of the neutralizing solutions from lot 128. Eight of the 13 eyes eventually required enucleation. Of the remaining five eyes, one eventually recovered 20/25 vision, one recovered 20/80 vision, one had light perception, and two lost light perception (one of these became phthisical). This surgically induced epidemic of fungal endophthalmitis clearly shows the major consequences of a breakdown in quality control for any substance or material used intraocularly.

Leukemic iris infiltration.

A 7-year-old boy with acute lymphocytic leukemia developed iris infiltration, blood-streaked hypopyon, and high intraocular pressure. A diagnostic anterior chamber paracentesis was performed, which provided a cytologic diagnosis. The aspirate was studied by both the millipore filter technique with Papanicolaou stain and a dried smear using the Wright's stain. Wright's stain provided more distinct cellular details. Irradiation of the anterior segment resulted in clearing of leukemic iris infiltration, hypopyon, and glaucoma.

HLA antigens in recurrent stromal herpes simplex virus keratitis.

HLA-A, HLA-B, and HLA-C antigens were typed on 48 patients with recurrent herpes stromal keratitis. The HLA-Aw30 antigen occurred three times more frequently in patients with herpes stromal keratitis than in those who lack the Aw30 antigen. When the data for the probability of the HLA-Aw30 were corrected for the number of variables studied, the corrected P value was not significant. A previous report of an increased frequency of HLA-B5 with recurrent herpes keratitis was not confirmed by our study. No significant associations with the HLA-C antigens were noted. HLA-DR antigen typing of 25 herpes stromal keratitis patients indicated that the HLA-DRw3 antigen appeared to have an increased frequency in herpes stromal keratitis. However, the small sample size studied precluded interpretation of the increased phenotypic frequency and the possible association of HLA-DRw3 and herpes stromal keratitis needs further study.

Intraocular invasion by papillary squamous cell carcinoma of the conjunctiva.

Two patients with incompletely excised invasive papillary squamous cell carcinoma of the corneoscleral limbal conjunctiva had subtle clinical signs suggesting incomplete transmural extension or localized intraocular tumor invasion. We attempted a wide en bloc resection procedure in each case. Histopathologic examination disclosed that the surgical margins were involved with tumor cells and the eyes were subsequently enucleated. The enucleated eyes disclosed more extensive microscopic intraocular involvement than had been anticipated.

Meningococcal conjunctivitis.

Meningococcal conjunctivitis is typically described as an acute purulent infection. An atypical case of mild catarrhal conjunctivitis occurred in a 19-year-old college student. The meningococci were identified as Neisseria meningitidis, group A, and were isolated from the throats of the patient and her roommate. The conjunctivitis responded rapidly to treatment with sodium sulfacetamide, and it was not treated systemically. A short review of the literature of meningococcal conjunctivitis is presented, and the current recommendation for prophylaxis is discussed.

Hazards of bilateral intraocular lens implantation.

Seven patients had bilateral intraocular lens implantation performed with a short interval between procedures, using intraocular lenses of unproven design or deficient quality control All seven patients suffered severe visual impairment, some to the level of legal blindness in both eyes. Lenses with unproven design characteristics should never be implanted in both eyes of a patients. The interval between the first and second procedure, even when using lenses of proven design and quality, should be long enough to insure the long-term successful outcome of the first procedure.

Chronic keratoconjunctivitis associated with ocular adenovirus infection.

Adenovirus (types 3, 4, 5) was implicated as the etiologic agent in three cases of persistent keratoconjunctivitis. In two, virus was isolated 25 and 29 months after the onset of symptoms; the third patient has suffered intermittent flares of kerato-conjunctivitis since the initial isolation of virus 13 weeks after the onset of the disease. Such patients may represent a previously unsuspected source of adenovirus in the population. All were given corticosteroids topically. This treatment may have prolonged the adenoviral disease.

Anaerobic and aerobic bacteriology of acute conjunctivitis.

Aerobic and anaerobic cultures and clinical data were obtained from 131 patients presenting with acute conjunctivitis. Similar cultures were obtained from 60 noninflamed individuals. Anaerobes were isolated from 66 patients, 51 times in mixed culture with aerobes and 15 times (11.5%) as the only isolates. Aerobes only were recovered in 54 patients. The organisms found to be statistically significantly more commonly recovered from eyes with conjunctivitis were Staphylococcus aureus, Hemophilus influenzae, Peptostreptococcus, and Propionibacterium acnes. No statistical difference was present between the bacterial flora of the eyes with unilateral conjunctivitis when compared to their uninvolved mates in 20 cases in which only one eye was involved.

Fungal endophthalmitis following intraocular lens implantation.

Endophthalmitis developed in a 61-year-pld man after an intraocular lens implantation. A whitish opacity developed, visual acuity decreased to light perception, and we enucleated the eye. Paecilomyces lilacinus (Thom) Samson, a saphrophyte that has contaminated laboratory solutions, was cultured.

Lacrimal immunoglobulins and complement quantified by counter-immunoelectrophoresis.

Immunoglobulin and complement concentrations of tears were measured using a sensitive electroimmunodiffusion assay. In the 13 subjects tested, the predominant immunoglobulin was secretory IgA (mean concentration 107 mug/mg protein; standard error of mean 15; n equal 17). The B1C component of complement was measurable in half of the subjects studied. The data illustrate that it is possible to quantify exactly certain humoral indices which may be important in the host defense of the eye.

Corneal intrastromal cyst following lamellar keratoplasty.

A case is reported documenting formation of an intrastromal corneal cyst following lamellar keratoplasty. A patient with keratoglobus sustained trauma to the left eye. The trauma resulted in rupture of the cornea, which was repaired with an overlay conreal graft. Two and one-half years later, an intrastromal fluid-filled cyst of the cornea was found in the visual axis. Cytologic examination of the aspirated cyst fluid revealed epithelial cells.

Pathogenesis of experimental allergic uveitis induced by retinal rod outer segments and pigment epithelium.

Experimental allergic unveitis (EAU) was produced in strain 13 guinea pigs after immunization with purified guinea pig retinal rod outer segments and pigment epithelium (PE) in mycobacterial adjuvant emulsion. The lesions of EAU appear as inflammatory infiltrates of the iris, ciliary body, and choroid, often with photorecptor degeneration. Specific antibodies are frequently detected in the serum of some but not of all animals with clinical uveitis. Immunohistochemical staining of normal and inflamed eyes with serum from the immunized animals with clinical disease demonstrated specific antigens localized in the retinal photoreceptor layers, whether or not circulating precipitating antibodies were also present in the serum. All immunized animals demonstrated delayed-type hypersensitivity (DTH) characterized by skin reactions of mononuclear cells and by the vitro inhibition of migration of cells from the sensitized animals in the presence of specific antigen whether or not clinical uveitis occurred. However, stronger DTH reactions were observed during clinical uveitis. Cellular immunity appears to correlate with clinical EAU, whereas, no correlation was found with serum antibody. These findings suggest that the retinal photoreceptor cell and PE are the source of the significant antigens in autoimmune uveitis and retinitis.
