Novel mechanism of post-transcriptional modification of tRNA. Insertion of bases of Q precursors into tRNA by a specific tRNA transglycosylase reaction.

The guanine insertion enzyme isolated from Escherichia coli (tRNA transglycosylase) catalyzed the incorporation of bases of Q (queuosine) precursors into E. coli undermodified tRNAAsn and tRNATyr. These bases of Q precursors were inserted in the first position of the anticodon of tRNASn and tRNATyr, replacing guanine originally located in that position. This is a novel type of post-transcriptional modification, inserting a modified base into the polynucleotide chain by cleavage of the N--C glycoside bond without breakage of the phosphodiester bond. One of the bases of Q precursors, 7-(aminomethyl)-7-deazaguanine, was found in the acid-soluble fraction of E. coli cells, supporting the conclusion that formation of Q, 7-(3,4-trans-4,5-cis-dihydroxy-1-cyclopenten-3-ylaminomethyl)-7-deazaguanosine, in tRNA in vivo actually proceeds by the tRNA transglycosylase reaction.

Therapy and prognosis of status convulsivus in childhood.

The record of 67 cases under 15 years of age who were hospitalized during status convulsivus from 1975 to 1978, the 348 cases who visited the hospital for the first time with epilepsy (Oct. 1977 to Sept. 1978) and the 32 cases who were hospitalized during status epilepticus from 1969 to 1974 and who are being followed up as outpatients were studied. The frequency of status epilepticus was 8% among epileptic children. There was no difference in the frequency of incidence between male and female. Patients with mental retardation, however, were revealed to have status epilepticus twice to three times more frequently as compared to cases without mental retardation. The major seizure types of status epilepticus in childhood were generalized tonic clonic convulsion and unilateral clonic convulsion. In 25% of the cases, status epilepticus was the first ictal manifestation. The major cause of status convulsivus was epilepsy, followed by encephalitis and encephalopathy, but cases due to brain tumor were rare. The drug of first choice for status convulsivus is diazepam. If there is any difficulty in controlling status convulsivus with diazepam, it may be worthwhile to consider what the problem is, causes of status convulsivus, seizure type, or basic disease of the patient. The effective dose of diazepam was within the range of 0.3--0.5 mg/kg. When the effect is not sufficient, the dose of diazepam should be increased to 1 mg/kg while watching the general condition of the patient. Factors affecting the prognosis of status convulsivus were its cause, duration, onset age and effectiveness of therapy during the acute stage. The frequency of cases who suffered disability after status epilepticus was 56%. (transient disability 43%, permanent disability 13%) The most frequent type of transient disability was hemiplegia. Most epileptic children who had repetitive status convulsivus revealed psychomotor retardation before first status. Factors which cause repetitive status seem to be hemispheric brain damage or diffuse corticocentrencephalic damage.

[Fundamental studies and clinical evaluation of PC-904 in urinary tract infection (author's transl)].

Measurement of MICs of PC-904, CBPC and GM against 135 strains of Pseudomonas aeruginosa isolated from patients with urinary tract infections was performed. Susceptibility to PC-904 of Pseudomonas aeruginosa was found to be 3 approximately 7 times more sensitive than that to CBPC and as same as that to GM with inoculum size of 10(8)/ml and 10(6)/ml. Serum level of PC-904 reached a peak of 38.5 microgram/ml 15 minutes after 0.5 g intravenous administration. Urinary recovery rate unit 6 hours after the administration was 12.5%. Overall clinical efficacy of 6 cases of acute simple cystitis and 3 cases of chronic complicated cystitis was judged to be 83.3% and 33.3% respectively by a criterion proposed by the UTI Committee in Japan.

[Retention of crowns as affected by film thickness of zinc phosphate cement and taper angle of crowns (author's transl)].

Dependence of the retention of crowns at their axial wall on the film thickness of zinc phosphate cement and the taper angle was investigated. Stainless steel dies, composed of a preparation and an occlusally perforated crown the taper angle of which was 2.9, 5.7, 8.5 and 11.3 degrees respectively (Fig. 1, (a) and (b)), were cemented within three min. after the start of mix with a Shofu Micro Cement having a powder-liquid ratio of 1.5 g/0.5 cc. The thickness of the cement layer at the axial wall was regulated to a given value between 9 and 55 mu through the use of a discrepancy measurer. A Shimazu Autograph tensile test machine was employed to measure 48-hr retention (kg/cm2). The retention of the crowns was strongly dependent on the cement film thickness and the taper angle when the film thickness was below a critical value (15 mu in this experiment), but not when the film thickness was above this value (Fig. 3). This may be explained by the term of mechanical interlocking of either of the unreacted powder grains and the matrix phase in the luting cement, as schematically drawn in Fig. 6. The same findings were more evidently appeared on the semilogarythmic diagram in Fig. 4. In this diagram, the critical value was shown as intersection of two straight lines which could be drawn through all the measurements for four taper angles, using the least squares.
