[Quantitation of absolute and relative renal uptake using 99mTc-DMSA: sequential change in time and correlation with 99mTc-DTPA uptake].

A renal uptake of 99mTc-DMSA as a function of time and correlation between 99mTc-DMSA and 99mTc-DTPA in renal uptake were discussed. The absolute renal uptake of 99mTc-DMSA calculated by complete correction of tissue attenuation and physical decay showed increase by 5 hours after the administration, except one case with congestive heart failure. The net renal counts attained a maximum between 2 to 3 hours after the administration. There was a good correlation (r -0.992, p less than 0.001) between the relative renal uptake of 99mTc-DMSA (2 hour) and that of 99mTc-DTPA (2-3 min). On the contrary, the absolute renal uptake of 99mTc-DMSA as a indicator of total renal function showed a poor correlation (r = 0.419) to GFR calculated from 99mTc-DTPA. The results indicate that the relative renal uptake of 99mTc-DMSA is a reliable quantitative parameter for split renal function. However, the absolute renal uptake of 99mTc-DMSA may be suggested to be a different renal functioning indicator from GFR calculated from 99mTc-DTPA.

[A case of hepatocellular carcinoma treated by intrahepatic arterial administration of anticancer agents: serial determination of the concentration of the chemotherapeutic agents in serum and dialysate during hemodialysis].

The concentration of adriamycin (ADR) and mitomycin C (MMC) were measured in serum and dialysate in a patient with hepatocellular carcinoma complicating chronic renal failure. ADR at a dose of 20 mg and MMC at 10 mg were administered through the hepatic artery or peripheral vein during hemodialysis. ADR and MMC became undetectable after 24 hours and 2 hours, respectively. On the other hand, while MMC was detected in dialysate, but ADR was not. Consequently, it was speculated smaller molecular weight of MMC (334.34) than that of ADR (579.99) might be responsible for the appearance of MMC in dialysate. These results suggest that hemodialysis may be effective in reducing side effects of certain anticancer agents. Serum concentration of the anticancer agents injected through hepatic artery was similar to that injected intravenously.

[Spontaneous complete remission in a patient with acute monocytic leukemia].

A spontaneous complete remission was observed in a 47-year-old female with acute monocytic leukemia. Resolution of all abnormalities, including systemic papule, thrombocytopenia, increased numbers of immature monocytoid cells in the peripheral blood and bone marrow, elevation of serum lysozyme and LDH, and trisomy 8 on chromosome analysis, occurred without any treatment. Moreover, the remission was not associated with any infection or blood transfusion, and is persistent for 12-month duration.

[A case of right ventricular dilated cardiomyopathy, which involved left ventricle].

A case of right ventricular dilated cardiomyopathy which also involved the left ventricle was reported. On health screening, a 16-year old woman was pointed out to have multifocal PVC and cardiomegaly. Subsequently, she was admitted to our hospital because of general fatigue. CTR was enlarged to 54.9% on chest X-ray. ECG showed LBBB-type PVC, right axis deviation, low voltage and T wave changes. On UCG, RVdD was dilated to 40 mm and LVdD was 37 mm. There was no finding of abnormality of the tricuspid valve. On cardiac catheterization, there was no shunt disease. Intracardiac pressure was normal. The end-diastolic volume index (ml/m2) of RV and LV was 196.7 and 67.4, respectively. And ejection fraction (%) was 20 and 40. Ventriculography revealed diffuse dilatation of the right ventricle. And lowered contractility existed not only in the right ventricle but also in the anterior and apical segment of the left ventricle. T(1)201 myocardial perfusion imaging showed irregular perfusion defect of the left ventricle. Endomyocardial biopsy revealed marked hypertrophy, partial atrophy, disarrangement of myocyte and interstitial fibrosis of the right ventricle. This case was considered to be right ventricular dilated cardiomyopathy. It seemed to be an intermediate form of dilated cardiomyopathy since it also involved the left ventricle. It was an interesting case to illustrate the spectrum of expression of cardiomyopathy.

[Protective effect of calcium channel blockers on the liver against halothane hepatitis in rats].

This study examined whether nifedipine or flunarizine, calcium channel blockers, could protect the liver against halothane hepatotoxicity in rats. Six groups of 102 enzyme-induced male Sprague-Dawley rats were studied. Group N1 and N2 rats received nifedipine 20 micrograms and 100 micrograms.kg-1.day-1, and group F1 and F2 rats received flunarizine 100 micrograms and 1mg.kg-1.day-1 in their drinking water for 24 hours prior to the experiment. They were then anesthetized with N2-O2-halothane 0.5% under the condition of hypoxia (FIO2 0.12) for two hours. Animals in group H were given clear water, and then anesthetized with halothane and hypoxia. Animals in group C were given clear water and exposed to hypoxia alone. Twenty-four hours after anesthesia, all animals were decapitated and their liver specimens were stained with H&E for the examination by light microscopy. Eighty percent of rats in group H showed centrilobular degeneration, but administration of nifedipine or flunarizine reduced the incidence significantly. Furthermore, histological score graded from 0 to 3, according to the degree of centrilobular degeneration was also significantly reduced by administration of nifedipine or flunarizine. These results suggest that oral administration of nifedipine or flunarizine could prevent the incidence of halothane-induced hepatotoxicity in enzyme-induced rats, probably by preventing the increase in cytosolic calcium concentration.

[Abnormal cluster formation in a patient with myelodysplastic syndrome with trisomy-11--periodical approach by colony assay].

59 year old female was admitted to Nagoya Memorial Hospital for anemia unknown etiology after the work up of the gastrointestinal tract. Peripheral blood count at admission was as follows: WBC 2,400/microliters, RBC 321 X 10(4)/microliters, Hb 9.8 g/dl, Ht 30.1%, Plt 8.2 X 10(4)/microliters, which showed pancytopenia with normocytic, normochromic anemia. She had no hepatosplenomegaly, vitamin B12 nor folate deficiency. Bone marrow was hyperplastic and showed trilineage megalodysplastic changes. The diagnosis of myelodysplastic syndrome (Refractory anemia) was made. Progenitor assay showed no colony formation of BFU-E but showed normal growth of CFU-GM colony and cluster. She had chromosomal abnormality of 47, XX, + 11. Administrated anabolic steroid, prednine and activated vitamin D3 were not effective and she died of brain hemorrhage in April 1987. Colony assay at this stage showed numerous leukemic clusters and no normal colonies. Re-performed chromosome assay showed 47, XX, + 11. There are only a few reports of trisomy-11 in a patient with MDS. Especially we could follow this case till her leukemic transformation by colony assay.

[Sudden cardiac death in the emergency hospital].

We investigated 303 (men: women = 2:1) cases who suffered sudden heart arrest in Yamagata city since 1984 to 1987. The incidence rate was 41.0/100,000/year, and increased markedly with increasing age. There was a tendency for sudden death to occur in the winter season, evening and early morning. Two major causes were cardiac disease (especially ischemic heart disease) (46.4%) and intracranial hemorrhages (18.6%). 20% of all the heart-arrest cases were able to be saved, but, depending on the kind of heart disease the survival rate varied greatly (18.8% in acute myocardial infarction and 71.4% in vasospastic angina), (40.0% in ventricular fibrillation and 13.3% in the bradycardic arrhythmias). Survival rate was also effected by the time interval from the onset till the beginning of cardio-pulmonary resuscitation. About one half of the cases had histories of cardiac disease. Premonitory symptoms were observed in at least one third of the cases.

Early and late periodic patterns of even skipped expression are controlled by distinct regulatory elements that respond to different spatial cues.

We have identified the regulatory sequences required for the periodic expression of the Drosophila pair rule gene even skipped (eve). We find that the gradually changing pattern of periodic eve expression during early embryogenesis is directed by two distinct regulatory programs. Initially, eve expression in individual stripes is established by different regulatory elements, each of which responds to nonperiodic spatial cues provided, at least in part, by the gap genes. Later, coordinate expression of eve in all seven stripes is directed by a single regulatory region that responds to periodic cues provided by primary pair rule genes, including eve itself. As a consequence of this two-step regulatory program, eve functions both in the establishment of the periodic pattern of gene expression and in the subsequent specification of parasegmental boundaries.

Ultrastructural features of the AIDS virus (HIV) and its morphogenesis.

HIV particles were usually seen on the surface of established lymphoid cells derived from AIDS patients or on CEM cells infected with HIV, and sometimes in cytoplasmic vacuoles. The virus particles were formed by a budding process from the plasma membrane of an infected cell. The budding particles were of a doughnut form. Various profiles of virus particles were seen extracellularly: type 1 had a bar-shaped, electron-dense core, type 2 had a central and type 3 an eccentric electron-dense round core, type 4 was doughnut-shaped, and type 5 had a layered core. However, projection patterns of the AIDS virus model suggested that type 1, 2 and 3 particles are similar. Therefore, the AIDS virus may be one of three main types: with or without a dense core, and with a layered core. It is thought that a particle with a layered core and a doughnut-type particle may be immature viruses.

Procoagulant activity of human neuroblastoma cell lines, in relation to cell growth, differentiation and cytogenetic abnormalities.

Procoagulant activity (PCA) was investigated in relation to cell growth, differentiation, and cytogenetics in seven human neuroblastoma cell lines. Before 5-bromodeoxyuridine (BrdUrd) treatment, PCA was notably heterogeneous, with the highest activity in NCG (S-type in morphology) 40- to 100-fold greater than the lowest activity in SK-N-DZ (N-type). PCA was not related to 1p abnormalities. After BrdUrd treatment at 5 micrograms/ml for 6 days, PCA increased 6.8-fold in GOTO and 2.7-fold in SK-N-DZ with associated growth inhibition and morphological changes (I-type morphology converted to S-type in GOTO and N-type converted to an advanced N-type in SK-N-DZ). In contrast, only growth suppression was observed in 2 other cell lines, and no changes in PCA, growth or morphology were induced in the remaining 3 cell lines.

[A cytochemical study of lysosomal system in rat junctional epithelium after intravenous horseradish peroxidase injection].

Lysosome formation was studied in rat junctional epithelium (JE) by detection of acid phosphatase (ACPase) using the cerium reaction method after intravenous injection of horseradish peroxidase (HRP). Intravenously injected HRP was endocytosed by the whole JE, especially at the coronal portion. ACPase activity was also observed in numerous organellae (vacuoles, dense bodies, multivesicular bodies and Golgi cisternae) in the junctional epithelial cells. These ACPase positive organellae often fused with each other, and their contents were mixed in the fused structures. The numbers of the ACPase-positive organellae in the HRP-exposed JE were more numerous than those in the JE without HRP, especially at the coronal portion. Thus, the distribution of the ACPase-positive products in the JE was closely related to that of HRP-positive products. These findings indicated that the whole JE has not only endocytotic ability but also intracellular digestion ability by the lysosomal enzymes.

[Nuclear DNA cytofluorometry of normal human laryngeal epithelia and squamous cell carcinoma].

Normal human laryngeal epithelia and laryngeal squamous cell carcinoma were assayed by Feulgen DNA cytofluorometry using free cell nuclei isolated from carnoy-fixed, paraffin-embedded specimens. In all of the 12 normal specimens, the epithelium showed typical diploid cell clones with low proliferative activity. Polyploid cells were seen in only two specimens from subjects aged 61 and 69 years respectively, and the number of polyploid cells seen in these two specimens was only two. Fifteen cancer cases were divided into three groups: an untreated group (5 cases), a chemotherapy group (5 cases) and a group of cases with recurrence after radiation therapy (5 cases). Among these three groups the DNA ploidy patterns were compared. In the untreated group, all cases showed a two-peak diploid pattern and a high proliferative activity, and polyploid cells were present. In the chemotherapy group, a wide one-peak histogram extending from 2C to about 5C was noted in 4 cases, and an aneuploid pattern in one case. Thus, the DNA ploidy pattern in the chemotherapy group differed from that in the untreated group. Of the 5 cases with recurrence after radiation therapy, one had a tetraploid pattern, but the remaining 4, a two-peak diploid pattern similar to that seen in the untreated group. Polyploid cells were observed in all these cancer cases. However, because they were also seen in some normal subjects, the finding of polyploid cells is not considered to be conclusive of cancer diagnosis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Facial bone fracture--statistical analysis and clinical aspects].

We experienced 102 cases of facial bone fracture during 16 months of 1986 to 1987. These cases were analyzed statistically concerning causes, age and locations of the fracture. These fractures have increased rapidly in number. The causes were classified into three types; occurrence during sport, traffic accident and fighting, which were equal in number. There were 85% males and 15% females in the patient cohort, which were concentrated at the ages of 10-20 years. A large part of the fractures was mostly consisted of maxillo-facial components (95%). These trends were similar to the previous report of our clinic (1972-1979). On the other hand, not only severe dysfunctioning cases but also complicated cases increased in number, so that the several clinical aspects were reported. Case 1: 17-year-old male presented with retraction of left cheek caused by Rugby foot ball, whose malar bone was dislocated backward and anticlockwise, was treated with oroantral reduction and with the intermaxillary packing of silicon blocks. Case 2: 10-year-old boy with complaint of double vision occurred by head blow to right eye. Pure type blowout fracture of the orbital floor was presented, which was reconstructed by silicon plate from the incision of the lower eyelid. Case 3: 59-year-old male presented with 6 month history of diplopia and retraction of left eye ball, had been under the conservative care by an eye doctor. X-ray examination showed the intraorbital soft tissue was blown out into the ethmoidal sinus. However the transethmoidal reduction was performed, the result was not satisfactory.(ABSTRACT TRUNCATED AT 250 WORDS)
