Liver function and gastric acid secretion in parkinsonian patients under prolonged treatment with L-dopa and a peripheral decarboxylase inhibitor.

Twelve patients with long-standing Parkinson's disease, treated successfully with a combination of l-dopa and an inhibitor of aromatic l-amino acid decarboxylase (Ro 4-4602), in the ratio 4:1, were screened for damage to various organ systems, in particular liver and skeleton. Among other tests, liver biopsies were obtained before and after 6 months of treatment. One patient discontinued therapy becuase of an accentuation of pre-existent liver damage, another because of psychomental manifestations. The remaining ten patients were followed for 8-15 months and longer. The liver biopsies remained practically unaltered. Elevation of alkaline phosphatase was found in 10 out of 12 subjects. In five patients this rise fluctuated around the upper limit of normal. In two patients, who discontinued the treatment, the raised alkaline phosphatase values soon returned to normal. Analysis of isozymes proved this phosphatase to be of liver origin. All other liver function tests remained unchanged, except for an increased retention of bromsulphalein in two patients. In the one patient with the initially damaged liver, all tests became normal soon after discontinuation of therapy. No changes could be found in gastric acid secretion. All other parameters studied remained within normal limits, including urinary excretion of calcium and phosphate.

Risk factors in ulcerative colitis.

In 504 Jewish patients with ulcerative colitis the following risk factors were evaluated: sex, age at onset of disease, community group, extent of disease, and duration of disease. The disease was more severe in females. Severe attacks, weight loss, iron deficiency, liver disease and arthritis were significantly more frequent in females than in mles. Mortality was higher in patients who contracted the disease above age 50. The disease appeared to be more severe in patiets of Ashkenazi origin as compared to Orientals. The difference failed to reach statistical signifcance except for mortality. We confirmed the well-known adverse effect of extensive colonic involvement. Severe attacks, a severe course of the disease and extensive colonic involvement are more frequent with increasing duration of the disease.

Ulcerative colitis in the Jewish population of Tel-Aviv Yafo. III. Clinical course.

The clinical course of ulcerative colitis has been studied and analyzed in 504 Jewish patients in the Tel Aviv area. The length of follow-up was 1 to 46 years, with a mean of 7.8 years. The data are compared with other published series of the disease in different geographical areas. Differences in the incidence of serveral complications are noted. The trend in recent years toward a lesser mortality associated with the disease is discussed.

Chymotrypsin output in the stools in pancreatic and other diseases.

Chymotrypsin output in the stools (u./24 hr.) has been calculated from analysis of single stool samples, using cuprous isothiocyanate as a continuous marker. The values thus calculated agreed favorably with those calculated from three-day stool collections. In 45 controls, with and without steatorrhea, chymotrypsin output was above 10,000 u./24 hr. In six patients with pancreatic insufficiency the values were considerably lower. In 10 patients with pancreatic disease without insufficiency the values were in the normal range. The method was found very useful in the diagnosis of pancreatic insufficiency. It was not sensitive enough to detect lesser degrees of pancreatic damage.

Fiberoptic endoscopic diagnosis and treatment of a congenital esophageal diaphragm.

A 9-month-old infant suffering from dysphagia to solids presented with esophageal bolus obstruction. Using an adult fiberoptic esophagoscope, a congenital web was found at the junction of the middle and distal thirds of the esophagus. Previous direct rigid esophagoscopy and roentgenologic examination had not visualized the web. The web was successfully ruptured and dilated by inserting a guide wire under fiberscopic control and then inserting metal olives, followed later by blind mercury bougienage. This report illustrates the use of adult fiberoptic instruments for diagnosis and treatment in pediatric patients.

EEG in ulcerative colitis.

Electroencephalographic tracings of 50 patients with ulcerative colitis (UC) were compared with those of 75 controls. In the UC patients a 24% incidence of abnormal tracings was found as compared with an 8% incidence in the controls. A higher incidence of abnormal electroencephalograms was found among active cases of UC than among those in remission. The meaning of these results is not yet clear.
