[A case of juvenile Parkinson disease with vocal cord abductor paralysis in the course of malignant syndrome].

We report a 60-year-old woman with juvenile Parkinson disease (PD) with vocal cord abductor paralysis (VCAP). She had suffered from juvenile PD for 30 years. She was admitted in February 1998 to our clinical unit, because of malignant syndrome induced by dehydration. Neurological examination revealed disturbance of consciousness, hand tremor, dyskinesia of the trunk and all extremities, and rigidity. Laboratory examinations disclosed leukocytosis, renal dysfunction, hypermyoglobinemia, and elevation of the serum creatine kinase. Six days after admission, dyspnea and inspiratory stridor were noted, and the respiratory distress worsened. Endoscopy of the upper airways revealed that the vocal cord was in the midline or paramedian position. There are some cases of PD with VCAP, but such a case is very rare in Japan. We discussed the pathogenic mechanisms of these conditions, and speculated that VCAP was associated with malignant syndrome in our case.

Expression of metalloproteinase-2 (gelatinase A) in labial salivary glands of patients with Sjögren's syndrome with HTLV-I infection.

OBJECTIVE: To determine whether gelatinase A (MMP-2) plays a significant role in the pathogenesis of Sjögren's syndrome (SS) with or without HTLV-I infection. METHODS: We examined 24 patients with SS (14 HTLV-I-seropositive and 8 HTLV-I-seronegative). Labial salivary gland tissue samples were analysed immunohistochemically using anti-MMP-2 monoclonal antibodies. RESULTS: In normal salivary glands, MMP-2 expression was not detected. All biopsy samples of 8 SS patients with HTLV-I-associated myelopathy (HAM) and 3 of 6 HTLV-I-seropositive SS patients without manifestation of HAM stained positively for MMP-2. However, the other samples were negative for MMP-2. CONCLUSION: Our study showed the MMP-2 expression in labial salivary glands of HTLV-I seropositive SS patients, especially in all SS patients with HAM. The presence of MMP-2 in the salivary glands of these patients suggests that it may play a role in cellular infiltration and destruction in salivary glands of SS.

Current understanding of the molecular basis of chloroquine-resistance in Plasmodium falciparum.

Chloroquine (CQ) is the most successful antimalarial drug ever discovered. Unfortunately, parasites resistant to the drug eventually emerged after its large scale use and are now widespread. Although great progress in our understanding of the mechanisms of CQ action and CQ resistance (CQR) has been achieved over the past two decades, including the identification of the molecules responsible for CQR (e.g., Plasmodium falciparum chloroquine resistant transporter, PfCRT) many questions remain unanswered. Here we highlight recent advances in our understanding of the genetics and molecular mechanisms of CQR, with particular emphasis on the role of genes such as pfcrt and pfmdr1 in the resistance to CQ and other drugs. New drug development and applications will undoubtedly benefit from a better understanding of CQR, eventually leading to more effective malaria control measures.

Laser scanning cytometry (LCS) allows detailed analysis of the cell cycle in PI stained human fibroblasts (TIG-7).

We have demonstrated a method for the in situ determination of the cell cycle phases of TIG-7 fibroblasts using a laser scanning cytometer (LSC) which has not only a function equivalent to flow cytometry (FCM) but also has a capability unique in itself. LSC allows a more detailed analysis of the cell cycle in cells stained with propidium iodide (PI) than FCM. With LSC it is possible to discriminate between mitotic cells and G2 cells, between post-mitotic cells and G1 cells, and between quiescent cells and cycling cells in a PI fluorescence peak (chromatin condensation) vs. fluorescence value (DNA content) cytogram for cells stained with PI. These were amply confirmed by experiments using colcemid and adriamycin. We were able to identify at least six cell subpopulations for PI stained cells using LSC; namely G1, S, G2, M, postmitotic and quiescent cell populations. LSC analysis facilitates the monitoring of effects of drugs on the cell cycle.

Rheumatoid neutrophilic dermatitis.

Rheumatoid neutrophilic dermatitis (RND) is a rare disorder seen in patients with severe rheumatoid arthritis. We present a 63-year-old Japanese with seropositive rheumatoid arthritis and rheumatoid lung who showed erythematous plaques and papules with pustules on his extremities and buttocks. Histological examination revealed a dense neutrophilic infiltration the upper dermis and neutrophilic abscesses in the dermal papillae. obvious vasculitis was seen. We suggest that RND could be included within the concept of a neutrophilic vascular reaction and that this condition develops through immune complex-mediated and neutrophil-induced vascular damage.

A primary hepatic carcinoid tumor: evaluation by computed tomography and magnetic resonance imaging.

A case of primary hepatic carcinoid tumor is reported. A 40-year-old woman was admitted to our hospital because of an epigastric mass. Ultrasonography demonstrated a heterogeneous, hyperechoic mass with hypoechoic area in the left lobe of the liver. CT showed a hypodense lesion with more hypodense areas. Enhanced CT showed slight retention of contrast medium and a low density area that suggested necrosis in the tumor. The mass appeared as a heterogeneous area of low signal intensity with sharp margin on T1-weighted MRI. T2-weighted MRI revealed a high intensity mass with higher intensity foci. Left lateral segmental hepatic resection was performed. Immunohistochemical and electron microscopic studies revealed the carcinoid tumor. Postoperatively, careful, intensive search revealed no other primary source of the tumor. The higher intensity foci were microscopically identified as intratumoral hemorrhage, corresponding to hemorrhagic areas on the cut surface. Therefore, T2-weighted MRI might reflect the pathological characteristics of the tumor.
