Effect of supernatants derived from T lymphocyte culture in minimal change nephrotic syndrome on rat kidney capillaries.

The effect on the kidney capillaries of T lymphocyte culture supernatants from patients with minimal change nephrotic syndrome was studied by infusion into the left renal artery of rats. The supernatants of T lymphocyte cultures from nephrotic patients induced significant proteinuria and a reduction of anionic sites in the glomerular basement membrane of the kidneys in the rats. These changes were not produced in rats receiving equivalent supernatants from healthy controls.

Influence of alpha-chloralose on muscle relaxant effect of chlorzoxazone in rats and pharmacodynamic analysis.

The Pharmacological response intensity of chlorzoxazone (CZX) was measured by the crossed extensor reflex (CER) method in anesthetized rats. The appropriate dose of alpha-chloralose (ACH) was examined to bring about a quantitative effect of CZX. The pharmacodynamics of CZX was also characterized in the anesthetized rats at three different doses of CZX. When rats were anesthetized with ACH at three different doses (80, 110, and 150 mg/kg i.p.), the CER response caused by CZX at the doses of 25, 50, and 75 mg/kg p.o. was depressed dose-dependently. Among these three different doses of ACH, 80 and 110 mg/kg gave similar results in describing the pharmacological response induced by CZX with time, whereas a dose of 150 mg/kg of ACH elicited a stronger and longer CZX response than the other two doses. The area under the intensity-time curve also showed the same tendency. These findings suggested that the dose size ranging from 80 to 110 mg/kg of ACH for surgical anesthesia was appropriate to evaluate the quantitative pharmacological response of CZX. Pharmacodynamic analysis offered the results that the dose-normalized biophase levels of CZX were coincident with each other when CZX was given at three different doses under ACH anesthesia at the dose of 80 mg/kg. This suggested that the disposition of CZX in rats obeyed linear kinetics and the theoretical values of CZX-induced response would be predictable. This implied that pharmacological response intensity of CZX was reasonably related to the relative biophase CZX level via Hill's equation.

Phenolic constituents of licorice. II. Structures of licopyranocoumarin, licoarylcoumarin and glisoflavone, and inhibitory effects of licorice phenolics on xanthine oxidase.

An anti-HIV (human immunodeficiency virus) phenolic constituent, licopyranocoumarin (4), and two other new phenolics named licoarylcoumarin (5) and glisoflavone (6) were isolated from Si-pei licorice (a commercial licorice; root and stolon of Glycyrrhiza sp. from the north-western region of China) using droplet countercurrent chromatography and centrifugal partition chromatography, and their structures were assigned based on chemical and spectroscopic data. Kaempferol 3-O-methyl ether (7) and licocoumarone (8) were also isolated from the licorice. The inhibitory effects of ten licorice phenolics on xanthine oxidase were examined. Licochalcone B (1), glycyrrhisoflavone (2), 8 and licochalcone A (19) showed 50% inhibition at the concentration of 1.3-5.6 x 10(-5) M.

[Synthesis of 2-oxooctahydroimidazo[1,2-alpha]pyridine-3-spiro-4'-piperidine derivatives possessing antiapomorphine activity].

4'-Carbamoyl-1,4'-bipiperidine 1 was dehydrogenated on Pd-C to give 2-oxo-2,3,5,6,7,8-hexahydroimidazo[1,2-alpha]pyridine-3-spiro-4'-p iperidine 2, which was reduced to 2-oxo-1,2,3,5,6,7,8,8a-octahydroimidazo[1,2-alpha]pyridine-3-spiro -4'- piperidine 3 with NaBH4. The iminodibenzyl and similar derivatives of 2 and 3 were synthesized and evaluated by using antiapomorphine test in mice. The derivatives of 3 had more potent antagonistic activity than those of 2 and the order of potency of 3 was: chloriminodibenzyl greater than chlordibenzocycloheptene greater than iminodibenzyl greater than iminostilbene greater than fluorene. Further chemical modification of the most active chloriminodibenzyl derivative 8, such as the substitution of Cl atom to Br atom, N-methylation on amine moiety, the replacement of imidazopyridine ring to imidazopyrrole ring, did not give any positive effect. Therefore, 8 may have potential usefulness as an antipsychotic drug.

[Neuroleptic properties of Y-20024, a new benzofurancarboxamide derivative].

N-[(1-butyl-2-pyrrolidinyl)methyl]-2-methyl-5-sulfamoyl-2,3- dihydrobenzofuran-7-carboxamide hydrochloride (Y-20024) was synthesized in our laboratories in anticipation of developing a new psychotropic drug. In the present study, the pharmacological properties of Y-20024 were compared with those of sulpiride (SPR) and haloperidol (HPD). Administered orally, Y-20024 was 10 times stronger than SPR in inhibiting apomorphine (0.5 mg/kg, s.c.)-induced hyperactivity in mice; administered intravenously or intracerebroventricularly, it was 2 times or one third as strong, respectively. Y-20024 was almost equipotent to SPR in antagonizing apomorphine (0.1 mg/kg, s.c.)-induced vomiting in dogs. Combined with methamphetamine (5 mg/kg, i.p.), Y-20024 and SPR induced mortality dose-dependently in rats, but HPD did not. The mammotropic activity of Y-20024 administered orally once a day for 5 days was almost equipotent to that of SPR. From these results, Y-20024, pharmacologically similar to SPR, but superior in bioavailability and penetration through the blood brain barrier, may have potential usefulness as an antipsychotic drug.

[Deformity of the fingers among women workers at a papermaking mill].

With regard to deformity of the fingers (chiefly Heberden's nodes) frequently occurring in Japan among workers preparing school lunches, an epidemiological study was carried out with a suspicion that this deformity might be an occupational disease. We studied the deformity of fingers among woman employees of a papermaking mill (262 mill workers in comparison with 108 office workers) who excessively use their fingers on the job like workers preparing school lunches. The results of this study showed that the number of mill workers suffering from deformity of the fingers (chiefly Heberden's nodes) was significantly (p less than 0.01) greater than that of office workers. Genetic factor, aging factor and mechanical factor have been cited as the etiological factors for the development of Heberden's nodes. However, our findings suggest that mechanical factor is a more important rather than the genetic or aging factor. As for mechanical factor, we have proposed for the first time that repetition of strong and precise pinching movement with the use of all fingers of both hands is an important factor.

Acute thrombosis of the inferior vena cava and hepatic veins in patients with Budd-Chiari syndrome: CT demonstration.

We reviewed the CT findings in 17 patients with angiographically proved Budd-Chiari syndrome to determine the ability of CT to show acute thrombosis of the inferior vena cava (IVC) and hepatic veins. In eight patients with membranes (web or band) in the IVC, no thrombus was detected with CT or angiography. In the other nine patients, thrombi in the IVC and/or hepatic veins were seen as intraluminal filling defects that did not change in appearance on precontrast and postcontrast CT scans. Attenuation values of intraluminal filling defects of the IVC ranged from 38 to 42 H in four patients. High-attenuation intraluminal filling defects (60-70 H) of the IVC (five patients) and hepatic veins (one of five patients) were detected. Of these five patients, four had acute symptoms and one had chronic vague symptoms. The underlying disease was a web or band in the IVC and hepatic veins in three patients, invasive hepatocellular carcinoma in one, and injury to the IVC wall during hepatectomy in one. Inferior venacavography showed occlusion of the hepatic segment of the IVC in all five patients. Additional angiograms obtained by injection of contrast medium after a catheter tip was placed in the occluded hepatic IVC showed numerous filling defects suggestive of thrombi of recent onset, which correlated with the high-attenuation thrombi seen on CT scans in two patients. In the remaining three patients, high-attenuation areas in the IVC and hepatic veins also were considered to represent thrombi of recent onset because the attenuation values later decreased to 33-42 H. Spontaneous reduction in diameter of the thrombosed segment of the IVC was observed in four of the five patients. Knowledge of the CT features of acute thrombosis of the IVC and hepatic veins is useful in the early diagnosis of Budd-Chiari syndrome.

Roles of endogenous leukotrienes and prostaglandins in the healing of gastric ulcers induced by acetic acid in rats.

We investigated the roles of endogenous leukotrienes (LTs) and prostaglandins (PGs) in the healing of gastric ulcers induced by acetic acid in rats. The mucosal levels of LTB4 and sulfidopeptide LT at the ulcer edge had increased by one day after the induction of ulcers. AA-861, a selective inhibitor of 5-lipoxygenase, did not affect the ulcer healing. Indomethacin delayed the healing. Cimetidine did not affect this delay, but ornoprostil, a PGE1, derivative, prevented it. These results suggest that endogenous LTs are not related to the healing of gastric ulcers and that a deficiency in endogenous PGs may be involved in the persistence of gastric ulcers.

Induction of cytochrome P-450 isozymes by chromanamine derivatives in rat liver.

1. Pretreatment of rats with 6-(3-picolyl)amino-2,2,5,8-tetramethylchromane (PATC) for 7 days resulted in a significant increase in the activities of benzphetamine N-demethylase, p-nitroanisole O-demethylase and aniline hydroxylase in liver microsomes prepared 24 h after the last treatment. 2. Analysis by Western blot showed that PATC induces cytochrome P-450 b, P-450 c and P-450 d, which are the major forms of cytochrome P-450 in liver microsomes of rats when pretreated with phenobarbital and 3-methylcholanthrene. 3. Exposure of liver sections to the antibodies to cytochrome P-450 b and P-450 c resulted in intense immunostaining within the centrilobular regions, but produced staining of considerably weaker intensity in the perilobular region. Semiquantitative immunochemical analysis, by image analyser, of cytochrome P-450 b and P-450 c showed that centrilobular hepatocytes were stained more intensively than perilobular hepatocytes. 4. These results indicate that PATC induces cytochromes P-450 b and P-450 c, in the centrilobular hepatocytes to a greater degree than those in the perilobular hepatocytes. 5. Co-administration of PATC with pentobarbital caused a significant increase in pentobarbital sleeping time. Furthermore, PATC was found to cause a decrease in the activity of benzphetamine N-demethylase in liver microsomes prepared 30 min after treatment with the drug.

[The role of catecholamine content in human platelets; clinical implications in patients with pheochromocytoma].

UNLABELLED: This study examined if the platelet catecholamine content (CA) in healthy adults and patients with adrenal tumors reflected the activity of the sympatho-adrenomedullary system. RESULTS: (1) In 44 healthy adults, platelet norepinephrine (NE) levels ranged from 72 to 1175 pg.mg-1.protein-1 (mean: 473pg.mg-1.protein-1) and platelet epinephrine (E) levels ranged from 23 to 273 pg.mg-1.protein-1 (mean: 68 pg.mg-1.protein-1). These were used as normal ranges for this study. (2) In patients with adrenal tumors, plasma CA level, urinary CA elimination and platelet CA content were higher in patients with pheochromocytoma than in those with other adrenal tumors. Mean platelet NE and E levels for patients with pheochromocytoma were 2188 and 336 pg.mg-1.protein-1, respectively. These results indicate that a large amount of CA is contained in platelets (particularly in patients with pheochromocytoma), and that platelet CA level is very useful for preoperative diagnosis of pheochromocytoma. The results also suggest that platelet CA level closely reflects the activity of the sympatho-adrenomedullary system.

[A case of reexpansion pulmonary edema during fenestration of a giant hepatic cyst].

We experienced a case of reexpansion pulmonary edema during the fenestration of giant hepatic cyst. The patient was a 56 year-old female who had been suffering from dyspnea due to a large abdominal mass for 3 months. The preoperative chest X-ray revealed remarkable elevation of diaphragm (right third and left fifth intercostal spaces). The result of blood gas analysis showed hypoxemia and respiratory compensation for metabolic acidosis. By the drainage of 12L of the cyst fluid for 30 minutes during operation, bloody and foamy bronchial secretion spurted out. Vigorous respiratory management using PEEP was promptly started and continued postoperatively in ICU. The endotrachial tube was withdrawn on the 2nd postoperative day. The ratio of protein in airway secretion in to plasma protein was 0.78, and it appeared that the pulmonary edema was mainly caused by increased permeability of pulmonary capillary vessels.

[MR measurement of normal brainstem cerebellum and corpus callosum on midsagittal section].

The dimensions of the brainstem, cerebellum and corpus callosum were measured on magnetic resonance (MR) images with sagittal spin-echo sequence. Eighty-two normal adults (average 49.6 years old) were measured. The mesencephalic, pontine or cerebellar diameters and lengths could be measured more accurately and reproducibly than medullary diameter and length. The anterio-posterior diameter of the pons and the cerebellum was 23.2 +/- 1.4 mm and 26.4 +/- 2.5 mm respectively. The length of the pons and the cerebellum was 27.8 +/- 2 mm and 45.8 +/- 3.5 mm respectively. We have observed focal thinning at the body of corpus callosum in 73%. This narrowing is almost unquestionably a normal variant.

[Effects of intravenous administration of bupivacaine on the renal sympathetic nerve activity in the cat].

The effects of an i.v. bolus injection of sub-seizure doses of bupivacaine on the renal sympathetic nerve activity (RSNA), mean arterial pressure (MAP) and heart rate (HR) were studied in cats. The cats received intravenous administration of bupivacaine (0.4 mg.kg-1, 0.8 mg.kg-1 and 1.2 mg.kg-1 at 50- and 110-min interval respectively in order of increasing doses) during nitrous oxide anesthesia and nitrous oxide-halothane anesthesia. During nitrous oxide anesthesia, MAP and HR did not change significantly with all doses of bupivacaine. RSNA increased at first and then decreased slightly following all doses of bupivacaine. In cats with denervated baroceptor, a change of RSNA after bupivacaine 0.8 mg.kg-1 i.v. was similar to that in intact cats. In contrast, RSNA, MAP and HR decreased significantly following bupivacaine 1.2 mg.kg-1 i.v. during nitrous oxide-halothane anesthesia. Cardiovascular depression following bupivacaine i.v. during nitrous oxide-halothane anesthesia may be caused by both a decreased sympathetic activity and a direct myocardial depression.

Malignant spindle cell tumor of the pericardium. Evidence of sarcomatous mesothelioma with aberrant antigen expression.

A case of malignant spindle cell tumor occurring in the pericardium is presented. The tumor arose from the pericardium of a 51-year-old Japanese woman with no history of exposure to asbestos. The tumor extended into the pericardial and left pleural cavities. The primary and metastatic tumors consisted of fusiform cells with frequent mitoses. Ultrastructurally, the tumor cells possessed a discontinuous external lamina, cytoplasmic processes, microfilaments and desmosomal intercellular junctions. Immunohistochemical examination showed that most tumor cells were positive for Leu 7, and several for S-100 and glial fibrillary acidic protein. Unexpectedly, most of the tumor cells also expressed keratin. These findings favor a diagnosis of sarcomatous mesothelioma with aberrant antigenic expression or heterogeneous differentiation of neoplastic cells.
