Regulatory or structural alterations of c-onc genes including amplification, rearrangement and point mutation was implicated in the causation of various malignant tumors. However, such changes of molecular levels have not been reported so far in choriocarcinoma cells. In the present study, thus, 5 choriocarcinoma cell lines were analyzed by hybridization using 16 oncogene probes. By Southern blot hybridization of DNA extracted from these cells, 8 fold amplification of c-myc gene and rearrangement of c-fms gene were shown in ENAMI cells, although the role of these alterations remained unknown. Northern blot hybridization performed simultaneously demonstrated multiple expression of c-onc genes. 4 choriocarcinoma cell lines (HCCM, CHl, CCl, ENAMI) expressed at least 11 c-onc genes (H-ras, K-ras, N-ras, c-myc, N-myc, fos, fms, src, yes, erb B and raf); though the degree of expression of H-ras, C-myc, erb B and fms in these cells was either similar or enhanced as compared with normal fibroblast, the expression of two c-onc genes (N-myc and fos) was extremely enhanced. However, expression of K-ras and myb was either low or not detected. The multiple expression of c-onc genes seems to reflect partly on growth advantages of trophoblast. Transfection assay using NIH3T3 cells failed to form any transformed foci. Since choriocarcinoma cells which derived from the transformation of trophoblast of complete mole possess the genetic characteristics identical to the one of cells of complete mole, chromosomal instability was assumed to play a major role for multiple oncogene expression in choriocarcinoma cells.

We studied larval nematodes of four genera of the Anisakidae using a scanning electron microscope (SEM). The anterior and posterior extremities and cuticular structures of the 3rd-stage larvae (L3) of Anisakis type I, Pseudoterranova decipiens, Contracaecum type B and Hysterothylacium were examined. The 4th-stage larvae (L4) of Anisakis type I, P. decipiens, recovered after infection into laboratory rats, and the L3 and L4 of Anisakis type I larvae from human were also examined in the same way. There were generic differences in the shape and size of the lip bulges, external papillary structures, the appearance of the boring tooth, the width and depth of the grooves and ridges of the cuticle and the caudal structures of the L3. In Anisakis type I and P. decipiens L3, changes were seen in the anterior extremity, cuticle and posterior extremity after molting to the L4. Similar changes can be expected in larvae infecting man. The L4 of Anisakis type I from rat and man were similar, while the L4 of Anisakis type I and P. decipiens showed differences. These ultrastructural differences might be of value in the identification of fragments recovered during endoscopy in man.
This report describes the successful use of an upper arm flap, pedicled on the cephalic vein and with microvascular arterial anastomosis. It was used for the reconstruction of a defect of the temple after excision of a recurrent malignant melanoma and a previous block dissection of the neck.
The effects of oral diltiazem (90-180 mg/day for four weeks) on ventricular premature contractions (VPCs) were studied in 16 patients with frequent VPCs using 24-hour ambulatory ECG recordings. VPC frequency was evaluated as a function of underlying heart rate. Plots of VPC frequency vs. heart rate were made at 1-beat/min intervals for all heart rates recorded for at least five minutes during 24 hours. Patterns of correlation between VPC frequency and heart rate observed before diltiazem therapy included: 1) a relatively linear increase in VPCs with heart rate (positive correlation) in ten patients, 2) a linear decrease (negative correlation) in one patient, and 3) an increase at low heart rates and a decrease at high heart rates (bidirectional correlation) in five patients. Diltiazem significantly reduced the mean VPC frequency per 24 hours for patients with a positive correlation, but induced no significant change for patients with a negative or a bidirectional correlation. At the 65% level of VPC reduction, diltiazem was effective in eight of ten patients with a positive correlation but was not effective in the six patients with other correlations (p less than 0.01). These results suggest that an evaluation of VPC frequency as a function of heart rate predicts the response of VPCs to diltiazem.
Comparative histochemical studies of glycosidase activity were carried out in Clonorchis sinensis, Eurytrema pancreaticum, Fasciola hepatica, Dipylidium caninum, Hymenolepis nana, Ascaris suum, Toxocara canis, Ancylostoma caninum, Trichuris vulpis and Dirofilaria immitis. The enzymes examined were: N-acetyl-beta-glucosaminidase (EC 3.2.1.30), beta-glucuronidase (EC 3.2.1.31) and beta-galactosidase (EC 3.2.1.23). There were variations in enzyme distribution and intensity among the species and also between trematodes and nematodes; no marked positive reaction for these enzymes occurred in cestodes. In some trematodes, the caeca, especially in the brush border, and the tegument, subtegumental cells and testes, were reactive to the enzymes. In nematodes, although there was variation in reactions among species, N-acetyl-beta-glucosaminidase and beta-galactosidase were localized in the hypodermis and lateral cords excluding the excretory canal, and coelomocytes, intestinal epithelium and the walls of the reproductive systems.
The success of fasciocutaneous flaps is based on the existence of epifascial vascular networks and reliable blood supply. However, there has been no thorough classification of the vascular anatomy of the fascia and skin and there is some confusion in regard to cutaneous vascular nomenclature. We divide the vascular systems involved in the cutaneous circulation into four categories. This permits classification of skin flaps into five types (cutaneous, fasciocutaneous, adipofascial, septocutaneous, and musculocutaneous flaps). Fasciocutaneous flaps can be further divided into six types, according to the patterns of the vascular input to the fasciocutaneous plexus. This classification has been demonstrated to be related to clinical effects. Nine new free and island flaps are discussed.
We have modified the original Weidmann method for the measurement of internal longitudinal resistance in ventricular muscle by using air rather than silicon oil to insulate guinea pig papillary muscles and by omitting the tetrodotoxin inactivation of a portion of the preparation and have examined the requirements necessary for the theoretical assumptions to be satisfied by this or similar preparations. We found a homogeneous depolarization wavefront beyond about 1 mm from the stimulating electrodes. The adequacy of the interelectrode spacing was identified by a discrete plateau in the extracellular potential recording. The extracellular resistance in this preparation was sensitive to changes in the volume of the extracellular compartment, which we manipulated by changing inflow and outflow rates, and to changes in total ionic content of the superfusate. Our results establish the essential nature of maintaining constant flow rates and total ionic content and suggest that changes in volume and ionic content of a restricted extracellular space could conceivably influence conduction in vivo.

Fifty-four patients with acute myocardial infarction (AMI) were treated by percutaneous transluminal coronary recanalization (PTCR) within six hours after onset of symptoms or at the time of emergency coronary angiography. Of these, six patients had neither good collaterals nor recanalization, and followed by mechanical failure due to rupture of the left ventricular free wall or interventricular septum in five patients despite antihypertensive therapy. In the remaining 48 patients with good collaterals or recanalization, mechanical failure was statistically infrequent and occurred in only two patients (p less than 0.001). Ninety-four patients treated by PTCR were assessed in terms of mortality, cause of death and their hemodynamic findings. Cardiac deaths occurred in nine patients (9.6%); mechanical failure, in four; and cardiogenic shock due to the occlusion of the left main trunk, in five. Cardiac death was more frequently encountered in the era of the initial stage of PTCR (six of 53 cases: 11.3%) and the main cause was mechanical failure (four of six cases). On the other hand, cardiac death in recent years of PTCR was less (three of 41 cases: 7.3%), and all three had pump failure due to the occlusion of the left main trunk. Risk of mechanical failure was successfully resolved using intra-aortic balloon pumping and beta-blocker in addition to antihypertensive therapy. At the present time, PTCR and other supplementary therapy mentioned above reduced the mortality from mechanical failure and cardiogenic shock, but occlusion of the left main trunk remains an important cause of death in patients with AMI.

From 1969 to 1984, we treated 178 patients with acute gastric anisakiasis caused by gastric mucosal penetration of the Anisakis larvae. Because the larvae of Anisakis can always be observed by endoscopy and removed with biopsy forceps, early performance of endoscopy is highly recommended for patients in whom acute gastric anisakiasis is suspected and for those who have eaten raw fish within 12 hours before onset of the gastric symptoms. Endoscopic removal is essential to relieve the intolerable pain; after relief, the administration of antacids will repair the damaged gastric mucosa. Ulcer formation is rare.
The localization of the gut-associated circulating anodic antigen in Schistosoma japonicum adults was revealed by means of the immunofluorescence and a peroxidase-anti-peroxidase method with the electron microscope. The reaction sites were confined to amorphous material in the cecal lumen. The cecal lumen generally was infolded with lamellae. The antigenic material appeared to be secreted by the rough endoplasmic reticulum, probably through the Golgi apparatus, into the lumen. Observations of male and female worms showed that there was a clear difference between the sexes in antigen concentration. The thick epithelium of the female worm, with well-developed cisternae on the endoplasmic reticulum, produced a lot more antigen than the male. Positive staining with ruthenium red confirmed that the antigenic material was a negatively charged polysaccharide as had been previously reported.
Reconstruction of cervical oesophagus with free revascularised small intestine is a reliable and effective technique but a common complication is dysphagia. This paper presents a method of splitting the wall of the intestine longitudinally, folding it and suturing it to double the size of the lumen of the reconstructed conduit and to abolish effective peristalsis. The method has proved to be useful in three clinical cases.
