Epidermolysis bullosa simplex and mottled pigmentation: a new dominant syndrome. I. Clinical and histological features.

A family is reported in which 11 members presented epidermolysis bullosa simplex with some unusual features, and 10 of whom had congenital mottled hyper- and hypopigmentation of the skin. Both anomalies are inherited together in an autosomal dominant fashion, although dominant X-linkage cannot be excluded. The epidermolysis without dyspigmentation in the 11th individual suggests that the syndrome in the other 10 members is due to genetic linkage of two independent genes and not to pleiotropism of a single mutant gene. The mottled pigmentation seems distinct from previously known dyspigmentations and is also suggestive of autosomal inactivation in man. (See addendum).

[Passive smoking].

Passive smoking is the involuntary inspiration of smoky indoor air. Based on the information available today, it may be assumed that passive smoking normally is no health hazard as far as the classical smoker's diseases (lung cancer, myocardial infarct, etc.) are concerned. Nevertheless, it is probable that irritations caused by tobacco smoke have an unfavorable influence on the health of small children and that of already sick persons. The main problem of passive smoking is annoyance due to odor and irritations of eyes and respiratory organs. Our investigations in a climatic chamber with healthy subjects show that air pollution caused by tobacco smoke as indicated by 5 ppm CO leads to marked eye irritations--objectively as well as subjectively--in 15 to 20% of the subjects. This corresponds to smoking 10 cigarettes per hour in a small room with an air ventilation rate of four times per hour. If air pollution caused by tobacco smoke lies below the level of 2 ppm CO, irritations and annoyance for healthy persons are regarded as low and tolerable. This corresponds to about four cigarettes per hour under the same circumstances.

Treatment of parapsoriasis en plaques, mycosis fungoides, and Sézary's syndrome with trioxsalen baths followed by ultraviolet light.

Three patients with parapsoriasis en plaques (PEP), fifteen with mycosis fungoides (MF), and one with Sézary's syndrome (SS) were given a bath to which a small amount of trioxsalen solution had been added, and then exposed to ultraviolet (UV) radiation from a bank of dysprosium lamps. Within 2--6 months of this treatment the skin lesions healed completely or almost completely in all 3 patients with PEP, in all 7 with MF stage II and in 4 of 5 with MF stage III. Two patients with MF stages IV--V showed a fair degree of improvement. One with erythrodermic form of MF responded, but poorly. The patient with SS and the one with erythrodermic MF responded with severe phototoxic reactions.

Absorption of 8-hydroxyquinolines through the human skin.

The skin absorption of clioquinol and chloroquinaldol in ointment or cream base was studied in fourteen patients with widespread dermatoses. A serum concentration in the range of 0.3--1.3 microgram/ml using clioquinol and 0.05--0.1 microgram/ml with chloroquinaldol was reached by the second day and persisted throughout the topical treatment. The mean serum half-life of clioquinol and of chloroquinaldol was estimated to 25 and 6 hours, respectively. Local application of zinc oxide ointment after the end of the treatment chelate-bound the skin deposits of clioquinol and lowered the serum half-life to 11 hours. The mean daily urinary excretion of the applied amount of clioquinol and chloroquinaldol was 4.5 and 6%, respectively.

Comparative treatment of psoriasis with UV-light, trioxsalen plus UV-light, and coal tar plus UV-light.

Areas within psoriasis plaques have been treated with UV-light alone, trioxsalen plus UV-light, and coal tar (liquid carbonis detergens) plus UV-light, using the 313, 365 and 405 nm wavelength bands in various doses. With the 313 nm band, the same degree of healing was obtained with all three types of treatment. With the 365 nm band, no healing was achieved with UV-light alone, but healing did occur with UV-light when the skin was sensitized with trioxsalen. Coal tar itself has a certain healing effect on psoriasis but the effect is so markedly enhanced by light of the 365 nm band that the healing is comparable to that obtained with the 313 nm band. No healing was seen with light of the 405 nm band. The patients who were healed with the 313 nm band alone, also healed with trioxsalen or coal tar plus UV-light of the 365 nm band. With each method there is an individual optimal light dose of 1--2 MED that gives healing.

Development of narrow-band lamps for testing and clinical use.

A compact narrow band lamp for high intensity irradiation has been developed. It consists of a high-pressure mercury lamp, a shutter and water cooled filters for spectral isolation of the light. A mixing device permits simultaneous irradiation with two different wavelenghts by using two lamps. The spectral distribution of the light within the bands is given for seven filter combinations. The construction and calibration of a power meter is also described.

Nonverbal dialogue with the brain-damaged elderly.

In a population of twelve 63- to 89-year-old residents of a nursing home, rank-ordered scores of copying errors on the revised Benton visual retention test were found to be significantly correlated with the rotational shifts in the Minnesota percepto-diagnostic test. Residents on both ends of the continuum of brain damage could paint interesting, expressive and/or original pictures, in spite of the fact that the first eight residents were on psychotropic medication and the last four were occasionally handicapped by constructional apraxia. Ten adjudicators ranked pairs of figure drawings for each resident in decreasing order of 'aesthetic pleasingness' and 'originality'. Degree of brain damage was found unrelated to both measures. Interadjudicator agreement was stronger on 'originality' than on 'aesthetic pleasingness'. Some of the beneficial and provocative results of art therapy with the elderly are elaborated ans a plea made for accepting their observed creative performance as part of the realm of legitimate art.
