[Digital mammo-radiography (DMR) in mass screening for breast cancer].

There are still many problems to be overcome in the development of mammographic apparatus suitable for mass screening purposes, but a certain number of these have been solved in the system discussed in the present paper. The equipment employs the technique of displaying I.I. images on a monitor by means of an image processor. This possesses the advantages of both film mammography and xeromammography, and the X-ray dose involved is small. The following advantages over film mammography are offered: The system is capable of clearly bringing out very minor differences of contrast. Tumor shadows can be easily displayed. The X-ray dose is small: 0.03-0.05 R. The equipment is inexpensive to use. Patient positioning is simple, and examinations take little time. Long operator training programs are unnecessary. The system uses digital signals, and so automatic diagnosis is possible. Theoretically, at least, tumors as small as 0.2 mm in diameter can be detected. The future goal is discovering unpalpable breast cancers. DMR is useful because of low dose, low cost and high safety. In our study as screening for breast cancer, DMR system shows high sensitivity (87.1%), specificity (78.7%) in 391 cases of out patients.

[5-FU/ADR/MMC combined hepatic infusion in liver metastases from gastric cancer].

Twenty patients with liver metastases from gastric cancer judged non-resectable and the factors limiting their survival were treated with 5-fluorouracil (5-FU), adriamycin (ADR) and mitomycin C (MMC) combined hepatic arterial infusion chemotherapy. The catheter was placed into the hepatic artery, via the left subclavian artery in every case, and 5-FU 334 mg/m2 qw, ADR 20 mg/m2 q4w, MMC 2.7 mg/m2 q2w was administered bolusly. Myelo-suppression (WBC less than 2,000 or platelet less than 50,000) occurred in 25% of the patients, but this therapy was tolerable in all patients without major complication. The response rate in evaluable cases by CT-scan was 76.5% (4CR + 9PR/17), and 50% survival was 407 days. A significant correlation between the existence of extra-hepatic lesions and 50% survival (extra-hepatic lesions (-): 196 days vs (+): 464 days) was noted. The comparison between responder and non-responder in terms of 50% survival and the cause of death showed the importance of control of liver metastases for improved survival.

Massive outbreak of antimicrobial-resistant salmonellosis traced to pasteurized milk.

Two waves of antimicrobial-resistant Salmonella typhimurium infections in Illinois totaling over 16 000 culture-confirmed cases were traced to two brands of pasteurized 2% milk produced by a single dairy plant. Salmonellosis was associated with taking antimicrobials before onset of illness. Two surveys to determine the number of persons who were actually affected yielded estimates of 168 791 and 197 581 persons, making this the largest outbreak of salmonellosis ever identified in the United States. The epidemic strain was easily identified because it had a rare antimicrobial resistance pattern and a highly unusual plasmid profile; study of stored isolates showed it had caused clusters of salmonellosis during the previous ten months that may have been related to the same plant, suggesting that the strain had persisted in the plant and repeatedly contaminated milk after pasteurization.

Structural differences found in the asparagine-linked sugar chains of human chorionic gonadotropins purified from the urine of patients with invasive mole and with choriocarcinoma.

Paper electrophoresis of the oligosaccharide fraction obtained by hydrazinolysis of human chorionic gonadotropin (HCG), which was purified from the urine of two patients with invasive mole, gave fractionation patterns different from those of normal and hydatidiform mole HCGs. Structural study of the oligosaccharides in each fraction revealed that the triantennary complex-type asparagine-linked sugar chains, specifically found in choriocarcinoma HCGs, were included in the two HCG samples. However, the unusual biantennary complex type sugar chains, which are also specific for the tumor HCGs, were not detected at all. This result indicated that abnormal expression of N-acetylglucosaminyl-transferase IV during the development of choriocarcinoma occurs in two steps: (a) ectopic expression of the regular N-acetylglucosaminyltransferase IV; and (b) modification of the substrate specificity of the enzyme.

Electron microscopic study on S-100 protein-immunoreactive cells in the guinea pig duodenum, with special reference to the interstitial cells of Cajal.

S-100 protein-immunoreactive cells were studied in the nervous system of the guinea pig duodenum by pre-embedding immunoelectron microscopic cytochemistry. Immunoreactivity of S-100 protein was found diffusely in the cytoplasm of enteroglial cells in the ganglia, nerve strands and autonomic ground plexus. There were fibroblast-like cells containing no S-100 protein immunoreactivity around the ganglia and nerve bundles. Relationship between the fibroblast-like cells and the interstitial cells of Cajal was discussed.

Systematic fractionation of oligosaccharides of human immunoglobulin G by serial affinity chromatography on immobilized lectin columns.

Human immunoglobulin G is known to contain 16 different biantennary complex-type asparagine-linked sugar chains, each of which occurs in a nonsialylated, monosialylated, or disialylated form. These oligosaccharides can be separated into 14 fractions by sequential affinity chromatography with Aleuria aurantia lectin (AAL)-Sepharose, RCA120-WG003, and E4-phytohemagglutinin-agarose columns. Twelve of them were found to contain a single oligosaccharide, while the fraction which passed through all three columns was shown to contain two oligosaccharides, GlcNAc beta 1----2Man alpha 1----6(+/- GlcNAc beta 1----4) (GlcNAc beta 1----2Man alpha 1----3)Man beta 1----4GlcNAc beta 1----4GlcNAcOT. The fraction, which bound to the AAL-Sepharose column and passed through the remaining two lectin columns, also contained two oligosaccharides, GlcNAc beta 1----2Man alpha 1----6(+/- GlcNAc beta 1----4) (GlcNAc beta 1----2Man alpha 1----3)Man beta 1----4GlcNAc beta 1----4 (Fuc alpha 1----6)GlcNAcOT. These results indicated that serial affinity chromatography with the three lectin columns can be used effectively to detect changes in the sugar chains of IgG resulting from diseases such as rheumatoid arthritis.

Regulation of Thy-1 gene expression in transgenic mice.

Genomic DNA fragments encompassing the human Thy-1 or mouse Thy-1.1 gene have been microinjected into pronuclei of mouse embryos homozygous for the Thy-1.2 allele. In the resulting transgenic mice, the human gene is expressed in a pattern characteristic of normal human tissues, and is not influenced by the pattern of endogenous mouse Thy-1 expression. The mouse Thy-1.1 gene fragment is expressed in a pattern typical of mouse Thy-1, although it is more limited in its distribution. The results indicate the presence of multiple cis-acting regulators of Thy-1 gene expression that have changed in both their character and arrangement over the course of Thy-1 gene evolution.
