[Lipid peroxidation induced by methylmercury in rats].

The ability of methylmercury (MMC) to induce lipid peroxidation was tested in rats. One group of 5 rats (MMC group) took a diet to which 10 ppm of methylmercury was added while another group of 5 rats was fed a diet containing no MMC as a control. During the experimental period of 14 weeks (98 days) on these diets, body weight gain and amount of food consumption were checked individually for each two-day interval. Samples of expired air and blood were obtained every two weeks and analysed for ethane and pentane production and for plasma fatty acid composition, respectively. Results obtained were as follows: 1. During the first half of the 14-week experimental period, there was no significant difference in the amount of food consumption between the MMC group and the control group. However, a significantly smaller amount of food consumption was found in the MMC group as compared to the control group during the latter half of the experiment. The amount of food consumption was directly reflected in the body weight gain throughout the experimental period. 2. The amount of expired ethane was generally higher in the MMC group than in the control group throughout the experimental period. During the last 2 weeks of the experimental period, the difference in the amount of expired ethane between the two groups was statistically significant. 3. The amount of expired pentane was also higher in the MMC group than in the control group during all the weeks of the experimental period.(ABSTRACT TRUNCATED AT 250 WORDS)

[Induction of lipid peroxidation and change of fatty acid composition in rats fed methylmercury].

Four groups of rats, with 5 animals in each, were fed a diet to which 1, 2, 4 or 8 ppm of methylmercury (MeHg) were added while another group of 5 rats was fed a diet containing no MeHg as a control. Moreover, each rat was daily supplied the same volume of these diets. During the experimental period of 40 days, body weights of the rats were daily checked individually. On day 40, expired air of the rats was collected to analyse for ethane and pentane production, and then the rats were sacrificed to obtain the liver, kidneys, brain and plasma. Thio-barbituric acid (TBA)-reactants in the selective organs and plasma were measured to investigate the effects of MeHg on lipid peroxidation. Further, gas-chromatographic analyses of fatty acids were carried out on the samples of the selective organs and plasma to examine the effects of MeHg on lipid metabolism. Results obtained were as follows: 1. The fifth day after the experiment started, the growth rates of the groups fed a diet containing MeHg (the MeHg groups) were higher than those of the control group. Moreover, the growth rates tended to increase with increases of MeHg concentrations in food from days 25 to 32 of the experimental period. Furthermore, the weights of the kidneys increased with increases in MeHg concentrations in food. 2. The amounts of expired ethane and pentane did not increase proportionally with increases in MeHg concentrations in food but those of expired ethane and pentane were higher in the MeHg groups than in the control group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fibromuscular dysplasia with cerebral infarction in children. Case report].

A 15-year-old boy suddenly developed a headache, followed by progressive weakness of the right extremities. A computed tomographic scan obtained on admission showed a low-density area in the left putamen extending to the internal capsule. No neurological deficits were observed other than slight dysarthria and right hemiparesis. Left carotid angiography showed the characteristic "string of beads" deformity of fibromuscular dysplasia (FMD) extracranially and complete occlusion of the internal carotid artery at the C2 portion. The right carotid artery and both vertebral arteries were normal. Anastomosis of the left superficial temporal and middle cerebral arteries was performed 1 month after the onset. The patient recovered quickly from the hemiplegia and was discharged with no neurological deficits. Angiograms obtained 3 months later showed not only patency of the bypass, but also recanalization of the intracranial internal carotid artery. Cervico-cephalic FMD mainly affects middle-aged women, and is uncommon in children; only nine childhood cases have been reported. This disorder may play an important role in the genesis of strokes among children.

Pathogenesis of platelet-activating factor-induced gastric mucosal damage in rats.

Platelet-activating factor (PAF), given intravenously, induced erosions and hyperemia to the rats stomachs. Gastric mucosal blood flow was decreased and thiobarbituric acid (TBA) reactants (an index of lipid peroxidation) in the gastric mucosa were increased 10 min after PAF injection. Superoxide dismutase plus catalase reduced the gastric mucosal lesions and TBA reactants, but had no influence on gastric mucosal blood flow. A reduction in the number of circulating polymorphonuclear leukocytes (PMN) reduced the gastric mucosal damage and TBA reactants, and inhibited the decrease in gastric mucosal blood flow, as observed 30 and 60 min after PAF injection. PAF induced superoxide production by rat PMN and enhanced that stimulated by opsonized zymosan or phorbol myristate acetate. These results suggest that microcirculatory disturbance and oxygen-derived free radicals generated by PMN play important roles in gastric mucosal lesions induced by PAF.

FK409, a novel vasodilator isolated from the acid-treated fermentation broth of Streptomyces griseosporeus. II. Structure of FK409 and its precursor FR-900411.

In a purification study of novel vasodilator FK409 from the fermentation broth of Streptomyces griseosporeus, we found the existence of FR-900411, a precursor of FK409 in the broth, suggesting that FK409 was derived from FR-900411 via a chemical reaction. We therefore isolated FR-900411 from the broth. The structure of FK409 and FR-900411 were determined to be 1 and 2 respectively, on the basis of spectroscopic and chemical evidence.

[Silent myocardial ischemia evaluated by ambulatory left ventricular function monitoring].

To determine whether left ventricular function is less disturbed in asymptomatic ischemia than in symptomatic ischemia, exercise-induced left ventricular function was measured in beat-to-beat using an ambulatory left ventricular function monitoring system. The study subjects were 22 patients with coronary artery disease. Supine and sitting ergometer exercise tests were performed. Of 44 exercise tests, 33 showed significant ST depression on electrocardiograms. Among these 33, 17 were asymptomatic: 16, symptomatic. The left ventricular functions during exercise of these 33 were analyzed and compared with those of five normal controls. The changes in end-diastolic volume were not so significant either in controls or in the asymptomatic and symptomatic groups, but the changes in end-systolic volume were reversed in the diseased groups, particularly in the symptomatic group. The changes in ejection fraction (EF), therefore, were significantly negative in the diseased group, particularly in the symptomatic group. The correlation between exercise-induced left ventricular dysfunction and symptoms was evaluated among the 33 patients. Symptoms were present in 35% (6/17) in less than 10% decrease in EF, 44% (4/9) in 10-15% decrease, and 85% (6/7) in greater than or equal to 15% decrease, respectively. Thus, asymptomatic ischemia represents a lesser degree of myocardial ischemia as indicated by mild left ventricular dysfunction, compared with symptomatic ischemia. However, some cases of severe myocardial ischemia did not develop symptoms. We concluded that silent myocardial ischemia is responsible for a lesser degree of myocardial ischemia and decreased pain perception.

[Combination chemotherapy with UFT, etoposide, CDDP, adriamycin (FEPA) in advanced gastric cancer].

Fifteen patients with inoperable advanced gastric cancer were treated with UFT.etoposide.CDDP.adriamycin (FEPA). Six were males and 9 were females with an average age of 58 (range 40 to 80 years). Nine patients were in P.S. 2 and 6 in P.S. 3. FEPA regimen was performed every 4 weeks as follows: UFT 400 mg/m2 (p.o.) every day, etoposide 50 mg/m2 (i.v.), CDDP 25 mg/m2 (i.v.) and adriamycin 10 mg/m2 (i.v.) on days 1, 8, 15 and 22. Among 15 patients, 6 partial remissions and 4 minor responses were obtained. Gastrectomy was performed in 2 of 6 PR patients after FEPA chemotherapy. The overall response rate was 40.0%. As for side effects, mild myelosuppression was the most frequent (66.7%), followed by alopecia and nausea. We concluded that combination chemotherapy of FEPA is useful for inoperable advanced gastric cancer.

[Role of polymorphonuclear leukocytes and oxygen-derived free radicals in the formation of gastric mucosal lesions induced by platelet-activating factor].

Gastric hemorrhagic erosions were induced by the administration of platelet-activating factor (PAF). Gastric wall blood flow was decreased and thiobarbituric acid (TBA) reactants in the gastric mucosa were increased 10 min after PAF injection. SOD and catalase reduced gastric mucosal lesions and TBA reactants, but had no influence on gastric blood flow. In polymorphonuclear leukocytes (PMN)-depleted rats, gastric mucosal lesions and TBA reactants in the gastric mucosa were reduced and gastric blood flow were increased. PAF induced the superoxide production from rat PMN in a dose dependent manner. These results suggest that oxygen-derived free radicals produced by PMN and lipid peroxidation may play an important role in the pathogenesis of gastric mucosal injury induced by PAF.

[Two cases of oral leukoplakia possibly associated with human papillomavirus].

Two cases of oral leukoplakia were studied immunohistologically and ultrastructurally. By means of avidin-biotin peroxidase complex technique using papillomavirus specific antibody, distinct nuclear staining of epithelial cells of middle and upper layers was observed in both cases. Further evidence for the presence of human papillomavirus was obtained by the electron micrograph, which revealed virus-like particles (30-40 nm in diameter) within the nucleus.

[Case report of a second resection of rectal leiomyosarcoma due to recurrence after 10 years].

A leiomyosarcoma of the rectum is a rare disease, and cases of a recurrence are even more rare. Reported is the case of a patient who, in 1987, underwent a per sacral resection of a rectal tumor which was diagnosed as leiomyosarcoma. In 1987, after symptoms that showed that the patient's feces had become narrower, a recurrence of this tumor was diagnosed, so that the patient again underwent an abdominoperineal rectomy. The postoperative course has been good. Details of this disease still remain unclear. In Japan, there have been only 10 such cases of a recurrence reported. Of these, a liver metastasis and a localized recurrence are the most common. Long term follow-ups are considered necessary.

[A retroperitoneal leiomyosarcoma--a case of non-concurrent double cancers with kidney cancer].

A retroperitoneal leiomyosarcoma in a 50-year-old man is reported. A laparotomy was performed on Dec. 17, 1987 and the tumor, weighing 120 g, was completely excised. The patient was followed up and has stayed healthy (1 year after operation). Although CT ultrasonography and arteriography are helpful in the diagnosis of retroperitoneal tumors, it is felt that a histological examination is indispensable for a qualitative assessment of the lesion. Surgical excision constitutes the fundamental treatment for a retroperitoneal leiomyosarcoma. However, the prognosis is usually poor and depends on successful radical surgery. This case involved non-concurrent double cancers with a retroperitoneal leiomyosarcoma and a kidney cancer.

[Detection and significance of calcified coronary arteries by computed tomography].

To investigate whether coronary artery disease is detected noninvasively, we correlated calcification on computed tomography (CT) with coronary angiographic (CAG) findings in 90 consecutive patients including 48 with angina pectoris or myocardial infarction. The mean age was 52.2 years and 83% were men. CT scans without contrast enhancement (3 sec scan time) were obtained at 1 cm thickness from the ascending aorta to cardiac apex to identify calcification in coronary arteries. Results; CAG revealed of 50 patients, and stenosis was present more often at calcified vessels on CT. (table; see text) Calcification occurred more often in 2 or 3 vessel disease. However, stenotic segments were not always calcified. We conclude that CT is sensitive for the detection of calcification and an important method for the diagnosis of coronary artery disease.

[Analysis of "rising up mechanism" in months-old infants--determination of contact area and center of gravity by pedoscope].

The author studied how the rising up mechanism changed with the development of motor function in healthy infants. 1. The rising up mechanism can be evaluated from the body support pattern characteristic to each stage of posture development in supine and prone positions, the body support (contact) area and shift in the center of gravity. 2. The support patterns in supine position were classified into 4 types; S-I ("squid type", 2-4 mo), S-II ("platform type", 4-6 mo), S-III ("bell type", 7-10 mo), S-IV ("separate type"), and their characteristics were described. 3. The contact area in supine position gradually decreased as the support pattern changed from S-I (area ratio, approx. 2/3) to S-III (area ratio, 2/5). 4. The center of gravity in supine position was located at 10th thoracic vertebra at 2-4 mo, then rose with age until it was stabilized at 7th thoracic vertebra at 10 mo. 5. The support patterns in prone position were classified into 7 types; P-I ("protector type", 2-3 mo), P-II ("joint square and round type", 3-4 mo), P-III ("elongated oval type", 4 mo-first half of 5 mo), P-IV ("spindle type", 4 mo-latter half of 5 mo), P-V ("ginseng type", 5-6 mo), P-VI ("tetrapod type", 7-10 mo), P-VII ("oval type", 9 mo onward), and their characteristics were described. 6. The contact area in prone position gradually increased as the support pattern changed from P-I (area ratio, 3/5) to P-V (area ratio, 9/slightly over 10), and gradually decreased as it changed thereafter to P-VII (area ratio, 7/slightly under 10). 7. The center of gravity in prone position swung over a wide range. It was located at 12th thoracic vertebra at 2-3 mo, then ascended as far as 9th thoracic vertebra at 4 mo-first half of 5 mo, and descended thereafter to be stabilized at 2nd lumbar vertebra (level of navel) at around 10 mo. 8. Particular attention was paid to the development of rising up mechanism in the upper extremities in observation of changes in the support pattern.(ABSTRACT TRUNCATED AT 250 WORDS)

[Role of singlet oxygen in pathogenesis of liver injury in rats treated with D-galactosamine].

A study was conducted to elucidate the possible role of singlet oxygen in pathogenesis of D-galactosamine-induced liver injury. Tissue and plasma levels of singlet oxygen were determined with chemiluminescence analysis. Following results were obtained: 1) Chemiluminescence as well as malondialdehyde, which is regarded as one of terminal products of lipid peroxidation, significantly increased in the liver and plasma of rats treated with D-galactosamine. 2) Elevation of plasma GPT and total bilirubin was also observed in rats with D-galactosamine-induced liver injury. Histological examination of the liver revealed submassive hepatic necrosis. 3) Administration of vitamin E, a radical scavenger of singlet oxygen, significantly inhibited the increases of chemiluminescence and MDA in the liver and plasma as well as the elevations of GPT and total bilirubin in the plasma. Histological changes of the liver were also found to improve significantly by vitamin E administration. In conclusion, singlet oxygen seems to be definitely involved, at least in part, in pathogenesis of liver damage induced by D-galactosamine. In addition, inhibition of the liver injury is possible, to some extent, by administration of vitamin E, one of the potent radical scavengers of singlet oxygen.

[Severe respiratory failure with pulmonary tuberculosis during initial phase of chemotherapy].

In seven patients with tuberculosis required ventilatory assistance, in our IRCU at the Habikino Hospital, we found worsening of the findings on chest X-ray film and clinical status during initial phase of chemotherapy, although their sputum became negative for acid-fast bacilli. On admission, six of them had large infiltration with cavities and discharged a large number of bacilli with elevation of ESR and hypoalbuminemia. We could get four patents off ventilator after start of PEEP therapy and high dose corticosteroid therapy. But three of them died at last, because they developed severe respiratory failure again. Pulmonary histology was available in five patients. We found interstitial and intra-alveolar pulmonary edema, intra-alveolar organization, hyperplasia of alveolar wall, and hyaline membrane formation. These findings were compatible with diffuse alveolar damage.
