Antigenic characterization of small, round-structured viruses by immune electron microscopy.

Small, round-structured viruses (SRSVs) detected from nonbacterial gastroenteritis outbreaks in Tokyo and Saitama Prefecture, Japan, during the period from 1977 to 1988 were tentatively classified into nine antigenic patterns from SRSV-1 (S-1) to SRSV-9 (S-9) by cross-immune electron microscopy (IEM). S-1 and S-2 appeared pattern specific, while S-3 to S-9, distinguishable from each other in their reactivity, appeared somewhat antigenically related. Their antigenic relatedness to the Norwal, Hawaii, and Otofuke agents was also examined by IEM by using antisera to these agents. S-3 appeared most closely related to the Norwalk agent. S-4 and S-5 were related to the Norwalk agent and, presumably, were distantly related to the Hawaii and Otofuke agents. S-6 and S-7 were related to the Hawaii and Otofuke agents. S-8 and S-9 were related to the Otofuke agent and, presumably, were distantly related to the Hawaii agent. The prevalence of each antigenic pattern in 38 outbreaks was examined: S-8 was implicated in 24% of the outbreaks S-5 in 16%, S-4 in 13%, S-9 in 13%, S-6 in 11%, and others in 5%.

Anti-ulcer effects of Trichosanthes fruits.

Anti-ulcer activities of fruits of Trichosanthes kirilowii Maximowicz var. japonica Kitamura (50% ethanolic extract, TKE) were investigated in rats. TKE, at doses of 100-1000 mg/kg, showed potent protection against experimental gastric lesions, namely, those induced by water-immersion, histamine, serotonin, HCl.ethanol, 0.6 N HCl, 0.2 N NaOH, 35% NaCl, and Shay' ulcer and acetic acid-induced gastric ulcer. At doses of 500-1000 mg/kg, TKE decreased the gastric secretion and acid output in pylorus-ligation (for 7 h), but 100 mg/kg of TKE had no influence on the gastric secretion. On the other hand, TKE exerted inhibition on the contractile responses of the isolated ileum of mouse to acetylcholine. These results suggested that TKE has an anti-ulcer effect.

A procedure for recording electroretinogram and visual evoked potential in freely moving cats.

A procedure for recording the electroretinogram (ERG) and the visual evoked potential (VEP) in freely moving cats was developed and used to test the visual toxicity of nalidixic acid. The electrodes for recording the ERG and VEP were implanted chronically in the sclera of the eye and on the surface of the visual cortex, respectively. Both the ERG and VEP were simultaneously recorded in freely moving cats using a slip ring that allows the cat to move freely without twisting the cables. After a 15-min dark-adaptation period, 20 responses to repetitive photic stimulation with an intensity of 2 joules and a 10-sec interstimulus interval were averaged by means of a minicomputer. The waveform of the ERG consisted of a negative wave (a-wave) followed by a positive wave (b-wave) with 2 to 3 oscillatory potentials on the rising slope. The VEP consisted of initial positive wave (P1 wave) followed by a negative wave (N1 wave) and late positive and negative deflections. The amplitude of the components of the ERG and VEP varied considerably from animal to animal, whereas within the same animal they were very stable. Using this technique, the effects of nalidixic acid, a retinotoxic compound, on the ERG and VEP were studied. Nalidixic acid produced a marked, but transient, decrease in the amplitude of the ERG b-wave after a single intravenous injection at a dose of 10 mg/kg. After repeated doses at 40 mg/kg/day for 2 weeks, the reduction in the amplitude of the b-wave was stronger, but remained transient. Upon ophthalmologic and histopathologic examination of the retina, no abnormalities indicating visual toxicity were observed. These results indicate that this procedure for recording the ERG and VEP is useful for evaluating the visual toxicity of drugs in freely moving cats.

[The clinical role of anaerobic threshold in physical training of patients with recent myocardial infarction].

Twenty-seven patients with recent myocardial infarction were studied to evaluate the clinical usefulness of the anaerobic threshold (AT) in physical training as cardiac rehabilitation at home. The 27 patients were divided into trained and control groups. Symptom-limited treadmill exercise tests were conducted before and six weeks after the prescribed aerobic training. Heart rate (HR), systolic blood pressure and respiratory gases (VO2, VCO2 and VE) were noninvasively measured throughout the exercise periods. AT was defined as systemic increase in the ventilatory equivalent O2 (VE/VO2) without an increase in the ventilatory CO2 (VE/VCO2) during the increasing work load. The patients in the trained group were obliged to perform aerobic walking training requiring 90-100% HR of the AT level for 30 min, four to six days per week. 1. In the trained group, exercise time, peak VO2, and AT on treadmill performance were significantly increased compared with those of the pre-training values. However, in the control group, these parameters were not significantly changed. A significant reduction in double products was observed during submaximal workload in the trained group. 2. Factors related to the training effects: The training effect expressed by delta peak VO2 correlated significantly with the initial peak VO2 (pre-training peak VO2) (y = 35.4 - 1.29x, r = 0.70), but there was no correlation between left ventricular function and delta peak VO2. These results indicated that aerobic exercise at home guided by the AT level is safe and beneficial for patients with recent myocardial infarction.

[Relationship between serum fatty acid composition and life style and/or health status in a rural population in Kagoshima].

Serum fatty acid compositions of 232 subjects (97 men and 135 women) living in a rural area of Kagoshima, were analyzed by gas-chromatography. Subjects were classified by sex into three groups (Group I, II and III) by cluster analysis according to concentrations of selected serum fatty acids. Dietary habits obtained through an interview and variable data related to health status presented by biochemical and biophysical measurements, were studied in relation to serum fatty acids of the three groups. The results are summarized as follows: 1) No significant differences in specific serum fatty acids were observed among the three groups for each sex, but males and females were characterized by the concentrations and compositions of omega-3 and omega-6 polyunsaturated acids, respectively. 2) Differences in urinary Na/K ratio among the groups were significant for males but no significant differences in urinary Na or K were observed between groups for either males or females. 3) For males or females significant differences in systolic blood pressure were seen between groups. Significant differences in distolic blood pressure were seen for female. Furthermore, the distribution among the three categories of blood pressure as defined by WHO, was significantly different among the groups for each sex. 4) Comparison of frequency distributions for dietary habits, showed that amount of salt and fruits consumed, and drinking habits for each sex, smoking habits among males, and the frequency of use of foods using fat or oil by females were significantly different. The amount of shellfish and vegetable consumed for either males or females was not significantly different among the three groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Cloning and sequencing of cDNAs corresponding to mite major allergen Der f II.

A cDNA library corresponding to mite protein was screened employing anti-Der f II antibody. Two possible clones were obtained, which contained plasmids, pFL1 and pFL11, respectively. Both plasmids had insertions of about 500 base pairs. The DNA sequences of the two insertions were determined, from which the amino acid sequences were deduced. The amino acid sequence of the purified native Der f II protein could be determined to 45 residues from the N-terminus. As a result of comparison, we concluded that the cDNAs prepared from live mite Dermatophagoides farinae corresponded to the mite allergen, Der f II.

[A case of unresectable pancreatic cancer that responded to UFT chemotherapy].

A 77-year-old patient with an unresectable pancreatic cancer was treated with a combination chemotherapy of UFT (600 mg/day, p.o.) and Lentinan (2 mg/week, i.v.). After three and a half months, a partial response (PR) was achieved, based on the criteria of Koyama, Saito, et al. The findings of the follow up CT scans suggested that 600 mg/day of UFT, rather than 300 mg/day, was effective as a chemotherapeutic agent. Although transient thrombocytopenia (62,000/mm2) and eruption were observed during this therapy, no serious side effects were experienced. After eighteen months of this chemotherapy, the patient has not succumbed and manifests a good quality of life.

[Clinical study of iohexol in excretory urography].

Excretory urography was performed using iohexol (Omnipaque 300 and Omnipaque 350) on 388 patients requiring urography in the Department of Urology, Nagoya University School of Medicine and 10 affiliated hospitals. Adverse reactions were observed in 3 cases (0.8%), but they were all mild. Good image efficacy was obtained in more than 95% of the patients injected 20 ml, 40 ml, and 100 ml of Omnipaque 300 and 40 ml of Omnipaque 350. There was no difference in the results of image efficacy between 40 ml (Omnipaque 300) and 100 ml (Omnipaque 300) groups. Therefore, the use of 40 ml of Omnipaque 300 for excretory urography was considered to be a very useful screening method for the urinary tract.

Preoperative assessment of the length of the stomach for esophageal substitution.

This method is easily applied and useful for the estimation of the actual length of the gastric tube in situations of reconstruction after subtotal esophagectomy. The actual length of the gastric tube was almost identical to that of the stomach which was measured from the preoperative roentgenogram. In the end, a safe reconstruction by this approach will decrease the operative mortality rate caused by the leakage of the anastomosis.

[Basic and clinical evaluation of rapid diagnosis of tuberculosis by detecting tuberculostearic acid].

Tuberculostearic acid (TSA) is known to be one of the characteristic lipid in the limited species of the order Actinomycetale, including Mycobacterium tuberculosis. We studied the significance of detecting TSA in the diagnosis of tuberculosis from clinical specimens collected from 791 patients with various respiratory diseases by using gas--chromatography/mass--spectrometry. By our method, the detectable limit of TSA was around 10(2)-10(3) bacilli of Mycobacterium tuberculosis, and our method is as sensitive as culture examination for tuberculosis. In sputa collected from patients with active pulmonary tuberculosis (n=169), TSA positively was around 90%, while less than 10% false positive ws also recognized. In pleural effusion (n=81) and bronchial washing (n=91) collected from patients with active tuberculosis, the positivity of TSA was around 70%. We could also detect TSA in about 30% of clinical specimens collected from patients suspected of tuberculosis. The diagnostic sensitivity of TSA for tuberculosis was similar to that of adenosine deaminase activity (ADA), while TSA was slightly superior to ADA in specificity. These findings indicate that detection of TSA from clinical specimens is useful to make rapid diagnosis of pulmonary tuberculosis.

[Cerebral blood flow of the non-affected brain in patients with malignant brain tumors as studied by SPECT; with special reference to adverse effects of radiochemotherapy].

The mean cerebral blood flow (mCBF) has been studied in 40 patients with brain tumors undergoing radiation and chemotherapy, and with ages ranging from 20 to 69 years, using 133Xe and single photon emission CT. A comparison was made of mCBF of 40 normal volunteers (34 males and 6 females) whose ages ranged from 25 to 82 years. The results were as follows: Some patients exhibited increased mCBF during external irradiation therapy. Over 3 months after the initiation of external irradiation, the average mCBF decreased significantly, ranging from 5.0 to 8.8 ml/100g/min, compared to the control group. A regression equation, which was obtained using multiple regression analysis, was delta mCBF = -3.140 + 0.181 A - 0.069 E - 0.022 V - 0.543 C (delta mCBF: subtract measured mCBF from presumed mCBF at the same age, A: age, E: dose of external irradiation, V: volume of lesion, C: CSF-cranial ratio, multiple regression coefficient: r = 0.536, P less than 0.002). This result means that the mCBF of non-affected hemispheres decreases in direct proportion to the volume of the lesion, degree of brain atrophy, and dose of external irradiation. In this study, the tendency for mCBF to decrease in direct proportion to these factors was found to be more pronounced among young patients. There were no side effects caused by the additional irradiated doses intraoperatively or caused by residual tumors on the mCBF of non-affected brains. Also, there was no significant difference in the average mCBF between cases in which whole and local brain irradiation was employed, as well as between cases in which there were different histopathological diagnoses.(ABSTRACT TRUNCATED AT 250 WORDS)

Distribution and forms of fluorine in whole blood of human male.

Distribution and absolute concentration of three kinds of fluoride/fluorine, i.e., free ionic, ionizable, and organic fluorine, in human whole blood and serum were determined. The method is based on low temperature oxygen plasma ashing, acid hydrolysis, gas chromatography using chlorotrimethylsilane, and fluoride ion-specific electrode. A large part (40%) of the total F is distributed in clot of the whole blood and 74% of the total F is in the form of organic fluorine.
